1.Modification of bioreactor for tissue-engineered heart valve and its application

Objective:To modify the pulsatile bioreactor we constructed previously for simulating the high-flow, high-pressure hemodynamics of heart valve in vivo, and to experimentally cultivate the tissue-engineered heart valves (TEHV) in the modified bioreactor. Methods: T-PLS system (NewheartBio Co., Ltd Korea) was used to generate pulsatile flow in the modified bioreactor and we designed a new air-exchange pathway to avoid contamination. The TEHV were made by seeding human bone mesenchymal stem cells (BMSCs) on de-cellularized porcine heart valve. After cultured under static condition for 4 d, the TEHVs were moved to the modified bioreactor and exposed to low-flow (0-600 ml/min) or high-flow(0-4 800 ml/min) pulsatile hydrodynamics for 7d, then the cells on TEHVs were evaluated. Results: After modification, the flow range expanded from (0-1 200) ml/min to (0-6 000) ml/min and the pressure range expanded from (0-40) mmHg to (0-180) mmHg. In culture experiments, 26.3% of the seeded cells remained under low-flow environment and cells were completely lost under the high-flow dynamics. Conclusion: The modified bioreactor can basically simulate the dynamics of heart valve in vivo and can be used in TEHV cultivation research. However, the current TEHV can not tolerate the high-flow pulsatile hydrodynamics.
2.Clinical observation of recombinant human interleukin-11 combined with prednisone in the treatment of adult recurrent primary immune thrombocytopenia

Objective To investigate the effect and adverse reaction of recombinant human interleukin-11 ( rhIL-11) combined with prednisone in the treatment of adults with recurrent primary immune thrombocytopenia ( ITP) .Methods The clinical and laboratory data of 34 patients of adult recurrent ITP were retrospectively analyzed.16 cases in A group were treated with prednisone,18 cases in B group were treated with rhIL-11 combined with prednisone.Results The effective rate of B group was 44.5%,which of A group was 50.0%,there was no sig-nificant difference between the two groups(P>0.05).After 7d,10d,14d treatment,platelet count in B group were (39.7 ±16.3) ×109/L,(55.3 ±27.6) ×109/L,(71.8 ±30.9) ×109/L respectively,which in A group were (24.3 ±6.7) ×109/L,(35.6 ±28.6) ×109/L,(47.3 ±29.2) ×109/L respectively,the differences between the two groups were statistically significant(t=2.008,2.090,2.431,all P<0.05).There were 5 cases(31.3%) in A group and 6 cases(33.3%) in B group showed adverse reactions,the difference between the two groups was no signif-icant (χ2 =2.117,P>0.05%),but there were 2 cases of capillary leak syndrome(CLS) in B group.Conclusion rhIL-11 combined with prednisone can promote significant rebound of platelet in the adult patients with recurrent IPT in a short term,effectively control bleeding,and the side effect is controllable.
3.Clinical application of ultrasound-guided femoral nerve and lateral femoral cutaneous nerve block for patients undergoing hip fracture surgery

Objective To observe the application of ultrasound-guided femoral nerve block (FNB) and lateral femoral cutaneous nerve block (LFCNB) for patients undergoing hip fracture surgery.Methods 60 patients scheduled for hip fracture surgery undergoing LMA general anesthesia were randomly divided into 3 groups,20 cases in each group.Before transfer patients from bed to operating table,A group received dezocine 5mg iv,B group received fascia iliaca compartment block(FICB),C group received FNB combined with LFCNB.40mL of 0.375% ropivacaine was injected guiding by ultrasound in B group and C group.The time of sufficient sensory block and awake,the dosage of propofol and remifentanil,MAP and HR at pre-block (T1),20min after block (T2),transfer bed (T3),LMA insert (T4),skin incision(T5),LMA remove(T6) and sober(T7) were recorded.Pain was assessed using visual analogue scale(VAS) pre-and post block.The incidence of using vasoactive drugs,agitation,pain and adverse reaction were also recorded.Results The time of sufficient sensory block and awake,the dosage of propofol and remifentanil in A,B and C groups were as following:A group (not measured),(20.3 ± 1.3) min,(835 ± 6.7) mg,(1 285 ± 18) μg;B group (i2.2 ±2.7)min,(13.3 ± 1.4)min,(610 ±9.9)mg,(835 ± 15) μg;C group (9.7 ± 2.4)min,(12.8 ± 1.5) min,(555 ± 6.5) mg,(785 ± 16) μg.The time of awake,the dosage of propofol and remifentanil in B group and C group were significantly lower than those in A group(F =2.62,2.41,2.45,all P < 0.05).The time of sufficient sensory block in C group was lower than that in B group (p < 0.05).The MAP and HR at T2,T3,T5 and T7 in A,B and C groups were:A group (115 ± 4) mmHg,(90 ± 8) beats/min,(135 ± 6) mmHg,(98 ± 8) beats/min,(104 ±6) mmHg,(87 ± 4) beats/min,(120 ± 5) mmHg,(88 ± 8) beats/min;B group (102 ± 3) mmHg,(81 ± 6) beats/min,(112 ± 5)mmHg,(82 ± 8)beats/min,(89 ±6) mmHg,(72 ± 3) beats/min,(100 ±6)mmHg,(76 ± 8) beats/min;Cgroup (100 ± 3) mmHg,(80 x 6) beats/min,(109 ± 6) mmHg,(83 ± 5) beats/min,(86 ± 5) mmHg,(70 ± 3) beats/min,(99 ± 5) mmHg,(75 ± 5) beats/min.The levels of MAP and HR in B group and C group were significantly lower thanthose in A group(F =2.25,2.85,2.87,2.91,all P < 0.05).The VAS scores at T2,T3,and T7in A,B and C groupswere:A group (3.9 ± 0.7) points,(8.2 ± 0.3) points,(6.0 ± 0.8) points;B group (2.3 ± 0.4) points,(4.1 ±0.4) points,(2.2 ± 0.7) points;C group (2.1 ± 0.5) points,(2.4 ± 0.4) points,(1.2 ± 0.4) points.The VAS scoresin B group and C group were significantly lower than those in A group (2.36,2.82,2.88,all P < 0.05).The VASscores at transfer bed and sober in C group were significantly lower than those in B group (F =2.32,2.38,all P <0.05).The incidence of using ephedrine/atropine,urapidil/esmolol,PONV,agitation,pain and incision pain in A,Band C groups were:A group 30%,30%,25%,25%,40%;B group 10%,10%,0%,0%,10%;C group 10%,5%,0%,0%,0%.The number of patients who required vasoactive drugs and adverse reaction in B group and C group were significantly lower than those in A group(x2 =7.58,8.81,9.11,9.11,8.89,all P <0.05).The incidence of incision pain at sober in C group was lower than that in B group(x2 =9.21,P < 0.05).Conclusion The ultrasound -guided FNB and LFCNB can obviously shorten the onset time,reduce the dosage of general anaesthetic and maintain the stability of henodynamics during the perioperative period.It has effective analgesia during transfer of patients from bed to operating table and sober.
4.Transplantation of autologous bone marrow mesenchymal stem cells transfected by angiogenin gene in a porcine chromic ischemic heart model

Objective To investigate the effect of the transplantation of autologous marrow mesenchymal stem cells transfected by angiogenin gene in a porcine chronic ischemic beart model.Methods Methods Mesenchymal stem cells were trnsfected byAd/Ang.The pigs underwent placement of amerold occluder around LCx and 4 weeks later sujected to transplantation of the trandected mesenchymal stem cells.The animals were evaluated by coronary angiography,echocardiography,mannetic resonance imaging end pathologic observation.Results All animal showed 95%occlusion fo LCx.4 weeks after treatment ,the perfusion of LCx,left ventricular ejection fracton were greatly evhanced.A large number of labeled mesenchymal stem cells were successfully uncorporated into blood vessels in the ischemic myocardial regions with increased vessels countin and survival of implanted cell.Conclusion Transplantaton of autologous mesenchymal stem cells trendected by angiogenin gene offere obvious advsntases of great improvement of blood supply and the heart funcition.
5.The value of right lateral decubitus position to decrease artificial defect of cardiac anterior wall in 99Tcm-MIBI SPECT myocardial perfusion imaging for women

Objective To explore the value of right lateral decubitus position MPI for differentiating myocardial perfusion defect from cardiac anterior wall attenuation artificial defect,caused by breast of woman.Methods Forty-nine patients(average age (61.5±8.4) years) who had low likelihood of coronary artery disease and had perfusion defect in the anterior wall after exercise stress 99Tcm-MIBI MPI were included.All underwent supine and right lateral decubitus position during resting SPECT images.The myocardial perfusion SPECT images at left ventricle were reconstructed and were measured by Bull's-eye,based on the counts.Results from both supine position imaging and right lateral decubitus position imaging were compared.Paired t test was used to statistically analyse the data by SPSS 13.0.Results Compared with supine position,the counts of the anterior,inferior,apex and lateral wall in right lateral decubitus position were significantly higher:(71.30±3.53) % vs (66.50±3.85) %,(70.06±4.45) % vs (65.44±4.16) %,(77.90±3.00)% vs (75.81±4.08)%,(79.30±2.26)% vs (72.60±3.87)% (t=6.731,5.286,3.555,10.885,all P<0.01).The counts of septal wall were significantly lower ((66.60±3.98)% vs (70.06±4.51)%,t=-4.625,P<0.01) in right lateral decubitus position than that in supine position.Among the different regions of anterior wall,the counts of the anterior-middle ((76.40 ± 3.80) % vs (68.60 ± 4.76) %) and anterior-apex region ((77.10±3.24) % vs (69.00±3.54) %) were significantly higher (t =9.916,8.870,both P<0.01) in right lateral decubitus position than those in supine position,but there was insignificance ((56.94±6.06)% vs (58.50±4.98) %,t =-1.493,P>0.05) at anterior-basal region.The artificial defect of different degrees in anterior wall was observed in all patients in supine position,23 cases (46.9%,23/49) showed artificial defect in the anterior-middle region and 16 cases (32.7%,16/49) in the anterior-apex region.All artificial defect showed radioactive filling in right lateral position imaging,12 cases(24.5%,12/49) filled markedly and 32 cases(65.3%,32/49) filled completely.Conclusions The artificial defects caused by breast of woman mainly present in anterior-middle region and in anterior-apex region of the anterior wall in supine position imaging.The right lateral decubitus position imaging can markedly improve these artificial defects.
6.Immunogenicity of decellularized porcine heart valve

BACKGROUND: It has been found that xenogenic extracellular matrix (ECM) may cause a strong inflammatory response in humans during clinical application of decellularized porcine heart valve (synergraft valves). An early inflammatory reaction severely weakens matrix structure of valve wall, leading to structural rupture and decay of grafts. From Synergraft's event, the decellularized porcine heart valves still had immunogenicity, especially for pediatric patients. The mechanisms by which the ECM triggers this immune process need to be further evaluated. OBJECTIVE: To find the difference of gene sequence between human and porcine ECM and to identify the ECM immunogenicity based on bioinformatics. DESIGN, TIME AND SETTING: A contrast study between human and porcine ECM based on type IV collagen was performed at the Laboratory of Cardiothoracic Surgery, Changhai Hospital, the Second Military Medical University of Chinese PLA from June 2008 to February May.MATERIALS: The fresh porcine heart valves were obtained from Shanghai Wufengshangshi Slaughter House. Decellularized porcine aortic valves, hybridoma cells, and monoclonal antibodies were provided by our laboratory. METHODS: Similar region and conservative site of gene sequence among human, porcine, and rat were compared so as to look for common similar region, site, and sequence difference and investigate the segment which caused common and different gene sequence. Type IV collagen monoclonal antibody was used to evaluate the persistence of ECM of decellularized porcine heart valve following immunohistochemical staining. MAIN OUTCOME MEASURES: Type IV collagen gene sequence; efficacy of self-made antibody using immunohistochemistry; effect of self-made antibody on type IV collage of decellularized porcine heart valve. RESULTS: The differential gene serial in type IV collagen protein was found out by bioinformatics method. Monoclonal antibodies were successfully produced by human-mouse hybridoma technique. Residual porcine ECM was observed on decellularized porcine heart valve. CONCLUSION: Residual porcine ECM was observed on decellularized porcine heart valve and had immunogenicity.
7.Efficacy of 90Sr- 90Y low dose applicator, topical timolol maleate, and the combination in the treatment of superficial infantile hemangioma: a prospective cohort study

Objective:To investigate the clinical efficacy and safety of 90Sr- 90Y β-ray low dose applicator, topical timolol maleate, and their combination in the treatment of superficial infantile hemangioma (IH). Methods:From May 14, 2013 to April 11, 2017, 400 children (126 males, 274 females, age 5.3(3.9, 7.1) months) with superficial IH in Department of Nuclear Medicine, the First People′s Hospital of Foshan were prospectively enrolled. All patients were randomly divided into 4 treatment groups according to the proportion of 1∶1∶1∶1 by the method of random number table: topical timolol maleate (group A, control group), 90Sr- 90Y β-ray low dose applicator (group B), single course applicator combined with timolol (group C), and multi-course applicator combined with timolol (group D). Lesions were followed up to the 104 th week (W104). Cure rate of W104 was considered as primary end point. Efficacy and safety of different treatment were compared. Kruskal-Wallis rank sum test, Mann-Whitney U test, and logistic regression analysis were used for statistical analysis. Results:Totally, 438 lesions in 400 cases were included in this prospective study. There was no significant difference in baseline characteristics among 4 groups ( χ2 values: 1.709-11.616, H values: 3.681-7.653, all P>0.05). As of W104, 11 lesions (2.51%, 11/438) were lost follow-up, 32 lesions (7.31%, 32/438) were with early withdrawal, 357 lesions (81.51%, 357/438) were cured, 15 lesions (3.42%, 15/438) were with residual, 23 lesions (5.25%, 23/438) were with rebound growth, and no serious adverse events occurred in the 4 groups. Multivariate analysis showed that lesions thickness (<3 mm vs ≥ 3 mm, odd ratio ( OR)=16.689, 95% CI: 7.908-35.223; χ2=54.555, P<0.001) and treatment (considering group A as reference category, OR (95% CI) of group B, C and D were 16.842(6.179-45.901), 4.801(2.167-10.638) and 39.127(10.468-146.243), respectively; χ2=47.663, P<0.001) were independent factors affecting the cure rate of W104. 90Sr- 90Y low-dose fractionation radiotherapy was significantly better than topical timolol maleate ( OR=16.842, 95% CI: 6.179-45.901), and the combination with timolol could significantly reduce the cumulative absorbed dose of radiotherapy (group D vs B: 16(8, 16) vs 16(16, 24) Gy; z=-4.947, P<0.001). Conclusion:90Sr- 90Y low dose applicator therapy is superior to topical timolol maleate for superficial IH, and the combination with timolol could significantly reduce the cumulative absorbed dose of applicator.
8.Comparison of the different reconstruction algorithms for Philips GEMINI PET/CT

Objective To evaluate the effects of different reconstruction algorithms on image quality for Philips GEMINI PET/CT. Methods Jaszczak phantom were scanned on the GEMINI PET/CT system, and the raw data were reconstructed using filtered-back projection with Hanning filter (FBP-Hanning), filtered-back projection with Butterworth filter (FBP-Butterworth), ordered subset expectation maximization (OSEM), row-action maximum likelihood algorithm (RAMLA) and three-dimensional row-action maximum likelihood algorithm (3D-RAMLA), respectively. The resolution, uniformity, contrast of images and the time of different reconstruction algorithms were compared. Results The reconstruction time was 180 s, 130 s, 120 s, 85 s and 80 s for 3D-RAMLA, RAMLA, OSEM, FBP-Hanning and FBP-Butterworth respectively in phantom studies. The smallest rods with diameter of 4.8 mm of the phantom could be observed for FBP- Butterworth and 6.4 mm for other algorithms in case of high counts. The image contrast of 3D-RAMLA were better than that other algorithms, and the image uniformity of 3D-RAMLA and RAMLA were better than those of other algorithms. The resolution, uniformity and contrast of images with all algorithms decreasd in case of low counts, and the image quality of FBP-Butterworth was not good enough for clinical studies. Conclusion Image quality is variable with different reconstruction algorithms. For clinical PET imaging, it is necessary to choose proper algorithms.
9.Comparative analysis of essential oil and fatty acid constituents of Abrus cantoni-ensis and Abrus mollis

Objective To utilize and evaluate the resources of Abrus cantoniensis Hance (AC) and Abrus mollis Hance (AM ) by analyzing and comparing their essential oils and fatty acids .Method The essential oils of AC and AM were extracted with the hydro distillation method .Fatty acids were obtained by petroleum ether extraction of the 95% ethanol concentrate . Fatty acid extract was further reacted with BSTFA for GC-MS analysis .Components were identified by searching NIST MS library .Result Forty-two and thirty-three chemical constituents were identified from the essential oil of AC and AM respec-tively ,which accounted for 56 .76% and 63 .45% of their volatile components .( ± )-α-terpinyl acetate are the common essential oils found in both AC and AM .Thirteen and fourteen chemical constituents were identified from the fatty acid extract of AC and AM respectively .AC and AM have different chemical components and compositions .Conclusion Our results provide a sci-entific basis for the bioactivities ,quality control and resource utilization of AC and AM .
10.Herbal textual research on Illicium lanceolatum A.C.Smith and Illicium henryi Diels

Illicium henryi Diels is a medicinal and aromatic plant native to China with Chinese name Mancao or Hondu-hui.The name of Mancao was first recorded in the"Classic of Mountains and Rivers"and the earliest documented medicinal function of this plant was found in"Shen Nong′s herbal classic".Its botanical origin includes Illicium lanceolatum A.C. Smith and Illicium henryi Diels while Miscanthus sinensis Anderss(Gramineae)and Tripterygium wilfordii Hook.f. (Celastraceae)were recorded as Mancao by mistake.Mancao is mainly distributed in Henan,Hebei,Sichuan,Zhejiang and Fu-jian Province.All those plants are toxic.In this paper,the name,habitats,origins,the characteristics and effects of Illicium lanceolatum A.C.Smith and Illicium henryi Diels were investigated to correct the origin of the textural records and provide a reference for future research,development and utilization of the medical resources.
