2.Reduced estimated glomerular filtration rate and proteinuria are associated with increased cardiovascular events rate in octogenarian population

Objective To determine the relationship between estimated glomerular filtration rate (eGFR) and proteinuria with cardiovascular events in subjects aged 80 years or older.Methods Data for this retrospective prognostic study were drawn from the patient database for routine checkup in Beijing hospital between January 2001 to December 2001.Baseline eGFR and proteinuria were evaluated in 340 subjects [mean age:(85.6 ± 4.0) years].eGFR was calculated using the modified abbreviated MDRD equations based on the Chinese chronic kidney disease patients.The subjects were divided into normal renal function group and reduced renal function group (eGFR < 60 ml · min-1 · 1.73 m-2).The subjects were divided into subjects without proteinuria and subjects with proteinuria group.Cardiovascular events included cardiovascular death,nonfatal myocardial infarction,nonfatal stroke.Results The proportion of reduced renal function was 36.8% (125/340).The proportion of proteinuria was 10.3% (35/340).The proportion of reduced renal function or proteinuria was 41.8% (142/340).Follow-up time was 79 months (40-114 months).Cardiovascular events rate was significantly higher in reduced renal function group than in normal renal function group [37.6% (47/125)vs.26.2% (55/210),P <0.05] and in proteinuria group than in without proteinuria group [50.0% (17/34) vs.28.2% (85/301),P < 0.01].Cox multivariate analysis revealed that both eGFR (HR =0.978,95% CI:0.961-0.994,P < 0.05) and proteinuria (HR =2.049,95% CI:1.132-3.709,P < O.05) were independent risk factors for cardiovascular events after adjusting for age,gender,triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,uric acid,hypertension,coronary heart disease,diabetes mellitus.Conclusions Reduced eGFR and presence of proteinuria are independent risk factors for cardiovascular event in subjects aged 80 years or older.eGFR and proteinuria can thus be used for cardiovascular event risk stratification in subjects aged 80 years or older.
3.Study on the prevalence rate of hypertension in Guangxi Hei Yi Zhuang population.

OBJECTIVETo study the prevalence rate of hypertension and the risk factors in Guangxi Hei Yi Zhuang population.
METHODSA total of 1068 people of Hei Yi Zhuang nationality aged 20 and over were surveyed. Blood pressure, height, weight, serum lipid and apolipoprotein levels were measured ,and both body surface areas and body mass index were calculated. Results were compared with those in 933 people of Han nationality living in the same district.
RESULTSThe prevalence rates of hypertension in Hei Yi Zhuang population were significantly higher than that in Han population (25.2% vs. 17.3%, P < 0.001) ,as well as the isolated systolic hypertension (12.5% vs. 3.9%, P < 0.001). The mean levels of systolic pressure and pulse pressure in Hei Yi Zhuang population were significantly higher than those in Han population (125.20 +/- 18.62 vs. 121.88 +/- 15.99 mm Hg, P < 0.001 and 48.64 +/- 14.75 vs. 44.98 +/- 11.12 mm Hg, P <.001; respectively). The prevalence rate of hypertension in Hei Yi Zhuang population was positively correlated with triglyceride, sex and age whereas the prevalence rate of hypertension in Han population was positively correlated with total cholesterol, sex, age, and alcohol consumption. Rates of awareness on hypertension in Hei Yi Zhuang and Han population were 8.6% vs. 21.1% (P <.001), patients with treatment rate of established hypertension were 4.5 vs. 15.5 (P < 0.001) with the control rates as 1.9 vs. 10.6 (P < 0.001).
CONCLUSIONThe prevalence rate of hypertension in Guangxi Hei Yi Zhuang population was significantly higher than that in Han ethnic group. The causes of high prevalence rate of hypertension might be ascribed to special geographical surroundings, unhealthy life style, high sodium intake, low education, and possibly genetic factors. The rates on awareness, treatment and control of hypertension were still under satisfaction.

4.The prevalence of body overweight and obesity and its changes among Chinese people during 1992 to 2002.

OBJECTIVETo describe the prevalence of body overweight and obesity and its changes among Chinese people during 1992-2002, and to provide scientific basis for developing intervention strategies for obesity in China.
METHODSThe data from "1992 China third National Nutrition Survey" (78,704 subjects, 38,323 male and 40,381 female) and from "2002 China National Nutrition and Health Survey" (209,849 subjects, 101,377 male and 108,472 female) were used in this study. The overweight and obesity were defined by using WHO Z-score criteria for children younger than 7 years old, Chinese age-, sex-specific BMI criteria for children aged 7-17 years, and Chinese BMI criteria for adults.
RESULTSThe overall prevalence of overweight and obesity of Chinese people was increased by 38.6% and 80.6%, respectively during the period of 1992-2002. The prevalence of overweight and obesity of Chinese people aged 0-6 years, 7-17 years, 18-44 years, 45-59 years and above 60 years was increased 31.7%, 17.9%, 66.7%, 45.2% and 43.7%, respectively. The increase of the prevalence of overweight and obesity was higher in girls younger than 6 years than their male counterparts, while among other age groups, that were higher in boys than girls. The prevalence of overweight and obesity among people living in rural areas was lower than that of their urban counterparts, while the increment of overweight and obesity prevalence among rural people was greater than that of their urban counterparts. It was estimated that another 70 million overweight and 30 million obese Chinese people emerged in China from 1992 to 2002.
CONCLUSIONThe prevalence of overweight and obesity of Chinese people was increased rapidly in the past decade, which had affected 260 million Chinese people. It would continue to increase in the near future if effective intervention measures have not been taken.

OBJECTIVETo study the relationship between the polymorphism of the variable number of tandem repeats region 3' of the apolipoprotein B gene (3'APOB-VNTR) and serum lipid levels in the Guangxi Heiyi Zhuang population.
METHODSA total of 548 people of Heiyi Zhuang nationatity were surveyed by a cluster sampling. Epidemiological data were collected and serum lipid and apolipoprotein levels were measured. The genotypes and alleles of the 3' APOB-VNTR were determined by polymerase chain reaction combined with gel electrophoresis, and then analyzed by direct sequencing in the most common alleles. The results were compared with those in 496 people of Han nationality also live in that district.
RESULTSThere were 19 alleles of the 3'APOB-VNTR in both ethnic groups. They were hypervariable elements (HVEs) 24, 26, 28, 30, 32, 34, 36, 38, 40, 42, 44, 46, 48, 50, 52, 54, 56, 58, 60, 62 and 64, but HVEs 56 and 58 in Heiyi Zhuang nationality and HVEs 48 and 62 in Han nationality were not be detected. The most common allele is HVE32 in Heiyi and Zhuang nationality (25.9%), and HVE34 in Han nationality (27.2%). The frequencies of HVEs 26, 30, 46, heterozygote, and short alleles (< 38 repeats, S) were higher in Heiyi Zhuang nationality than in Han nationality, whereas the frequencies of HVEs 34, 38, 40, homozygote, and long alleles (>or= 38 repeats, L) were lower in Heiyi Zhuang nationality than in Han nationality. The levels of total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C) and apolipoprotein (apo) B in Heiyi Zhuang nationality were higher in VNTR-LS (carrier of one long and one short alleles) than in VNTR-LL genotypes (the individual carrying two long alleles) genotypes. The levels of TC, triglycerides, HDL-C and apo B in Heiyi Zhuang nationality were also higher in homozygotes than in heterozygotes. There were no significant differences in the detected lipid parameters between the VNTR-SS (carrier of two short alleles) and VNTR-LS or VNTR-LL genotypes in both ethnic groups.
CONCLUSIONThe 3'APOB-VNTR polymorphism is found to be significant difference between Heiyi Zhuang nationality and Han populations, and is associated with the serum lipid levels in Heiyi Zhuang nalionality but not in Han nationality.

6.Prognostic analysis of transarterial chemoembolization combined with a traditional Chinese herbal medicine formula for treatment of unresectable hepatocellular carcinoma.

BACKGROUNDTransarterial chemoembolization (TACE) is the most widely used primary treatment for unresectable hepatocellular carcinoma (HCC) due to its survival benefit, though its clinical effect is still far from satisfactory. Jiedufang (JDF) granule preparation is a commonly used Chinese herbal medicine formula for HCC. The aim of this study was to evaluate the effect of combined therapy with TACE and JDF granule preparation in treatment of unresectable HCC on survival.
METHODSA retrospective study of TACE was performed in 165 patients with unresectable HCC who were admitted between January 2002 and December 2007 in Changhai Hospital, Shanghai, China. Of the 165 patients, 80 patients (study group) received combined therapy consisting of TACE and a long-term maintenance treatment with oral JDF granule preparation, and the remaining 85 patients (control group) received TACE alone. The survival rates of both groups were calculated by the Kaplan-Meier method. Factors possibly affecting survival were assessed by multivariate analysis in the Cox proportional hazard model, such as maximum tumor size, number of lesions, portal vein invasion, and etc.
RESULTSThe median overall survival was 9.2 months (95% CI: 6.94 - 11.46) in the study group versus 5.87 months (95% CI: 4.21 - 7.52) in the control group. In the study group,survival rates of the 1-, 2- and 3-year follow-up were 41.2%, 18.4%, and 9.6%, respectively. Significant independent prognostic factors identified by the Cox regression analysis were as follows: serum hepatitis B surface antigen (HBsAg) (P = 0.014), maximum tumor size (P = 0.027), number of lesions (P < 0.001), portal vein invasion (P < 0.001), and the therapy model (P = 0.006).
CONCLUSIONCombination therapy of TACE and JDF granule preparation may significantly prolong survival of patients with unresectable HCC.

7.Prevalence of hypertension in the Guangxi Bai Ku Yao populations

Objective To observe the prevalence of hypertension and associated risk factors in the Guangxi Bai Ku Yao populations.Methods A toLal of 1170 subiects of Bai Ku Yao aged 15 and over were surveyed by a stratified randomized cluster sampling.Blood pressure,body height,weight,waist circumference,serum lipid and apolipoprotein levels were measured, and body mass index(BMI)were calculated,matched 1173 subjects of Han Chinese from the same region served as control.Results The standardized prevalence of hypertension in Bai Ku Yao was significantly lower than that in Han(11.53% vs.16.79%,P<0.01).The mean levels of systolic,diastolic blood pressure,and pulse pressure in Bai Ku Yao were also signifieanfly lower than those in Han[(115.7 ±16.3)vs.(120.0±16.3)mm Hg (1 mm Hg=0.133 kPa),P<0.01;(74.1 ±9.4)vs.(75.9 ±10.4)mm Hg,P<0.01;and(41.6 ± 12.0)vs.(44.2 ±11.2)mm Hg,P<0.01;respectively].Hypertension was positively correlated with male,age,physical activity,BMI,waist circumference,and the intakes of total energy,total fat,and sodium,and negatively associated with education level in both ethnic groups(P<0.05-0.01),but was positively associated with alcohol consumption only in Han.The rates of awareness,treatment and control of hypertension were significantly lower in Bai Ku Yao than those in Han population[(11.81%vs.21.76%),P<0.05;(5.51%vs.12.95%),P<0.05;and(2.36%vs.8.29%),P<0.05;respectively].Conclusion The prevalence of hypertension was significantly lower in Bai Ku Yao population than in Han population and diet,low sodium inmke,life style,and genetic factors might be responsible for the lower hypertension prevalence in Bai Ku Yao population.

OBJECTIVETo investigate the levels of blood pressure and serum lipids, and examine the relationship between hypertension and hyperlipidemia in Hei Yi Zhuang Chinese living in Guangxi.
METHODSA total of 1056 people of Hei Yi Zhuang ethnicity were studied. Blood pressure, body height, body weight, and serum levels of lipids and apolipoprotein were measured. The data were compared with those in 925 people of Han ethnicity, who live in the same region.
RESULTSSystolic blood pressure and pulse pressure were significantly higher in Hei Yi Zhuang than Han Chinese (P < 0.001). The prevalence of isolated systolic hypertension and hypertension was also significantly higher in Hei Yi Zhuang than Han Chinese (P < 0.001). Serum concentrations of total cholesterol, triglycerides, low-density lipoprotein cholesterol, and apolipoprotein (Apo) B, and the prevalence of hypercholesterolemia and hyperlipidemia were significantly lower in Hei Yi Zhuang than Han Chinese (P < 0.05). Serum concentrations of high-density lipoprotein cholesterol and the Apo A1 to Apo B ratio were significantly higher in Hei Yi Zhuang than Han Chinese (P < 0.001). The prevalence of hypertension in Hei Yi Zhuang Chinese was positively associated with triglycerides (r = 0.425, P < 0.05), whereas the prevalence of hypertension in Han Chinese was positively correlated with total cholesterol (r = 0.623, P < 0.001).
CONCLUSIONThe present study revealed a significant difference in blood pressure and serum lipids between Hei Yi Zhuang and Han ethnic groups, and an association between hypertension and hyperlipidemia.

9.Genetic counseling and instruction for deaf couples directed by genetic testing.

OBJECTIVETo analyze the molecular pathogenesis of deaf couples by means of genetic testing. To provide accurate genetic counseling and instruction for deaf couples with different etiology based upon results of genetic testing.
METHODSFour deaf families from July 2005 to May 2006. Each subject was with moderate to profound hearing loss. Genomic and mitochondrial DNA (mtDNA) of each subject were extracted from whole blood. Genetic testing of GJB2, SLC26A4 (PDS) and mtDNA A1555G mutation were offered to each individuals.
RESULTSThe husband from family 1 didn't carry GJB2, SLC26A4 and mtDNA A1555G mutation while his wife was confirmed to carry compound SLC26A4 mutations. The possibility of their offspring's to be SLC26A4 single mutation carrier was 100%. The couple from family 2 both didn't carry GJB2, SLC26A4 and mtDNA A1555G mutation. The possibility of their offspring's having hereditary deafness caused by GJB2, SLC26A4 and mtDNA A1555G mutation was excluded. The husband from family 3 was confirmed to carry homozygous GJB2 mutations and a single SLC26A4 mutation while his wife who was diagnosed with enlarged vestibular aqueduct syndrome (EVAS) by CT scan was proven to carry a single SLC26A4 mutation. The risk of their offspring's suffering EVAS was 50%. The husband from family 4 was mtDNA A1555G positive while his wife who was diagnosed with cochlear malformation by CT scan didn't carry GJB2, SLC26A4 and mtDNA A1555G mutation. The risk of their offspring's having hereditary deafness caused by GJB2, SLC26A4 and mtDNA A1555G mutation was excluded.
CONCLUSIONSGenetic testing could be applied to offer the more accurate genetic counseling and instruction to deaf couples.

10.Study on corresponding areas the liver and lung channels in brain with fMRI.

OBJECTIVETo explore distribution of the Liver and Lung Channels in the brain so as to provide imaging basis for construction of channel theory in the brain.
METHODSSixty healthy student volunteers were randomly divided into a Liver Channel group (I) and a Lung Channel group (II), and the each group was further divided into five subgroups with 6 volunteers in each subgroup, based on five-shu-point principles which, were Dadun (LR 1, I 1), Xingjian (LR 2, I 2), Taichong (LR 3, I 3), Zhongfeng (LR 4, I 4), Ququan (LR 8, I 5), Shaoshang (LU 11, II 1), Yuji (LU 10, II 2), Taiyuan (LU 9, II 3), Jingqu (LU 8, II 4), and Chize (LU 5, II 5), respectively. In order to observe the brain activating patterns during acupuncture at the different acupoints, functional magnetic resonance imaging (fMRI) technique was adopted. All image data were then analyzed with SPM 2 software. The statistical parameter gram was composed of the pixel P < 0.01, and anatomic location was made according to Talairach coordinate, attaining experimentally activated areas, and the commonly activated area of five-shu-point of each channel was considered as the brain distribution of the Liver and Lung Channels.
RESULTSThe common areas activated by the five-shu-points of the Liver Channel were homolateral Brodmann area (BA) 34, BA 47, red nucleus, contralateral BA 19, BA 30, BA 39, the superior parietal lobule, cerebellum decline, and bilateral BA 3 and culmen. The common areas activated by the five-shu-points of the Lung Channels included homolateral BA 2, BA 18, BA 35, and contralateral BA 9 and substania nigra.
CONCLUSIONThere are relatively specific corresponding brain areas for the Liver and Lung Channels, indicating that there is possible relatively specific connection between channels and the brain.
