1.Regulation of Pituitary Tumor Cell Prolieration by Epidermal Growth Factor and Genistein

Recombinant human epidermal growth factor (rhEGF, 20 ng/ml) and Genistein (740 μmol/ml) were added to the cells from 12 cases of human pituitary tumors and co-cultured in vitro. The 3H-thymidine uptake of tumor cells was measured by scintillation counter and tyrosine kinases activity was detected by using immunocytochemical method. The results showed that 3H-thymidine uptake and tyrosine kinases activity were increased in 10 out of 12 cases of human pituitary tumor by EGF and both declined by Genistein. It was suggested that EGF had a strong mitogenic effect on majority of pituitary tumors. Genistein could abolish the effects of EGF promoting the proliferation of the tumor cells, inhibit the growth of the tumor and might be useful in pituitary tumor treatment.
2.Effects of 1,25-dihydroxyvitamin D_3 on T-lymphocyte subsets,mixed lymphocyte reaction and natural killer cell following skin transplantation in mice

Objective To evaluate the mechanism of immunosuppressive activity of 1,25-dihydroxyvitamin D_3 and provide a theoretical basis for clinical use. Methods Different inbred strain male BALB/C (H-2d) and male C57BL/6(H-2b) mice were used as skin transplantation donors and recipients, respectively. After operation C57BL/6 mice were conditioned with 1,25-dihydroxyvitamin D_3 2.5 ??g/kg every day, Cyclosporine A (CsA) po 25 mg/kg every day separately or unitedly. Ten days after transplantation, the recipients were sacrificed, and the spleens were collected. The mouse splenic T lymphocytic subsets, mixed lymphocyte reaction (MLR), the activity of natural killer (NK) cells were determined. Results The mean survival time (MST) of skin allografts was prolonged from ( 9.75 ? 0.89 ) days to ( 13.13 ? 1.13 ) days by treatment of the recipient mice with 1,25-dihydroxyvitamin D_3. 3.Quantitative electroencephalographic analysis as a tool for predicting the depth of sedation of sedation induced by propofol, midazolam and ketamine

Objective To evaluated the acccuracy of the two quantitative electroencephalographic parameters-bispectral index (BIS) and 95% specrral edge frequency (95% SEF) for measuring the depth of sedation induced by propofol, midazolam and ketamine. Methods Forty-five ASA Ⅰ - Ⅱ patients aged 30-59 yr weighing 46-80 kg scheduled for elective general thoracic or abdominal surgery were randomized to receive an infusion of propofol at a rate of 8 mg?kg-1?h-1 (group P , n = 15) or midazolam at 0.5 mg?kg-1?h-1 (group M, n = 15) or ketamine at 4 mg?kg-1? h-1 (group K, n = 15) . The patients were unpremedicated. The depth of sedation was assessed using OAAS scale (5 = wide awake , 1 = no response to prodding or shaking ) at 3 min intervals. BIS and 95 % SEF were continuously monitored. The BIS and 95% SEF values at each OAAS score (5-1) were recorded. The relations between BIS, 95 % SEF and sedation scores were determined in each group. The ED50 values of BIS and 95% SEF50 for loss of consciousness and their 95% confidence internals were calculated. Prediction probability(Pk) values for BIS and 95% SEF were compared among the drugs. Results There were no significant differences among the 3 groups with respect to age, body weight, sex and duration of drug infusion. With increasing sedation there was a progressive decrease in BIS and 95 % SEF values in group P and M but no significant changes in BIS and 95 % SEF values were seen in group K. The BIS and 95 % SEF positively correlated with OAAS score in group P and M but not in group K. The BIS50 was 65.9 in group P and 70.7 in group M,but inestimable in group K.The 95% SEF50 was 20.4 in group P and inestimable in group M and K. The Pk values for BIS and 95 % SEF were higher in P group than in M group and were not significantly different from 0.5 indicating a very poor predictive performance . Conclusion The accuracy of BIS and 95 % SEF for assessing the depth of sedation is greater with propofol. BIS is more sensitive than 95% SEF for the same anesthesia.
4.Expression of Epidermal Growth Factor, Transforming Growth Factor-αand Epidermal Growth Factor Receptor in Human Pituitary Tumors

Objective:To explore the mechanism of growth factor autocrine stimulation in human pituitary tumors. Method: Immunohistochemical analyses of EGF、TGF-αand EGFR were studied on paraffin-embeded sections of 30 pituitary tumors. Results: Expression of EGFR and its ligand EGF、TGF-αhas a variable stained cells intensity,density and type. EGFR and the ligands expressed in majority of pituitary tumors.Conclusion:The EGF autocrine stimulation exists in pituitary tumors,and tyrosine kinases inhibitors may be useful for pituitary tumors treatment.
5.Effects of dexamethasone on toll-like receptor(TLR)2 and 4 in CD14+ monocyte of patients during and after cardiac surgery with cardiopulmonary bypass

Objective To investigate the kinetics processes of TLR2 and TLR4 in CD14~+ monocyte of patients during and after cardiac surgery with cardiopulmonary bypass(CPB) and the effects of dexamethasone(DXM) on the regulation of TLR2 and 4 in CD14~+ monocyte. Methods Twenty patients undergoing elective atrial/ventricular septal defect correction were randomized to received 1 mg/kg dexamethasone or placebo before induction of anesthesia. The CD14~+ monocyte surface TLR2 and TLR4 and the intracellular HSP70 were stained and analyzed by flow cytometry, and plasma level of TNF-?, IL-6, IL-10, NO and MDA were measured at following times: before the dexamethasone or placebo were administer(T1), before starting CPB(T2), immediately after aortic declamping(T3), 30min after aortic declamping(T4), 5h after skin closure(T5) and 24h after skin closure(T6). 6.The clinical evaluation of the bispectralindex of electroencephalogram in predicting movement during skin incision in patients under sevoflurane anesthesia

Objective: To evaluate the efficacy of the bispectral index (BI) in predicting patient movement on skin incision under sevoflurane anesthesia with the PK statistic. Method: Twenty-nine adult patients. scheduled for elective upper abdominal surgery, were selected. Anesthesia was induced with propofol and 4%-5% sevoflurane in oxygen and maintained with one of four randomized sevoflurane concentrations (0.6MAC, 1.0MAC. 1.5MAC and 2.0MAC) for fifteen minutes, then skin incision was performed by the surgeon at the planned site of the surgery, and each patient was observed carefully about 2 minutes to detect purposeful movement. The data of BI, 95% SEF, MAP, SABP and HR in one minute before skin incision were applied to statistically analysis. Result: The P_K was significantly greater than 0.5 for the sevoflurane level and the BI. The PK of 95% SEF. SABP and MAP were less than that of the level of sevoflu or Bl. but was significantly greater than 0.5 The P_K of HR was close to 0.5. Conclusion: These indicators of B, BP and 95% SEF may predict patient movement to skin incision under sevoflurane anesthesia. of which the BI is the most sensi tive.

Objective To investigate the protective effect of phycocyanin on focal cerebral ischemia reperfusion in rats.Methods The model of the middle cerebral artery occlusion and reperfusion(MCAO/R) was established using the intraluminal filament occlusion with 60 healthy adult male Wistar rats,and treated by phycocyanin.The expressions of NF-?B,IL-6 and neuronal apoptosis were detected to evaluate the effects of phycocyanin on above indexes.Results The expression level of NF-?B increased successfully from reperfusion 6h,peaked at reperfusion 1d,and then decreased in cortex and striatum in ischemic group.The expression site and change principle of IL-6 were similar to those of NF-?B except of expression peak at reperfusion 2d.The neuronal apoptosis increased successfully from reperfusion 6h,reached peak at reperfusion 1d and 2d in cortex and striatum respectively,and the changed character was coincident with NF-?B and IL-6 expressions.The expressions of NF-?B and IL-6 reduced and the number of neuronal apoptosis decreased significantly after applying phycocyanin compared to the control group in the same time.Conclusion Phycocyanin could inhibit neuronal apoptosis and down-regulate the expressions of NF-?B and IL-6 after cerebral ischemia-reperfusion.
10.Application of anterolateral thigh myocutaneous flap using computed tomography angiography for mouth-floor reconstruction after resection of middle-late stage carcinoma of mouth floor.

OBJECTIVEThe aim of this study was to investigate the value of free anterolateral thigh myocutaneous flap (ALTMF) and computed tomography angiography (CTA) for the reconstruction of mouth-floor defects after the resection of middle-late stage carcinoma of the mouth floor.
METHODSSixteen cases of middle-late stage carcinomas of the mouth floor underwent radical resection, and mouth-floor and tongue defects were reconstructed with ALTMF. CTA was applied to plan the lateral circumflex femoral artery (LCFA) and its perforating vessel, which was verified during the operation.
RESULTSThe position of the perforating vessel in the operation was fully consistent with that designed by the preoperative CTA. All 16 flaps completely survived. The appearance and function of all cases were both satisfactory. All donor sites were primarily closed and healed without functional morbidity. During the follow-up period of 6-36 months, 15 cases survived with acceptable aesthetic and functional results in mouth floor and tongue reconstruction, except for 1 case (T4N2M0) that died of metastasis carcinoma 10 months after operation.
CONCLUSIONCTA can accurately locate the LCFA and artery perforator. Preoperative perforator planning using CTA in ALTMF transplantation is a reliable and useful method thatresults in safe operation with optimal outcome. The ALTMF is an ideal choice for the reconstruction of soft tissue defects after the resection of middle-late staie carcinoma of the mouth floor
