1.Observation on therapeutic effect of electroacupuncture plus acupoint-injection for nerve root sciatica

Objective:To observe the clinical effects of electroacupuncture (EA) plus acuoint-injection for nerve root sciatica.
Methods:A total of 114 cases were randomly divided into an observation group and a control group, 57 cases in each group. The observation group was given EA plus acupoint-injection therapy with Cobamamide. The control group was given oral administration of Diclofenac Sodium Sustained-release Tablets plus muscular injection of Cobamamide. After one-course treatment, the clinical effects were compared between the two groups.
Results:The total effective rate was 96.5%in the observation group and 91.2%in the control group, without a statistical significance in the difference between the two groups (P>0.05). The curative rate was 57.9%in the observation group and 21.1% in the control group, with a statistical significance in the difference between the two groups (P<0.05). In the follow-up on the fourth day after the treatment, the recurrence rate was 23.1%in the observation group and 47.9%in the control group, with statistical difference between the two groups (P<0.05).
Conclusion: EA plus acupoint-injection therapy for sciatica of nerve roots is remarkable in the clinical effect and needs promotion in clinic.
2.Effects of Electroacupuncture on the Neuronal Apoptosis and the Expression of NF-?B Protein in Cerebral Ischemia Area of Cerebral Ischemia-reperfusion Rats

Objective To observe the effect of electroacupuncture (EA) on the expression of NF-?B in cerebral ischemia area of cerebral ischemia-reperfusion rats and the neuron apoptosis in cortex by EA on shuigou and neiguan (left) point, and study the protective effect and its mechanism of EA on the cerebral ischemia. Method Cerebral ischemia reperfusion model was established by occlusion of the middle cerebral artery for 30 min and then removal, then EA shuigou and neiguan left points of experimental rats. Neuronal apoptosis in cortex was observed by the terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick end labeling (TUNEL), NF-?B protein in the ischemia cortex was detected by immunohistochemistry. Result The neuronal apoptosis and the expression of NF-?B protein in the ischemia cortex was deteded of ischemia side of EA treatment groups were lower comparing with model group. Conclusion EA can lighten the neurons injury after cerebral ischemia-reperfusion by decreasing the expression of NF-?B protein and the neuronal apoptosis in the ischemia cortex.
3.Early diagnosis of contrast-induced renal damage and the protective effect of hydration therapy

Objective To investigate the diagnostic value of urine neutrophil gelatinase apolipoprotein (NGAL) and kidney injury molecule 1 (KIM-1) as markers of CIN,and the effectiveness of hydration therapy in the prevention of CIN.Methods One hundred and twenty patients were randomly divided into control group and treatment group.The patients of treatment group received hydration therapy through intravenous fluid infusion.Urine samples were taken for detecting the value of albumin (mAlb),NGAL,and KIM-1 before surgery (T0),after surgery 12 h (T1),24 h (T2),48 h (T3),72 h (T4) by ELISA assay.The levels of urinary mAlb,Scr,BUN and cystatin C were detected at the same time.Results (1) The urine NGAL/Cr and KIM-1/Cr significantly increased and were more than twice the baseline value at the time of 12 h after PCI in 87 of 120 cases of the participants.There are eight cases occurred CIN (6.67%) and one case occurred in hydration treatment group (1.7%),seven cases were in control group (11.7%).The difference was statistically significant.(2) There were no significant difference in BUN,Scr,mAlb/Cr,Cys-C and GFR between two gToUps (P > 0.05).(3) NGAL/Cr,KIM-1/Cr were elevated at T1 in both groups (P < 0.01).In hydration treatment group,levels of NGAL/Cr and KIM-1/Cr decreased substantially to the level of T0 at T4(P < 0.01),while in the control group they didn't.(4) Area under the ROC curve (AUC) of NGAL/Cr and KIM-1/Cr 12 h after PCI were 0.931 [95% CI (0.889,0.973)] and 0.811 [95% CI(0.736,0.886)] respectively (all P<0.05).Conclusions NGAL and KIM-1 are sensitive and specific indicators for predicting early renal injury induced by contrast medium and can be used for early diagnosis of CIN.Hydration therapy can prevent the contrast agent-induced renal damage.
4.Clinical characteristics of elderly patients on maintenance hemodialysis in Anhui Province

Objective To understand the clinical characteristics of elderly patients aged 60 years and over in Anhui Province who receive maintenance hemodialysis (MHD).Methods Clinical data of MHD patients in hemodialysis centers of 26 hospitals in Anhui Province from March 31, 2014 to January 1, 2014 were collected.With non-elderly MHD patients (aged over 18 old and under 60) serving as controls, similarities and differences in the primary disease, vascular access, nutritional status, mineral and bone disorders (MBD), and other related aspects in MHD patients were compared.Results The top three original diseases for elderly patients were chronic glomerulonephritis (CGN) (32.8 %), hypertensive nephrosclerosis (HNS) (27.7 %) and diabetic nephropathy (DN) (26.2%).The proportion of patients with DN and HNS in the elderly was higher than in the control group (P < 0.001).Vascular access types in elderly MHD patients were arteriovenous fistula (AVF) (89.4 %), tunneled cuffed catheter (TCC) (8.9 %), and temporary venous catheter (1.3%).The proportion of patients with AVF was lower (P<0.001) and the proportion of patients with TCC was higher (P<0.001) in the elderly group than in the control group.The incidence of anemia was higher in the elderly patients than in the control group (P<0.05);the proportion of elderly patients with hemoglobin (Hb) levels was 30.3%, in which had no significant difference as compared with the control group (P > 0.05).The incidence of hypoalbuminemia was 21.2% in elderly patients, which was higher than in the control group.The proportions of elderly MHD patients with normal levels of adjusted serum calcium, serum phosphorus, and intact parathyroid hormone (iPTH) were 52.0%, 27.9% (higher than in the control group, P<0.001), and 47.9%, respectively, while there was no difference in adjusted serum calcium and iPTH between the two groups (P>0.05).The treatment rates for low serum calcium,hyperphosphatemia and secondary hyperparathyroidism (SHPT) in elderly patients were 50.6%,51.6% and 71.5%, respectively, which had no significant difference as compared with those in the control group (P>0.05).Conclusions Elderly patients with MHD show distinct characteristics in primary diseases, vascular access types, and nutrition and MBD status, including higher proportions of HNS and DN as their primary diseases.The type of vascular access is still dominated by AVF, and the proportion of TCC is higher.The nutritional status is poor, with a lower incidence of high phosphorus and a higher incidence of low iPTH.Efforts should be made to promote compliance in elderly patients to better control MBD, improve nutritional status and increase safety and effectiveness of treatment.
5.Clinical Observation of Multimodal Analgesia for Treatment of PTPS

Objective To observe the curative effect and adverse reactions of intercostals nerve block combined with fentanyl transdermal system in treatment of post-thoracotomy pain syndrome (PTPS). Methods Intercostals nerve block combined with fentanyl transdermal system was used for treatment of 141 patients with PIPS. The degree of pain relief was evaluated by numerical rating scale (NRS),the quality of life of patients was evaluated by the quality of life score standard,and the adverse reactions were also observed. Results The overall remission rate was 97.85%,there were 51 cases (36.43%)with complete remission, 80 cases (57.14%)with significant remission,and 6 cases (42.85%) with moderate remission. The adverse reactions included nausea,vomiting, constipation,dizziness,skin pruritus and dysuria,no serious liver and kidney function damage and respiratory inhibition. The quality of life of all patients was improved. Conclusion Multimodal analgesia has better curative effect and less adverse reactions in treatment of PTPS,so it deserves clinical promotion.
6.Methods of hepatic vascular exclusion on recovery of postoperative liver function in patients undergoing hepatectomy

One hundred and ten patients with primary hepatocellular carcinoma underwent hepatectomy from 2005 to 2010.Different methods of hepatic exclusion were applied in the surgery,including 54 patients with total hepatic vascular exclusion,22 with half hepatic vascular exclusion and 34 with regional hepatic vascular exclusion.The recovery of postoperative liver function was retrospectively analyzed.The results showed that the postoperative liver function of regional hepatic vascular exclusion [ALT(311 ± 80) U/L,total bilirubin (TB) (22.2 ± 8.3) μmoL/L at 1 st day and ALT (58 ± 17) U/L,TB (11.3 ± 3.1)μmol/L at 7th day] was better than that of total hepatic vascular exclusion [ALT(874 ±299)U/L,TB (42.9± 19.1) μmol/L at 1st day (P<0.05) and ALT (108-±52)U/L,TB (14.6±9.2) μmol/L at 7th day] (P < 0.05,> 0.05),indicating that regional hepatic vascular exclusion can effectively reduce hepatic injury during the operation and promote recovery of liver function after operation.
7.Modulation of Interleukin-15-induced Suppression of Human Neutrophil Apoptosis by TNFα

Human interleukin-15 (IL-15) is a proinflammatory cytokine to suppress neutrophil apoptosis, which is a potential therapeutic agent. The modulatory effect of TNFα was investigated in IL-15-induced suppression of human neutrophil apoptosis. TNFα was shown to reverse the ability of IL-15 to delay neutrophil apoptosis within certain time course. Moreover, this reverse effect by TNFα might be associated with a reduction of the expression of the anti-apoptotic Bcl-Xl protein detected by Western blotting. It is concluded that TNFα can be used to modulate IL-15-induced suppression of neutrophil apoptosis within certain time course.
8.Therapeutic evaluation of endoscopic treatment of acute biliary pancreatitis

Objective To evaluate the therapeutic effect of endoscopic treatment for acute biliary pancreatitis. Methods Thirty-six cases of acute biliary pancreatitis were treated in our hospital in recent 4 years. Tweent cases (group E) underwent emergency endoscopic retrograde cholongiopancreatography(ERCP),endoscopic sphincterotomy(EST) and endoscopic naso-biliary drainage (ENBD) within 24h of hospitalization; 16 cases (group C ) were treated conservatively or by operation. Results Successful rate of endoscopic management was 95.24%. There were no severe complications nor mortality in group E. The duration of symptoms and hospitalization in endoscopic group was significantly shorter than those in group C(P0.05). But the serum and urine amylase in group E were significantly lower than those in group C in the second day(P
9.Pharmacokinetics Study of Puerarin and Puerarin Lobate Extract in Rats

OBJECTIVE:To investigate the pharmacokinetic characteristics of puerarin and puerarin lobate extract in rats and compare the difference of pharmacokinetic behavior of them. METHODS:24 SD rats were randomly divided into high-dose and low-dose groups of puerarin (300,100 mg/kg) and puerarin lobate extract (equal to 300,100 mg/kg of puerarin). 0.2 ml blood sample were collected from posterior venous plexus of bulbus oculi before intragastric administration and 0.083,0.17,0.33,0.5, 0.67,1,1.5,2,3,4,6,8,10 h after administration,respectively. The pharmacokinetic parameters were calculated by using DAS 3.0 software. RESULTS:Compared with puerarin high-dose group,AUC0-t and cmax of puerarin low-dose group increased sig-nificantly,while CL decreased significantly;MRT0-t of puerarin low-dose group and puerarin lobate extract high-dose and low-dose groups decreased significantly (P<0.01). There was no statistical significance in pharmacokinetic parameters between puerarin lo-bate extract low-dose group and puerarin lobate extract high-dose group (P>0.05). CONCLULSIONS:There is difference in the pharmacokinetic behavior between puerarin and puerarin lobate extract in rats. Some components of puerarin lobate extrats maybe speed up the elimination of puerarin in rats,so as to shorten the average retention time of it.
10.High glucose induces expression of osteopontin through activation of PI3 K/AKT/mTORC1 pathway in human renal tubular epithelial cells

Aim Toexplorethemechanismofupregu-lation of osteopontin ( OPN ) expression induced by high glucose in human renal tubular epithelial cells (HK-2cells).Methods Afterstimulationwithhigh-glucose (25 mmol·L-1 ) culture medium, HK-2 cells were then treated with the specific inhibitors or siRNA to inhibit the activity of PI3K and/or mTOR. Subse-quently, Real-time PCR was used to investigate the mRNA level of OPN, and Western blot was performed to detect the protein expression of OPN, p-AKT, p-S6,RaptorandRictor.Results Theexpressionlevel of OPN was increased in a time-dependent manner in HK-2 cells followed by high-glucose stimulation. The mRNA level of OPN peaked at 48 h; while the protein expression of OPN reached the highest level at 72h. Meanwhile, high glucose activated the PI3K/AKT/mTOR signaling pathway. Moreover, inhibition of the PI3 K/AKT/mTOR pathway by LY294002 and/or rapa-mycin led to significant down-regulation of OPN. Addi-tionally, the treatment with Raptor siRNA, but not Rictor siRNA resulted in reduction of OPN expression. Conclusion Highglucoseincreasestheexpressionof OPN through the activation of PI3 K/AKT/mTORC1 signaling pathway in HK-2 cells.
