1.Effect of high volume hemofiltration combined with mechanical ventilation on seawater respiratory distress syndrome in canine models

ObjectiveTo investigate the effect of high volume hemofiltration (HVHF) combined with mechanical ventilation (MV) on seawater respiratory distress syndrome (SW-RDS) canine models.MethodsTen nomal hybrid dogs were randomly assigned into two groups:MV group(MV group,n=5),all the animals only received MV after establishing model successfully; HVHF combined with MV group (HVHF+MV group,n=5),all were received HVHF plus MV after establishing model successfully.Both groups were observed for 4 hours.Mean arterial pressure (MAP),heart rate (HR),central venous pressure(CVP),arterial blood gas and venous plasma osmotic pressure were detected at baseline,0 min(model establishment),60 min,120 min,180 min,240min after treatment.Venous blood was collected to detect inflammatory mediators (IL-8,IL-6,TNF-α)at baseline,0 min,120 min,240 min after treatment.The lung pathology was examined at the end of the experiment.Results(1)All the animals were suvival after four hous of treatment in both groups. (2)Pattial pressure ofoxygen(PaO2) and O2 saturation(SaO2) rised after four hours of treatment in both groups(P<0.05), and HVHF+MV group was better than MV group.After 4 hours of treatment,pH,actual bicarbonate (AB),bases excess(BE) in HVHF+MV group were significantly better than those in MV group(P<0.05),recovering to the baseline values.(3)MAP,HR,CVP were stable during the four hours of treatment,and compared with 0 min,there was no significant differences after 4 hours of treatment in bothgroups. There were no significant differemces at the same time of treatment in both groups. (4)Plasma osmotic pressure were stable during the four hours of treatment,and compared with 0 min,there was no significant difference in MV group.But in HVHF+MV group,osmotic pressure was significantly higher after 4 hours of treatment than that at the same time in MV group(P<0.05),and compared with 0 min and 180 min,those were higher too(P<0.01). (5)Compared with those at the same time in MV group,plasma inflammatory mediators (IL-8,IL-6,TNF-α) were significantly decreased after 4 hours of treatment in HVHF+MV group(P<0.01).After 4 hours of treatment IL-8,TNF-α in MV group were higher than those at the same time in 0 min (P<0.05).(6)Compared with those in MV group,there were less infiltration of neutrophils,edema and injury of alveolar epithelium from pulmonary pathology.ConclusionsHVHF combined with MV can significantly improve hypoxemia and correct acidosis of SW-RDS model in canines.HVHF can effectively clear plasma inflammatory mediators and redundant water,improve pulmonary pathology changes.HVHF has no impact on MAP,HR and CVP of SW-RDS.
2.Inhibitory Effects of Phosphatidylinositol 3-kinase Inhibitor LY29400 Combined with Indole-3-carbinol on the Proliferation of Human Anaplastic Thyroid Carcinoma FRO Cells

OBJECTIVE:To study the inhibitory effects of phosphatidylinositol 3-kinase (PI3K) inhibitor LY29400 (“LY”) combined with indole-3-carbinol (I3C) on the proliferation of human anaplastic thyroid carcinoma FRO cells. METHODS:RFO cells in logarithmic growth phase were divided into blank control group,I3C (250 μmol/L) group,LY (10 μmol/L) group and combination (I3C of 250 μmol/L + LY of 10 μmol/L) group,which were subject to 24,48 and 72 h drug action. MTT method was used to determine and calculate the inhibitory rates of the cells in all groups,flow cytometry assay to determine the apoptosis rates after 48 h action,Western blot method to determine the expressions of Capspase-3 proteins thereafter and immunohistochemi-cal method to determine the expressions of Bcl-2 and Bax thereafter. RESULTS:The inhibitory rate obviously increased with the ex-tension of drug action time,with statistical significance at each time point in combination group (P<0.05). Compared to I3C group and LY group,combination group had higher inhibitory rate,apoptosis rate and the expressions of Capspase-3 protein and Bax,and lower Bcl-2 expression and ratio of Bcl-2 to Bax,with statistical significance (P<0.01). CONCLUSIONS:LY com-bined with I3C has synergistic inhibitory effects on the proliferation of FRO cells by a mechanism which may be related to apopto-sis induced by down-regulating Bcl-2 expression,up-regulating Bax expression and activating Caspase cascade reaction.

4. Anticoagulant Treatment After Transjugular Intrahepatic Portasystemic Shunt: A Systematic Review and Meta-analysis

Background: Transjugular intrahepatic portasystemic shunt (TIPS) is widely used for reducing portal hypertension. Post-TIPS anticoagulant treatment is controversial because of lack of obligatory evidence. Aims: To systematically review the effect of anticoagulant treatment on patients with liver cirrhosis after TIPS. Methods: Randomized controlled trials (RCTs) of liver cirrhosis patients after TIPS with anticoagulant treatment (anticoagulant treatment group) or without anticoagulant treatment/placebo (control group) were retrieved from PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang, CBM and VIP databases in March 2020. Meta-analysis was conducted by RevMan 5.3. Results: Three RCTs involving 157 liver cirrhosis patients were enrolled. These studies mainly reported the effects of anticoagulant treatment on gastrointestinal rebleeding, stent patency, mortality and incidence of hepatic encephalopathy (HE). Meta-analysis revealed that no significant differences in total gastrointestinal bleeding rate (OR=1.04, 95% CI: 0.25-4.38, P=0.96), variceal bleeding rate (OR=1.04, 95% CI: 0.14-7.68, P=0.97), stent stenosis (OR=1.88, 95% CI: 0.73-4.79, P=0.19), occlusion (OR=0.07, 95% CI: 0.00-1.44, P=0.09), shunt dysfunction (OR=0.67, 95% CI: 0.10-4.29, P=0.67), mortality (OR=2.12, 95% CI: 0.06-72.77, P=0.68) and incidence of HE (OR=1.18, 95% CI: 0.45-3.06, P=0.74) were found between anticoagulant treatment group and control group. Conclusions: Post-TIPS anticoagulant treatment is safe and without increasing the rate of gastrointestinal rebleeding, mortality and incidence of HE. However, anticoagulant treatment does not further improve the stent patency. Therefore, anticoagulant treatment appears to be unnecessary in patients with liver cirrhosis after TIPS.
5.Flushing-free film test of 192Ir accuracy of position and step distance for afterloading systems

Objective To study the method of measuring the position accuracy and the step distance accuracy of afterloading system with192Ir source by using flushing-free film.Methods The position accuracy and the step distance accuracy of a China-made afterloading system with192Ir source was measured by using GAFCHROMIC (R) EBT3 flushing-free film.The film was scanned to proper image format,required by dose analysis software,by EPSON PREFACTION V700 PHOTO scanner.Then images are analyzed by using film dose analysis software in SNC Patient 5.2.Results With focus on the center of active section of source,the position accuracy of this afterloading system with192Ir source was-0.75 mm.Using film analysis could make the step point to tell apart if the step distance was 5 mm away by the method of film analysis,but couldnot make it to tell apart if the step distance was 2.5 mm away.The 2.5 mm step distance accuracy could be judged if the distance between the 1 st point and the 3rd point was 5 mm,then the 2.5 mm step distance could be deemed to no deviation.The 5 mm step distance of this afterloading system had no deviation in continuous 9 step points measured by flushing-free film.The indirect measuring results of the 2.5 mm step distance had no deviation as well.The position accuracy of this afterloading system measured with the flushing-free film accorded with the national standards.Conclusions The method of measuring the position accuracy and the step distance accuracy of the afterloading system with192Ir source by using flushing-free film is technically feasible.
6.Effects of different irradiators on the establishment of osteoradionecrosis model of rat mandible.

OBJECTIVES:
To compare the effects of different irradiators on the establishment of osteoradionecrosis of jaw model (ORNJ) to explore an ideal modeling method.
METHODS:
A total of 33 adult SD rats were included and randomly divided into three groups according to the radiation equipment, namely, the blank control (CN, 3 rats), group A (linear accelerator irradiation, 15 rats), and group B (small-animal irradiator irradiation, 15 rats). At 3 weeks after dental extractions, complete gingival healing was found in the regions of dental extractions in groups A
CONCLUSIONS

7.Current status of the pathogenesis, diagnosis, and treatment of sarcopenia in patients with liver cirrhosis

Sarcopenia is a common complication in patients with liver cirrhosis, with an incidence rate of up to 40%-70%, which increases the incidence rate of complications in patients with liver cirrhosis and seriously affects patients' quality of life and survival time. Sarcopenia in liver cirrhosis has a complex pathogenesis, and its clinical manifestations are easily ignored. No consensus has been reached on diagnostic criteria, and there are few targeted therapeutic drugs. Therefore, it should be taken seriously by clinician and the research on pathogenesis and therapeutic drugs should be strengthened.
9. The clinical thinking of diagnosis and treatment of cryptogenic cirrhosis

Cryptogenic cirrhosis (CC), cirrhosis of unknown cause, is common in clinic. With the improvement of the level of diagnosis and the diversification of the diagnosis and treatment, the incidence of CC is gradually declining. In the process of diagnosis and treatment, perfect clinical thinking can help clinicians to analyze and evaluate the existing clinical data, and it is expected to further improve the diagnostic rate of CC. In this paper, the clinical thinking of CC diagnosis and treatment is expounded from the aspects of disease spectrum thinking, anatomy thinking and macroscopic micro thinking.
10.Evidence-based pre -prescribing review to standardize off -label drug use for sleep disorders

OBJECTIVE To provide reference for standardizing off -label drug use for sleep disorders in hospitals . METHODS The off -label drug use for sleep disorders in our hospital was analyzed statistically ,and then was evaluated with evidence -based medical evidence . The records that met the filing standards for off -label drug use should be filed ,and the drug review rules should be modified in the pre -prescribing review system . The blocking rules in the pre -prescribing review system were set up for unreasonable off -label drug use . RESULTS Off-label drug use for sleep disorders in our hospital accounted for 36.92% of total number of prescriptions ,including drug use of off -indication,over-frequency,over-dose. After the pharmacists modified the drug review rules ,the rational off -label drug use system would be automatically reviewed and approved ,and the unreasonable off -label drug use system would be intercepted . CONCLUSIONS There is a certain rationality in off -label drug use for sleep disorders . The pharmacists can further standardize off -label drug use for sleep disorders by continuously improving the standardized management of off-label drug use and revising the drug review rules of pre -prescribing review system ,so as to ensure the safe ,reasonable and effective use of drugs related to sleep disorders .
