1.Combined use of thoracoscopy and laparoscopy for the treatment of esophageal carcinoma:A report of 2 cases

Objective To explore the feasibility and short-term efficacy of combined use of thoracoscopy and laparoscopy for the treatment of esophageal carcinoma. Methods The study included 2 patients.The operation was conducted under general anesthesia.The esophagus was disconnected and the diaphragm was opened under a 4-port thoracoscopy.Then the stomach was dissociated under a 4-port laparoscopy.The esophagus was mobilized outside the thorax through a!cervical incision after the imple mentation of laparoscopic performance.Under direct vision,the lesion was removed and an anastomosis was made.A naso-intestinal tube and a naso-gastric tube were placed,respectively.Results The operation time was 450 and 470 min,and the intraoperative blood loss was 150 and 200 ml,respectively.The lesion was thoroughly removed with negative cutting edges.Postoperative pathological reports showed well-differentiated squamous cell carcinoma(stage T_1N_0M_0) in both of patients.Follow-up for 4 months in two cases found no recurrence.Conclusions Combined use of thoracoscopy and laparoscopy for the treatment of esophageal carcinoma ensures the feasibility and safety of esophagectomy,with minimal invasion and low postoperative complication incidence.
2.The effect of enteral nutrition on the patients suffered from esophageal rupture

Objective: Enteral nutrition was used to correct the malnutrition in the patients suffered from esophageal rupture postoperatively. Methods: The naso intestinal tube was placed during operation and the enteral nutrition was used postoperatively. The albumin, prealbumin and transferrin were measured before and day1, 5, 8 and 12 after operation. Results: All 27 patients were discharged with no death. Albumin, prealbumin and transferrin decreased on the 1st day postoperatively and reached the preoperative level on the fifth postoperativeday. Conclusion: Enteral nutrition plays an important role in the postoperative treatment for esophageal rupture.
3.Influence of siRNA interfering USP7 expression on cytobiological characteristics of laryngeal cancer cells

Objective This study is to investigate the influences of USP7 on the cytobiological characteristics of laryngeal cancer cells by small interfering RNA (siRNA) interfering the USP7 expression in the laryngeal cancer cells .Methods The self‐de‐signed highly efficient siRNA was used to conduct the specific interference on USP7 expression in laryngeal cancer HEP2 cells . Then the influence on the capacity of cell proliferation and migration ,as well as apoptosise after USP7 interference were observed by using the CCK‐8 method ,Transwell chamber migration test and flow cytometry .Results The self‐designed siRNA could effi‐ciently inhibit the expression of USP7 mRNA in laryngeal cancer cells ,furthermore markedly suppressed the proliferation and mi‐gration of laryngeal cancer cells ,enhanced the cell apoptosis in laryngeal cancer HEP2 cells in vitro .Conclusion The siRNA inter‐fering USP7 can inhibit the proliferation and migration capacity of laryngeal cancer cells ,and promoted their apoptosis .
4.Investigation of Dissolvability of Ephedra Alkaloid in Compatibility Ephedrae Herba- Aconiti Lateralis Radix Praeparata

Objective To determine equilibrium solubility and apparent oil/water partition coefficient of ephedra alkaloid in the compatibility Ephedrae Herba-Aconiti Lateralis Radix Praeparata; To provide a basis for transdermal delivery.Methods The extract was prepared by 70% ethyl alcohol and D101 macroporous absorbent resins. Dissolvability of its main effective components (ephedrine and pseudoephedrine) in the compatibility Ephedrae Herba-Aconiti Lateralis Radix Praeparata was determined by precipitation method and HPLC method; the oil/water partition coefficient of ephedrine and pseudoephedrine in n-octanol-water buffer solution system were determined by shaking flask method.Results The extract had optimum solubility in methyl alcohol and acetonitrile, and ephedrine and pseudoephedrine had optimum solubility in buffered solution of pH 7.4. Oil/water partition coefficient of ephedrine and pseudoephedrine in n-octanol-water system was 0.101 with lgP=-0.99 and 0.076 with lgP=-1.12. Oil-water partition coefficients of ephedrine and pseudoephedrine in the extract were affected by pH.Conclusion The extract has optimum solubility in high polar solvents. Ephedrine and pseudoephedrine have certain fatsoluble and water-soluble in suitable pH, which was beneficial for transdermal absorption.
5.Effects of Long-term Atenolol Treatment on Cardiac Adrenoreceptors in Rats

PURPOSE: To know whether the cardiac function of the patients with heart disease is impaired or improved after long-term taking ?-AR antagonists. METHODS: After giving the rats ?-AR selective antagonist, atenolol, for 12wk, the method of radioligand binding assay and the experiment of isolated left-atrium contractive function were carried out to observe the quantity and distribution of ?-AR, its subtypes, ?-AR, and the change of left - atrium contractive function. RESULTS: After long-term administration of atenolol, there were no any obvious changes in ?-AR, the density of the total ?-AR, ?1-AR, and the proportion of ?2-AR in tota1 amount of ?-AR, but the con centrat ion - response cu rves shi fted l e ftwa rd s si gn ifi cant ly as compared with control group- The PA2 value of isoprotenol(ISO) - induced positive inotropic effect after antagonized by ?1-AR selective antagonist CGP20712A in atenolol group was increased significantly as compared with control group. But the PKB value after antagonized by ICI 118511 showed no obvious difference between two groups. HPLC detection showed that the level of plasma atenolol was 3. 5pmol/L in atenolol group and the leveIs of plasma norepinephrine had no significant difference between two groups. But the level of plasma adrenaline in atenolol group was obviously lower than that in control group. CONCLUSIO- N: The long-term administration of ?l-AR antagonist will cause significant increase of the sensitivity of heart ?-AR, especially ?1-AR to excitant ISO. But the number of ?-AR and its subtypes did not change significantly. Besides, after the long-term administration of ?1-AR antagonist atenolol, the level of plasma adrenaline in rats was much lower than that in control group.
6.Perioperative nutrition support for lung transplantation

Objective: To evaluate the effect of nutritional support for lung transplantation patients.Methods: The lung transplantation patient received perioperative enteral nutrition(EN) .Exogenous glutamine(Gln) and recombinant human growth hormone(rhGH) were postoperatively used for 7-14 days.Results: The patients weight increased from 53 kg to 55 kg. No respiratory failure and acute rejection occurred postoperatively. The patient recovered fluently.Conclusion: Appropriate perioperative nutritional support and postoperative metabolic intervention can facilitate the recovery of lung transplant patient.
7.Application of contrast-enhanced ultrasound in preoperative diagnosis of anal fistula

Objective To evaluate the clinical value of contrast-enhanced ultrasound (CEUS) in preoperative diagnosis of anal fistula. Methods Forty-five patients with fistula in ano were evaluated by physical examination, then CEUS were peformed by injecting SonoVue through the external opening to enhance the detection of the fistulous track and the internal opening. The results of CEUS were matched with surgical features to establish their accuracy in preoperative assessment of anal fistula. Results Simple typing fistula was found in 19 of 45 patients and 26 patients had complex fistulas. The accurate diagnostic rates by conventional ultrasound of simple typing fistula and complex fistulas were 89. 5% and 61.5%,respectively,the accurate diagnostic rates by CEUS of simple typing fistula and complex fistulas were 94. 7% and 92.3%, respectively, there was no significant difference between conventional ultrasound and CEUS about simple typing fistula( P >0.05), and there was significant difference about complex fistulas ( P <0. 05). Conclusions CEUS has a good visibility and accurate rate for diagnosis of anal fistula,It plays an important role for operation.
8.Super-minimally invasive bilateral thoracoscopic extended thymectomy

Objective To present the technique of super-minimally invasive bilateral thoracoscopic extended thymectomy,and evaluate the early clinical results by using of this technique.Methods Twenty-three patients with myasthenia gravis (MG) with thymoma underwent with super-minimally invasive bilateral thoracoscopic extended thymectomy in our institution between July 2014 and January 2016.Two operate-poles are 5mm trocar,one is three intercostal space at the anterior axillary line,and the other is four intercostal space at the midclavicular line.A 10 mm trocar is inserted through the 6th intercostal space in the mid axillary line.The perioperative variables and outcomes were collected and analysed retrospectively.Results In the 23 patients who underwent Super-Minimally invasive bilateral thoracoscopic extended thymectomy,the mean operation time was (163.2 ± 14.4) min and the average blood loss was (148.2 ± 39.5) ml.The chest tube duration was (4.14 ± 0.27) days.There were no mortalities.Conclusion Our preliminary report showed that Super-Minimally invasive bilateral thoracoscopic extended thymectomy for mediastinal tumour resection was a promising and safe technique with regard to short-term clinical outcome.
9.Application of new ERCC1 antibody for molecular diagnosis of platinum chemotherapy in non-small cell lung cancer

Background and purpose:High expression of excision repair cross-complementing 1 (ERCC1) is related to resistance in patients treated with platinum-containing regimens. The ERCC1 antibody 8F1 was usually used in past studies, but it was found to have no-speciifcity recently. This study aimed to investigate the predictive role of a new ERCC1 antibody 4F9 to platinum chemotherapy in non-small cell lung cancer (NSCLC) patients. Methods:Expression of ERCC1 was detected using antibody 4F9 by immunohistochemistry (IHC) in 72 NSCLC tissues. The relationship between the expression of ERCCl and the clinical pathological parameters, the efficacy of platinum chemotherapy and overall survival of patients were explored by statistical analysis. Results: The high expression of ERCCl protein was 55.5%in 72 cases. There was no signiifcant correlation between the ERCC1 expression with gender, age, pathological type, clinical stage and lymphatic metastasis (P>0.05). Patients with low expression of ERCC1 had signiifcantly higher response rates to platinum chemotherapy, longer median survival time and 2-years survival rate comparing with those with high expression of ERCC1 (62.5%vs 37.5%;22.9 vs 18.4 month;46.9%vs 37.5%), respectively (P<0.05). Conclusion:The expression analysis of ERCC1 using new ERCC1 antibody 4F9 by IHC method is helpful to assign chemotherapeutic regimen, and guide individual platinum chemotherapy for post-operation patients.
10.The use of percutaneous endoscopic gastrostomy and percutaneous endoscopic jejunostomy in enteral nutrition

Objective: To establish the method of percutaneous endoscopic gastrostomy(PEC) and percutaneous endoscopic jejunostomy (PEJ) for enteral nutrition. Methodes: PEG tubes were placed in 114 patients with Pull method. On the foundation of PEG, PEJ tubes were placed in 26 patients by pushing endoscopy to send tubes through Treitz ligment with usingthe the clip. Results: All PEG insertion was performed successfully. PEJ tubes were placed successfully with a new method in 26 patients. 15 patients had a little blooding and 8 patients had slight infection. 21 patients had respiratory tract infection and had been cured by using antibiotic. There was no severe complication. Conclusion: PEG is simple、safe、efficient. The new method of PEJ is feasible.
