Objective To study the risk factors for contralateral breast cancer(CBC) in women after regular treatment of the primary breast cancer. Methods Between January 1997 to December 2002,the clinical data of 340 breast cancer patients at our institution were retrospectively analyzed. In all the patients a detailed analysis was carried out with respect to age, operation type, radiation therapy technique and dose, the use of chemotherapy or hormone therapy, and other clinicopathologic characteristics. The Kaplan-Meier method was used to estimate the actuarial rate of CBC.The Cox proportional hazard regression model was used to estimate the relative risk factors of CBC.Results Fourteen cases were diagnosed to be CBC,thus overall incidence of CBC was 4.1%.Ten-year CBC incidence(2.7%) was higher than 5-year incidence of CBC(1.4%).Univariate analysis showed that the risk factors of CBC at 5-year and 10-year included: ≤45 years old, medullary carcinoma, family history of breast cancer and being taken without endocrine therapy(P0.05).Mutivariate analysis showed that ≤ 45 years old and being internal breast radiotherapy were independent risk factors of CBC at 5-year and 10-year(P

Objective To investigate the relationship between axillary lymph node status and molecular pathological features in breast cancer and its clinical significance. Methods A retrospective analysis was performed to study the relationship of axillary lymph node metastasis (LNM), positive lymph node (PLN), and lymph node ratio (LNR) with clinical and molecular pathology features in 300 patients with breast cancer. Results Of 300 cases of breast cancer, the positive rate of axillary LNM was 31.0% (13/42) in the Luminal A subtype of breast cancer, yielding a statistically significant difference (P < 0.05), compared with that of other molecular subtypes. Of 144 patients with positive axillary LNM , there were no significant difference of LNR and PLN between different molecular subtypes of breast cancer (P > 0.05). PLN and LNR were all negatively correlated with E-cadherin expression in breast cancer (P < 0.01 and P < 0.05, respectively), correlated with tumor size and TNM staging (P < 0.05), but not with age, menopausal status, and histological grade (P > 0.05). Conclusion The combined phenotype expression of ER, PR, HER-2, and Ki67 (molecular subtypes) are associated with the incidence of axillary lymph node metastases , and E-cadherin expression deficiency and tumor size growth promote the progression of lymph node metastases in breast cancer.

AIM:To investigate the changes of retinal microcirculation after phacoemulsification and the influencing factors of visual acuity.METHODS: Retrospective analysis. A total of 264 cataract patients(264 eyes)who underwent phacoemulsification in our hospital from January 2022 to December 2022 were selected as the study objects. Patients were divided into < 0.3 group(66 eyes)and ≥0.3 group(198 eyes)according to the recovery of best corrected visual acuity(BCVA)at 3 mo after surgery. The changes of retinal microcirculation indexes were compared before and after treatment. Logistic regression and LASSO regression models were used to screen the influencing factors of postoperative BCVA. A nomogram prediction model of postoperative BCVA was constructed and verified. A restricted cubic spline Logistic regression model was established to analyze the dose-response relationship between end-diastolic velocity(EDV), peak systolic velocity(PSV)and the risk of BCVA recovery.RESULTS: At 3 mo postoperatively, EDV and PSV were significantly improved compared with those before treatment, and resistance index(RI)levels were significantly lower than those before treatment(all P<0.05). Preoperative EDV, PSV, aqueous humor cell grade, fundus lesion grade, advanced age and Emery grade were influencing factors for poor BCVA recovery after phacoemulsification in cataract patients(P<0.05). The AUC before and after validation of the nomogram model by Bootstrap method were 0.869(95%CI: 0.815-0.903)and 0.866(95%CI: 0.802-0.895), respectively. The sensitivity was 88.36% and 88.27%, and the specificity was 91.82% and 91.78%, respectively. Restricted cubic spline model analysis showed no nonlinear dose-response relationship between EDV and PSV levels and the risk of poor BCVA recovery in either male or female(P>0.05).CONCLUSION: After phacoemulsification, retinal microcirculation in cataract patients improved significantly. EDV, PSV, aqueous humor cell grade, fundus lesion grade, advanced age and Emery grade are all factors influencing poor BCVA recovery after cataract surgery.
