2.Role of radiosynovectomy in the treatment of rheumatoid arthritis and hemophilic arthropathies

Radiosynovectomy is a novel method of treatment for several acute and chronic inflammatory joint disorders. A
small amount of a beta-emitting radionuclide is injected into the affected joint delivering a radiation dose of 70 to 100
Gy to the synovia. The proliferative tissue is destroyed, secretion of fluid and accumulation of inflammation causing
cellular compounds stops and the joint surfaces become fibrosed, providing long term symptom relief. The radionuclides
are injected in colloidal form so that they remain in the synovium and are not transported by lymphatic vessels causing
radiation exposure to other organs. Complete reduction of knee joint swelling has been seen in above 40% and pain
relief in 88% of patients. Wrist, elbow, shoulder, ankle and hip joints showed significant improvement in 50-60% and
restoration of normal function and long term pain relief has been achieved in about 70% of small finger joints. In
hemophilic arthropathies complete cessation of bleeding in about 60% and improved mobility in 75% of patients has
been reported.
3.Clinical and hematological study of canine Ehrlichiosis with otherhemoprotozoan parasites in Kolkata, West Bengal, India

Objective: To observe other hemoprotozoan diseases with canine ehrlichiosis and to evaluate the clinical and hematological aspects of dogs naturally infected with ehrlichiosis with other hemoprotozoan diseases.Methods:Blood was collected for hematological value and Giemsa stained blood smear was made for diagnosis of Ehrlichia sp. and other hemoprotozoan parasites from naturally infected dogs. Case history was taken from the owner and clinical signs and symptoms were noted.Results:A total of 47 cases of ehrlichiosis in dogs were reported with babesiosis (8.51%) and hepatozoonosis (6.38%) hemoprotozoan diseases. Ehrlichia canis, Ehrlichia ewingii, Brucella canis,Babesia gibsoni and Hepatozoon canis were observed under oil immersion lense of microscope in Giemsa stained peripheral blood smears. Marked anaemia and neutrophilic leukocytosis were observed.Conclusions:The results of this study stated that clinical and haematological changes occurred in canine ehrlichiosis with babesiosis and hepatozoonosis due to parasitemia. In mixed infection, the disease more severe, and also it depended on immunity of animals. Babesia gibsoni andHepatozoon canis with Ehrlichia sp. were first reported from West Bengal state of India by this study.

No screening test is ideal for detecting rheumatic diseases; diagnosis depends on appropriate history and thorough physical examination. Sometimes, laboratory investigations may be useful in confirming or ruling out rheumatic disease after a clinical diagnosis is considered. Once a rheumatic disease has been diagnosed, certain laboratory tests can
help in assessing prognosis or determining the extent of the disease. Laboratory tests may also help the physician
monitor certain rheumatic diseases, guide treatment or assess potential drug toxicity.
5.Verteporfin therapy for subfoveal choroidal neovascularisation associated with choroidal osteoma

· AIM: To report a case of verteporfin therapy for subfoveal choroidal neovascularisation associated with choroidal osteoma.· METHODS: A 32-year-old patient was diagnosed as subfoveal choroidal neovascularisation (CNV) associated with choroidal osteoma in the right eye, with present visual acuity (VA) of 20/80; N24. He underwent 2 sessions of photodynamic therapy (PDT) in the right eye for 12mo.· RESULTS: At the end of 12mo, the patient's VA was 20/200 with complete regression of the CNV.· CONCLUSION: PDT could be a treatment option in subfoveal CNV associated with choroidal osteoma.
6.An Unusual Presentation of Adult Onset Still’s Disease

Adult onset still’s disease usually presents with high grade intermittent fever, polyarthritis, salmon pink evanescent rash and hepatosplenomegaly. Pulmonary involvement in the form of pneumonitis, as a presenting feature is very rare.
We report a case of a young lady who presented with fever,
cough and respiratory distress. Chest X-ray revealed patchy
infiltration in left upper lung zone. She was subsequently
diagnosed as Adult onset Still’s disease. There was no
improvement in clinical condition despite five days of
antibiotics. On trans-bronchial lung biopsy (TBLB) proved
she had interstitial pneumonitis and responded dramatically
to steroid treatment.

Leucodystrophies are a group of white matter diseases caused by an abnormality in the formation or
maintenance of one or more components of the myelin sheath. The devastating course of the disease
coupled with the lack of any defi nitive treatment render them potentially fatal and incurable. Van der
Knaap Syndrome, also known as megaloencephalic leucoencephalopathy with subcortical cyst, is a
relatively new and rare entity. Although reported sporadically from Europe, Asia and the Agarwal
community in India, there is practically no case report from the eastern part of India. We present a
case of van der Knaap syndrome in an adult female with some atypical features.
