1.Vitrectomy for Giant Traumatic Avulsion of Vitreous Base

Objective To describe the clinical features and observe the effect of vitrectomy on traumatic avulsion of vitreous base with giant retinal dialysis.Methods Seven cases with giant retinal dialysis(rang 120~150?) secondary to traumatic avulsion of vitreous base and PVR grade B to C 3 were retrospectively reviewed.All eyes had recieved vitrectomy,scleral buckle and lenses removed,and six eyeballs underwent postoperative sequential fluid-gas exchange.Results In most cases,the dialyses and avulsed vitreous base developed in the temponate quadrants and complicated lenses dislocation,pupilloparalysis,and vitreous hemorrhage.All cases attained success after disappearance of the intraocular tamponate.The major complication was elevated intraocular pressure,which was transient.Conclusions Giant retinal dialysis may result from traumatic avulsion of vitreous base.These dialysis often become clinical manifestation for several days or longer following ocular contusion.Vitrectomy is a very effective management for these cases.
2.Nerve growth factor prevents apoptosis induced by indomethacin in cultured human fetal retinal pigment epithelial cells

Objective To investigate the protective effect of nerve growth factor (NGF) on apoptosis of cultured human fetal retinal pigment epithelium (HFRPE) cells induced by indomethacin (IN) in vitro. Methods Subcultured HFRPE cells were treated with different concentrations of IN to establish apoptotic model. The protective effect of NGF on apoptosis of cultured HFRPE cells were assessed using an acridine orange (AO) staining method and transmission electron microscopy (TEM). Results HFRPE cells exposed by 200 600 ?mol/L IN for 24 hours elicited typical apoptosis morphological changes, including condensed chromation, nuclear fragmentation and reduction of nuclear size and cell volume. There was a statistically difference in HFRPE cells with apoptosis between 200 ?mol/L IN+500 ?g/L NGF and 200 ?mol/L IN groups ( q=3 9204,P=0.0320) ; there was a significant difference in HFRPE cells with apoptosis in 400 ?mol/L IN+500 ?g/L NGF and 400 ?mol/L IN as well ( q=9 7915,P=0 0001). Conclusion NGF has an protective effect on IN induced HFRPE cells apoptosis.
3.Expression of NF-?B and ICAM-1 in rat′s retina injured by ischemia-reperfusion and the effect of pyrrolidine dithiocarbamate on the expression

Objective To investigate the expression of nuclear factor(NF)-?B and intercellular adhesion molecule (ICAM)-1 in rat′s retina injured by ischemia-reperfusion, and the effect of pyrrolidine dithiocarbamate (PDTC) on the expression of NF-?B and ICAM-1. Methods The model of retinal ischemia-reperfusion was set up in 60 SD rats, which were divided into two groups with 30 rats in each: ischemia-reperfusion group and ischemia-reperfussion with injection of PDTC group. The left cephalic artery of each rat was ligated, and the right side was the control. Every group was subdivided into group 1 hour, 6, 12, 24, 48, and 72 hours after ischemia-reperfusion injury, and with 5 rats in each group. mRNA of NF-?B and ICAM-1 mRNA was measured by in situ hybridization (ISH) method in rat′s retina. Every rat underwent electroretinography (ERG) at the corresponding time before executed by neck breaking. Results In ischemia-reperfusion group, expression of NF-?B and ICAM-1 was detected at the 6th hour after ischemia-reperfusion, reached the highest level at the 24th hour, and weakened gradually later. In ischemia-reperfusion with injection of PDTC group, expression of NF-?B and ICAM-1 was detected at the 12th hour after ischemia-reperfusion, and reached the highest level at the 24th hour but lower than that in ischemia-reperfusion group. No expression of NF-?B and ICAM-1 was found in the control group. The relative recovery rate of ERG a and b wave amplitude in ischemia-reperfusion groups was lower than that in ischemia-reperfusion with injection of PDTC group at every stage(P
4.Utilization Analysis of National Essential Medicines in Antibiotics in 3 Second-level General Hospitals from Chongming Area of Shanghai from 2011 to 2013

OBJECTIVE:To provide reference for promoting the rational use of national essential medicines in antibiotics. METHODS:The use of national essential drugs in antibiotics in 3 second-level general hospitals from Chongming area of Shanghai from 2011 to 2013 were statistically analyzed. RESULTS:The sales amount of antibiotics was in an increasing trend,with an aver-age growth rate of 28.16%;while the proportion of sales amount accounted for the total sales amount of drugs were still relatively low,with an average growth rate of 2.16%. The proportion of total sales amount of top 3 antibiotics from 2011 to 2013 accounted for the total sales amount of drugs were respectively 87.92%,93.60% and 95.54%,with an increasing trend;while compared with 2011,the DDDs of national essential medicines in antibiotics was decreased a little during 2012-2013;the top 2 DDDs were Cefuroxime axetil tablets and Amoxicillin capsules,the top 1 sales amount was Cefuroxime sodium for injection. Varieties with the sales amount/ DDDs close to 1 were relatively less,indicating that the sales amount of drugs and the number of drug use was not synchronous. CONCLUSIONS:There are still some problems in the utilization of national essential medicines in antibiotics. It needs to be continuously improved in the related work to further improve medication rationality and economy,and progressively re-alize the institutionalized and normalized antibiotics in clinical use management.
5.Change of anatomic configuration of proximal tibia following high tibial osteotomy

[Objective]To measure the change of anatomic configuration of proximal tibial after high tibial osteotomy and probe its clinical consequences.[Method]From 2001 to 2005,there were 59 knees of 35 patients with medial unicompartmental osteoarthritis treated with closing-wedge high tibial osteotomy.Anatomic configuration of proximal tibia were measured both pre and postoperatively on radiographs.They included posterior slope angle of proximal tibia,tibial angle,lateral shift of articular surface of proximal tibial and level of the joint line.The alteration of the above features was calculated and analyzed statistically with paired t test.[Result]The mean tibial posterior slope angle were(8.9??2.6?)preoperatively and(5.0??2.3?)postoperatively.The tibial posterior slope angle was averagely decreased about(3.9??1.7?).Tibial angle were(99.1??4.3?)and(91.1??3.8?)before and after high tibial osteotomy respectively.Lateral shift of articular surface of proximal tibial were(46.2?3.6)% and(53.1?3.9)% in pre and postopratively.Level of joint line was(41.2?3.6)mm and(38.0?3.2)mm in pre and postopratively.Statistical analysis indicated that there was significant difference of posterior slope angle or tibial angle or lateral shift of articular surface of proximal tibial or level of joint line between pre and post operative(P

Objective To investigate the effect of advanced glycation end products (AGEs) on the catalase activity and the levels of malondialdehyde in cultured bovine retinal capillary pericytes (BRPs), and to investigate the relationship between oxidative stress and diabetic retinopathy. 8.Observation of the Effect of the Photocoagulation and Vitrectomy Treatment on Eales Disease

Objective To evaluate the effect of argon laser photocoagulation (photocoagulation group) and endolaser photocoagulation during vitrectomy (vitrectomy group) on the eyes with Eales disease by fundus fluorescein angiography(FFA) and visual acuity (VA).Methods Sixty-seven eyes with Eales were treated by photocoagulation and 31 eyes with Eales were treated by vitrectomy. The changes of FFA and VA before and after treatment were observed.Results In the photocoagulation group, retinal non-perfusion disappeared in 31eyes(55 4%),alleviated in 17eyes(30 4%),unchanged in four eyes(7 1%)and deteriorated in four eyes(7 1%).Retinal neovascularization disappeared in 30 eyes(50 8%),alleviated in 19 eyes (32 2%),remained in six eyes(10 2%)and deteriorated in four eyes(6 8%).The first FFA after treatment, non-perfusion was existed in 25 eyes in photocoagulation group, and only was existed in two eyes in vitrectomy group.Neovascularization was existed in 29 eyes in photocoagulation group, and was existed in three eyes in vitrectomy group.The best visual correction(VA≥3 0) in photocoagulation and vitrectomy groups respectively was 30 eyes and zero eye before treatment,and after treatment respectively was 47 eyes and five eyes.There were 54 eyes and 11 eyes with VA≥0 05 in photocoagulation and vitrectomy groups respectively before treatment,and 60 eyes and 20 eyes with VA≥0 5 respectively after treatment.Conclusions The better result of FFA in vitrectomy group was related to the sufficient photocoagulation by endolaser. The higher rate of low vision in vitrectomy group was related to the impairment of macula by large amount of vitreous hemorrhages,retina detachment and vitrectomy. So the patients with Eales should be checked by FFA as early as possible.If the patients had the indicator to be treated by laser,they should be treated early and rechecked by FFA,to avoid the visual function harm by repeated vitreous hemorrhage due to incomplete treatment.
9.Effect of hypoxia on expression and function of integrin rece ptor ?_v?_3 of bovine retinal v ascular endotheliocytes

Objective To investigate the effect of hypoxia on the e xp ression and function of integrin receptor ? v? 3 of bovine retinal vascular endotheliocytes. Methods Bovine retinal vascular endotheliocy tes in the culture dishes coated by vitronectin was put into the normal and hypo xemic condition, respectively. Enzyme linked immunosorbent assay and cel l-adhesion analysis were used to detect the expression and function of integrin receptor ? v? 3 in bovine retinal vascular endotheliocytes, respectively. Results Under the condition of hypoxia, the expression of ? v? 3 increased gradually, and reached the peak at the 48th hour. The expressio n of ? v? 3 at the 60th and 72nd hour in hypoxia group was higher than that in the normal group. Bovine retinal vascular endotheliocytes absorbed more Vn of extra-cellular matrixes (ECM) after cultured under hypoxemic condition for 24 hours. Conclusion Hypoxia may up-regulate the expression of ? v? 3, which promote the adsorbability of endotheliocytes.
10.Effects of Zhaoke defibrase and anti-?_v?_3 mAb on the adhesion and immigration of bovine retinal vascular endothelial cells

Objective To explore the effects of Zhaoke defibrase and anti-? v? 3 mAb (23C6) on the adhesion and immigration of bovine retinal vascular endothelial cells. Methods The culture dishes coated with vitronectin (Vn) and collagen,assays of adhesion and immigration were performed 60 minutes after different concentration of Zhaoke defibrase and anti-? v? 3 mAb was added to the bovine retinal vascular endothelial cells. The apoptosis of bovine retinal vascular endothelial cells induced by Zhaoke defibrase and anti-? v? 3 mAb was detected by electron microscopy. Results Both Zhaoke defibrase and anti-? v? 3 mAb inhibited the adhesion and immigration of bovine retinal vascular endothelial cells in a dose-dependent manner. The inhibited concentration (IC 50) of Zhaoke defibrase was less than 0.05 ?mol/L, while IC 50 of anti-? v? 3 mAb was more than 2.5 ?mol/L. 81.8% endothelial cells adhering to Vn were inhibited by 0 1 ?mol/L Zhaoke defibrase, while 76.3% by endothelial cells adhering to Vn were inhibited by 10 ?mol/L anti-? v? 3 mAb. Typical apoptosis cells were found in bovine retinal vascular endothelial cells after affected by Zhaoke defibrase and anti-? v? 3 mAb. Conclusion Both Zhaoke defibrase and anti-? v? 3 mAb can significantly inhibit the adhesion and immigration of bovine retinal vascular endothelial cells to extracellular matrix, and the mechanism may lie in inducing the apoptosis of endothelial cells.
