1.Uncemented total hip arthroplasty-for the developmental dislocation of the hip in adults

[Objective]To evaluate the operative technique and the short-term clinical results of total hip arthroplasty(THA) for developmental dysplasia of the hip(DDH) in adults. [Method]Sixty-one patients(77 hips) with DDH who received THR without cement between December 1997 and October 2006 were followed up.In this group,53 cases were female,and 8 cases were male.The average age was 31.4 years(range,17~56 years).According to the classification of Hartofilakidis,fifteen cases were type I(eighteen hips);twenty-three cases,type Il(thirty hips);and twenty-three cases,type III(twenty-nine hips).The average Harris Hip Score was 40 points(range,16~61).All of patients were exposed through the.posterolateral approach.The threaded cup with coating were put in or near the level of the true acetabulum in all of cases.Full coating stems were used in femoral side,shortening osteotomy below lesser trochanter of femur were performed in twelve cases. [Result]The mean duration of follow-up was 49 months(range,12~118 months),the average Harris Hip Score increased from 40(range,16~61) preoperatively to 91(range,82~95) at the time of the latest follow-up.All the hips acquired union of osteotomy and bone ingrowth.None of the patients had radiographic evidence of aseptic loosening of prosthesis. [Conclusion]Uncemented total hip arthroplasty for the developmental dislocation of the hip in adults can get better results significantly,although it presents greater technical difficulty.

3.Open reduction and multi-pin internal fixation of type Ⅲ supracondylar humerus fractures with late callus formation in children

Objective To evaluate the effect of open reduction and multi-dimensional several pin fixation of Gartland type Ⅲ supracondylar fractures with late callus formation in children.Methods The study involved 37 children of Gartland type m supracondylar fractures with late callus formation undergone posteromedial elbow incision,open reduction via bilateral triceps sparing approach,several K-wire internal fixation and early functional rehabilitation from May 2009 to May 2011.There were 23 males and 14 females at 3-13 years of age (mean,7 years).Postoperative results were evaluated according to Flynn standard.Mean 9.5 months of follow-up was made.Results The results were excellent in 28 cases,good in five,fair in two and poor in two.No serious complications occurred after operation.Conclusion Open reduction and multi-dimensional several K-wire fixation is an effective treatment for Gartland type Ⅲ supracondylar fractures with late callus formation in children and hence provides a choice for clinical use.
4.Role of miR-122 in bone marrow derived stem cell therapy for acute liver injury in rats

Objective To investigatet the effects of miR-122 in the therapy of bone marrow derived stem cells (BMSCs)for acute liver injury in the rats,and to clarify the mechanism.Methods The BMSCs were isolated from the bone marrow of male rats by density gradient centrifugation.The BMSCs were divided into transfection group and control group.The BMSCs in transfection group were transfected with miR-122 mimics by liposome,while the BMSCs in control group were not.60 SD rats with acute liver injury induced by 10%CCl4 were randomly divided into control group (the saline was injected through mainline),normal treatment group (the normal BMSCs were injected through mainline) and experimental therapy group (the BMSCs transfected with miR-122 mimics by liposome were injected through mainline)(n=20).The liver function and tissue pathology were examined at 1 d, 7 d and 14 d after transplantation.Results The expression level of ALB in BMSCs was up-regulated,while the AFP expression level was down-regulated after the transfection of miR-122 mimics.At 1 d after transfection of BMSCs,the serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) activities had no significant difference between normal treatment group and experimental therapy group.At 7 d and 14 d after transfection of BMSCs,the serum ALT and AST activities in experimental therapy group were obviously lower than those in normal treatment group (P <0.05).The liver congestion,cytoplasm degeneration and liver cell necrosis in experimental therapy group were improved compared with normal treatment group.Conclusion The up-regulation of miR-122 expression in BMSCs would promote its differentiation into hepatocyte like cells,which plays a role in promoting the recovery of liver injury.
5.Endovenous laser treatment,radiofrequency endovenous occlusion and conventional stripping combined with transilluminated powered phlebectomy for lower extremity varicose vein

Objective To compare the clinical results of endovenous laser treatment(EVLT),radiofrequency endovenous occlusion(RFO)and conventional stripping combined with transilluminated powered phlebectomy(TIPP)for lower extremity varicose vein.Methods From Jun 2004 to Jan 2007,200 cases(232 limbs)were treated by EVLT with TIPP,80 cases(88 limbs)by RFO with TIPP,and 180 cases(202 limbs)by conventional stripping with TIPP.Operation time,number of the incision made,intraoperative bleeding,postoperative hospital stay,complications,and one-year recurrence rate were compared with each other. Results Operation time was longer(41±8)min in RFO group than that in other two groups.Postoperative hospital stay was shorter in EVLT group(1.2±0.4 d)and RFO group (2.1±0.8 d)than that in stripping and TIPP group(P<0.05).Patients in stripping group also suffered from more intra-operative bleeding more often incidental nervus saphenus injury and more incision numbers when compared with other two groups(P<0.05).There was no significant difierence in one-year recurrence rate among patients in the three groups. Conclusions The clinical efficacy is almost the same among the three groups in terms of eradication of the varicose veins.EVLT and RFO are safe and minimal invasive for the treatment of lower extremity varicose vein.
6.Construction, prokaryotic expression and immunogenic analysis of HXB2 subtype Tat mutant of human immunodeficiency virus type-1

Objective To construct shifting mutant of cysteine-rich region to 3?@terminal of Tat gene of human immunodeficiency virus type-1 (HIV-1) HXB2 strain, and to analyze the immunogenicity of mutant protein (Tat-cct) after prokaryotically expressed and purified. Methods The cysteine-rich region (nucleotides 64--111) of Tat gene was shifted to 3'terminal of Tat of HIV-1 HXB2 strain by polymerase chain reaction (PCR) and Tat mutant DNA sequence was obtained. Prokaryotie express plasmid pET32a-Tat-cct was constructed and transformed into E. coli BL21 (DE3), then Tat-cct protein was expressed and purified. BALB/c mice were immunized with the fusion protein Tat-cct, and immunogenicity of the immunized serum was detected by enzyme-linked immunosorbent assay (ELISA). Results The recombinant plasmid pET32a-Tat-cct expressed in E. coli BL21 (DE3) and the relative molecular mass of the purified fusion protein was 31 000. The serum antibody titer of mice immunized with Tat-cct recombinant protein was 1 : 1600, which binded specifically with both Tat-ect protein and Tat protein (amino acids 1-101). Conclusions The recombinant protein Tat-cct of Tat mutant strain can be expressed efficiently in E. coli and well retains immunogenicity, which provides valuable information for basic research of HIV-1 Tat vaccine.

8.The effect of expression level of IL-23R mRNA on Th17/IL-17 immune response in mycobacterium tuberculosis infection

Objective To investigate the effect of interleukin 23 receptor (IL-23R) on T helper cell 17 (Th17) call-mediated immune response in mycobacterium tuberculosis (TB) infection,and to explore the role of IL-23R in the pathogenesis of pulmonary tuberculosis.Methods 21 active lung tuberculosis (ATB) patients were enrolled in Beijing chest hospital from July to October in 2015,21 cases of latent tuberculosis infection (LTBI) and 21 healthy Healthy Donors (HD) were selected from Beijing Changping center for tuberculosis control and prevention from May to July in 2015.The peripheral blood mononuclear cells (PBMCs) were isolated and cultured.The expression of IL-23R mRNA in PBMCs was detected,IL-23 and IL-17A levels in the supernatant of PBMCs were measured.The expression of IL-23R mRNA in different groups and the effect of IL-23R expression on IL-17A level were analyzed.Results The expression of IL-23R mRNA in ATB group was lower than that in LTBI group (Z =-2.528,P =0.011),and in ATB group was higher than that in HD group (Z =-3.849,P < 0.001).The expression of IL-17A in ATB group was lower than that in LTB group (t =2.238,P =0.031),and ATB group was higher than that in HD group (t =4.733,P < 0.001).There was no significant difference in IL-23 level between the three groups (F =0.432,P =0.651).IL-23R mRNA expression was positively correlated with IL-17A level (rs =0.438,P =0.047).Conclusions The expression level of IL-23R in mycobacterium tuberculosis infection can regulate the immune response mediated by Th17 cells,which may affect the susceptibility and infection outcome of pulmonary tuberculosis.

OBJECTIVETo analyze the results of pathogenic bacteria culture on chronic periprosthetic joint infection after total knee arthroplasty (TKA) and total hip arthroplasty (THA).
METHODSThe medical data of 23 patients with chronic periprosthetic joint infection after TKA or THA from September 2010 to March 2014 were reviewed. Fifteen cases of TKA and 8 cases of THA were included in this study. There were 12 male and 11 female patients with the mean age of 62 years (range from 32 to 79 years), and among them 9 patients with sinus. All patients discontinued antibiotic therapy for a minimum of 2 weeks before arthrocentesis, taking pathogenic bacteria culture and antimicrobial susceptibility test by using synovial fluid taken preoperatively and intraoperatively of revision. Common pathogenic bacteria culture and pathological biopsy were taken on tissues intraoperatively of revision. Culture-negative specimens were prolonged the period of incubation for 2 weeks.
RESULTSThe overall culture-positive rate of all 23 patients for 1 week before revision was 30.4% (7/23), and the positive rate of culture-negative samples which prolonged for 2 weeks was 39.1% (9/23). The overall culture-positive rate of patients for 1 week intraoperatively of revision was 60.9% (14/23), and the positive rate of culture-negative samples which prolonged for 2 weeks was 82.6% (19/23). The incubation results of 7 cases (30.4%) preoperatively conformed to that of intraoperation.
CONCLUSIONThe culture-positive rate of pathogenic bacteria culture can be increased evidently by discontinuing antimicrobial therapy for a minimum of 2 weeks prior to the definite diagnosis.

10.Cure ofpancreaticoduodenal anastomotic leakage after simultaneous pancreas-kidney transplantation with enteric drainage:a case report and literature review

Objective To summarize the experience of one case of anastomotic leakage after simultaneous pancreas-kidney transplantation (SPK ) with enteric drainage .Methods One case of type 2 diabetes mellitus complicated with end-stage nephropathy undergoing SPK was retrospectively analyzed .Iliac venous systemic circulation was employed for pancreatic venous reflux ,transplanted pancreas exocrine via enteric drainage and side-to-side anastomosis between donor pancreaticoduodenum and recipient jejunum . Pancreatoduodenal anastomotic leakage occurred at 12 days post-operation .During re-operation ,Roux-en-Y anastomosis was established between donor pancreaticoduodenum and recipient jejunum .And the relevant domestic and foreign literatures were searched .Results The follow-up time was 3 month after a second operation .Recipient pancreas and kidney transplantation survived well . There was no onset of enteric leakage .The incidence of anastomotic leakage varies greatly between different transplantation centers both at home and abroad .The incidence ranged from 3 .6% to 11 .3% .And the risk of pancreatic loss was as high as 54 .6% .Conclusions As a severe postoperative complication ,anastomotic fistula after SPK may cuase abdominal infection . Even after reparing enteric fistula , the risk of leakage remains high . Roux-en-Y anastomosis is other therapeutic option .
