1.Analysis of the situation of pre-pregnancy health check in rural areas of Sichuan province

Objective To investigate the situation of pre-pregnancy health check in rural areas of Sichuan province and get the key difficult points during the process of carrying out the work.Methods According to the economic situation in Sichuan province and geographic distribution,random eight areas in Sichuan province (Chengdu,Neijiang,Nanchong,Ziyang,Meishan,Zigong,Les-han and Suining),investigate pregnant women in maternal and child health care institutions and family planning service institutions at all all levels in the areas above.Results Only 334 people (33.26%)underwent the pre-pregnancy health check during the 974 pregnant women in rural areas.The rate of planned pregnancy is low;the rate of unintended pregnancy is close to 40.00%.55.29%of the planned pregnancy women didn′t undergo the pre-pregnancy health check.And the factors that affect their behavior of under-going pre-pregnancy health check are whether planned pregnancy and personal attitude towards pre-pregnancy health check.Conclu-sion The rate of pre-pregnancy health check is low in the pregnant women from rural areas of Sichuan province,the health sector needs to make efforts to promote and enhance the awareness of the rural pregnant women to prevent birth defects and the initiative in participation.
2.Replantation of amputated toe:report of 60 cases

Objective To report the clinical effect,characteristic and functional recovery of replantation of amputated pediphalanx. Methods Sixty cases (70 toes) of amputated toes had been taken the replantation which methods were anterograde and changing postural. Results Sixty-eight were successful in all the 70 amputated toes.The survival rate was 97%.The shape of replanted toes was good.About the range of flexion,the great toes were 25?~60? and the second and third toes were 36?~85?.The two-point discrimination on the toe tip was 1.1~1.8 mm.In the gait 60 cases had been normal and 10 cases had closed to normal. Conclusion The replantation of amputated toe must be performed with taking great pains.The successful operation can reduce the degree of disability of patient and remain the function and shape of the foot.To raise the rate of success of the operation,the skilled technology of microsurgery was the most important.
3.Clinical features of ischemic stroke in patients with nasopharyngeal carcinoma.

OBJECTIVETo investigate the clinical features of ischemic stroke in patients with nasopharyngeal carcinoma.
METHODSThe clinical data of the nasopharyngeal carcinoma patients with ischemic stroke treated at the First Affiliated Hospital of Guangxi Medical University between January 2006 and December 2013 were collected.
RESULTSAmong 3 822 patients with nasopharyngeal carcinoma 10 patients suffered from acute ischemic stroke. The 10 patients were males and their age ranged from 39 to 70 years old, with an average age of 51 years. All of the patients were found with squamous cell carcinoma, with metastasis in 7 of them. Among the 10 patients, only 3 patients had with traditional risk factors for ischemic stroke, but no traditional risk factor was found in other 7 patients; 2 patients showed single lesion in brain and 8 patients with two or more lesions due to the blockage of multiple cerebral arteries; 7 patients showed an elevated plasma D-dimer level.
CONCLUSIONIt is suggested that the acute ischemic stroke occurring in the patients with nasopharyngeal carcinoma commonly lacks traditional risk factors, with elevated plasma D-dimer levels and multiple lesions due to involvement of several cerebral arteries.

4. Clinical features and prognosis of 17 patients with primary cold agglutinin disease

Objective:
To explore the clinical characteristics, treatment and prognosis in 17 patients with primary cold agglutinin disease (CAD) .
Methods:
Clinical data, treatment and survival status of 17 patients diagnosed with primary cold agglutinin disease in Peking Union Medical College Hospital during April 2007 to October 2016 were retrospectively analyzed. The MYD88L265P mutation was tested in 4 patients.
Results:
The median age of 17 patients was 67 years (range, 51-86 years) , and male- to female ratio was 1.1∶1. Seven patients were diagnosed with indolent lymphoma, including 3 Waldenstrom macroglobulinemia/lymphoplasmacytic lymphoma (WM/LPL) , 2 small lymphocytic lymphoma/chronic lymphocytic leukemia (SLL/CLL) , and 2 splenic marginal zone lymphoma (SMZL) . 15 patients experienced anemia. The median HGB level was 67 (35-127) g/L. 11 patients had cold agglutinin (CA) titers ≥1∶64, with median CA of 1∶1 024. MYD88L265P mutation was detected in 1 patient. 12 patients received drug therapy: 7 were treated with glucocorticoid-based therapy and 1 patient responded to treatment; 5 received rituximab-based therapy and 3 patients responded to treatment. With a median follow-up of 14 (0.5-96) months, the median overall survival was not reached.
Conclusion
Clinical manifestations of CAD are various, and diagnosis is dependent on CA testing. The efficacy of glucocorticoid-based therapy is limited, and rituximab is recommended for CAD treatment.
5.Clinical diagnosis and treatment of autoimmune pancreatitis

Objective:To investigate the clinical manifestations ,diagnosis and treatment of autoimmune pancreatitis (AIP).Methods:The clinical data of 17 AIP patients admitted at the First Affiliated Hospital of Soochow University from Apr 2021 to Jan 2023 were retrospectively analyzed.Results:Among the 17 AIP patients, 12 were male (70.6%). Ten complained abdominal pain (58.8%), and 8 had jaundice (47.1%). Fifteen patients had elevated IgG4 levels more than twice the normal value (88.2%), and 14 patients had elevated liver enzymes (82.3%). The imaging manifestations of 17 patients were all diffuse or limited enlargement of the pancreas. All patients had diffuse or limited enlargement of the pancreas, some were accompanied by a dilatation of the pancreatic duct or bile duct. Fourteen patients were diagnosed by combining the clinical manifestations, imaging examination, laboratory examination, and puncture pathology, and in 3 cases the diagnosis was confirmed by postoperative pathology. Fourteen patients showed significant improvement in clinical symptoms and imaging manifestations after hormone therapy, 2 patients stopped hormone medication on their own after the improvement of the imaging, and 1 suffered recurrence,which was responsible to hormone readministration.Conclusions:AIP, as a rare and easily misdiagnosed immune disease, involves the pancreas leading to an inflammatory response and often encroaches peri-pancreatic areas such as the bile ducts, causing biliary stenosis and jaundice. Most patients respond well to glucocorticoid therapy and surgery was usually not indicated in those with definite AIP diagnosis.

Objective:
To evaluate the feasibility and potential value of comprehensive geriatric assessment (CGA) in elderly (≥60 years) patients with newly diagnosed acute myeloid leukemia (AML) in China.
Methods:
The CGA results of 83 newly diagnosed AML (non-APL) patients from 16 hospitals in Beijing and Tianjin between March 2016 and December 2017 were prospectively collected and analyzed. The clinical data, treatment and follow-up information were also collected.
Results:
Of 83 newly diagnosed elderly AML patients, 81 patients (97.6%) completed all designated CGA assessment. The median number of impaired scales of the CGA assessment in the studied population was 2(0-6). Sixteen patients (19.3%) showed no impairments according to the geriatric assessment scales implem ented by this study. The distributions of impaired scales were as follows: impairment in ADL, 55.4%; IADL impairment, 42.2%; MNA-SF impairment, 48.2%; cognitive impairment, 15.7%; GDS impairment, 31.7%; HCT-CI impairment, 19.5%, respectively.
