1.Advances in the study on FAK-ERK and its applicable value in forensic medicine

Focal adhesion kinase(FAK)and extracellular signal-regulated protein kinase(ERK),which are thought to play major roles in transducing surface receptor-derived signals into nucleus,are pivotal signal transduction molecules in mammalian cells.FAK is activated through phosphorylation by extracellular stimuli.p-FAK activates the downstream of ERK which changes cell action.FAK-ERK signal conduction factors are involved in a variety of biological responses,such as cell proliferation,differentiation,stress,apoptosis and malignant change.The biologic characters of FAK-ERK and advances in the study on its roles in tissue damage are reviewed in this article,which may provide a more effective way for estimation of wound age in forensic medicine.

Oncogene c-jun is a member of jun family,the immediately early genes(IEGs),and belongs to one of the nuclear transcription factors of basic leucine zipper(bZIP)family.Combined with many gene promotors,c-jun is involved in the regulation of gene transcription.Its products play important roles in regulating gene expression,cell proliferation,differentiation and apoptosis.The structure,biological funetion,regulation of c-jun and its roles contributing to tissue damage are reviewed in this article,which may provide understanding for severity of tissue injury and wound age estimation in the field of forensic pathology.
3.Role and value of functional magnetic resonance imaging in brain function

OBJECTIVE: To introduce the research progression of fMRI in brain functional localization in China or abroad in the recent 15 years, so as to further comprehend the effect and value of fMRI.DATA SOURCES: A computer-based online search of Medline database was undertaken to identify English articles about fMRI published from 1990 to 2005 with the keywords "functional magnetic resonance imaging (fMRI), brain".STUDY SELECTION: The articles about experimental studies of fMRI articles irrelative to motor function, sensory function, hearing function, visuEnglish title.DATA EXTRACTION: Totally 6 144 relevant articles were collected, 18 articles were involved according to the inclusive criteria, and the others were excluded. cles about sensory function, including 1 about simple tactile stimulation,ry function, including 2 about simple visual stimulation, and 1 about binoction, including about hearing stimulation by various methods, and 1 about comparison between normal and abnormal subjects by the same stimulation.language function, 1 about visual language function, and 1 about listening basic principle.CONCLUSION: fMRI was more accurate, convenient and direct than other methods. It can not only show localization, size and dimension of the activated areas in different cerebral regions, but also show accurate anatomical position. Many aspects still need to be consummated, for example, exploration of the stimulating method, design of advanced hardware, post processing of the image and the effect of visualization. With the development of hardware, software and the endless endeavor of scientists, fMRI dooms to play an important role in illustrating the function of human brain.
4.Relationship between brucellosis arthritis of the hip and femoral head necrosis

Objective To investigate the relationship between brucellosis arthritis of the hip and femoral head necrosis.Methods In August 2007 to August 2013,16 diagnosed and hospitalized patients with brucellosis complicated with femoral head necrosis in Heilongjiang Land Reclamation Bureau General Hospital were chosen as the objects of observation,and the patients' hospitalization information was analyzed retrospectively.Patients' medical records were investigated and analyzed.Observations included general condition of the patient,clinical symptoms and signs,imaging,pathology,treatment and curative effect.Results All the 16 patients had definite clinical manifestations of hip arthritis and femoral head necrosis including hip pain,limited mobility,positive 4 test,limp or bedridden,and sick limb muscle atrophy.Imaging examination:X-ray computer tomography (CT) showed cystic change and deformation collapse of femoral head.Magnetic resonance imaging(MRI) showed low signal on weighted T1 images,high signal on weighted T2 and soft tissue granulation hyperplasia could be seen in the hip joint.Pathological examination:most femoral bone tissue necrosis,dissolved,bone cells disappeared,a few bone cells present,and some fibrous tissue hyperplasia.Patients were treated with antibiotics according to the treatment principles of brucellosis,and the effect was obvious.Conclusions There is a causal relationship between brucellosis arthritis of the hip and femoral head necrosis.Therefore,positive and effective treatment of brucellosis arthritis of the hip in the early stages is crucial to prevent the occurrence of femoral head necrosis.
5.Clinical efficacy of autologous dendritic cells and cytokine-induced killer cells combined with transcatheter arterial chemoembolization in the treatment of moderate- and advanced hepatocellular carcinoma

Objective To investigate the clinical efficacy of autologous dendritic cells and cytokine-induced killer cells (DC/CIK) combined with transcatheter arterial chemoembolization (TACE) in the treatment of moderate and advanced hepatocellular carcinoma (HCC). Methods Sixty patients with moderste and advanced HCC were randomly divided into two groups: the experimental group (n = 32), in which the treatment of DC/CIK combined with TACE was used, and the control group (n=28), in which TACE treatment was used only. The parameters of tumor size , serum alpha-fetoprotein , survival rate , the median survival time and quality of life , were detected in patients of the two groups before and after corresponding therapy . Results ( 1 ) After receiving corresponding treatments, the efficient rates of DC/CIK combined with TACE and TACE only were 87.50% and 64.29%, respectively, with significant difference;(2) The level of serum AFP decreased in the two groups after corresponding treatment, with no significant difference; (3) The 6-month survival rate was 96.88%and 92.85%, and the 1-year survival rate was 84.38%and 64.29%, the 2-year survival rate was 65.63%and 42.86%in the experimental group and in the control group, respectively. And the median survival time was 21 months and 17 months in the experimental group and the control group, respectively, with no significant difference; (4) The quality of life was improved significantly in the DC/CIK combined with TACE group after treatment. Conclusions Administration of DC and CIK combined with TACE can prolong the survival time, increase the survival rate and especially improve the life quality of HCC patients. It is a promising approach for the treatment of patients with moderate and advanced HCC.
6.Early application of examethasone improves the survival of autogenous island flap and prognosis of patients with burn scar after transplantation

BACKGROUND:Autologous island flap transplantation is the main therapy for burn scars. Floating and necrosis of transplanted flaps mainly impact the therapeutic effects. Dexamethasone has immunoregulatory function during trauma and stress process. OBJECTIVE:To investigate the effect of dexamethasone on immune balance of Th1/Th2 and prognosis in patients with burn scars undergoing autologous island flap transplantation. METHODS:Sixty-eight patients with burn scars undergoing transplantation of pedicled island flap were selected and randomly divided into control group and experimental group. Patients in the two groups were treated with negative pressure drainage under skin flap, infection prevention, improvement of microcirculation. After that, dexamethasone sodium phosphate (0.2 mg/kg) was immediately given in the experimental group, and the dose gradualy decreased after 3 days until drug withdrawal at 7 days. Levels of Th1, Th2, Th1/Th2 and interferon-γ, interleukin-4 were compared between the two groups before and 1 week after transplantation. In addition, incidence rates of skin flap necrosis, hemodynamic disorder, and effusion were compared between the two groups. RESULTS AND CONCLUSION:The levels of Th1, Th2, Th1/Th2, interferon-γ and interleukin-4 had no obvious differences between the two groups before flap transplantation. At 1 week after transplantation, the levels of Th1 and Th1/Th2 were higher, while the levels of Th2, interferon-γ and interleukin-4 were lower in the experimental group than the control group. Moreover, the incidence of complications was also lower in the experimental group than the control group. In conclude, dexamethasone can decrease the incidence of complications after island flap transplantation, which is maybe the result of adjusting immune balance of Th1/Th2.
7.Feasibility of prospective electrocardiographically-gated dual-source CT coronary angiography in patients with high heart rate

Objective To explore the optimal reconstruction windows in patients with heart rate (HR) over 91 (beats per minute) bpm, and to explore the feasibility of prospective ECG-gated DSCT coronary angiography.Methods Two hundred and thirty-two patients[body mass index (BMI):23-25 kg/m2, with stable HR, and average HR ≥91 bpm]with suspected or known coronary artery disease underwent retrospective ECG-gated DSCT coronary angiography.They were divided into 3 groups (A-C) according to the average HR of scanning.Images were reconstructed from 29% to 80% of the R-R interval in 3% increments.Two independent readers assessed the overall image quality by a five-point scale and determined the optimal reconstruction windows of each coronary segment and the ranges.Coronary arteries were segmented according to the guideline of the American Heart Association (AHA).The degree of interobserver agreement was determined by Kappa statistics.Results Three thousand three hundred and fortythree segments were considered to have diagnostic image quality in 232 patients.The ranges of optimal reconstruction windows of images were concentrated on 81%-61% and 51%-31%.In group A(91-95 bpm), there were 1183 segments in 83 patients, and the according proportions were 5.49%, 94.51%,respectively; In group B(96-100 bpm), there were 986 segments in 68 patients, and the according proportions were 0.20%, 99.80%, respectively; In group C(≥ 101 bpm), there were 1174 segments in 81 patients, and the according proportions were 0.17%, 99.83%, respectively.The optimal construction windows in 3274 segments out of 3343 segments in 232 patients were concentrated in 41%, ranged from 51%-31%.The image quality assessment in 3343 segments in 232 patients have a high inter-observe agreement (Kappa=0.883,P <0.05).Conclusion The optimal reconstruction windows of patients with stable HR(≥91 bpm) was concentrated in 41%, ranged from 51%-31%.When nothing except the window of data acquisition is considered, the prospective ECC,-gated DSCT coronary angingraphy can be used in patients with stable HR(≥91 bpm).
8.An Initial Study of Dual-source CT Coronary Artery Angiography

Objective To explore the clinical application of dual-soure CT(DSCT) in coronary artery.Methods 51 patients suspected with coronary artery disease underwent both DSCT coronary arteriography and selective coronary angiography(CAG), the DSCTA results were compared with that of CAG.Results DSCT coronary arteriography in 51 patients at any heart rate was successful.Coronary artery branches and part sub-branches were clearly displayed by DSCT angiography.Stenosis or obstruction of 166 coronary artery branchs in 51 patients were showed ,of them,150 branches were confirmed by CAG. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of DSCTA were 100%, 90%, 90.4%, 100% and 94.8% respectively.Conclusion DSCT is a valuable method in detecting the diseases of coronary artery with lower dose. The accuracy of the DSCT is approximate to the CAG.
9.Application of FLOW-PRA detection in clinical organ transplantation

Objective To apply flow cytometry-panel reactive antibody (FLOW-PRA) and compare the application of traditional (enzyme-linked imrnunosorbent assay) ELISA-PRA in clinical organ transplantarion,so as to evaluate the concordance,sensitivity,accuracy and practicability of FLOW-PRA.Methods PRA was detected in 212 serum samples from 185 patients awaiting organ transplantation using FLOW-PRA and ELISA-PRA.Results It took 1.5 h and 3 h for FLOW-PRA vs ELISA-PRA.Concordance correlation coefficient for the results of the two methods was 94% (class Ⅰ)and 89% (class Ⅱ),respectively.Of all sera,24.5% (in comparison to ELISA-PRA,P<0.005)were class I positive,18.4% (P<0.05) class Ⅱ positive by flow cytometry,and 17.9% and 14.6% by ELISA,respectively.The positive incidence in Flow group was higher than in ELISA group.Low titer of antibodies was detected positively only by flow eytometry,furthermore,the antigen specificity of PRA could only be discriminated by FLOW-PRA.Conclusion Flow cytometry is more sensitive and more accurate than ELISA in PRA detection.FLOW-PRA is easy to operate and time-effective,and suitable for clinical application.
10.Evaluation of the effects caused by different doses of N-nitrosodiethylamine on establishing multistep carcinogenesis model of cirrhotic liver in rats

ObjectiveTo evaluate the effects caused by different doses of N-nitrosodiethylamine (DENA) on establishment of a multistep carcinogenesis model of liver cirrhosis in SD rats.MethodsFifty male SD rats were randomly divided into control group (n =5 ) and experimental group (n =45 ).Experimental group consisted of 3subgroups(high,median and low dosegroup ),which were intragastrically induced DENA once a week for 10 weeks,by using 70,50,35 mg/kg DENA solution,respectively.MRI was performed for inspecting pathological changes of rat livers in the following weeks.The mortality rates of different groups were calculated.The differences of the incidence of dysplastic nodule (DN)found in three groups were analyzed with Pearson Chi-square test.ResultsThe multistep carcinogenesis development from regenerative nodule (RN) to DN to hepatocellular carcinoma (HCC) was showed in the SD rat model.Two rats in median dose group and 4 rats in high dose group died.The incidence of DNs found in 3 different groups was 20.7% ( 6/29 ),30.6% ( 19/62 ),14.3% (9/63),respectively,which showed no significant difference among three groups(x2 =4.901,P > 0.05).However,significant difference existed between median and high dose group( x2 =4.811,P < 0.05 ).Combined with the MRI presentation,the intragastric dose of 50 mg/kg could be more appropriate among three different doses.ConclusionsThe median dose DENA could induce an ideal multistep carcinogenesis model of HCC which is suitable for investigating DN on MRI.
