1.Development and application of a new type of assisted micro implant anchorage 3D guide plate

Objective:To establish a computer-aided design and 3D printing system for precise implantation of micro implant anchorage, and accurately calibrate the position and direction of micro implant implantation.Methods:A retrospective selection was conducted on 15 patients (30 in total) who underwent micro implant implantation surgery from the Department of Stomatology, the Affiliated Hospital of Jiangnan University from November 2019 to November 2021, including 6 males and 9 females, aged (17.1±6.3)years old. The preoperative patient was photographed with cone beam computed tomography (CBCT) and the collected DICOM data format was output. A 3D scan was performed on the patient′s preoperative analysis model to obtain the STL file of the model scan. The CBCT data and model data were fitted and matched using 3Shape Implant Studio software, and the thickness of the guide plate, the amount of undercut compensation, and the size of the key component collar were designed. The 3D printer was used for printing after resizing. Using the assist method to implant micro implants, CBCT was taken postoperatively to compare the preoperative design with the postoperative results.Results:After fitting the postoperative CBCT with the designed CBCT of the micro implant, it was found that the micro implant was consistent with the preoperative design, maintained a safe distance and parallel to the adjacent tooth root, and did not damage the maxillary sinus and other areas. No detachment of the micro implant anchorage was observed 1 or 3 months after surgery. The application of assisted micro implant anchorage 3D guide plate was reliable, with accurate implantation position and direction, and can be implanted in most parts of the oral cavity.Conclusions:The use of computer-aided design and 3D printing system to create an assistive micro implant anchorage 3D guide plate can accurately locate the position and direction of the micro implant, which is worthy of clinical promotion and application.
2.Analysis of epidemiological characteristic of incidence and mortality of stroke in Chongqing

Objective To analyze the epidemiological characteristic of stroke incidence and mortality in Chongqing, and to provide suggestions for targeted prevention and control of stroke. Methods Data of reported cases of ischemic stroke (I63), hemorrhagic stroke (I61-I62), subarachnoid hemorrhage (I60), and unclassified stroke (I64) in Chongqing in 2018 was collected. SPSS 25.0 was used to calculate the incidence, age-standardized incidence rate (ASIR), mortality, age-standardized mortality rate (ASMR) and proportion of stroke. The incidence and mortality of stroke were compared by Chi-square test between males and females, and between central urban areas and suburban counties. Results In 2018, the incidence and ASIR of stroke were 294.61/105 and 222.43/105 in Chongqing, respectively. The mortality and ASMR of stroke were 116.34/105 and 85.81/105. The incidence, ASIR and proportion of ischemic stroke were 186.63/105, 139.17/105 and 63.35%, respectively. The incidence of ischemic stroke in male was higher than that in female (χ2=18.52, P<0.001). The incidence of ischemic stroke in central urban areas was higher than that in suburban counties (χ2=29.50, P<0.001). The mortality of ischemic stroke in male was higher than that in female (χ2=6.67, P=0.010). The mortality of ischemic stroke in suburban counties was higher than that in central urban areas (χ2=17.55, P<0.001). The incidence, ASIR and proportion of hemorrhagic stroke were 78.15/105, 60.44/105 and 26.53%, respectively. The incidence of hemorrhagic stroke in male was higher than that in female (χ2=27.64, P<0.001). The incidence of hemorrhagic stroke in suburban counties was higher than that in central urban areas (χ2=18.60, P<0.001). The mortality of hemorrhagic stroke was the highest among all subtypes of strokes. The mortality, ASMR and proportion of hemorrhagic stroke was 64.44/105, 48.28/105 and 55.39%, respectively. The mortality of hemorrhage stroke in male was higher than that in female (χ2=23.46, P<0.001). The mortality of hemorrhage stroke in suburban counties was higher than that in central urban areas (χ2= 43.57, P<0.001). Conclusion The incidence and mortality of stroke in Chongqing were lower than the national average levels. The incidence of ischemic stroke was higher than that of hemorrhage stroke. The mortality of hemorrhage stroke was higher than that of ischemic stroke. Men and suburban counties were the focus of stroke prevention and control in Chongqing.
3.Analysis on the prevalence of hypertension and its risk factors among residents aged 35-75 years old in Chongqing

Objective To investigate the prevalence of hypertension and its risk factors among residents with age between 35 and 75 years old in Chongqing and provide evidence for the prevention and control of hypertension. Methods During 2016-2018, 101036 eligible subjects aged 35-75 years from 8 sites of China Patient-centered Evaluative Assessment of Cardiac Events in Chongqing were interviewed and examined. The data were analyzed by SPSS 20.0. The difference of the prevalence of hypertension of different population were compared chi-square test. Risk factors of hypertension was identified by step backward multivariate Logistic regression model. Results The crude prevalence and age standardization prevalence of hypertension was 40.80% and 33.91% respectively. The results of multivariate logistic regression indicated that the risk factors of hypertension were residence, ethnicity, age, medical insurance ,drinking, present smoking , indoor passive smoking , overweight , obesity , central obesity , diabetes and dyslipidemia ,while the protective factors of hypertension of those were education level and income. Conclusion The prevalence of hypertension was high among residents with age between 35 and 75 years old in Chongqing ; Residence, ethnicity, age, medical insurance, education level, income, drinking, present smoking , indoor passive smoking , overweight , obesity , central obesity , diabetes and dyslipidemia are the related factors of hypertension.
4.Predictive value of cardiac biomarkers of low cardiac output in children undergoing congenital heart surgery

Objective:To assess the predictive value of atrial natriuretic peptide, b-type natriuretic peptide(BNP), cardiac troponin I(cTn-I) and central venose oxygen saturation(ScvO 2)as indicators of low cardiac output syndrome(LCOS) in children with congenital heart disease undergoing cardiopulmonary bypass(CPB). Methods:After corrective surgery for congenital heart disease under CPB, 70 children(aged 5 days to 160 months) were enrolled in a prospective observational pilot study during 1-year period from April 2017 to March 2018. The patients were classified according to whether they developed low cardiac output syndrome. Biomarker levels were measured at 2, 12, 24, and 48 h post-CPB. The clinical data and outcome variables were analyzed by a multiple logistic regression model. Results:Twenty-two(31.4%) patients developed low cardiac output syndrome(group 1) and the remaining 48(68.6%) patients were included in group 2. cTn-I levels >13 ng/ml at 2 hours after CPB( OR=40.2, 95% CI: 9.4-171.4, P<0.001) and ScvO 2 levels<0.37 at 12 hours following CPB( OR=124.7, 95% CI: 28.7-538.2, P<0.001) were independent predictors of low cardiac output syndrome. Conclusion:Our results suggest that cTn-I at 2 h and ScvO 2 levels at 12 h post-CPB are evident early predictors of low cardiac output syndrome. This predictive capacity is, moreover, reinforced when cTn-I combined with ScvO 2 following CPB. These 2 cardiac biomarkers would aid in therapeutic decision-making in clinical practice and would also enable clinicians to modify the type of support to be used in the pediatric intensive care unit.
5.Observation of nab-paclitaxel as first-line treatment in 40 elderly patients with ad-vanced lung squamous carcinoma

Objective:To evaluate the clinical efficacy,toxicity,and prognostic factors of nab-paclitaxel as first-line treatment for elderly patients with advanced lung squamous carcinoma.Methods:This was a prospective study.Forty patients enrolled in the Second Affili-ated Hospital of Fujian Medical University were treated with nab-paclitaxel(260 mg/m2,ivggt d1),and a period of three weeks was considered as one session.The effects were evaluated after two cycles.Results:All 40 patients were followed up and appraised.Two patients achieved complete remission,13 achieved partial remission,13 achieved stable disease,and 12 achieved progressive disease. The objective response rate was 37.5% and the disease control rate was 70.0%.The progression-free survival(PFS),median overall sur-vival,and 1-year survival rate was 6.3 months,12.6 months,and 62.5%,respectively.The main hematologic toxicities were neutrope-nia and anemia,and the main non-hematologic adverse events were fatigue,constipation,nausea,vomiting,muscle aches,and hear-ing loss.Most patients could tolerate these toxic reactions.Moreover,Cox multivariate regression analysis showed that the neoplasm stage,Eastern Cooperative Oncology Group performance status,response rate,and PFS were independent factors for the survival rate (P<0.05),while age was not related to patient prognosis(P>0.05).Conclusions:Nab-paclitaxel as single drug and first-line therapy for elderly patients with advanced lung squamous carcinoma is effective and safe.
6.Ardipusilloside-I stimulates gastrointestinal motility and phosphorylation of smooth muscle myosin by myosin light chain kinase.

Ardipusilloside-I is a natural triterpenoid saponin, which was isolated from Ardisia pusilla A. DC. The aim of the study was to evaluate the stimulation of ardipusilloside-I on gastrointestinal motility in vitro and in vivo. The experiment of smooth muscle contraction directly monitored the contractions of the isolated jejunal segment (IJS) in different contractile states, and the effects of ardipusilloside-I on myosin were measured in the presence of Ca²⁺-calmodulin using the activities of 20 kDa myosin light chain (MLC₂₀) phosphorylation and myosin Mg²⁺-ATPase. The effects of ardipusilloside-I on gastro emptying and intestinal transit in constipation-predominant rats were observed, and the MLCK expression in jejuna of constipated rats was determined by western blot. The results showed that, ardipusilloside-I increased the contractility of IJS in a dose-dependent manner and reversed the low contractile state (LCS) of IJS induced by low Ca²⁺, adrenaline, and atropine respectively. There were synergistic effects on contractivity of IJS between ardipusilloside-I and ACh, high Ca²⁺, and histamine, respectively. Ardipusilloside-I could stimulate the phosphorylation of MLC₂₀ and Mg²⁺-ATPase activities of Ca²⁺- dependent phosphorylated myosin. Ardipusilloside-I also stimulated the gastric emptying and intestinal transit in normal and constipated rats in vivo, respectively, and increased the MLCK expression in the jejuna of constipation-predominant rats. Briefly, the findings demonstrated that ardipusilloside-I could effectively excite gastrointestinal motility in vitro and in vivo.

7.The occurrence of chemotherapy-induced amenorrhea in postoperative and premenopausal patients with breast cancer and its influencing factors

Objective To study the influence of postoperative chemotherapeutic regimens on menstruation in pre-menopausal patients with breast cancer. Methods 120 pre-menopausal patients with breast cancer from Jan 2010 to Dec 2012 were followed-up. The occurrence rate and recovery rate of chemotherapy-induced amenorrhea (CIA) were analyzed. The relationship between different chemotherapeutic regimens and CIA of various age groups was analyzed. Results 120 women were followed-up, 94 (78.3 %) patients developed CIA. The frequency of CIA in younger patients (≤ 35 years) was significantly lower than that in patients older than 35 years (P < 0.05). 50 (53.2 %) patients returned to normal menses. The recovery situation of CIA was significantly different among various age groups (P < 0.05). The difference of the occurrence rate of CIA was not statistically significant between the patients who received anthracycline-based chemotherapy and docetaxel-based chemotherapy (P > 0.05). Conclusions Age is an important influence factor of the CIA. Chemotherapy has less influence on the menstruation of younger patients (≤35 years) and most amenorrhea is reversible. The effects of sequential addition of taxanes in chemotherapy and anthracyclines alone on menstruation have no significant difference.
8.The clinical and pathological characteristics of breast cancer in women ≤ 40 years of age

Objective To retrospectively analyze the clinical and pathological characteristics of breast cancer in women ≤ 40 years of age.Methods One hundred and thirty one young (≤ 40 years) female patients with breast cancer pathologically confirmed in the General Hospital of PLA from January 1 st 2008 to December 31st 2012 (young group) were collected,and 262 elderly (41-69 years) female patients with breast cancer in the same period (old group) were collected as control using the random number table method.The clinicopathological characteristics involved TNM staging,histological grade,the expression levels of estrogen receptor (ER),progesterone receptor (PR),human epidermal growth factor receptor-2 (Her-2) and clinical stage of the two groups were contrastively analyzed.Results Compared with elderly patients,young female patients with breast cancer had lower positive rate of ER or PR (64.9% vs.76.7%;x2 =6.171,P =0.013),higher positive rate of Her-2 (26.7% vs.15.3%;x2 =7.415,P =0.006) and higher histological grade (grade Ⅱ 38.2% vs.35.1%;grade Ⅲ 55.7% vs.49.6%;x2 =6.835,P=0.033).There were no significant differences in T stage (x2 =1.764,P =0.623),N stage (x2 =0.129,P =0.988),clinical stage (x2 =4.916,P =0.178),molecular subtype (x2 =7.475,P =0.058) and different surgical procedures (x2 =0.913,P =0.339) between the two groups.Conclusion Young (≤ 40 years) female patients with breast cancer have specific clinical and pathological characteristics including higher histological grade,higher positive rate of Her-2,lower positive rate of ER or PR and higher degree of malignancy,who should be diagnosed and treated as soon as possible.
9.Clinical efficacy of patient-controlled intravenous analgesia for intractable cancer pain

Objective:To compare the clinical efficacy of oxycodone hydrochloride controlled-release tablets (OHCT) and pa-tient-controlled intravenous analgesia (PCIA) in the treatment of intractable cancer pain. Methods:Retrospective analysis was conduct-ed to evaluate the intractable cancer pain of 89 elderly patients who were admitted to the medical oncology departments of The Second Affiliated Hospital of Fujian Medical University and the Fuzhou General Hospital of Nanjing Military Command between September 2012 and March 2014. Among the 89 patients, 47 were treated with OHCT, and 42 received PCIA. The total dosage ranged from 60 mg/d to 400 mg/d PO q12h for patients in the OHCT group, whereas abackground dose+patient-controlled dosemode was adopted for patients in the PCIA group. The therapeutic efficacy, presence of adverse reactions, cost of treatment, and degree of patient satisfaction were compared between the two groups. Results:The average dosages of analgesics in the two groups were almost the same (P>0.05). Visual analogue scale (VAS) values and daily average VAS values were both lower in the PCIA group than in the OHCT group at 24 h after analgesia (P<0.05). The incidence of adverse reactions, such as nausea and vomiting, was also lower in the PCIA group than in the OHCT group (P<0.05). The cost of treatment and degree of patient satisfaction were the same in both groups (P>0.05). Conclusion:Pa-tients who received PCIA attained better analgesia and exhibited less adverse reactions than those who received OHCT whereas the treatment cost and patient satisfaction did not differ in both groups.
10.Preparation of Clindamycin Loaded Chitosan Sustained-Release Microspheres and Drug Release Study in Vitro

Objective:To prepare clindamycin loaded chitosan sustalned‐release microspheres ,determine drug loading and drug encapsulation efficiency of the microspheres ,and observe the effect of sustalned‐release and bacteriostasis in vitro .Methods:The microspheres were prepared by emulsified chemical cross‐linking method . The morphology and particle size of microspheres were observed by transmission electron microscopy and laser diffraction particle size analyzer .The drug loading and drug encapsulation efficiency of clindamycin loaded chitosan sustalned‐release microspheres were tested by UV spectrophotometry method .The effect of drug sustalned‐release and bacteriostasis in vitro was studied by establishing release system in vitro .The bacteriostasis of microspheres in vitro was observed by plotting the area‐time curve of inhibition ring . Results:The size of clindamycin loaded chitosan sustalned‐release microspheres was uniform ,while the dispersion was good . The average particle size of the microspheres was (153 .2 ± 13 .7)μm .The average drug loading was(39 .8 ± 1 .6)% .The mean encapsulation efficiency was (79 .6 ± 3 .5)% .The cumulative release percentage of clindamycin loaded chitosan sustalned‐release microspheres in pH 7 .4 phosphate buffer saline(PBS) was 16 .5% at the first hour ,21 .6% at the second hour ,33 .2%at the third hour and 53 .7% at the twenty‐fourth hour .The cumulative release percentage of clindamycin control was 99 .6% at the third hour .The microspheres still had obvious bacteriostasis after 3 days’ release ,as the bacteriostasis time area curve of the drug loaded microsphere showed .Conclusions:The encapsulation efficiency of clindamycin loaded chitosan sustalned‐release microspheres was good ,so that the sustalned‐release effect and bacteriostsis in vitro was also good .
