1.Clinical application of distally reversed sural neurofasciocutaneous flap with muscle

[Objective]To introduce the effect of reversed sural neurofasciocutaneous flap with muscle repairing the soft tissue defect of foot and ankle.[Method]Reversed sural neurofasciocutaneous flap with muscle was applied for repairing the soft tissue defect simultaneous open fracture of tibia lower section 6 cases,the soft tissue defect simultaneous chronic osteomyelitis of tibia lower section 5 case,the soft tissue defect of sole of foot simultaneous calcaneus epicarp 3 case, the soft tissue defect of sole of foot simultaneous calcaneus osteomyelitis or dead space 2 case. The scope of the flap was 18 c m?13 cm~11 c m?7 c m and that of the gastrocnemius flap was 4 cm?3 cm~9 c m?6 cm,the thickness was 1~3 cm,the skin flap was bigger than the muscle flap.[Result]Muscle side of the muscle flap was errhysising actively and the skin flap was good circulation duing operation, the muscle flaps were used clinically in 16 patiens all survived completely after operation. All cases were followed up from 5 to 17 months ,incision of open fracture healed primarily, osteomyelitis without reoccurring, the muscle flaps were satisfactory in appearance,feel recovered and walk well.[Conclusion]Reversed sural neurofasciocutaneous flap with muscle has sufficient blood supply and high survival rate,so it is an effective and feasible method in repairing the soft tissue defect of foot, ankle and tibia lower section in special cases.

3.The investigation of the treatment of infection, complex defect of skin and bone in crus

Objective To investigate the effective method of the treatment of the infective, complex defect of skin and bone in crus. Methods On the basis of through anti-infection, the infective surface of wound with bone and skin complex defect was completely debrided. The infective and necrotic skin and bone were moved away, and the wound surface was covered with flaps having abundant circulation. The anti-infection therapy was continued after operation. When the infection healed and the normal skin covered the wound, the bone defect was renovated at the second stage. Result The transplanted flaps all survived and the skin covering the wound remained normal. After the follow-up of four to five years, all the transplanted bones healed completely. No limbs were amputured because of wound infection or the complex defect of bone and skin. Conclusion After the infective surface of wound was completely debrided, the skin defect was renovated with flaps having abundant circulation. On the basis of infection being totally cured, the lone-piece-bone defect was renovated. This method can maintain the limbs and creat conditon for the functional recovery.
4.Simultaneous Spectrophotometric Determination of Three Components in Satongfeng Injection by Flexible Tolerance Simplex Method

OBJECTIVE:To establish a method for simultaneous determination of the contents of three components in synthesis and actual satongfeng injection.METHODS:A flexible tolerance simplex method was used for multi-wavelength spectrophotometry with interval of 1nm in 258~290nm wavelenth,33 wavelenths were selected.RESULTS:The linear range and average recoveries rate of sodium salicylate,phenazone and caffeine were 0~42,0~12,0~6?g?mL-1 and(99.93?0.22)%,(99.85?0.24)% and(100.19?0.40)% respectively.All were agreed with those by standard method.CONCLUSIONS:This method is easy in operation,fast and reproducible,applicable to automatic analysis,and therefore can be used for the quality control of the preparation.

8.Effects of Resveratrol Combined with Radiotherapy on Human Hepatoma Cell Line Bel-7404 in Vitro

Objective To investigate the effects of radiotherapy combined with resveratrol( Res)on proliferation, invasion and apoptosis of hepatoma cell line Bel-7404. Methods Bel-7404 cell line was treated with Res(25 μmol·L-1 ) combined with radiotherapy,then divided into four groups( group 1:the control;group 2:radiation at dose of 2 Gy;group 3:radiation at dose of 4 Gy;group 4:radiation at dose of 6 Gy). Cell proliferation and invasion were detected by MTT assay. Cell apoptosis were observed by fluorescence microscopy. Expressions of MMP-2 and VEGF proteins were determined by Western blotting. Results Compared with the control group,cell proliferation and invasion were significantly inhibited,while cell apoptosis was increased in all radiation groups(P〈0. 05). Conclusion The sensitivity of hepatoma Bel-7404 cells to radiation can be enhanced by resveratrol. Radiation therapy combined with resveratrol can inhibit proliferation and invasion of hepatoma cells and increase the cell apoptosis,which may be related with the down-regulation of MMP-2 and VEGF proteins.

From October 1985 to April 1989, free flaps using post tibial vessel of the healthy leg as a bridgework were applied to 7 cases of extensive traumatic wounds, including damage of main vessels, exposure of bone and large soft tissue defect, of the lower limbs. Success was obtained in all cases. The operative method, indication, selection of the skin donor area and the key factor of operative success were introduced and discussed in the article.
10.Effects of acarbose versus fenofibrate on insulin sensitivity and β cell secretion in impaired glucose tolerance with hypertrigtyceridemia

Objective To investigate the effects of acarbose versus fenofibrate on insulin secretion and insulin resistance in the subjects with impaired glucose tolerance (IGT) and hypertriglyceridemia. Methods Eighty subjects were allocated to acarbose group (28 cases),fenofibrate group (30 cases),and control group (22 cases) without intervention for 3 months,and also divided into elderly (46 cases) and younger groups (34 cases). Fasting blood samples were collected for measuring fasting plasma glucose and lipid.Oral glucose tolerance test (OGTT) were carried out with measurement of plasma insulin and glucose before ad after treatment.Early insulin secretion indexes(△I30/△G30),insulin secretion indexes (HOMA -β) and insulin resistance indexes (HOMA-IR)were calculated. Results After 3-months of treatment,the lipid profile was evidently improved in fenofibrate group. Levels of triglyceride (TG) and total cholesterol (TC) were significantly reduced ( both P < 0.01 ),△I30/△G30 was significantly increased (P < 0.05) and HOMA-IR was decreased (P<<0.01).In acarbose group,levels of fasting plasma glucose (FPG),2hours postprandial plasma glucose (2 hPG) and HOMA-IR were reduced (all P<0.01),△I30/△G30 and HOMA-β were enhanced (P<0.01 or P<0.05).No change of above indicators was found in the control group.Compared with fenofibrate group,acarbose group had higher △ I30/△G30 (P< 0.05),HOMA-β (P< 0.01 ) and lower HOMA-IR ( P < 0.01 ).The improvement of △△I30/△G30 was correlated with the decreasing of plasma FPG,2 hPG and TG(r=0.5812,0.6327,0.3872,P<0.01),while HOMA-1R was related with the decreasing of plasma 2 hPG,TG and TC(r=0.8126,0.4671,0.2895,P< 0.01). HOMA-IR,△I30/△G30 and HOMA-β were lower (P<0.01) and acarbose improved insulin resistance better (P<0.05) in the elderly than in the younger with type 2 diabetes. Conclusions Pancreatic β-cell function declines gradually with aging,acarbose may have advantage over fenofibrate in improving early insulin secretion and resistance which was more relieved by diminishing glucotoxity in comparison with lipotoxity.
