1.Effect of Jiedu Huayu Granules on Inflammatory State of Patients with Chronic Hepatic Failure with Toxic Heat and Stasis Syndrome

ObjectiveTo evaluate the curative effect of Jiedu Huayu granules (JDHY) in the treatment of chronic liver failure (CLF) with the syndrome of toxic heat and stasis and investigate the influence on the inflammatory state. MethodA total of 136 patients were randomly divided into a control group and an observation group with 68 cases in each group. In addition to the comprehensive western medicine treatment, patients in the control group received Yinchen Haotang granules orally at 1 dose/day and those in the observation group received JDHY at 10 g/time,3 times/day. The treatment lasted for eight weeks. The endotoxin (ET),diamine oxidase (DAO),aromatic amino acids (AAA),branched chain amino acids (BCAA),blood ammonia,calcitonin (PCT),tumor necrosis factor-α (TNF-α),interleukin (IL)-1,IL-6,IL-17,regulatory T cells (Treg cells),helper T cells 17 (Th17),Th17/Treg ratio,total bilirubin (TBil),albumin (Alb),alanine aminotransferase (ALT),aspartate aminotransferase (AST),prothrombin activity (PTA), and D-dimer (D-D) levels before and after treatment were detected. The Child-Pugh grading scores of liver function, toxic heat and stasis syndrome scores, and the model scores of end-stage liver disease(MELD) before and after treatment were recorded. The fatality rate and survival were recorded at the follow-up for 48 weeks. ResultCompared with the control group after treatment, the observation group showed decreased ET,DAO, and blood ammonia, increased BCAA/AAA ratio (P<0.01), reduced PCT,TNF-α,IL-1,IL-6, and IL-17 (P<0.01), elevated Treg cells, dwindled Th17 and Th17/Treg ratio (P<0.01), diminished TBil,ALT,AST, and D-D levels, and up-regulated Alb and PTA(P<0.01). The Child-Pugh grading score,MELD score, and toxic-heat and stasis syndrome score of the observation group were lower than those of the control group (P<0.01). The total response rate in the observation group was 93.65% (59/63),which was higher than 79.03% (49/62) in the control group (χ2=5.683,P<0.05). The fatality rate of the observation group eight weeks after treatment was 6.35% (4/63),which was lower than 19.35% (12/62) of the control group (χ2=4.757,P<0.05). There was no significant difference in mortality between the two groups 16,24, and 48 weeks after treatment. As revealed by the Log-rank test,the difference in survival curves between the two groups was not statistically significant. ConclusionJDHY can remove toxins from the body,regulate immune function,relieve inflammation,improve liver function, and reduce the severity of the disease in CLF patients with the syndrome of toxic heat and stasis. It is significant in clinical efficacy and worthy of clinical application.
2.Simultaneous determination the concentration of risperidone, paliperidone,olanzapine and quetiapine in human serum by HPLC-MS/MS

Objective To develop a simultaneous determination method of risperidone,paliperidone,olanzapine and quetiapine in human serum for therapeutic drug monitoring.Methods Clinical serum samples were prepared by protein precipitation and analytes were determined using HPLC-MS/MS.An Eclipse XDB C1s column (4.6 mm ×50 mm,1.8μm) was combined with mobile phase consisted of methanol,water and acetrile (37.5:12.5:50,plus 2.5 mmol · L-1 ammonium formate).The flow rate was 0.5 mL · min-1 The column temperature was 35 ℃ and the injection volume was 2 μL.MS detection was carried out using an electrospray ionization source operated in multiple reaction monitoring (MRM) mode.The ion transitions were m/z 427.3 → m/z 207.2(paliperidone),m/z 431.3--m/z 211.2 (paliperidone-D4),m/z 411→m/z 191 (risperidone),m/z 415.4→m/z195.2 (risperidone-D4),m/z 384.3→m/z 253.1 (quetiapine),m/z 392.3→m/z 258 (quetiapine-D8),m/z313→m/z 256 (olanzapine),m/z 316→m/z 256 (olanzapine-D3),respectively.Results The calibration curves of risperidone and paliperidone were y =0.67x +4.94 × 10-4 (R2 =0.999 4) and y =0.68x +0.14 × 10-2 (R2 =0.999 6),with the linear ranges of 1-100 ng · mL-1.The linear ranges were 2-200 ng · mL-1(calibration curve:y =1.33x-0.65 × 10-2,R2 =0.998 6) for olanzapine and 8-800 ng· mL-1 (calibration curve:y =1.80x-2.08 × 10-2,R2 =0.999 4) for quetiapine.The inter and intra-batch precisions of risperidone,paliperidone,olanzapine and quetiapine were all within 15 % in lower limit of quantitation (LLOQ),low,middle and high levels of quality control samples.The extraction efficiency was more than 90% for four analytes.The concentration of analytes in quality control samples were less than ± 15 % bias compared with nominal concentration after storage at room temperature for 24 h,-70 ℃ for 34 d or after freeze-thaw for 3 cycles.Conclusion The validated method was simple,fast and sensitive for the simultaneous determination of risperidone,paliperidone,olanzapine and quetiapine in human serum.The method was applied to the therapeutic drug monitoring for patients administered with the above drugs.The stability of mixed quality control samples is reliable and can be used for therapeutic drug monitoring.

Insomnia and depression are common mental disorders and there exists bidirectionality between sleep symptoms and depressive symptoms.Cognitive behavioral therapy for insomnia (CBT-I) is recommanded first in patients with accute or chronic insomnia.Phannacotherapy treatment is recommended when CBT-I is insufficient or patients are comorbided with depression or other mental diseases.Pharmacotherapy treatment options include antidepressants,benzodiazepines,non-benzodiazepines,atypical antipsychotics as monotherapy or cotherapy.The aim of the present review was to discuss the selection and clinical application of sedative-hypnotic drugs in patients with comorbid depression and insomnia.
4.Influential factors of serum concentration/dosage ratio for schizophrenia patients after oral administration of risperidone

Objective To investigate the influential factors of serum drug concentration/dosage ratio (C/D) for schizophrenia patients after oral administration of risperidone.Methods Medical records of inpatients who received oral administration of risperidone in Guangzhou Huiai Hospital during 2016 were consulted and the C/Ds were calculated.Effects of age,gender,dosage form,combined medication,comorbidity and serum concentration ratio of 9-OH risperidone and risperidone were evaluated.Results C/D in patients aged 60 or older was (10.06 ± 6.61) ng · mL-1 · mg-1 · d.It was significantly larger than those under 60 years old,which were (7.82 ± 3.31) and (8.02 ±4.05) ng · mL-1 · mg-1 · d for 40-59 and under 40 years old,respectively.In addition,C/D was decreased with the increase of 9-OH risperidone/ risperidone ratio.The C/D were(10.96 ±6.46),(8.71 ±4.45)and (6.30 ± 2.99) ng · mL-1 · mg-1 · d for 9-OH risperidone/risperidone ratio < 1,1-10 and > 10,respectively.Gender,dosage form andthe major combined medications and comorbidities in the recruited patients hardly affected the C/D of oral risperidone.Conclusion The dose of risperidone that administered to the aged should be reduced properly.Serum concentration ratio of 9-OH risperidone and risperidonehas a potential to be used as an indirect index of the activity of metabolic enzymes.
5.Uncertainty evaluation of the determination of lamotrigine in human serum by HPLC

Objective To evaluate the uncertainty in the determination of lamotrigine (LTG) in human serum with high performance liquid chromatography (HPLC).Methods It was analyzed that the influences of various factors for detection results,including weighing,solution preparation,serum extraction,calibration curve fitting repeatability and so on.The expanded uncertainty was evaluated and combined by each component of uncertainty.Results The expanded uncertainty of lamotrigine at low (1.50 μg · mL-1),middle (8.00 μg · mL-1),high (20.00 μg · mL-1) concentrations were UL =0.30,UM =0.53,UH =0.98,respectively (P =95%,k =2).The corresponding test concentration were (1.51 ±0.30),(8.22±0.53),(20.40±0.98) μg · mL-1,respectively.Conclusion The relative uncertainty on the determination of lamotrigine in human serum by HPLC was mainly caused by repeatability,calibration curve fitting and extraction recovery;repeatability and calibration curve fitting were the important factor in influencing the results of low concentrations.
6.A Pilot Study of Quantitative Loop-mediated Isothermal Amplification-guided Target Therapies for Hospital-acquired Pneumonia.

BACKGROUNDIt is important to achieve the definitive pathogen identification in hospital-acquired pneumonia (HAP), but the traditional culture results always delay the target antibiotic therapy. We assessed the method called quantitative loop-mediated isothermal amplification (qLAMP) as a new implement for steering of the antibiotic decision-making in HAP.
METHODSTotally, 76 respiratory tract aspiration samples were prospectively collected from 60 HAP patients. DNA was isolated from these samples. Specific DNA fragments for identifying 11 pneumonia-related bacteria were amplified by qLAMP assay. Culture results of these patients were compared with the qLAMP results. Clinical data and treatment strategies were analyzed to evaluate the effects of qLAMP results on clinical data. McNemar test and Fisher's exact test were used for statistical analysis.
RESULTSThe detection of Staphylococcus aureus, Escherichia coli, Pseudomonas aeruginosa, Klebsiella pneumonia, Stenotrophomonas maltophilia, Streptococcus pneumonia, and Acinetobacter baumannii by qLAMP was consistent with sputum culture (P > 0.05). The qLAMP results of 4 samples for Haemophilus influenzae, Legionella pneumophila, or Mycoplasma pneumonia (MP) were inconsistent with culture results; however, clinical data revealed that the qLAMP results were all reliable except 1 MP positive sample due to the lack of specific species identified in the final diagnosis. The improvement of clinical condition was more significant (P < 0.001) in patients with pathogen target-driven therapy based on qLAMP results than those with empirical therapy.
CONCLUSIONqLAMP is a more promising method for detection of pathogens in an early, rapid, sensitive, and specific manner than culture.

7.Treatment of type C pelvic fracture with Stoppa approach and posterior percutaneous plate fixation.

OBJECTIVETo explore the clinical effects of Stoppa approach and posterior percutaneous plate in treating type C pelvic fracture.
METHODSFrom June 2009 to July 2011,16 patients with type C pelvic fracture were treated with reconstruction plate fixation in Stoppa approach for anterior lesions combined with posterior percutaneous plate fixation for posterior lesions.There were 11 males and 5 females,with an average age of 38.8 years (ranged, 22 to 59 years). According to the Tile classification,10 cases belonged to C1,4 belonged to C2,2 belonged to C3. Tometta and Majeed score standards were used to evaluate clinical results.
RESULTSSixteen patients were followed up from 4 to 13 months with an average of 7.3 months. Operative time was from 80 to 140 min with an average of 100 min;blood loss volume was from 200 to 500 ml with an average of 280 ml; and the healing time of fracture was from 12 to 16 months with an average of 14 months. According to the Tometta score classification, 9 cases got excellent results, 6 good, 1 fair. According to the Majeed score classification, 9 cases obtained excellent results, 5 good, 2 fair.
CONCLUSIONReconstruction plate fixation in Stoppa approach for anterior lesions combined with posterior percutaneous plate fixation for posterior lesions is an ideal minimally invasive operation in treaing type C pelvic fracture. It can early exercise and has the advandages of small trauma, safe operaton,less complication, stable fixation.

8.Surgical options and clinical outcomes of cervical ossification of the posterior longitudinal ligament.

OBJECTIVETo discuss the options and clinical outcomes of surgical treatment of cervical ossification of the posterior longitudinal ligament (OPLL).
METHODSFrom January 2006 to December 2010, 63 patients of cervical OPLL treated surgically by same team were retrospectively analyzed. There were 49 males, 14 females, with a mean age of 56.2 years (from 38 to 76 years). The duration of symptoms ranged from 2 months to 20 years (mean 3.2 years). The ossified ligament was classified via sagittal and coronal images on CT scan. The cervical curvature was measured. Choice of surgical methods was determined according to the type of ossified ligament and cervical curvature. All complications occurred was recorded. The Japanese Orthopedic Association (JOA) scoring system was used to evaluate the neurological status. The severity of cervical pain was assessed with visual analogue scale (VAS) method. The results of preoperative, postoperative and follow-up were statistically compared with student t test.
RESULTSThe JOA scores of 35 patients performed with anterior approach improved from 9.8 ± 2.3 preoperatively to 11.7 ± 2.1 at final follow-up (t = 3.28, P < 0.05). The VAS sore was significantly decreased at final follow-up compared with preoperative (t = 3.15, P < 0.05). While the JOA scores improved from 10.4 ± 2.7 preoperatively to 12.5 ± 2.3 at final follow-up (t = 3.81, P < 0.05) in 12 patients with laminectomy and from 9.7 ± 2.5 to 11.6 ± 2.6 in 14 patients with laminoplasty (t = 3.56, P < 0.05). The VAS score either in laminectomy or in laminoplasty has not significantly difference between preoperative, 3 month postoperative and final follow-up (P > 0.05). Two patients underwent a combined anterior-posterior approach procedure. Complications in patients included 1 patient of postoperative neurologic deterioration and 2 cases of cerebrospinal fluid leakage in patients with anterior approach, and 2 cases of C(5) nerve palsy in patients with laminectomy.
CONCLUSIONSSurgical options of cervical OPLL should be determined by detailed study of type and range of ossified ligament, as well as the cervical curvature of patients. Good neurological function can be expected in case of appropriate choosing the method for treating the cervical OPLL.

10.Comparison of color reappearance between metal-ceram restoration and foundry-ceram restoration using crystaleye spectrophotometer.

OBJECTIVETo study the color reappearance effect of metal-ceram restoration and foundry-ceram restoration using Crystaleye spectrophotometer.
METHODS58 metal-ceram restorations and 58 foundry-ceram restorations according to the result of the Crystaleye spectrophotometer were made respectively. The deltaE between restorations and natural teeth as referenced were analyzed. And satisfaction of dentists and patients were evaluated.
RESULTSThe deltaE between metal-ceram restorations and natural teeth was 7.13 +/- 0.74. The deltaE between foundry-ceram restorations and teeth was 1.47 +/- 0.84. There were statistical differences between the deltaE (P < 0.01). The satisnatural faction of dentists and patients to the foundry-ceram restorations were higher than to the metal-ceram restorations (P < 0.05).
CONCLUSIONCrystaleye spectrophotometer can provide accurate reference for foundry-ceram restoration, but for metal-ceram restoration it is not accurate.
