1.Effects of Midazolam Combined with Tramadol on Prognosis of Patients Underwent Tumor Resection in Gastrointestinal Surgery Department

OBJECTIVE:To investigate the effects of tramadol combined with midazolam on prognosis of patients underwent tumor resection in gastrointestinal surgery department. METHODS:In retrospective study,200 tumor patients of gastrointestinal sur-gery department were divided into observation group(100 cases)and control group(100 cases). Observation group was given Tra-madol hydrochloride tablet 100 mg,3 times a day+Midazolam injection 30 mg,added into 0.9% Sodium chloride injection 60 mL,via intravenous pump,2.5 mL/h,for consecutive 24 h,for consecutive 3 d. Control group was given Tramadol hydrochloride injection 100 mg,intramuscular injection,3 times a day,for 3 d. The hospitalization duration,postoperative ambulation time, sleep quality score,pain degree score,the occurrence of postoperative hypostatic pneumonia,and the incidence of adverse reactions in 2 groups were observed. RESULTS:The hospitalization duration and postoperative ambulation time of observation group were significantly shorter than those of control group;the sleep quality score of observation groups was significantly higher than that of control group;the severity of pain,the incidence of hypostatic pneumonia were significantly lower than control group,with statisti-cal significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLU-SIONS:Midazolam combined with tramadol can significantly reduce the postoperative pain of tumor patients in gastrointestinal tu-mor surgery department,shorten hospitalization time and postoperative ambulation time,improve the sleep quality of patients and reduce the incidence of hypostatic pneumonia with good safety.
2.Induction of extracellular hydroxyl radical in striatum of rats by intracerebral perfusion of 6-OHDA

Aim To provide an approach to drug filtration and the research into damage of brain cell caused by senile retrograde affection and oxidative stresses,a model of increased extracellular hydroxyl radical in striatum of rats was established by intracerebral perfusion of 6-hydroxy dopamine(6-OHDA).Method Microdialysis was used to establish the model,hydroxyl radicals captured by salicylic acid yielded 2,3-dihydroxy benzyl acid(2,3-DHBA) and 2,5-dihydroxy benzyl acid(2,5-DHBA) which were measured by HPLC-ED.Results After 75 minuets of the perfusion with 6-OHDA in brain of rats,the levels of 2,3-DHBA and 2,5-DHBA in the model group increased to 6.6 and 3.4 times compered with the control group respectively;and the 2,3-DHBA was still higher than that of the control group in the whole course of observation(P
3.Clinical analysis of functional delayed gastric emptying after pancreatodudenectomy:a report of 28 cases

Objective To study the etiology,diagnosis and treatment of functional delayed gastric emptying(FDGE) resulting from pancreatoduodenectomy.Methods From June,1990~June,2003,136 patients received pancreatoduodenectomy,whose clinical data were retrospectively analyzed.Upper gastrointestinal(radiography) and endoscopy were the main methods of examination.Results Twenty-eight cases were(complicated) with FDGE in the 136 patients(20.6%) after operation.The occurrence of FDGE was(correlated) with hyperbilirubinemia,diabetes,duodenal obstruction,pancreatic fistula and abdominal infection.All patients were cured with conservative treatment.The recovery time of gastric motility was 14-42 days,average time was 28 days.Conclusions Hyperbilirubinemia,diabetes,duodenal obstruction,pancreatic(fistula) and abdominal infection were the main causes of FDGE.
5.Effects of Duxil on striatum extracellular acetylcholine,glucose and lactic acid levels in rats of cerebral energy metabolism impairment

7.The method of continuous measuring extracellular acetylcholine and choline levels in striatum of freely moving awake rats

AIM To establish the method of continuous measuring extracellular Acetylcholine and Choline levels in[FQ(10*2。46,X-WZ]-striatum of freely moving awake rats. METHODS The microdialysis and high performance liquid chromatography(HPLC)-post column Immobilized enzyme reactor(IMER)-electrochemical detection(ED) were used to continuously measure extracellular acetylcholine and choline levels in striatum of freely moving awake rats. RESULTS Extracellular acetylcholine and choline levels were higher at the beginning of perfusion, then decreased gradually. The ACh concentration reached a stable level after 240 min while the Ch continuously decreased in the whole perfusion period of 450 min. The detection limit for ACh and Ch, at a S/N ratio of three, was found to be 150 fmol per injection. In the range of 0.125~1.0 ?mol?L -1 , the relation between the amounts and the peak amplitude was linear. CONCLUSION HPLC-IMER-ED is a sensitive and stable method to measure the ACh and Ch. Coupled with microdialysis, it can been successfully used for monitoring the extracellular ACh and Ch levels of freely moving awake rats. -
8.The current status and evaluation of comprehensive therapy of pancreatic carcinoma

The incidence of pancreatic carcinoma is increasing obviously in recent years with a serious threat to the people's life, and yet there is not a single treatment for obtaining satisfactory prognosis. At present, the radical resection is the primary method for resectable pancreatic carcinoma, together with radiotherapy can improve the surgical resection rate and reduce the dissemination of tumors. Intraoperative radiotherapy can alleviate the pain and increase the survival rate, but the role of postoperative radiotherapy and chemotherapy is still in controversial. There is a lot of advantages for regional chemotherapy theoretically, but lack of evidence in practice. Physical therapy and biological therapy in the treatment of pancreatic carcinoma have been recognized extensively. It is possible to change fundamentally the current status of treatment and to improve the long-term survival rates with the quality of life until establishing a comprehensive treatment system mainly depended on surgical resection with combination of radiotherapy, chemotherapy, physical and biological treatment. The interventional therapy has been significantly developed as an important palliative treatment during recent years.

The ?-radioactive seed brachytherapy source has been widely employed in the implantation therapy for the prostatic carcinoma and the ophthalmic lesions.In this study the dosimetric parameters for characterization of a low-energy interstitial brachytherapy source 125I were calculated according to dose calculation formalism recommended by AAPM TG-43U1.For data processing,a 0.28 cm active length was used for the geometry function.The dosimetry parameter air-Kerma strength,dose rate constant,radial dose function and anisotropy function were estimated by means of the EGS5 Monte Carlo code.The results obtained from this study are in good agreement with the corresponding values recommended by TG-43U1 and with the data reported by Dolan,et al.
10.Comparison between microinvasive puncture and small bone window craniotomy in the treatment of moderate cerebral hemorrhage

Objective To evaluate and compare the curative effect between the microinvasive craniopuncture therapy and the clearance of hematoma by craniotomy with small bone flap in treating patients with moderate cerebral hemorrhage (30-60 ml)in the basal ganglion part of the brain. Methods Ninety-five patients with intracerebral hemorrhage were randomly divided into treatment group (microinvasive craniopuncture therapy) and control group (the clearance of hematoma by craniotomy with small bone flap). The main indexes of evaluation were the neurological impairment degree (NID) on the 14th day after treatment, activities of daily living (ADL) by the end of the third month, the incidence rate of complications, and the case fatality during 3 months. Results On the 14th day after treatment, there was no significant difference between the two groups in the NID and the ADL of patients. The incidence rate of respiratory tract infection, gastrointestinal hemorrhage, electrolyte disorder in treatment group [16.33% (8/49), 6.12% (3/49), 6.12% (3/49), respectively] was significantly reduced than those of control group [56.52% (26/46), 21.74%(10/46), 21.74% (10/46),respectively] during hospitalization (P < 0.05). By the end of the third month, there was significant difference in favorable outcomes (Barthel index 95-100) (χ~2 = 18.7524,P =0.0009) and in improving the ADL (MRS)(t =5.2723,P =0.0001) between the two groups [39.13% (18/46), 4.65% (2/43),respectively]. In ease fatality, there was no significant difference between the two groups [6.12% (3/49),6.52% (3/46),respectively]. Conclusion As compared with the clearance of hematoma by craniotomy with small bone flap, the microinvasive craniopuncture therapy can remarkably reduce the incidence of complications, and improve the ADL of patients with moderate cerebral hemorrhage (30-60 ml) in the basal ganglion, and decrease disability without increasing fatality.
