1.Treatment of Posterior Ankle Impingement Syndrome with Arthroscopy

Objective To discuss and summarize the effects of arthroscopic treatment of posterior ankle impingement syndrome.Methods The study enrolled 30 patients with posterior ankle impingement syndrome treated from January 2008 to November 2015.There were 18 males and 12 females,aged 19-58 years old (mean,37.6 years old).Right ankle injury was noted in 19 patients and left in 11 patients.A total of 27 patients had a history of obvious ankle injury (7 cases of acute sprains and 20 cases of chronic sprains).Arthroscopic surgery,including osteophyte removal,cartilage formation and joint clearance,was performed through the posterolateral and posteromedial approaches.Postoperative outcome was evaluated by using the American Orthopedic Foot and Ankle Society (AOFAS) scores and visual analogue scale (VAS) scores.Results All the patients were followed up for 8-27 months (mean,22 months).These patients got a good healing without nervous and vascular injuries.At the last follow up,the AOFAS score was (89.8 ± 3.8) points,significantly higher than before operation [(35.0 ± 10.1) points,t =-26.456,P =0.000].The VAS score was (2.5 ± 1.3) points,significantly lower than before operation [(6.6 ± 1.9) points,t =12.427,P =0.000].The ankle joint flexion angle was improved significantly from 29.4° ± 2.3° to 43.4° ± 1.4° as compared to preoperative status (t =-32.625,P =0.000).There were 21 cases of excellent result and 9 cases of good result,with an excellent and good rate of 100%.Conclusion Debridement and treatment of complications under arthroscopy is one of the effective methods for the posterior ankle impingement syndrome,which has advantages of minimal invasion,safety,targeted operation,and fast postoperative recovery.

Objective To explore the internal jugular vein liver inside door body bypass surgery (Transjugularintrahepatie portosystemic shunt,TIPS) and part of the spleen embolization (PaniM splenicembolization,PSE) after laparoscopic resection combined with spleen schistosomiasis compounds with cirrhosis of the liver function of spleen hyperthyroidism curative effect.Methods Fifty-two cases had schistosomiasis hepatocirrhosis with spleen function in patients with hyperthyroidism,doing any operation after a week PSE TIPS.Postoperative liver function and adopted to monitor changes,two weeks left hand done help type laparoscopic splenectomy.Results Fifty-two patients underwent TIPS and PSE after surgical treatment,blood picture and liver function back to normal,in two weeks or so do help laparoscopic resection of the hand of the spleen,five cases were transfered to open surgery,patients were cured and discharged.And the previous similar average hospitalization days compared with patients who shorten,average hospital expenses dropped,and less complications,little pain.Conclusion TIPS and PSE joint laparoscopic splenectomy are schistosomiasis compounds with cirrhosis of the liver function of spleen hyperthyroidism,definite effect not only,and safety,minimally invasive,has a good value of clinical application.
3.Effects of Tongluo recipe on experimental diabetic peripheral neuropathy in rats

OBJECTIVE: To investigate the effects of Tongluo Recipe (TLR), a compound Chinese herbal medicine, on experimental diabetic peripheral neuropathy in rats. METHODS: Diabetes was induced by streptozocin in rats. Eight weeks later, the diabetic rats were assigned to 3 groups according to different body weights: untreated group, low-dose TLR-treated group and high-dose TLR-treated group. Eight normal rats were assigned to normal control group. Systemic and sciatic nerve intrinsic antioxidant enzyme activities, lipid peroxide levels, hemorheology and sciatic nerve conduction velocity and ultrastructure changes were evaluated. RESULTS: Glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) activities were lower, and lipid peroxide levels were higher in plasma and sciatic nerve of the untreated group than those in the normal control group (P<0.05). Blood glucose and viscosity in the untreated group were higher than those in the normal control group. Sciatic nerve showed demyelination changes and decrease of conduction velocity. Vasa nervorum expanded and erythrocyte deposited in the capillary in the untreated group. TLR obviously improved sciatic nerve conduction velocity and ameliorated morphology changes, decreased blood viscosity, improved SOD and GSH-Px activities. CONCLUSION: TLR can obviously improve sciatic nerve function and ameliorate morphology changes in rats with diabetic peripheral neuropathy, probably through depleting free radical production, improving endothelial cell function and ameliorating hemorheology changes.

5.Changes of plasma fibrinogen level among acute ischemic stroke subtypes according to TOAST criteria and effects of Songling Xuemaikang.

OBJECTIVETo investigate the changes of plasma fibrinogen level among acute ischemic stroke (ACI) subtypes according to Trial of Org10172 in Acute Stroke Treatment (TOAST) criteria and effects of Songling Xuemaikang.
METHODThe 160 patients with acute ischemic stroke were divided into two groups randomly: treatment group 85 cases (Songling Xuemaikang + Shuxuetong + Aspirin enterie coated tablets), control group 75 cases (Shuxuetong + Aspirin enterie ccoated tablets). The plasma fibrinogen was detected before and after treatment.
RESULTCompared with OC subtype, Fbg was higher in LAA, CE and SAO subtypes (P < 0.05). Compared with UE subtype, Fbg was higher in LAA, CE and SAO subtypes (P < 0.05). There was a significantly difference between LAA and SAO (P < 0.05). In LAA, SAO, CE of treatment group,the Fbg level were lowered significantly at the 15th day compared with pretherapy (P < 0.05). There was a significantly difference of Fbg between treatment group and control group In LAA, SAO and CE subtypes at the 15th day (P < 0.05).
CONCLUSIONFbg produces a marked effect at the pathomechanism of LAA, SAO and CE subtypes. Songling Xuemaikang can depress the plasma fibrinogen level of ACI, and be an effective adjunctive therapy on ACI.

6.Growth characteristics of A549 lung adenocarcinoma multicellular spheroid.

BACKGROUNDOwing to the limitation of monolayer tumour cells or transplanted tumor in radiation biology, multicellular spheroid (MTS) as a model of entity tumor had a wide range of application. This research aims to study the growth characteristics of A549 lung adenocarcinoma MTS so as to choose the best ones to perform experiments.
METHODS1.75×10⁷ A549 cells in 35mL were transfered into the 100mm incubator sealed with 2% agarose to have a rotating culture with 60r/min. After 40 hours every MTS was transfered into each of the 96 wells of multi-well plates coated with 2% agarose to have a static culture.
RESULTSDuring the rotation stage the MTS was round, the size was uniform, and the conjunction between cells was loose. After about 10 days of static culture, when the diameter of MTS at 429 to 570 μm the conjunction was close. After 26.5 days of static culture, the diameter of MTS became 1358.5 μm which could not be used for experiment. Observed under electron microscopy the cells in the MTS grew well without any apoptosis and necrosis. The cell cycle of MTS was dominated by G0 and G1 stage.
CONCLUSIONSCombination of rotating and static culture is a simple method to produce MTS which has similar growth characteristics with entity tumor and is best suitable for experiment research when the diameter is about 500 μm.
7.Clinic and Rehabilitation Pathway Recommendation for Spine and Spinal Cord Injury

Objective To establish the proposals of clinic and rehabilitation pathway for spine and spinal cord injury. Methods The experiences in clinic and rehabilitation treatment fields, the flow path of admission and the costs of hospitalization for spine and spinal cord injury patients in Chinese Rehabilitation Research Center were retrospectively summarized and analyzed. Related data in International Classification of Functioning, Disability and Health (ICF) core sets were also referred. Results The present proposals were applied to patients suffered from thoracic, lumbar or sacral spine injury with spinal cord injury. The proposals consisted of two parts: the surgical interventions in acute stage (within 2 weeks after injury) and clinic and rehabilitation treatments of spinal cord injury in sub acute and later stages (2 weeks to 6 months after injury). The criteria of surgical interventions in acute stage materialized the core therapies such as reconstruction of spine stability and neural decompression, etc. The criteria of treating spinal cord injury in subacute and later stages demonstrated the characteristic of combined therapy in which rehabilitation was the central element. Conclusion The establishment and implement of the present pathways were based on the time course of disease development and the staging feature of neural recovery and treatment after spinal cord injury. The purpose of this work is to improve the quality of medical service and reduce medical consumption.
8.Video-assisted thoracoscopic removal of esophageal leiomyomas with intraoperative tumor location by endoscopy.

OBJECTIVETo summarize our experience with video-assisted thoracoscopic (VATS) removal of esophageal leiomyoma located with endoscopy during operation.
METHODSBetween January 2006 and December 2010, 55 patients with esophageal leiomyoma underwent VATS enucleation. The surgical procedure was similar to that of open thoracotomy with intraoperative endoscopic location of the tumor and examination of the mucosal integrity especially for tumors less than 1 cm in diameter.
RESULTSOf the 55 patients undergoing VATS tumor removal, 54 patients completed the procedures smoothly, and 1 patient experienced ventricular fibrillation during the operation to require an open thoracotomy. Endoscopy was used in 38 patients during the operation. VATS enucleation differed significantly from open thoracotomy in the volume of bleeding, postoperative fasting days and postoperative hospital stay (P<0.05). The symptoms were completely relieved after the operation without postoperative complications. The patients were followed up for 8 to 59 months (mean 23.0 months) and no recurrence was found.
CONCLUSIONVATS removal of esophageal leiomyomas is minimally invasive, safe and effective and can serve as the primary option for surgical removal of esophageal leiomyomas.

Spinal cord injury is a catastrophic injury causing lifelong severe disabilities, and poses a great burden to the individuals, families and society. In order to promote the standardization in treatment of traumatic spinal cord injury, the consensus on the evaluation, treatment and rehabilitation of traumatic spinal cord injury was suggested by experts, who came from authoritative multicenter in China. The expert consensus, which formed a standardization process from the first aid clinical treatment to rehabilitation of spinal cord injury, shall give a better practical guide for clinic and rehabilitation physicians.
