1.Evaluation and Analysis of Drug Resistance of Escherichia Coli and Relationship of the Antibacterial Use

Objective:To investigate the variation of the drug resistance of Escherichia coli(Eco)against antibacterial in clinical specimens in the hospital, and to evaluate the simultaneous use of antibacterial, to explore to provide the reference for the reasonable clinical use of antibacterial. Method:Eco was separated from all kinds of clinical specimens of out-patients and in-patients in the hospital from August of 2005 to April of 2006,and ESBLs was detected with Kirby-Bauer (K-B) disk diffusion method. It was also counted that the antibacterial drugs ranked the first 30 in sales per month and their use frequencies in the same period in the hospital. Result:The detectable rate of producing ESBLs Eco was 58.6%. Eco was completely sensitive to imipenem. It had lower resistance rates, only 2% and 4% , against piperacillin/tazobactam and cefoperazone/sulbactam; and had 59% and 55% resistance rates respectively against cefotaxime claforan and ceftazidime pentahydrate in the third generation cephalosporin. The third generation cephalosporin and quinolones kept high in sales and their use frequencies ranked the first three. The use of the third generation cephalosporin with anhydrase inhibitors also had an uptrend. Conclusion:There is a direct relation between the use of a great deal of antibacterial and the imitative drug resistance of Eco. It is necessary to strengthen its clinical management and to raise the level of drug use.
2.The correlation of intra-abdominal fat area with insulin resistance in male with abdominal obesity

Objective To investigate the relationship between intra-abdominal fat area(IAFA)in male with abdominal obesity and glycometabolism related indicators. Methods Ninety-four obesity males,aged from 21 to 62 years old,were selected as our subjects. They were divided into abdominal obesity group(n = 70)and non-abdominal obesity group( n = 24). The general information of all subjects was recorded. The indexes of height,weight,waist circumference,hip circumference,body mass index(BMI)were measured. The fasting oral glucose tolerance test was performed. The levels of blood glucose(0,1,2 h)and insulin(0,1,2 h)were measured,and insulin resistance index was calculated. MRI measurement was applied to calculate the IAFA. Compared the difference between the two groups in term of the level of blood glucose,insulin and insulin resistance index,and analyzed the correlation between glycometabolism related indicators and IAFA. Results BMI,waist circumference,hip circumference,insulin resistance index and IAFA in abdominal obesity group were (28. 67 ± 4. 20)kg/ m2 ,(99. 75 ± 4. 07)cm,(104. 42 ± 7. 62)cm,2. 60 ± 1. 80,(153. 06 ± 53. 23)cm2 respectively,higher than those in non-abdominal obesity group((21. 80 ± 1. 97)kg/ m2 ,(79. 50 ± 10. 05)cm, (91. 86 ± 4. 49)cm,1. 52 ± 0. 73,(71. 78 ± 25. 48)cm2 ),and the differences were statistically significant(t= - 7. 704,- 9. 583,- 7. 618,- 2. 877,- 7. 184,P < 0. 05). The level of blood glucose at 0,1,2 h of patient in abdominal obesity group were(5. 89 ± 1. 36)mmol/ L,(10. 55 ± 3. 07)mmol/ L,and(8. 41 ± 3. 63) mmol/ L,higher than that in non-abdominal obesity group((5. 29 ± 0. 53)mmol/ L,(8. 76 ± 1. 96)mmol/ L, (6. 54 ± 1. 50)mmol/ L). Meanwhile,The insulin at 0,1,2 h of man in abdominal obesity group were(9. 71 ± 5. 05)mU/ L,(83. 29 ± 64. 51)mU/ L,(63. 56 ± 21. 09)mU/ L),significantly higher than those in non-abdominal obesity group((6. 42 ± 2. 96)mU/ L,(33. 00 ± 19. 82)mU/ L,(63. 56 ± 21. 09)mU/ L),and the differences were significant( t = - 2. 098,- 2. 671;- 2. 447,- 3. 010;- 3. 784,- 3. 089;P < 0. 05). The IAFA in abdominal obesity was positively correlated with age,BMI,waist circumference,hip circumference,blood glucose(0,1,2 h)insulin(0,2 h)and insulin resistance index(r = 0. 254,0. 533,0. 521,0. 615,0. 245,0. 315, 0. 294,0273,0. 249,0. 225,P < 0. 05 ). After adjustment for confounding factors,age( x1 ),abdominal circumference(x2)and insulin resistance index(x3)were related to IAFA(y = 1. 369x1 + 4. 472x2 + 25. 072x3- 333. 626). Conclusion Compared with patients with non-abdominal obesity,the IAFA of patient with abdominal obesity with abdominal fat area size is associated with insulin resistance.
3.Relationship between the gene polymorphisms of transforming growth factor beta 1 and interleukin 1 beta and primary knee osteoarthritis in Xinjiang Han and Uygur populations

BACKGROUND:Transforming growth factor β1 (TGF-β1) and interleukin 1β (IL-1β) have been reported to play an important role in the occurrence and development of primary osteoarthritis. OBJECTIVE:To investigate the association between gene polymorphisms of TGF-β1 and IL-1β and primary knee osteoarthritis. METHODS: Han and Uyghur elderly populations in Xinjiang Uygur Autonomous Region, China were surveyed on the prevalence of osteoarthritis, followed by allotted to osteoarthritis and health groups according the symptoms and radiography. Genotyping TGF-β1-509C/T and -1348C/T and IL-1β-511C/T was performed to analyze the relationship between the gene polymorphisms of TGF-β1 and IL-1β and osteoarthritis. RESULTS AND CONCLUSION:Alleles T and C with genotypes CC, CT and TT were detected in both two groups. In the Uygur population, the genotype frequency of TGF-β1-509C/T and IL-1β-511C/T showed significant difference between osteoarthritis and health groups (P < 0.05). In the Han population, the genotype frequency of TGF-β1-1348C/T showed significant difference between two groups (P < 0.05). These results suggest that the Uygur individuals carrying TT genotype of TGF-β1-509C/T and IL-1β-511C/T, and Han people carrying the TT genotype of TGF-β -1348T are more susceptible to osteoarthritis.
4.Clinical observation of intra-operative PTH assay in hyperparathyroidism due to parathyroid tumors.

OBJECTIVE:
To investigate the clinical manifestations and diagnostic method of hyperparathyroidism due to parathyroid tumors and to evaluate the intra-operative detection of parathyroid hormone in surgical treatment.
METHOD:
Thirty-seven cases with functional parathyroid tumors from January 2003 to October 2012 were retrospectively analyzed. The clinical manifestation, examination and operation method, changes of parathyroid hormone before and after operation were collected.
RESULT:
All cases were definitely diagnosed before operation. The sensitivity and the positively predictive values of neck ultrasonography were 86.5% and 97.6% respectively, and the same data of Tc-99m-MIBI was 97.2% and 100.0%. The PTH levels declined by 84.9% ten minutes after tumor resecting compared with the level before operation. The serum calcium and PTH returned to normal levels and symptomatic relief occurred after operation.
CONCLUSION
Recurrent bone disease, long-term urinary calculus and obscure gastrointestinal symptoms were common symptoms of hyperparathyroidism due to parathyroid tumors. The neck ultrasonography and Tc-99m-MIBI were suitable for location of parathyroid tumors. Surgical operation was an effective treatment for parathyroid tumor. Intra-operative PTH assay would be able to ensure the radical excision and the operative safety for functional parathyroid tumors.

5.SCD1 over-expression inhibits palmitic acid-induced apoptosis of rat BRL hepatocytes.

OBJECTIVETo investigate the protective mechanism of stearoyl-CoA desaturase 1 (SCD1) over-expression against the pro-apoptotic affects of palmitic acid on hepatocytes using the rat BRL cell line.
METHODSConcentration effect curves were generated using the trypan blue exclusion test to assess the death rate of BRL cells upon exposure to a dilution series of palmitic acid. The multiplicity of infection (MOI) of a lentiviral expression vector, pGC-FU-GFP, was determined for the BRL cells. Unmanipulated BRL cells were divided into two groups: the non-palmitate groups were composed of ordinary cultured cells (CON) alone, infected with lentivirus empty expression vector (negative control, NC), and infected with lentivirus overexpressing SCD1 (SCD1-LV); the palmitate groups were composed of ordinary cultured cells plus palmitate (CON+) alone, infected with lentivirus empty expression vector plus palmitate (NC+), and infected with lentivirus overexpressing SCD1 plus palmitate (SCD1-LV+). SCD1 mRNA expression was detected by real-time PCR. Propidium iodide (PI) single-staining was used to detect apoptosis and assess the cell cycle. Inter-group differences were analyzed statistically.
RESULTSThe death rate of BRL cells increased significantly after 72 h of exposure to 400 mumol/L palmitate (P less than 0.01). The MOI of pGC-FU-GFP in BRL cells was 20. The expression of SCD1 was significantly higher in the SCD1-LV and SCD1-LV+ groups than in the respective controls (vs. CON: F = 289, P less than 0.01; vs. CON+: F = 1522, P less than 0.01). Palmitate exposure led to decreased expression of SCD1 (CON+ vs. CON, F = 22, P less than 0.05 and NC+ vs. NC: F = 34, P less than 0.05). The ratio of S stage cells was similar in all non-palmitate groups (CON, NC and SCD1-LV, P = 0.137). However, there was a significant apoptotic peak and lower ratio of S stage cells in the control palmitate groups (CON+ and NC+) and the activity of cell proliferation was decreased as well. The ratio of apoptotic cells was decreased significantly in the SCD1-LV+ group compared to the CON+ group (P less than 0.01).
CONCLUSIONThe expression of SCD1 and its desaturation activity increased in BRL cells upon infection with the pGC-FU-SCD1-GFP lentiviral vector, suggesting that SCD1 over-expression can decrease palmitic acid-induced toxicity and apoptosis in hepatocytes.

6.The clinical value of " Push-pull traction-relax homing-Repeatedly confirmed" in the prevention of bile duct injury in LC

Objective To study the value of the technique of "Push-pull traction-relax homingrepeatedly confirmed" in the prevention of bile duct injury in LC. Methods From March 2001-August 2009, we applied this technique in 4800 cases of LC. The technique of "Push-pull traction" showed the structures of in the Calot's triangle. The technique of "relax homing" was to restore the cystic duct,hepatic duct and common bile duct to their original anatomical positions. The technique of "repeatedly confirmed" repeatedly identified the positions of the cystic duct, the common hepatic duct and the common bile duct. Results There was no bile duct injury. Conversion to open surgery happened in 118patients due to difficulties in identifying the Calot's triangle structures, bile duct stones, gallbladder cancer, and gallbladder-duodenal fistula. Conclusions The "Push-pull traction-relax homing-repeatedly confirmed" technique could effectively prevent bile duct injury in LC. The method is simple, easy to master and worthy of promotion.
7.Study on correlation of waist circumference with intra-abdominal fat area

To explore the best measurement of waist circumference related with intra-abdominal fat area evaluated by magnetic resonance imaging ( MRI). Totally 207 participants aged 20-60 years were enrolled. Waist circumference were measured at the levels of navel ( WC1) and the midpoint between costal brim and iliac cest (WC2). Intra-abdominal fat area was evaluated by MRI scan. Intra-abdominal fat area was significantly higher in men than in women [(132. 17 ± 59. 49 vs 70. 56 ± 35. 33)cm2 , P<0. 01]. Pearson correlation analysis showed that WC1 and WC2 were positively correlated with intra-abdominal fat area (r = 0. 779, r = 0. 809, both P<0. 01). Multiple linear stepwise regression analysis demonstrated that WC1 and WC2 were independently associated with intra-abdominal fat area(β=0. 553, R2 =0. 714, P<0. 01; β = 0. 603, R2 = 0. 735, P<0. 01). All of the two different measurements of waist circumference parameters may reflect intra-abdominal fat area, while WC1 seems to be the simpler one.
8.Treatment of upper urinary calculi with MPCNL : experience of 10,452 cases of 19 years in a single-center

Objective To analyze the clinical indications,efficacy and safety of Chinese minimally invasive percutaneous nephrolithotomy (MPCNL) in treating upper urinary calculi based on our experience.Methods From June 1992 to September 2010,a total of 10,452 patients (6060 males and 4392 females)with a mean age of (47.6 ± 13.7) years (7 months-93 years) received MPCNL in our center.The mean stone burden was (777.4 ± 740.3) mm2 (20 - 4 080 mm2 ).The data of stone burden,operative techniques,operating time,stone-free rate,major complication,hospital stay and stone composition were investigated. Results Of the 10 452 cases,11 801 procedures were performed on 10 876 (5493 left and 5383right) renal units,including 10 102 first stage procedures,1604 secondary procedures,86 third procedures and 9 fourth procedures.There were 11 830 tracts established,including 373 (3.15% ) tracts of 14 F,7867 (66.50%) tracts of 16 F and 3590 (30.35%) tracts of 18 F.There were 1207 (10.20%),9174(77.55%) and 1449 (12.25%) punctures located in upper,middle and lower pole,respectively.956(8.79%) renal units were managed with multiple tracts,which including 2 tracts in 846 (7.78%) units,3tracts in 85 (0.78%) units,4 tracts in 18 (0.17%) units and 5 tracts in 7 (0.06%) units.Pneumatic lithotripsy was used in 8563 (72.56%) procedures,Holmium:YAG laser lithotripsy was used in 2981(25.26%) procedures and Pneumatic lithotripsy + Holmium: YAG laser lithotripsy was used in 257(2.18%) procedures.762 (7.29%) cases needed ESWL to clean the stone after MPCNL.The average operating time was ( 101.3 ± 44.2) min ( 10 -240 min).The stone-free rate of MPCNL was 89.9%,which increased to 93% by adjunctive ESWL.And the mean hospital stay was ( 13.2 ± 6.4) days (2 - 72 days).The major complications happened on 321 (3.07%) cases,including 294 (2.81% ) cases of blood transfusion,12 (0.11% ) cases of sepsis,2 (0.02%) cases of renal abscess,9 (0.09%) cases of pleura injury,2 (0.02%) cases of colon injury and 2 (0.02%) cases of death.53 (0.51%) cases needed selective renal arterial embolization to achieve hemostasis.The main stone compositions were analyzed in 4345 cases.Calcium oxalate,calcium phosphate,magnesium ammonium phosphate,uric acid,ammonium urate,carbapatite and cystin were 91.74%,90.33%,14.91%,17.77%,4.83%,8.47% and 0.51%,respectively. Conclusions MPCNL is an effective and safe treatment option for all kinds of upper urinary calculi in patients at all ages with a high stone free rate and low major complication rate.
