1.Study on the antiviral effect of C gene truncated HBV mutant in HBV infection

Objective To explore the effect of the C gene truncated HBV mutant on HBV replication. Methods The C gene truncated HBV vector pHBV-?C was constructed through the molecular clone in vitro,and then transfected transiently into HepG2 cell.The expression of S protein was assayed by ELISA and SDS-PAGE Western blot.After co-transfection with pHBV-?C and wild HBV genome adwR9 into HepG2 cell the DNA was detected quantitatively by real-tine fluorescence quantitative PCR in the culture medium and the cell. Results There was no significant difference in expression of S protein assay by ELISA and Western blot.The DNA of the cotransfected group with pHBV-?C and adwR9 was lower than that of control group in the culture medium and the cell. Conclusions(C gene) truncated HBV mutant can cause the reduction of HBV replication.
2.Angiotensin Ⅱ and receptors in rheumatoid arthritis

Angiotensin Ⅱ and type 1 receptor ( AT1R) are ab-solutely therapeutic targets in the hypertension and cardiovascular disease, and angiotensin Ⅱ gets involved in monocytes chemo-taxis and migration, T lymphocytes activation and proliferation, Th1 and Th17 immune function, and synoviocytes apoptosis inhi-bition in patients with rheumatoid arthritis ( RA) through binding to AT1R in the autocrine and/or paracarine manner. Drugs that limit angiotensin Ⅱ production by angiotensin converting enzyme inhibitors ( ACEIs) or blockade of angiotensin Ⅱ-AT1R action by AT1R blocker ameliorate immune and inflammation responses in RA patients. Additionally, angiotensin Ⅱ type 2 receptor (AT2R) expression increases in RA and experimental model, and activation of AT2R exerts a therapeutic effect on adjuvant-in-duced arthritis. AngiotensinⅡand its receptors in RA and auto-immune diseases are retrospectively reviewed in this paper.
3.Determination of ginsenoside Rg1 and ginsenoside Re in Wushenqi Oral Liquid by RP-HPLC

AIM: To develop a RP-HPLC method of determining ginsenoside Rg1 and ginsenoside Re in Wu-(shenqi) Oral Liquid(Radix et Rhizoma Ginseng,Radix Astragali,Fructus Ligustri Lucidi,etc.). METHODS: Hypersil-C_(18) column was used at 30 ℃.The mobile phase consisted of acetonitrile-water(103∶400) with 0.05% H_3PO_4.The detection wavelength was set at 203 nm.The flow rate was 1.0 mL/min. RESULTS: The linear ranges for ginsenoside Rg1 and ginsenoside Re were 0.188-6.016 ?g(r=0.999 9) and 0.197-6.323 ?g(r=(0.999)),respectively.Its average recoveries were 99.53% with RSD of 0.96% and 99.13% with RSD of 0.62%,respectively. CONCLUSION: This method is simple,accurate,reproducible,and can be used for the determination of ginsenoside Rg1 and ginsenoside Re in Wushenqi Oral Liquid.
4.Association analysis of IL-23R polymorphisms in allergic rhinitis.

OBJECTIVE:
Polymorphism of interleukin-23 receptor (IL-23R) gene was found being related with the susceptibility to several immune-related diseases. For the terminal differentiation of IL-17-producing effecter T-helper cells in vivo, the IL-23R gene is very important. As proved recently, Th17 cells might have a great influence to the pathogenesis of allergic airways disease. Our intention was paid to find any association between the polymorphisms in the IL-23R gene and allergic rhinitis(AR) in Chinese population.
METHOD:
A group of patients was involved in a case-control comparison, consisted of 239 AR patients and 271 control Chinese subjects. The study claimed to take some blood samples for DNA extraction and select 3 single-nucleotide polymorphisms in IL-23R individually genotyped by the PCR-RFLP method.
RESULT:
Comparison with the controls, a great growing prevalence of the homozygous rs7517847 GG genotype and G allele appeared in AR patients (P < 0.05, respectively). Besides, a great increased frequency of the GGA haplotypes was presented in AR patients in comparison with the controls (P < 0.05).
CONCLUSION
The results demonstrated a important association pattern between polymorphisms in IL-23R and AR in Chinese population. Between rs7517847 in an SNP of IL-23R and AR, a great association was identified.

5.Vascular intervention joint foot plastic surgery in the treatment of diabetic foot

Objective To evaluate the chnical value of intervention therapy combined with the foot plastic surgery in the treatment of diabetic foot.Methods 67 patients who accepted the intervention therapy combined with the plastic surgery for occlusive lesion of lower limbs arteries were selected.The clinical symptom,ankle brachial pressure index and hemodynamics were observed before and after treatment.All clinical indicators were compared between the therapeutic group and the control group.Results Among 67 patients,63 cases were successful in clinical treatment (the success rate:94.0%).One month,three months,six months after operation,the skin temperature,anklebrachial index,limp distance,clinical symptom,ankle brachial pressure index and hemodynamics were increased.Compared with preoperation,all above-mentioned indicators were improved obviously (P < 0.05,P < 0.01).Conclusion Intervention therapy for diabetic foot has the merits of less trauma,higher effectiveness of treating diabetic foot ulcers,defect repair,maximally reserved limbs and better shapes of limbs,which made both doctors and patients satisfied.It is a new direction for minimally invasive therapy.
6.Sociodemographic and clinical factors associated with interictal depression symptoms in adults with epilepsy

Objective To investigate the prevalence of adult epileptic patients with interictal depression symptoms(IDs) and identify early predictors of IDs. Methods Adult patients with epilepsy were recruited ( n =110,45 females and 65 males) ,age between 16 and 67 years ( median 24 years). The sociodemographic and clinical factors of patients were recorded. Hamilton Depression Scale ( HAMD ) were applied to evaluate interictal symptoms of depression ( at least 72 hours after the last epileptic seizure). According to HAMD score,the epileptic patients were divided into IDs ( ≥8 ) and non-IDs(<8) groups. The sociodemographic and clinical factors were compared between the two groups to identify the prevalence and early predictors of IDs in adult epileptic patients.Results The prevalence of IDs in adult patients with epilepsy was 38.2% ,49.0% in active epilepsy and 12.1 %in seizure freedom. 30.0% ,5.5% ,and 2.7% were experiencing mild-to-moderate (HAMD score≥8),moderateto-severe ( ≥ 18 ) and severe ( ≥25 ) depression. 42 patients who met the HAMD score≥8 were classified as IDs group,and the remaining 68 patients were classified as non-IDs group. With multiple stepwise backward logistic regreasion, independent predictors of IDs were epileptic seizures ( OR = 8. 845, P = 0. 003 ); symptomatic or cryprogenic epilepsy ( OR = 3.132, P = 0. 045 ); prolonged duration of illness ( OR = 1. 106, P = 0.004 ) and employment status (OR =0. 154, P=0.001 ). There were no relationship between seizure frequency and severity of IDs ( Kruskal-Wallis test, x2 = 4.5, P = 0. 104). Conclusion IDs is a frequent psychiatric comorbidity in adult patients with epilepsy. The prevalence of IDs is higher in those with active epilepsy compared with those in seizure freedom and most of them are mild-to-moderate. Epileptic seizure, symptomatic or cryprogenic epilepsy, prolonged duration of illness and employment status are independent predictors of IDs, but seizure frequency has nothing to do with the IDs severity of patients.
7.Neuropsychological characteristics in active epilepsy and its risk factors

Objective To assess the neuropsychological characteristics in active epileptic patients and investigate itsrisk factors. Methods Ninety adult epileptic patients included 60 active epileptic patients (two or more unprovoked seizures within 12 months) and 30 age-, sex-, education-, course of disease- and seizure type-matched seizure-free subjects (without epileptic seizure for at least 1 year) . The neuropsychological tests including trail making test,digit symbol test, verbal fluency test,digit span test and hamilton depression scale( HAMD) ,were used to detect mental and motor speed, attention, language, working memory and depression symptoms respectively. The neuropsychological tests were compared between active and seizure-free epileptic patients and identified the risk factors of neuropsychological deficits in active epileptic patients. Results Compared to seizure-free subjects, active epileptic patients had significantly worse scores in digit symbol test, verbal fluency test, digit span test ((47.45 ±18. 812) vs(56.40 ±13. 631), (25. 25 ±8. 163) vs(30.40 ±8. 414), (10. 39 ±2. 228) vs( 11. 80 ± 2.074) respectively) ; more time to accomplish the trail making test A and B((64. 35 ±31.710) vs( 45. 47 ± 16. 309) , ( 133. 18 ± 47. 331 ) vs ( 98. 00 ± 35. 003 ) respectively) ; and higher scores in depressive symptoms ((9.12 ±6.219)vs(3.77 ±3.997) ,all P<0.05). Within active epileptic group,significant predictors of neuropsychological deficits were identified in a stepwise linear regression analysis: advancing age was significantly negatively correlated with digit symbol test(β = -0. 468, P = 0. 000) , digit span test (β = -0. 439, P = 0. 000), trail making test A (β =0.365, P = 0.003) and B(β = 0.346, P=0.002) ; higher scores on depressive symptoms was significantly negatively correlated with digit symbol test (β = -0.244, P = 0.015) ; mental work,high-education level and monotherapy were positively correlated with some of the cognitive function subscales. Conclusion This study suggests that active epilepsy can have a direct adverse effect on cognition and depression symptoms. Multi-drug therapy, severity of depression symptoms, advancing age, low-education level and non-mental work are the predictors of neuropsychological impairment in active epilepsy. In addition, good seizure control even after 1 year can have a beneficial impact on cognitive and depression prognosis.
8.Factors and neonatal outcomes associated with histologic chorioamnionitis after premature rupture of membranes in the preterms

Objective To investigate factors and neonatal outcomes associated with histologic chorioamnionitis (HCA) in preterm premature rupture of membranes (PPROM).MethodsFrom Jan.2008 to Jun.2011,230 women with PPROM at 28 -33 +6 weeks of gestation undergoing deliveries in the Second Affiliated Hospital of Wenzhou Medical College were studied retrospectively.According to placental histopathologic findings,those patients were categorized into two groups,including 138 cases in histologic chorioamnionitis (HCA group ) and 65 cases in non-chorioamnionitis (control)group.Age,parity,gestational age of PPROM and delivery,latency period,oligohydramnios,white blood cell (WBC) count and serum C-reactive protein (CRP) level at admission and before delivery,the incidence of neonatal respiratory distress syndrome (NRDS),neonatal pneumonia,bronchopulmonary dysplasia,necrotizing enterocolitis,early-onset neonatal sepsis,abnormal brain sonography findings and mortality were compared between two groups.Results( 1 ) The incidence of HCA was 68.0.% ( 138/203 ) in all 203 cases with PPROM.(2) The occurring ruptured membrane gestation in HCA group was ( 31.1 ± 1.5 ) weeks,which were significantly earlier than (32.0 ± 1.3 ) weeks in control group ( P < 0.05 ).The level of CRP of (8.2 ± 14.9) mg/L before delivery in HCA group was significantly higher than (5.5 ±7.2) mg/L in control group (P < 0.05).The rate of oligohydramnios and cesearean sections were 55.1% (76/138) and 45.7% (63/138) in HCA group,which were significantly higher than 30.8% (20/65) and 29.2% (19/65) in control group (P <0.05).There were no significant difference in patient's age,parity,WBC count and CRP at admission between two groups (P > 0.05 ).The latency period did not show significant difference between (140± 116) hours in HCA group and (129 ± 125) hours in control group (P > 0.05).(3) Using multivariable logistic regression models,oligohydramnios ( OR =2.937 ),gestational age of PPROM < 32 weeks ( OR =2.352),serum CRP level > 8 mg/L before delivery ( OR =4.923 ) and latency period > 48 -168 hours (OR =4.439) were significantly associated with HCA (P <0.05).(4) The gestational age of delivery and birth weight of HCA group were significantly lower than those of control group [ ( 32.0 ± 1.5 ) weeks vs.( 32.7 ± 1.5 ) weeks,( 1680 ± 379) g vs.(2017 ± 333) g,respectively,P < 0.05 ].The incidence of Apgar <7,abnormal brain sonograhy findings, neonatal pneumonia,bronchopulmonary dysplasia,early-onset neonatal sepsis and mortality in HCA group were significantly higher than those in control group [20.3% (28/138) vs.7.7% (5/65),14.5% (20/138) vs.4.6% (3/65),12.3% (17/138) vs.3.1%(2/65),5.8% (8/138) vs.0,6.5% (9/138) vs.0,12.3% (17/138) vs.3.1% (2/65),respectively,P < 0.05 ].The incidence of necrotizing enterocolitis ( 1.5%,2/138 ) in HCA group was higher than that of controlgroup(0) and the incidence of NRDS ( 18.8%,26/138) in HCA group did not show statistical difference with 21.4% ( 14/65 ) in control group ( P > 0.05 ).ConclusionsIt was found that HCA was significantly correlated with lower gestational age of PPROM,higher serum CRP level before delivery,prolonged latency period and oligohydramnios in PPROM.HCA could increase the neonatal morbidity and mortality.
10.Preliminary design of the database and registration system for the national malignant tumor interventional therapy

Objective This research is one of the sub-researches of"The comparative study of the standards of interventional therapies and the evaluation of the long-term and middle-term effects for common malignant tumors",which is one of the National Key Technologies R&D Program in the eleventh five-year plan. Based on the project,the authors need to establish an international standard in order to set up the national tumor interventional therapy database and registration system.Methods By using the computing programs of downloading software,self-management and automatic integration,the program was written by the JAVA words.Results The database and registration system for the national tumor interventional therapy Wag successfully set up,and it could complete both the simple and complex inquiries.The software worked well through the initial debugging.Conclusion The national tumor interventional therapy database and registration system can not only precisely teU the popularizing rate of the interventional therapy nationwide,compare the results of different methods,provide the latest news concerning the interventional therapy,subsequently promote the academic exchanges between hospitals,but also help as get the information about the distribution of the interventional physicians,the consuming quantity and variety of the interventional materials,so the medical costs can be reduced.
