1.Repair of bone defect caused by bone tumor with OsteoSet:Osteoset absorption and new bone growth characters

BACKGROUND:In recent years,bone defect caused by tumor,trauma,infection and so on is always the troublesome problem in the orthopaedics field. Many kinds of bone graft substitute have been exploited to conquer this problem.OsteoSet,a new substitute for bone graft,with the advantage of excellent biocompatibility,degradation,bone conduction and induction of ossification,is an ideal product.OBJECTIVE:To investigate the effect of OsteoSet in repairing bone defects due to tumor.DESIGN:Retrospective study and case analysis.SETTING:Department of Orthopaedics,First Hospital of Jilin University.PARTICIPANTS: Twenty-two patients with bone defect caused by bone tumor who received operation in the Department of Orthopaedics,First Hospital, Jilin University between June 2004 and September 2006.The involved patients,14 male and 8 female,averaged 23 years old ranging from 5 to 68 years.Inclusive criteria:① Diagnosed as benign tumor in bone according to the bone tumor classification criteria instituted in China in 1983.②Had the indications of tumorectomy and bone grafting with OsteoSet substitute.③Informed consent was obtained.The position of tumor was described as 6 in shaft of humerus,2 in neck of femur, 9 in inferior segment of femor, 4 in superior segment of tibia and 1 in celcaneus respectively.METHODS:①Operation procedure:After the effective anesthesia,the tumor was exposed as usual.Then the tumor was resected as thoroughly as we could.When OsteoSet substitute in the bone defect was filled carefully in order to avoid crushing into pieces.At last the substitute should be covered with periosteum or soft tissue.②Postoperative evaluation: The area of substitute absorption was observed by X-ray examination in 4, 8, 12 and 24 weeks after oporation.Postoperatively.incision healing,pain and complications of paients were observed at 6,7,8 and 9 months.MAIN OUTCOME MEASURES: OsteoSet substitute absorption rate and new bone formation rate as well as postoperative follow-up results.RESULTS:Twenty-two patients were involved in the final analysis.①OsteoSet substitute absorption rate and new bone formation rate:The evident absorption of OsteoSet substitute could be found in the X-ray film 4 weeks after operation.At the same time,the slender bone trabecula formed and bone density ncreased.At the end of 12 weeks after operation,about 97.1 percent OsteoSet substitute was absorbed and approximate 95.3 percent new bone infiltrated the defect area. In the 6 months, the substitute absorption rate and the new bone formation rate were 99.1 percent and 98.6 percent respectively. Stable bone trabecula formed in the bone defect area and the new bone connected with the surrounding health bone naturally by remolding.②Follow-up results:All the operative incisions healed well and there were no pain and discomfort in the duration of hospital stay; All the patients had no fever, erythra or other allergic responses during the follow up;The OsteoSet substitute were absorbed well and new bone also formed well in all the cases;All the patients were satisfied with the recovery of limb function.CONCLUSION:The substitute absorption was accordance with the new bone formation.For its good absorption and new bone reconstruction,OsteoSet is an ideal substitute for bone defect caused by tumor.
2.Comparison of total hip replacement and bipolar artificial femoral head replacement for elderly patients with femoral neck fractures

AIM: There is a controversial argument about the choice of total hip replacement (THR) and bipolar artificial femoral head replacement (FHR) for elderly patients with femoral neck fracture. This study was designed to explore the long-term biocompatibility and functional recovery of joints in patients over 70 years with femoral neck fracture by THR and bipolar artificial FHR. METHODS: Sixty patients with femoral neck fracture over 70 years were enrolled from the First Department of Bone and Joint Surgery in the First Hospital of Jilin University from January 2002 to December 2006. There were 21 cases (8 males and 13 females) receiving THR, with a mean age of (72.1?3.5) years, and 18 of them were complicated with internal diseases. While 39 cases (16 males and 23 females) received bipolar artificial FHR, their average ages was (75.1?6.4) years, and 35 of them were complicated with internal disease. ①Choice of artificial prosthesis: Sixteen prosthesis were produced by Germany Link Company and American Zimmer Company, 44 artificial joints were offered by Beijing Mengtaiyin Medical Instrument Co., Ltd. ②All the patients were operated by senior doctors who were qualified for post technique owing to more than 10-years working experience about artificial joints. ③The mean time of follow-ups for all patients was 37 months. The difference was compared in biocompatibility between materials and host, joint function, Harris scoring, time of operation, blood loss, time of hospitalization, cost of hospitalization and adverse effect. RESULTS: ①Comparison of biocompatibility: Two revisions for abrasion of acetabular wall were carried out 5 years after bipolar artificial FHR, and there was no infection of incision, dislocation of hip joint or loose of artificial joint in other patients of these two groups. ②Comparison of adverse effect: There was one patient with complications of pain and red swelling of hip in each group. No infection occurred around the prosthesis, and these two patients recovered after symptomatic treatment. ③Comparison of follow-ups for joint function: The average Harris scores was 89.8?8.6 in THR group and 72.7?9.5 in bipolar artificial FHR group, and there were significant differences in these two groups (P 0.05). ⑤Comparison of operation time and blood loss: The average operation time were (150.2?24.3) minutes in THR group and (105.8?22.1) minutes in bipolar artificial FHR group, with significant differences (P
3.Human osteosarcoma multidrug resistance-related protein identified by differential in-gel electrophoresis

Objective:To analyze the differential protein expression between multidrug-resistant human osteosarcoma MG-63/DXR100 and its parental cell line by differential in-gel electrophoresis,so as to lay a foundation for exploring the mechanism of multidrug resistance(MDR) of osteosarcoma.Methods:Multidrug-resistant clone of human osteosarcoma MG-63 was established by stepwise exposure to increasing doses of doxorubicin(DXR).The total proteins of the above two cell lines were separated with two-dimensional electrophoresis.The proteins of differential expression(increased or decreased by more than 30%) were analyzed with image scanning and DeCyder software.Then they were identified in MALDI-TOF Pro and Mascot database.Results:Thirty proteins with differential expression were identified by proteomic analysis,and 18 of them were further identified by MALDI-TOF Pro.Five protein spots were successfully identified:matrix metalloproteinases 1(MMP1) related with tumor cell metastasis,alcohol dehydrogenase(ADH6) with function of detoxication,FERM domain containing protein 3(FRMD3) which belongs to anti-oncogenes and two agnoproteins composed of 128 and 300 amino acid residues.MMP1,ADH6 and the two agnoproteins were over-expressed in MG-63/DXR100 group,and FRMD3 expression was lower than that in the MG-63 group.Conclusion:Five proteins including MMP1,ADH6,FRMD3 and two agnoproteins have been found abnormally expressed in MDR osteosarcoma cells by differential in-gel electrophoresis;they might participate in MDR of osteosarcoma.
5.Clinical significance of serum interleukin-15 in the patients of severe type B hepatitis

Objective To observe the level changes of interleukin-15 (IL-15) and its role and clinical sig-nificance in severe type B hepatitis(HB). Methods IL-15 levels of 47 cases of severe HB and 20 cases of healthy subjects were detected by ELISA,meanwhile the alanine aminotransferase (ALT),total bilirubin (TBIL) and pro-thrombase activity (PTA) were measured as well. The correlation between IL-15 and ALT,TBIL and PTA were ana-lyzed. Results IL-15 in severe HB eases were higher than in control group [(18.9±7.5 ) ng/L vs. (5.9±2.0) ng/L,P <0.01] ,which was higher in death group than in survival group[(24.1±7.5) ng/L vs. (15.7±5.4) ng/L, P<0.01]. IL-15 level was decreasing with the improvement of general condition and liver function recovery. In addition, IL-15 in severe HB was positively correlated with TBIL (r=0.570,P<0.01) and negatively correlated with PTA(r=-0.529,P<0.01) but was not significantly correlated with ALT(r=0.099,P>0.05). Conclusion IL-15 may take part in the pathogenesis of severe HB ,which is consistent with disease condition and is closely re-lated to the improvement of disease. The detection of IL-15 may exert a predicting role in the prognosis of severe HB.
6.Effects of thyroxine on the level of serum homocysteine and urinary albumin excretion rate in elderly patients with early diabetic nephropathy and subclinical hypothyroidism

Objective To investigate the Effects of thyroxine on the level of serum homocysteine and urinary albumin excretion rate in elderly patients with early diabetic nephropathy and subclinical hypothyroidism.Methods Seventy-five patients with early diabetic nephropathy and subclinical hypothyroidism were randomly divided into levothyroxine treatment group and conventional treatment group.Urinary albumin excretion rate (UAER),serum levels of homocysteine,creatinine,and lipids were measured at both pre-and post-treatment of 48 weeks.Results After treatment,serum total cholesterol,low density lipoprotein-cholesterol,triglyceride,thyrotropin,homocysteine,UAER,and serum creatinine in the levothyroxine treatment group were significantly lower than those in the conventional treatment group [(-0.52 ± 0.12 vs 0.31 ± 0.40) mmol/L,(-0.33 ± 0.22 vs 0.24 ± 0.36) mmol/L,(-0.16±0.18 vs0.19±0.29)mmol/L,(-4.4 ± 1.2 vs 1.2 ±0.8)mIU/L,(-1.4 ±2.0 vs0.9± 1.0)mmol/L,(-13 ± 13 vs 10 ± 7) pg/ml,(-2 ± 2 vs 3 ± 2) μmoL/L,respectively,all P<0.01].Conclusions Treatment with levothyroxine could significantly improve serum lipid profiles and reduce homocysteine,UAER,and creatinine,and exert a protective effect on the kidney in the elderly patients with early diabetic nephropathy and subclinical hypothyroidism.
7.Expressions of C-Met and HGF in tongue squamous cell carcinoma and their clinical significance

ABSTRACT:Objective To study the expressions of C-Met and hepatocyte growth factor (HGF)in tongue squamous cell carcinoma and their clinical significance.Methods We analyzed 46 patients (the treatment group) with tongue squamous carcinoma and 27 patients (the control group)with benign tumor treated in our hospital between June 201 1 and May 2013.The expressions of C-Met and HGF were detected by immunohistochemistry. Results In the experimental group,C-Met negative expression rate was 21.7% (10/46),weak positive expression rate was 26.1% (12/46),and strong positive expression rate was 52.2% (24/46).The above expression rates were 77.8% (21/27),1 5.8% (5/27),and 3.7% (1/27)in the control group.The two groups differed significantly (P <0.001).In the experiment group,the negative expression of HGF protein was 1 7.4% (8/46 ),weak positive expression rate was 58.7% (27/46),and strong positive expression rate was 23.9% (1 1/46).The above expression rates in the control group was 63.0% (1 7/27 ),37.0% (10/27 ),and 0.0% (0/27 ).The two groups differed significantly (P <0.001).The expression of C-Met protein was significantly related to lymph node metastasis (P <0.05).The expression of HGF protein was significantly related to pathological grading and lymph node metastasis (P <0.05).Conclusion The expressions of C-Met and HGF in tongue squamous cell carcinoma are significantly higher.And the occurrence and development of tongue squamous cell carcinoma are closely related to the expressions of C-Met and HGF.
8.Effects of telmisartan on the serum levels of homocysteine and transforming growth factor-β 1 in patients with early diabetic nephropathy

Sixty patients with early diabetic nephropathy (DN) and normal blood pressure were randomly divided into telmisartan treatment and control groups.And the blood biochemical parameters,urinary albumin excretion rate (UAER),the serum levels of homocysteine (HCY) and transforming growth factor-β1 (TGF-β1) were measured at both pre- and post-treatment of 24 weeks.After treatment,the levels ofUAER,HCY [(61 ±26) vs.(98±38) mg/L,(11.70 ±2.86) vs.(14.4±2.8) μmol/L (P< 0.01 ) ]and homeostasis model of insulin resistance index,TGF-β1 [ ( 3.17 ± 0.66) vs.( 3.60 ± 0.87 ),(66 ± 14) vs.(77 ± 17 )U/L respectively (P < 0.05 )]in the telmisartan group were significantly lower than those in the control group.It indicated that telmisartan could improve the insulin sensitivity,significantly decrease the serum levels of HCY and TGF-β1,reduce UAER and exert kidney protection effects in the patients with early DN.
9.Influence of Transdermal Electric Stimulation of Neiguan (PC 6) On TCD in Patients with Acute Migraine

Purpose: In order to explore the clinical mechanism of Neiguan (PC 6) in the treatment of acute seizure of migraine and the relationship between Neiguan (PC 6) and cerebral blood flow. Methods: Clinically, 30 cases of the patients with acute seizure of migraine (ascending phase) were selected, in TCD abnormality, and divided randomly into the treatment group and control group, 15 cases in each group. In acute seizure of migraine, Neiguan (PC 6)was punctured with electric stimulation on the healthy side of the patients in the treatment group and Xuehai (SP 10) was punctured with electric stimulation on the healthy side of the patients in the control group. TCD was determined in the patients 30 min before and after the treatment. Results and conclusion: After transdermal electric stimulation on Neiguan (PC 6),the velocity of intracranial blood flow was obviously slowed down, indicating that transdermal electric stimulation on Neiguan (PC 6) can effectively relieve the spasm of intracranial blood vessels and also shows its relative specificity.
10. One patient with chronic refractory wound caused by bamboo sticks in lower leg

One patient with chronic refractory wound in right lower leg was admitted to our department in June 2017, which experienced repeated ulceration for 4 years. On 4 days post admission, two bamboo sticks were taken out from the deep muscle of the right lower leg. The wound was repaired by perforator flap of posterior tibial artery in the right lower leg. The donor site was covered with split-thickness skin in the upper leg of the same side. On 7 days post operation, the flap and skin graft survived. During follow-up of 12 months, the flap had good appearance. This case suggests that the first thing for the treatment of chronic refractory wound is to find out the cause of wound.
