Objective Summarized the therapeutic effect of colonic irrigation by mannitol in one-stage resection and primary anastomosis for left hemicolon obstruction. Method For 31 cases of left hemicolon obstruction in our hospital, After process was cut, firstly we had done coloclysis with normal saline, metron-idazole and 20% mannitol,then one-stage anastomosis. Results With the exception of three cases of in-cision infection, the rest who start eating 4～5 days after operation were cured and no anastomotic fistula. Conclusion During the colon resection in left hemicolon obstruction, coloclysis with normal saline, met-ronidazole and 20% mannitol, then one-stage anastomosis, the intestinal function were quick recovery and can prevent colonic anastomotic fistula.

Objective To investigate the diagnostic value of 18F-FDG PET and 99Tcm-MDP imaging in detection bone metastases. Methods 93 patients were undergone 18F-FDG PET and 99Tcm-MDP imaging within 2 weeks. The results of the 2 different modalities were analyzed. Results Of the 93 patients, 64 patients were found to have bone metastases by PET and MDP imaging. Among the 64 patients, 43 patients were found both positive PET and MDP. 16 patients had positive PET whereas negative MDP. 5 patients had negative PET whereas positive MDP. BACKGROUND:Studies have shown that placement, needling point, angle and length of pedicle screws for thoracic vertebral fractures have been unified. Many studies concerned axial pul-out force and biomechanical behavior of a single screw, but few reports addressed the biomechanical behavior after the replacement with three kinds of screw connecting to rods. OBJECTIVE:To further observe and compare biomechanical behavior after three kinds of thoracic screw placement connecting rod using biomechanical testing, to understand the strength of thoracic pedicle screw replacement connecting rods, and the effects of its puling out on the stress of the bone surrounding the placement site. METHODS:The fresh frozen specimens of thoracic vertebra were divided into three groups at random: the pedicle fixation group, the transverse process-vertebral body fixation group, and modified rib transverse process fixation group. Thoracic pedicle screw system was inserted by the standard placement method in each group. The experiment was measured on the mechanical equipment. During puling out, the strength that bone bore was described with dynamic curve, folowed by statistical analysis. The difference in the gripping force on the thoracic vertebra among three kinds of screw placement method was compared. RESULTS AND CONCLUSION: Among three kinds of screw placement methods and among three kinds of screw placement methods on the injured vertebral body, the gripping force was significantly bigger in the pedicle fixation group than in the transverse process-vertebral body fixation group and modified rib transverse process fixation group (P < 0.01). No significant difference in the gripping force was detected between the transverse process-vertebral body fixation group and modified rib transverse process fixation group. Results verified that the stability of loading ability was apparently better in the pedicle fixation group compared with the transverse process-vertebral body fixation group and modified rib transverse process fixation group.

Objective To investigate the effect and the mechanism of parecoxib sodium pretreatment on permeability of blood-brain barrier in a rat model of focal cerebral ischemia-reperfusion injury.Methods Sixty male SD rats weighing 300g were randomly divided into 5 groups (n=12 each):sham operation group (group S);focal cerebral I/R group (group I/R);parecoxib sodium 5 mg/kg pretreatment group (group L);parecoxib sodium7.5mg/kg pretreatment group (group M);parecoxib sodium 10 mg/kg pretreatment group (group H) Middle cerebral artery occlusion models were made by reforming Longa suture method in SD rats.Thirty minutes before ischemia,rats in group L,M and H were injected with 5 mg/kg、7.5 mg/kg and 10 mg/kg parecoxib sodium through the internal jugular vein.Group S and group I/R received equal volume of normal saline.ELISA technique was used to determine the content of S100 β,TNF-α,IL-1 β in Plasma.The changes of cerebral water content and the Evans Blue exudation from brain capillaries were observed.Results Pretreated with parecoxib sodium (5mg/kg、7.5 mg/kg and 10 mg/kg),the content of S100 β,TNF-α,II-1 β in plasma were reduced.The cerebral water content and the EB in brain were reduced.Pretreated with parecoxib sodium 10 mg/kg,Longa scores were reduced.Conclusion Pretreatment with Parecoxib can protect blood-brain barrier against focal cerebral I/R injury by inhibition of the inflammatory reaetion.

Objective To evaluated diagnosis yield of cytological and histological analysis of endoscopic ultrasound guided fine needle aspiration (EUS-FNA).Methods A total of 43 patients who were clinically diagnosed as having mediastinal lesions,retroperitoneal lesions or pancreatic lesions by CT were recruited to the study.EUS-FNA was performed with standard 22G needle.Specimens were placed in formalin for histological analysis,and residual materials were prepared for cytologic analysis.Results Lesions were located in the mediastinum (n =9),the retroperitoneum (n =1),and pancreas (n =33).Cytologic diagnoses included malignancy (n =22),suspicious malignancy (n =2),atypical cell (n =3),absence of malignancy (n =16).Histologic diagnoses included malignancy (n =30),suspicious malignancy (n =1),sarcoidosis (n =2),chronic inflammation (n =10).The positive rates,sensitivities and accuracies of cytology and histology were 51％,59％,65％ and 74％,81％,84％,respectively.Conclusion The positive rate,sensitivity and accuracy of histology of EUS-FNA is superior to cytology.

Objective To investigate whether radial augmentation index (AI) associates with cardiovascular risk as well as arterial stiffness.MethodsA total of 4985 subjects, 2417 men and 2568 women, aged 18-96 (50.9 ± 14.7)years,were recruited.AI was measured in the left radial artery using tonometry Colin HEM-9000AI.Carotid-femoral PWV (CFPWV) and carotid-radial PWV (CRPWV) were examined by automatic pulse wave velocity (PWV) measuring system.Framingham risk score and Chinese ischemic cardiovascular disease risk score were applied upon subjects without cardiovascular disease to calculate individual cardiovascular risk respectively.ResultsRadial AI (rAI) was significantly higher in women than in men[(83.18 ± 12.36)％ vs (71.93 ± 15.22)％, P ＜0.01].Simple correlation analysis showed rAI was significantly correlated to two cardiovascular risk scores or CFPWV and CRPWV.After adjusted for multi-factors, rAI was still significantly correlated to Framing, ham risk score or Chinese ischemic cardiovascular disease risk score (r = 0.17 and 0.12) in men respectively (P ＜ 0.05), while r were 0.09 and 0.08 in women respectively (P ＜ 0.05).In multivariate analysis, there was a significant association between CFPWV and rAI (r = 0.14 in men, r = 0.10 in women, P ＜ 0.01), whereas the relation ship between CRPWV and rAI was not found.The relationship between rAI and cardiovascular risk or PWV became weaker for those aged above 50 years.ConclusionAugmentation index might be a useful marker of cardiovascular risk and arterial stiffness, which could be more feasible for younger subjects as a tool for risk differentiation.

Objective To study the changes of mesenteric arterioles on the contractile response to norepinephrine in patients with cirrhosis and portal hypertension (PHT) caused by hepatitis B after treatment by high-dose vitamin C.Methods Eleven PHT patients and 6 non-PHT volunteer were those undergoing their respective surgeriesat in our hospital from January 2010 to June 2011.The PHT patients were randomly assigned to vitamin C treatment group and placebo treatment group.After admission and before induction of anesthesia,blood taken to mesure plasma concentration of malonaldehyde (MDA).Intraoperative portal pressure was measured.We took the third level intestinal mesentery vessels with their mesentery next to the jejunum and studied the contractile response to norepinephrine of the intestinal mesenteric arterioles.We also measured RhoA/ROCK pathway associated proteins' expression and activity changes by Western-blot method.Results The average value of the plasma MDA of PHT patients with cirrhosis was much higher than that of non-PHT patients.After treatment by vitamin C,the value decreased.The dose-response curve of isolated mesenteric arterioles to norepinephrine from PHT patients with cirrhosis shifted to the right,and the EC50 value was higher than that of non-PHT patients.But this phenomenon was reversed after the patients treated with vitamin C.The protein expression of RhoA in mesenteric arteries did not change significantly among the three groups.But the protein expression and activity of ROCK-1 was significantly reduced in PHT patients with cirrhosis.After treatment with vitamin C,the expression and activity of ROCK-1 in mesenteric arteries of cirrhosis was significantly increased.Conclusion The PHT patients with cirrhosis are in the situation of oxidative stress.The high-dose vitamin C treatment can reduce oxidative stress level and improve contractile hyporeactivity of mesenteric arterioles of PHT patients with cirrhosis to norepinephrine.The protein expression and activity increasement of ROCK in RhoA/ ROCK pathway is involved in thisphenomenon.

Objective To investigate the value of integrated PET-CT with 18F-FDG in the diagnosis of non-small cell lung cancer.Methods 91 patients with non-small-cell lung cancer(NSCLC)and 10patients with benign disease underwent integrated PET-CT from February 2004 to May 2005.Other diagnostic procedures were analysed. Results 91 patients with lung cancer were found by positive SUV and pathologically confirmed.Of the 10 patients with benign disease.positive SUV were found in 3 cases.2 cases with tuberculosis and 1 case with inflammatory pseudotumor were confirmed.Negative SUV were found in 7cases.1 case was incorrectly diagnosed by integrated PET-CT.The sensitivity,specificity and accuracy of integrated PET-CT in the diagnosis of NSCLC were 100%,90%,99%,respectively.Forty-eight of 91patients with lung cancer underwent operation.There were 30 cases with and 18 cases without mediastinal nodal metastases.There were two false negative cases and one false positive case.The sensitivity,specificity and accuracy of integrated PET-CT in the diagnosis mediastinal nodal metastases were 93.3%,94.4%,93.8%.respectively.Metastases were identified in 33 cases.Conclusion Functional metabolism and anatomy imaging are fused in integrated PET-CT.The primary site and metastases lesion are correctly diagnosed by integrated PET-CT.The accuracy of integrated PET-CT in the diagnosis of NSCLC is over 90%.
