1.Application values of multislice spiral CT in children malformation of large blood vessels outside the heart

Objective To analyze the application value of multislice spiral CT in diagnosis of children malformation of large blood vessels outside the heart.Methods The diagnosis results were compared between echocardiography and multislice spiral CT in 110 children with congenital heart disease,and the children had been confirmed by operation.Congenital heart disease was divided three kinds according to the region:heart,connections between heart and large blood vessels and large blood vessels outside the heart.Results One hundred and ten children with congenital heart disease were selected,and they had 366 malformations.The accuracy rates of echocardiography and multislice spiral CT were 90.7% (332/366) and 95.6% (350/366) respectively,and there was no statistical difference (x2 =3.284,P =0.070).In 366 malformations,160 malformations were in heart region.The accuracy rates of echocardiography and multislice spiral CT were 98.1% (157/160) and 97.5% (156/160) respectively,and there was no statistical difference (x2 =0.776,P =0.378).There were 55 malformations in connections between heart and large blood vessels.The accuracy rates of echocardiography and multislice spiral CT were 94.5% (52/55) and 100.0% (55/55) respectively,and there was no statistical difference (x2 =3.083,P =0.243).There were 151 malformations in large blood vessels outside the heart.The accuracy rates of echocardiography and multislice spiral CT were 81.5%(123/151) and 92.1% (139/151) respectively,and there was statistical difference (x2 =7.377,P =0.01).Conclusion Multisltce spiral CT has great application value in diagnosis of children congenital heart disease,especially in malformation of large blood vessels outside the heart.
2.Clinical Observation of Bloodletting plus Narrow Band Ultraviolet B for Prurigo Nodularis

Objective To observe the efficacy of bloodletting therapy plus narrow band ultraviolet B (NB-UVB) in treating prurigo nodularis.Method According to the randomized controlled principle, the enrolled patients were divided into a treatment group and a control group. The treatment group was intervened by bloodletting cupping at the selected acupoints and the topical areas plus NB-UVB once every other day; the control group was by orally taking Mizolastine sustained release tablets and external application of Halometasone cream.Result The total effective rate was 85.7% in the treatment group versus 61.9% in the control group, and the difference was statistically significant (P<0.01).Conclusion Bloodletting therapy plus NB-UVB can produce a content efficacy in treating prurigo nodularis, with few adverse reactions.
3.Epidemiological characteristics of human brucellosis in Nangang District of Harbin City in 2006-2015

Objective To analyze the epidemic characteristics of brucellosis in Nangang District of Harbin City,and to provide a scientific basis for making strategies in prevention and control of the disease.Methods Descriptive epidemiological methods were used,epidemic characteristics (time distribution,regional distribution and population distribution) of the disease in Nangang District of Harbin City in 2006-2015 were studied,and brucellosis cases were reported online through the national epidemic network.Results A total of 105 cases of brucellosis were reported,the average annual incidence rate was 7.84/100 thousand and no death case.The number of cases was the highest in 2014,it was 21 cases,the incidence rate was 1.57/100 thousand;the number of cases was the least in 2010,it was 3 cases,the incidence rate was 0.22/100 thousand;in 2006,the number of cases was 9,the incidence rate was 0.67/100 thousand;in 2015,the number of cases was 17,the incidence rate was 1.27/100 thousand.The incidence of the disease was mainly concentrated in summer and autumn,accounting for 65.71% (69/105),the number of summer onset was 38,accounting for 36.19% (38/105);the incidence of the autumn was 31,accounting for 29.52% (31/105).In 2006-2012,the incidence of the disease was mainly concentrated in Wanggang and Hongqi townships,accounting for 60.00% (30/50);after 2013,the cases in urban areas were on the rise,Wanggang and Hongqi townships accounted for 34.55% (19/55).Incidence of male youth was the highest,male and female incidence ratio was 2.18:1.00 (72:33);incidence peak was 30-60 years old,accounting for 60.95% (64/105);after analyzing the incidence of occupational groups,farmer was the highest,accounting for 43.81% (46/105).Transmission route was contact with cattle and sheep and lived in the environment cattle and sheep were breed,the number of cases was 34.Conclusions The incidence of brucellosis in Nangang District of Harbin City is rising after 2013.Disease season focus in summer and autumn,occupation focus in farmers,and the scope of the outbreak has been expanded.We should further strengthen publicity and education in disease focus season,improve the awareness of protection,in order to effectively control the epidemic of brucellosis.
4.Effects of transcranial magnetic stimulation on motor function after acute cerebral infarction

ObjectiveTo investigate the effects of transcranial magnetic stimulation(TMS)on motor function after acute cerebral infarction.Methods60 patients who suffered acute cerebral infarction were assigned to experiment group and control group(30 cases each group). Both groups received rehabilitation theropy. Meanwhile,experiment group received TMS. Fugl-Meyer Assessment(FMA), National Institute of Health Scale of Stroke(NIHSS), Barthel Index(BI) were used to measure the outcome.ResultsThe scores of FMA and BI increased significantly(P<0.05) while that of NIHSS decreased significantly(P<0.05) in both groups after treatment. However, these scores in experiment group improved more compared with those of control group(P<0.05).ConclusionTMS is helpful to improve the motor function, ability of daily living and neurological function.
5.Analysis of efficacy evaluation scales for anxiety treated with acupuncture-moxibustion.

Scales are important tools to measure and evaluate the severity degree and treatment effect of anxiety, but objective index with high quality is insufficient. Clinical researches of anxiety treated with acupuncture and moxibustion from the domestic and the oversea in recent 10 years are retrieved. The applications of all kinds of scales for anxiety treated with acupuncture and moxibustion in clinical research are analyzed, and problems needed to be paid attention to about scales are further explored. The establishment of effect evaluation system combining clinical symptoms with the quality of life is raised, so as to provide reference to further research.

6.Analysis the therapy effect of clavicular hook plate on Neer Ⅱ distal clavicle fractures and tossy Ⅲ acromioclavicular joint dislocation

Objective To investigate therapy effect and its clinical complications of the clavicular hook plate internal fixation for the treatment of Type Neer Ⅱ distal clavicle fracture and tossy Ⅲ acromioclavicular joint dislocation.Methods Twenty-two patients of Neer Ⅱ distal clavicle fracture and 16 patients of Tossy Ⅲ acromioclavicular joint dislocation were selected as our subjects.All patients were hospitalized and treated by using clavicular hook plate fixation form August 2007 to Februay 2012.The clinical effect and complications were analyzed retrospectively.Results All patients were showed the primary healing in terms of incision.Of all patients,30 cases were being following up,and follow-up periods was 6.0 to 18.0 months (average of (10.45 ±3.78) months).All patients underwent implant removal surgery at postoperative 6-14 months.No refracture or acromioclavicular joint separation again occurred.Four patients were with complications,of which,2 patients were clavicular hook plate decoupling at 7 days or 3 weeks after operation; One patient occurred clavicle stress fracture at more than 3 months after operation.All mentioned 3 cases were treated with re-operation.1 patient was with shoulder abnormal sound after operation and abnormal sound disappear after internal fixation removed.The shoulder functions were evaluated according to University of California-Los Angeles (UCLA) score system at before and after clavicular hook plate was taken out.The scores of pain and function of the shoulder,forward lateral flexion range were significant difference between before and after clavicular hook plate removed ((7.90 ±1.20) vs.(9.20±1.03),t =-2.327,P=0.045;(8.00±0.94) vs.(9.40 ±0.97),t=-3.280,P=0.001 ; (4.00 ±0.47) vs.(4.70 ± 0.48),t =-4.583,P =0.001).After the hook plate was removed,all shoulder functions showed improvement.The overall efficacy was excellent in 15 cases,good in 13 cases,and poor in 2 cases,and the recovery of excellent and good rate were 93.33%.Conclusion The clinical efficacy is well in terms of using clavicular hook plate internal fixation to treat Type Neer Ⅱ distal clavicle fracture and Tossy Ⅲ acromioclavicular joint dislocation.Prevention and treatment of complications should paid more attention.
7.Evaluation of right heart function in patients with acute pulmonary embolism by 256-slice spiral pulmo-nary artery angiography

Objective:To explore value of 256-slice spiral CT pulmonary artery angiography (CTPA)evaluating right heart function in patients with acute pulmonary embolism (APE).Methods:According to death risk assessment of APE patients, a total of 67 APE patients were divided into high risk APE group (n=41)and non-high risk APE group (n=35).CTPA was used to analyze and compare CTPA obstructive index and right heart function indexes between two groups.Correlations among CTPA obstructive index and right heart function indexes were also analyzed.Results:Compared with non-high risk APE group,there were significant rise in CTPA constructive index [8.58% vs.24.69%],percentages of straight or left protruded ventricular septum (31.46% vs.73.17%)and bronchial artery dilation (5.71% vs.24.69%);Except the supe- rior vena cava diameter,the rest right heart function parameters had difference significance in two groups,P<0.05 or<0.01. Spearman correlation analysis indicated that CTPA obstructive index was significant positively correlated with right ventricular maximal short axis (RVMSA),RV:LV,diameters of main pulmonary artery,superior vena cava and azygos (r=0.684~0.954),and significant inversely correlated with LVMSA (r=-0.786),P<0.01 all.ROC curve analysis indicated that AUCs of CTPA RV:LV (0.949)was the biggest diagnosing APE severity.Conclusion:Right heart func- tion indexes measured by multi-detector CT are accurate and practical indexes evaluating APE severity,and it possesses im- portant significance.
8. Analysis of incidence and mortality for upper digestive tract cancers and results of early detection and treatment program in Feicheng, Shandong Province

Objective:
To investigate the trend of esophageal and stomach cancer incidence and mortality in Feicheng during 2000 to 2013, and to evaluate the effects of the national project of screening for upper digestive tract cancers from 2006 to 2015 in Feicheng.
Methods:
We collected 26 569 cases, which were coded of C15 and C16 in the 10th edition of international classification of diseases (ICD) during the period of 2000-2013. The data came from cancer registration system of Feicheng, Shandong Province, China. These cases already covered 21 679 882 person-year. The annual percent change (APC) of incidence and mortalit were analyzed. The risk factors of upper gastrointestinal cancer were investigated and evaluated for people aging 40-69 in three street offices and eleven townships of Feicheng, and endoscopic screening was conducted among people who was in high risk. The corresponding data were used to analyze the early detection project of upper gastrointestinal cancer screening and early diagnosis, with the index of detection rate, early diagnosis rate, early treatment rate and other indicators.
Result:
There were 26 569 new cases of esophageal cancer and gastric cancer occurred in Feicheng from 2000 to 2013. 9. Analysis of clinical diagnosis and treatment in 43 cases of adult small intestinal torsion

Objective:
To investigate the diagnosis and treatment of adult small bowel torsion in order to improve early diagnosis and improve prognosis.
Methods:
Clinical data of 43 cases of small bowel torsion from January 2012 to December 2017 were collected. All of them were confirmed by surgery as small bowel torsion. After admission, white blood cell count > 18 × 109/L was found in 5 patients; and hemoglobin<100 g/L was found in 5 patients; abdomen CT examination in 39 patients showed 33 cases had intestinal torsion; 14 cases suggested possible mesenteric and root torsion.
Results:
All patients underwent surgical treatment. During the operation, there were 11 cases with intestinal torsion ≥ 720°; 13 cases underwent simple bowel torsion in surgical operation; 4 cases underwent decompression combined with reduction of the bowel; 1 case was treated with reduction and jejunostomy; and small intestine resection was performed for intestinal necrosis. Twenty-two patients had small bowel resection and jejunostomy was performed in 3 cases. The small intestine was resected 7-240 cm, with an average of about 66 cm.
Conclusions
Small bowel torsion should be diagnosed and operated as soon as possible. The suspected necrotic bowel should be removed and more healthy intestine should be reserved. The patient′s vital signs should be observed dynamically after operation to prevent the occurrence of intestinal necrosis and septic shock, to avoid excessive intestinal necrosis that would lead to postoperative short bowel syndrome, and to improve the patient's cure rate and reduce hospital stay.
