1.The Epidemic Characteristics of Diabetes among Residents in Rural and Urban Area in Sichuan

Objective To understand the diabetes prevalence of urban and rural residents in Sichuan,as the basis for prevention and control of diabetes. Methods By multiple stage cluster sampling method,4 475 residents were selected from 3 240 families of six districts in this survey. The content of blood sugar was determined and disease history of diabetes was investigated in the sampled population. Results The diabetes prevalence of population aged over 3-years was 2.2%,the standardized rate was 2.6%,and it was 4.0% and 3.4% respectively among population aged over 18-years. The diabetes prevalence of population aged over 18-years in urban area was 8.8%,the standardized rate was 6.22%,and it was 1.1% and 1.0% respectively in rural area,the standardized rate in urban area was higher than that in rural area (u=7.04,P
2.Structural identification of one new phenolic alloside from stems of Schisandra propinqua var. intermedia

Objective To study the bioactive components from stems of Schisandra propinqua var. intermedia. Methods Compounds were separated with acombination of multi-chromatography. Their che- mical structures were determined on the basis of spectral analysis and chemical evidence. Results Nine compounds were isolated from S. propinqua var. intermedia. The structures were elucidated as vanillic acid- 4- O-?-D-allopyranoside (Ⅰ), salidroside (Ⅱ), 2-hydroxy-5-(2-hydroxyethyl)-phenyl-?-D-glucopyranoside (Ⅲ), 3, 4-dimethoxyphenyl-4-?-D-glucopyranoside (Ⅳ), 3, 5-dihydroxybenzoic acid-4-O-?-D-glucopyranoside (Ⅴ), 3, 5-dimethoxy-4-hydroxy-benzoic acid (Ⅵ), 4-methoxy-benzoic acid (Ⅶ), vanillic acid (Ⅷ) and protocatechuic acid (Ⅸ). Conclusion Compound Ⅰ is a new phenolic alloside and others are isolated from S. propinqua var. intermedia for the first time.
3.High-normal blood pressure and carotid atherosclerotic plaque formation: The results from stroke population screening in Dongying, Shandong province

Objective To investigate the correlation between different blood pressure levels and carotid atherosclerotic plaque formation.Methods The population participated in stroke screening were selected from September 2012 to January 2013 in Dongying,Shandong province.The subjects met the diagnostic criteria of ideal blood pressure (systolic blood pressure [SBP] < 120 mm Hg and diastolic blood pressure [DBP] <80 mm Hg,1 mm Hg =0.133 kPa),normotensive (SBP 120-129 mm Hg and/or DBP 80-84 mm Hg),and high-normal blood pressure (SBP 130-139 mm Hgand/or DBP 85-89 mm Hg)were selected.The questionnaires,physical examinations,blood biochemical tests,and neck vascular ultrasound examination were performed.The detection rates of carotid plaque among the normotensive,high-normal blood pressure and ideal blood pressure groups were compared.Multivariatelogistic regression analysis was used to identify the independent risk factors for carotid plaque.Results The proportions of men and diabetes,as well as the levels of body mass index (BMI) and fasting blood glucose (FBG) of the normotensive and high-normal blood pressure groups were significantly higher than those of the ideal blood pressure group (all P <0.05).The detection rates of carotid plaque of the normotensive,high-normal blood pressure and ideal blood pressure groups were 12.41%,38.14% and 49.45%,respectively.The proportions of men and age,as well as the levels of SBP,FBG,and homocysteine (Hcy) of the carotid plaque formation group were significant higher than those of the non-carotid plaque formation group (all P <0.05).Multivariable logistic regression showed that age,SBP,FBG,and Hcy were the independent risk factors for carotid atherosclerotic plaque formation,while female gender was an independent protective factor.After adjusting the risk factors such as age,FBG,and Hcy,etc,the risk of plaque formation of the high-normal blood pressure group was significantly higher than that of the ideal blood pressure group (odds ratio 1.354,95% confidence interval 1.028-1.783; P =0.031),while there was no significant difference between the normotensive group and the ideal blood pressure group.Conclusions High-normal blood pressure was significantly associated with the increased risk of carotid plaque formation,which was an independent risk factor for carotid plaque formation.
4.Effect and mechanism of dexmedetomidine on propofol-induced apoptosis of cortical neurons in rats

Objective To investigate the mechanisms of the protective effects of dexmedetomidine against the propofol-induced neuroapoptosis in primary cultured cortical neurons.Methods The neurons were cultured 7days and then divided into four groups: vehicle-control group (treated with equal volume of intralipid), propofoltreated group (treated with 500 μmol/L propofol), propofol plus dexmedetomidine treated group (treated with 500 μmol/L propofol and 0.1 μmol/L dexmedetomidine), and LY294002 pretreated group (treated with 500 μmol/L propofol ,0.1 μ mol/L dexmedetomidine and 10 μmol/L LY294002).12 hours after different treatments, neuron viability was measured by MTT assay,neuroapoptosis was detected by Hoechst33258 staining, and the levels of pAkt and Bcl-2 protein were detected by Western blot.Results Compared with the vehicle-reduced group,propofol reduced neuron viability greatly((53.4±4.2)% vs (99.9±6.3)%;P<0.01), but increased neuroapoptosis greatly((44.6±4.3)% vs (5.8±0.4)%;P<0.01).The levels of pAkt((0.41±0.03) vs (0.86±0.07))and Bcl-2 ((0.15±0.02) vs (0.72±0.03)) were decreased greatly (both P<0.01).Compared with propofol treatment group, the neuron viability of propofol plus dexmedetomidine group were increased greatly((86.4±5.3) % , P<0.01) ,the neu roapoptosis was decreased greatly ((23.1 ± 3.5) %, P< 0.01), and the levels of pA kt (0.8 ± 0.03) and Bc1-2 (0.52 ±0.05) were increased greatly (both P<0.01).Compared with propofol plus dexmedetomidine treated group,LY294002 inhibited the protective effects of dexmedetomidine, decreased neuron viability greatly ((64.3±5.1) % ,P<0.01), increased the number of apoptotic neurons((38.8±4.9) %, P<0.01), and reduced the levels of pAkt (0.52±0.04) and Bcl-2(0.31±0.02) significantly (P<0.01).Conclusion Dexmedetomidine exerts the neuroprotective effects against propofol-induced neuroapoptosis by activating the PI3K-Akt-Bcl-2 signalling pathway.
5.Protective effect of 17β-estradiol against propofol-induced apoptosis of primarily cultured cortical neurons

Objective 17β-estradiol is known to have a neuroprotective effect.The aim of this study was to investigate the effects of 17β-estradiol on propofol-induced neuroapoptosis in primarily cultured cortical neurons. Methods Rat cortical neurons were primarily cultured for 7 days and randomly divided into groups A ( vehicle control) , B, and C, treated with equal volume of 20%intralipid, 500 μmol/L propofol, and 500 μmol/L propofol +0.1 μmol/L 17β-estradiol, respectively.At 12 hours after treatment, the morphology of the neurons was observed under the microscope, their survival rate calculated by MTT, their apoptosis was deter-mined by FCM assay, and their mitochondrial membrane potential measured by fluorescent dye rhodamine 123. Results Compared with group A, group B showed a significantly reduced number of neurons, lack of 3-dimensional appearance, unclear contour, and fractured neuron axons, but a remarkable improvement was observed in the propofol-induced morphological damage in group C.The survival rate of the neurons and the mitochondrial membrane potential were markedly decreased in group B ([52.3 ±5.2]% and [59.1 ± 5.3]%) as compared with groups A ( [99.9 ±3.6]%and [99.6 ± 5.8]%) and C ([90.1 ±7.2]%and [89.2 ±7.1]%) (both P<0.01 ) , while the rate of neuroapoptosis significantly increased in group B ([43.4 ±4.6]%) in comparison with A ([3.1 ±0.2]%) and C ([22.3 ±3.2]%) (both P<0.01). Conclusion 17β-es-tradiol can protect against propofol-induced apoptosis of primarily cul-tured neurons by inhibiting the reduction of their mitochondrial membrane potential.
6.DWI-ASPECTS predicts collateral circulation compensation in patients with acute middle cerebral artery infarction after intravenous thrombolysis

Objective To evaluate the value of Alberta Stroke Program Early CT Score on diffusion weight-ed imaging(DWI-ASPECTS)in predicting the leptomeningeal collateral circulation(LMA)compensation of isch-emic stroke with middle cerebral artery stenosis or occlusion after intravenous thrombolysis. Methods A total of 178 patients with ischemic stroke confirmed as severe middle cerebral artery stenosis or occlusion by imaging were enrolled in the study.All the patients were treated by rt-PA intravenous thrombolysis.The baseline clinical date, DWI-ASPECTS,mRS and NIHSS were collected. LMA was assessed by cranio-cervical CTA. Results Compared with the poor collateral circulation group,the triglyceride level and DWI-ASPECTS were significantly increased in good collateral circulation group(P<0.05).The proportion of hypertension,NIHSS score,mRS score in good col-lateral circulation group were significantly lower than those in the poor collateral circulation group(P < 0.05). Compared with the poor collateral circulation group,insula,the ASPECTS areas M1 to M4,M6 and insula showed significantly fewer infarctions in good collateral circulation group(P<0.05).ROC analysis showed the area under ROC curve(AUC)of DWI-ASPECTS to predict LMA compensation were 0.932,the cutoff point of DWI-ASPECTS was 7.5.the sensitivity and specificity were 81.% and 94.1%.Conclusions The tissue protective role of good lepto-meningeal collateralization seems to be more pronounced in cortical and subcortical areas M1 to M4,M6 and the in-sula.DWI-ASPECTS can effectively predict the collateral circulation compensation in patients with acute middle ce-rebral artery infarction.
7.Identification of aberrantly expressed miRNAs in rectal cancer.

OBJECTIVE:
To identify aberrantly expressed miRNAs in rectal cancer.
METHODS:
We used the miRCURY™ Array® LNA microRNA chip (v.14.0) to evaluate miRNA expression levels between rectal cancer tissues and adjacent non-tumor tissues; an average change more than 2-fold (and P value less than 0.05) was set as a cutoff level. All 6 paired rectal cancers were classified pathology stage C or D.
RESULTS:
Eighty-eight miRNAs were up-regulated and 46 miRNAs have been reported in colorectal cancer; 40 miRNAs were down-regulated in rectal cancers and 15 miRNAs have been reported in colorectal cancer. To compare the relative miRNA expression levels as measured by RT-qPCR and chip analysis, we analyzed expression levels of these miRNAs in the cancer tissues. The results showed that miRNA expression (increased or decreased) in the paired benign and tumor tissue was consistent between the two methods in all cases. Expression levels of 6 up-regulated miRNAs (by chip analysis compared to RT-qPCR) varied in a range from -11.9% to 39.1% . Expression levels of 5 down-regulated miRNAs varied in a range from 1.4% to 29.4%. The Pearson correlation of relative miRNAs expression levels was analyzed by cDNA array versus RT-qPCR, and found to be 0.96 (P<0.01).
CONCLUSION
miRNA profile in rectal cancer showed unique characteristics, and identified a series of new, aberrantly expressed miRNAs.

8.Effect analysis of multi-interventional modes mainly with mechanical thrombectomy for large artery occlusive acute cerebral infarction

Objective To investigate the effectiveness and safety in patients with largeartery occlusive acute cerebral infarction who received multi-interventional modes mainly with mechanical thrombectomy and its related factors affecting prognosis. Methods The clinical data of 56 patients with large artery occlusive acute cerebral infarction were analyzed retrospectively. The clinical characteristics (gender,age,and underlying diseases),timing of treatment (time from ictus to puncture,time from puncture to recanalization), multi-interventional mode therapies (intra-arterial thrombolysis,thrombectomy,balloon dilation,and stenting, etc. ),and distribution of offending vessels were observed. The modified Thrombolysis in Cerebral Ischemia Scale (mTICI)grade was used to evaluate revascularization. The National Institute of Health Stroke Scale (NIHSS)score was used to observe the neurological function at 24 h before and after procedures. The modified Rankin scale (mRS)was used to evaluate the prognosis at 3 months after procedure. The safety of the treatment was evaluated with operative complications (mainly symptomatic intracranial hemorrhage)and mortality. The patients were divided into either a good prognosis group (n = 34;mRS≤2)or a poor prognosis group (n =22;mRS≥3)according to the prognosis at 3 months after procedure. They were analyzed with univariate analysis. The factors influencing the prognosis were further analyzed with multivariate logistic regression analysis. Results (1)The recanalization rate in 56 patients was 78. 6%(n = 44),in which basilar artery was the highest,reaching 93. 8% (15 / 16),middle cerebral artery was 87. 0% (20 / 23). The NIHSS score at 24 hours was 10 ± 7,it was lower than 16 ± 6 on admission. There was significant difference (t =6. 401,P <0. 01). At 3 months,34 patients (60. 7%)had good prognosis,4 (7. 1%)died,and 8 (14. 3%) had symptomatic intracranial hemorrhage. (2)Multiple factor analysis showed that the high level of recanalization was a protective factor for good prognosis (OR,0. 465,95% CI 0. 267 -0. 809,P =0. 007). Diabetes was an independent risk factor for poor prognosis (OR,5. 535,95% CI 1. 101 -27. 835, P = 0. 038). Conclusion Acute large artery occlusive cerebral infarction treated with the intra-arterial multi-interventional modes may quickly and effectively restore intracranial blood flow. It has the characteris-tics of high recanalization rate and good prognosis,and the higher the level of recanalization,the better the prognosis. Diabetes is an independent risk factor for poor prognosis.
9.Reexploration of glucose metabolism in childhood obesity

Objective :
To analyze the status of glucose metabolism in obese children.
Methods :
266 cases of obese children were included in the study,fasting plasma glucose ( FPG) ,fasting insulin ( FINS) ,glycosylated hemoglobin ( HbA1c) were measured ,Oral glucose tolerance test ( OGTT) and insulin release test ( IRT) were carried out,insulin resistance index ( HOMA-IR) ,quantified insulin sensitivity index ( QUICKI) and peak insu- lin /fasting insulin (Ip / I0 ) were calculated.Two hundred non-obese healthy children were used as control group, and glucose metabolism indexes were compared between the two groups. Glucose metabolism indexes and the incidence of insulin resistance (IR) were compared in children with different degrees of obesity .Prediabetes risk factors and body mass index(BMI) correlation were analyzed in obese children.
Results :
The proportion of predia- betes and type 2 diabetes mellitus in obese children was 18. 0% (48 /266) .FINS,HOMA-IR and HbA1c in obesi- ty group were higher than those in the control group,but QUICKI was lower than that in the control group (P < 0. 05) .There was no significant difference in FPG between the two groups (P = 0. 423) .FINS and HOMA-IR of severe obesity group were higher than those of mild to moderate obesity group,Ip / I0 was lower than that of mild to moderate obesity group (P<0. 05) ,and there was no significant difference in FPG,QUICKI,HbA1c and 2hPG of severe obesity group.There was no significant difference in IR incidence between mild to moderate obesity group and severe obesity group (P = 0. 163) .Logistic regression indicated that BMI and acanthosis nigricans was great influential on prediabetes with no statistical significance(P>0. 05) .Correlation analysis showed that BMI had no correlation with FPG (P = 0. 160) ,but was positively correlated with FINS,HOMA-IR , 2hPG and HbA1c (P < 0. 05) ,and negatively correlated with QUICKI and Ip / I0 (P<0. 05) .
Conclusion
Abnormal glucose metabolism is available in nearly 20% of obese children.The prevalence of insulin resistance in obese children is not affected by the degree of obesity.
