1.Linkage of allergic rhinitis with HLA-DRB alleles polymorphism

Objective:To investigate whether susceptibility or resistance to allergic rhinitis associated with HLA-DRB alleles.Method:Using PCR-SSP (sequence-specific primer polymerase chain reaction),we tested the frequency distribution of HLA-DRB alleles in 41 patients with allergic rhinitis (AR) and 41 healthy controls from Beijing China.Result:The frequence of HLA-DRB10301.2 and HLA-DRB 40101 was lower in AR than in controls (2.44% vs 17.07%, P<0.05;29.27% vs 51.22%, P<0.05).Conclusion:HLA-DRB10301.2 and HLA-DRB 40101 alleles might confer protection against AR.
2.Application value of transcranial Doppler diagnosis for intracranial posterior circulation artery stenosis or occlusion

Objective To investigate the sensitivity and specificity of transcranial Doppler (TCD) diagnosis for intracranial posterior circulation artery stenosis or occlusion.Methods Seventy-two cases of patients treated for posterior circulation intracranial artery stenosis or occlusion were chosen,TCD and CT angiography (CTA) tests were carried out and the results were compared and analyzed.Results Compared with CTA,the sensitivity for period of posterior circulation intracranial arteries by TCD was 82.50%,while specificity reached 94.64%.Positive predictive value attained 91.67%,while negative predictive value reached 88.33% and the accuracy was 89.58%.Conclusions TCD diagnosis for period of posterior circulation intracranial arterial has high specificity,normal sensitivity,and the highest diagnosis accuracy for intracranial segment of vertebral artery.TCD can serve as early inspecting method for intracranial artery.
3.Clinical observation and intervention of somatostatin combined with omeprazole in the treatment of liver cirrhosis with upper gastrointestinal bleeding

Objective To explore the curative effect and nursing of gastrointestinal bleeding of somatostatin combined with omeprazole in the treatment of liver cirrhosis. Methods 40 cases of gastrointestinal bleeding in patients with cirrhosis using somatostatin combined with omeprazole treatment, at the same time using conventional nursing (control group), the other 40 patients used somatostatin combined with omeprazole treatment, and comprehensive nursing (observation group); two group intervention. The patients were in our hospital from March 2015 to January 2017 were treated. Results The effective rate of treatment group was two, obviously higher in the observation group, the observation group was 95.0 %, the control group was 70.0 %; the satisfaction rate is obviously higher in patients in the observation group, the observation group total the satisfaction rate was 97.5 %, the control group was 72.5 %; comparison shows obvious differences(P<0.05). Conclusion Somatostatin combined with omeprazole in the treatment of liver cirrhosis of digestive tract hemorrhage, the treatment can improve the patient's condition, but also the implementation of comprehensive nursing, is conducive to improve the treatment effect of patients, and improve the satisfaction rate of patients for nursing care, it is worthy of reference.
4.Hemodynamic analysis of subclavian steal syndrome in patients undergoing transcranial doppler examination

Objective To improve the theoretical basis for the typical hemodynamic changes in subclavian steal syndrome (SSS) patients underwent transcranial doppler (TCD) by detailed hemodynamic analysis.Methods Patients with SSS were examined by transcranial doppler and some patients performed tourniquet test.Flow spectrum,flow velocity and pulsatility index (PI) of vertebral artery,basilar artery and middle cerebral artery (MCA) were recorded.Results The flow spectrum of steal side vertebral artery showed that there were 14 cases with systolic notch,18 cases with systolic reverse flow and 3 cases with flow completely reversed.The flow spectrum showed high resistance spectrum in vertebral artery and basilar artery and ipsilateral basilar artery vortex or turbulence in 32 cases.The PI value was higher in contralateral vertebral artery and basilar artery than in anterior circulation (q =9.56,9.98,both P < 0.01),and there was no difference between contralateral vertebral artery and basilar artery (q=0.416,P>0.05).The tourniquet test were positive in all 22 patients.Conclusions TCD is a convenient,fast,accurate and non-invasive method for hemodynamic examination,which is the first choice for the diagnosis of SSS.
5.Influence of different oxygen partial pressures on cytokines secreted from human adipose-derived stem cells

BACKGROUND:Effects of different oxygen partial pressures on cytokine secretion of human adipose-derived stem cells have been differently reported. These differences may be caused by varying oxygen partial pressures.
OBJECTIVE:To investigate the influence of different oxygen partial pressures on cytokines secreted from human adipose-derived stem cells.
METHODS:Human adipose-derived stem cells were cultured in vitro and identified by its immunophenotype. Human adipose-derived stem cells were divided into five groups and cultured under different oxygen partial pressure conditions (1%, 3%, 5%, 10%, 21%) for 24 hours, respectively. With quantitative real-time PCR and enzyme linked immunosorbent assay, the secretion of cytokines, vascular endothelial growth factor, hepatocyte growth factor, nerve growth factor, keratinocyte growth factor, from human adipose-derived stem cells were analyzed on the gene and protein levels.
RESULTS AND CONCLUSION:Human adipose-derived stem cells were positive for CD71, CD73, CD90, CD105 and negative for CD34, CD45, CD54, HLA-DR. From the aspect of gene level, hypoxia (1%, 3%O 2 ) promoted the expression of vascular endothelial growth factor and nerve growth factor from human adipose-derived stem cells (P<0.01), and significantly elevated the expression of hepatocyte growth factor (P<0.05);however, there was no significant influence on keratinocyte growth factor under hypoxia (P>0.05). Based on the protein level, protein secretion of hepatocyte growth factor and vascular endothelial growth factor from human adipose-derived stem cells was increased under hypoxia (P<0.01), but no changes occurred in nerve growth factor and keratinocyte growth factor. After cultured under hypoxic environment, human adipose-derived stem cells were promoted to express gene vascular endothelial growth factor, hepatocyte growth factor and nerve growth factor, as wel as to secrete protein keratinocyte growth factor and vascular endothelial growth factor.

Objective To summarize the experience of early management in elderly patients with serious burning in order to raise cure rate. Methods Elderly patients (63 cases) with serious burning (burned area exceeding 30% or Ⅲ? area exceeding 10% ) admitted from 1990 to 2002 were retrospectively studied. Patients were divided into two groups according to the admission date, before and after January 1, 1996 since new management was extensively applied after 1996. These measures consisted of rapid and adequate fluid resuscitation, early enteral feeding and autograft after eschar excision. OBJECTIVE To study the correlation between the serum-specific IgE and IL-13 single nucleotide polymorphism (SNP) among patients with allergic rhinitis who were allergic to Dermataphagoides pteronyssinus(Derp1) or Artemisia Pollen. METHODS Sixty nine patients who were allergic to Derp1 and 87 patients who were allergic to pollen were examined for detecting serum-specific IgE by a fluorescent enzyme immunoassay. The IL-13 SNP of the coding sequence (Arg130Gln) and the promoter region -1112(C/T)was measured by polymerase chain reaction /restriction fragment length polymorphism (PCR-RFLP) techniques. RESULTS Among patients who were allergic to Derp1, there was a significant correlation between the IL-13 promoter -1112(C/T)SNP ,but not of Arg130Gln ,and serum-specific IgE. Among patients who were allergic to pollen, there were no significant association both of the IL-13 promoter -1112(C/T)and Arg130Gln SNPs with serum-specific IgE. CONCLUSION The results showed a significant association of the IL-13 promoter -1112(C/T)SNP with serum-specific IgE in patients who allergic to Derp1. That suggested a possible involvement of IL-13 promoter -1112(C/T)SNP in the regulation of serum-specific IgE response to Derp1 allergens.
8.The hypoglycemic effect and mechanism of Laminaria japonica on diabetic model induced by alloxan in rats

Objective To investigate the hypoglycemic effects of Laminaria japonica (L. japonica) on diabetic model induced by alloxan in rats. Methods Sixty healthy female rats were used to establish diabetic models by injecting alloxan peritoneally, and L.japonica was applied as raw materials for potential marine drugs.The levels of fasting blood glucose (FBG) were detected by automatic blood glucose device. Enzyme linkedimmunoabsorbant assay was applied to determine the insulin level in serum. The shape and structure of isletcells were observed with histopathological staining, and the expression of superoxide dismutase (SOD) and inducible nitric oxide synthase (iNOS) in islet cells were detected by immunohistochemical technique. Results After the treatment, the levels of FBG of L.japonica treated group B [(9.37±1.70) mmol/LandC (9.18±1.65 ) mmol/L, F= 32.81, q=6.35~11.72, P<0.05 ] reduced, while the serum levels of insulin in treated group A, Band C (0.0378±0.0026, 0.0378±0.0027, 0.0367±0.0035) increased(F= 11.40, q=4.28~8.47, P<0.05) significantly than those of diabetic model group (0.0456 ±0.0057) . The shape and structure of islet cells improved with the up-expressing SOD(t=4.73~4.76, P<0.05)and down-expressing iNOS (t=4.81~5.30, P<0.05) in L.japonica treated group B and C than those in diabetic model group. Conclusion L.japonica might decrease the serum level of FBG through promoting the islet cell recovery by an anti-oxide effect.
9.Risk factors of septic shock after mini-percutaneous nephrolithotripsy

Objective To evaluate the risk factors of septic shock after mini-percutaneous nephrolithotripsy (mPCNL). Methods Clinical data of 1 590 cases who underwent mPCNL from January 2013 to December 2014 were retrospectively analyzed. The x2 test and logistic regression were used to identify the key risk factors for septic shock after mPCNL. Results Of the 1 590 patients, 18 patients suffered septic shock, including 6 male patients and 12 fe﹣male patients. Their mean age was (45.6 ± 13.5) years (28 ~ 69 years). White cell in urine was 100 percent, the stone diameter ranged from 1.5 to 5.0 cm, unichannel for 15 cases while multichannel for 3 cases, the operation du﹣ration ranged from 45 to 200 min, mean (87.0 ± 56.0) min. 2 in 18 cases died in multiple organ failure, the others recovered till discharged. In x2 test, female gender (P = 0.001), (+++ ~ ++++) white cells in urine (P= 0.042), un-preoperative nephrostomy drainage (P=0.041) had significant association with septic shock after mPCNL. While in multivariate analysis, female gender ( O? = 5.471, 95 % CI: 0.756~21.452, P< 0.05) and un-preoperative nephrostomy drainage (O? =3.106, 95%CI:1.283~7.907, P<0.05) were identified as independent risk factors for septic shock after mPCNL. Conclusions Female gender and un-preoperative nephrostomy drainage are the key risk factors for septic shock after mPCNL.
10.Treatment of upper ureteral calculi by an ureteroscopy approach in a low-head lithotomy with right or left lateral tilt

ObjectiveTo evaluate the methods in treatment of upper urinary calculi in a low-head lithotomy with right or left lateral tilt by an ureteroscopy approach. Methods From September 2009 to May 2015, 110 patients with upper ureteral calculi (after failed ESWL) were underwent holmium laser lithotripsy by a ureteroscopy approach in a low-head lithotomy with right or left lateral tilt. Their clinical data and complications were analyzed retrospectively. Results Surgical effect of patients was satisfied with the success of gravel 91 patients, with a total rate of 82.7 %gravel. Conclusion Low-head lithotomy with right or left lateral tilt is a good body position to perform ureteroscopic lithotripsy for upper ureteral calculi. It is safe and effective.
