1.Comparison of the clinical efficacy of azithromycin in the different course treatment of children with myco-plasma pneumonia

Objective To explore the clinical efficacy of azithromycin in the different course treatment of children with mycoplasma pneumonia.Methods 1 20 children with mycoplasma pneumonia were divided into two groups according to the treatment plan.Adding different treatment course azithromycin treatment based on conventional treatment,treatment group 1 (42cases)for a single course of 3 days,the treatment group 2 (78cases)single course more than 3d,treatment group 2 including group 2 -A (22cases)and group 2 -B (41 cases)and group 2 -C (1 5cases),the single course of treatment,respectively,5d,7d,8d.Patients'body temperature returned to normal time, and the disappearing time of cough,lung rale and the time in the hospital were observed.Results The fever and lung rale disappearing time of the treatment 2 group were lower than those of treatment 1 group,while there were no signifi-cant differences[(2.99 ±2.84)d vs.(3.24 ±3.03)d,(6.57 ±4.33)d vs.(6.84 ±4.67)d,t =0.449 3,0.31 6 9, all P >0.05 ].The hospitalization time and cough time of treatment 2 group were lower than those of treatment 1 group,the differences were statistically significant[(8.32 ±4.1 6)d vs.(1 0.21 ±5.1 6)d,(9.52 ±3.67)d vs. (1 3.33 ±6.77)d,t =2.1 78 7,4.004 4,all P <0.05].The fever disappearing time of the A,B and C groups had no statistical differences(all P >0.05),while the cough time,lung rale disappearing time and hospitalization time of B group were obviously lower than A and C group,the differences were statistically significant(all P <0.05),there were no statistically significant differences between treatment A group and treatment C group(all P >0.05).Conclusion Azithromycin in the different course treatment of children with mycoplasma pneumonia has no obvious difference for fever disappearing time.However,for the clinical symptoms such as cough and lung rale selecting 7 days as a course of treatment is the best.
2.Clinical study of high-frequency oscillatory ventilation in the treatment of pulmonary hemorrhage in newborn infants

Objective To investigate the effects of high frequency oscillatory ventilation in the treatment of neonatal pulmonary hemorrhage curative.Methods 56 neonates with pulmonary hemorrhage were selected,28 cases in high frequency mechanical ventilation(HFOV) group and 28 cases in conventional mechanical ventilation(CMV)group.On the basis of conventional treatment,the HFOV group received high-frequency mechanical ventilation therapy,the CMV group treated with conventional ventilation.The breathing machine parameters of the two groups were observed and recorded,calculated oxygenation index (OI) and arterial/alveolar oxygen partial pressure ratio (a/APaO2) The duration of pulmonary hemorrhage,the duration of mechanical ventilation,the length of hospital stay,and the duration of oxygen therapy were compared between the two groups.Results Before treatment,there were no statistically differences of in OI and a/APaO2 between the two groups (t1 =0.28,P1 =0.78;t2 =0.47,P2 =0.64),1h treatment later,declining value of the OI and the rising of a/APaO2 in surviving children,and the therapeutic effect of the HFOV group were more obvious than those of the CMV group (t1'=2.14,P1'=0.04;t2'=2.26,P2'=0.03).In the HFOV group,23 cases cured(82.1%),5 cases died(17.9%).And in CMV group,15 cases cured(53.6%),13 cases died (46.4%).The duration of pulmonary hemorrhage,the duration of mechanical ventilation,the length of the hospital stay,and the duration of oxygen therapy of the HFOV group were significantly lower than those of CMV group,the differences were statistically significant (P =0.046,0.044,0.048,0.035).Conclusion HFOV can better improve oxygenation function in infants with pulmonary hemorrhage,shorten the course of the disease and improve the cure rate,recommend the application and clinical treatment.

BACKGROUND:Implant related problems such as loosening,dislocation and infection often come along with joint replacement.Nanotechnology provides a new insight into the preparation of joint replacement implants.OBJECTIVE:To elaborate the research progress of nanotechnology in improving joint replacement implants.METHODS:The first author used the computer to retrieve PubMed databases using the key words of joint replacement,artificial implant,nanotechnology,nano-materials,nanoscales,biocompatibility in English,to find relevant literature on nanotechnology and joint replacement implants.All data were primarily screened to exclude repeated and irrelevant articles,and finally 51 articles related to the study were retained.RESULTS AND CONCLUSION:Through reviewing these 51 articles,we make a detailed introduction about some of the biological responses that occur with nano-materials,and some of the biocompatibility problems that have been raised in relation to materials,as well as the ways that have been employed to improve biocompatibility of nano structured materials.But the long-term effect of nanotechnology on the human body is still worthy of further research in medicine or related fields.
5.The analgesic effect of dezocine on patients after laparoscopic cholecystectomy

Objective To investigate the efficacy and safety of different doses of dezocine at different administration time on analgesia after laparoscopic cholecystectomy.Methods 80 patients elected from ASA Ⅰ-Ⅱ grade laparoscopic cholecystectomy were randomly divided into four groups,20 cases in each group.Group Ⅰ was anesthetized by intravenously injected dezocine 0.10mg/kg before the surgery,group Ⅱ was anesthetized by dezocine 0.15mg/kg before the surgery,group Ⅲ was anesthetized by intravenously injected dezocine 0.10mg/kg after stopping anesthetic surgery,and group Ⅳ was anesthetized by intravenously injected dezocine 0.15mg/kg after stopping anesthetic surgery.The postoperative pain scores were observed 1,6,12,24 hours after operation in the four groups by using visual analog scale (VAS),comfort score (BCS),and anesthesia recovery score (modified Aldrete score).Results TheVAS of the four groups 1 hour after surgery:group Ⅰ and group Ⅲ was significantly different(t =2.308,P =0.036),group Ⅰ and group Ⅳ was significantly different (t =2.106,P =0.042),group Ⅱ and group Ⅲ was significantly different (t =2.711,P =0.014),group Ⅱ and group Ⅳ was significantly different (t =2.317,P =0.037).The BCS 1 hour after surgery:group Ⅰ and group Ⅲ was significantly different(t =2.108,P =0.042),group Ⅰ and group Ⅳ was significantly different(t =2.069,P =0.048),group Ⅱ and group Ⅲ was significantly different (t =2.353,P =0.033),group Ⅱ and group Ⅳ was significantly different (t =2.361,P =0.036).The VAS 6 hours after surgery:group Ⅰ and group Ⅲ was significantly different (t =2.084,P =0.045),group Ⅱ and group Ⅲ was significantly different(t =2.309,P =0.038),group Ⅱ and group Ⅳ was significantly different(t =2.303,P =0.040).The BCS 6 hours after surgery:group Ⅰ and group Ⅲ was significantly different (t =2.294,P =0.041),group Ⅱ and group Ⅲ was significantly different(t =2.322,P =0.035),group Ⅱ and group Ⅳ was significantly different (t =2.070,P =0.048).The BCS 12 hours after surgery:group Ⅱ and group Ⅲ was significantly different(t =2.518,P =0.047).VAS and BCS scores at other time points had no significant difference (P > 0.05).Conclusion The analgesic after laparoscopic gallbladder surgery using dezocine 0.10mg/kg-0.15mg/kg,especially 0.15mg/kg administered anesthesia before surgery,can effectively relieve postoperative pain and improve postoperative comfort,reduce postoperative analgesic(pain pump) and has less adverse reactions,which is worthy of promotion.
6.Construction and application of clinical microbiology laboratory data management expert system

Objective To introduce the construction and application of clinical microbiology laboratory data management expert system.Methods Firstly, the process management was introduced to clinical microbiology laboratory. Then the characteristics of data on each node of work process were analyzed, and SQL Server data table was created as the knowledge base of the expert system.Finally, VB6.0 was used to compile the knowledge acquisition module, reasoning desktop module and input/output interface procedures to finally construct the expert system.Rates of defect report, errors in bacterial identification and drug sensitivity test, delay in culture results reporting and average delayed days were compared before and after the application of the expert system.Results The expert system could be used for data management in process nodes like sample reception, information collection and input, bacteria culture medium selection, bacterial identification and drug sensitive test, interpretation of drug sensitivity results, comprehensive evaluation in bacterial identification and drug sensitivity results, report of negative result, report of positive result, blood culture, Mycoplasma culture, time limit of detection, and nosocomial infection indicators.No defect report was found after the application of expert system; rate of errors in selection of drug sensitivity test medium was reduced from 0.81% ( 31/3 836 ) in 2012 to 0.02%(1/5 433) in 2014;rate of delay in culture results reporting was reduced from 1.78% (320/17 983) to 1.18%(232/19 692), and the average delayed days was also reduced (3.8 d vs.3.2 d).Conclusion Clinical microbiology laboratory data management expert system can improve work efficiency and reduce errors, which can enhance the overall management of laboratory and the quality of clinical service.
7.An investigation of the happiness and influencing factors of the elderly in nursing homes

Objective To understand the present situation of happiness of the elderly living in nursing homes and discuss the related factors.Methods Using the method of questionnaire,the happiness of the elderly over sixty years old and its influencing factors in eleven nursing homes in Changchun were discussed.Results Among many influencing factors,health status,the reasons for living,marital status and self-care status affected the happiness of the elderly in nursing homes.Above all,the health status had the most significant effect on happiness.Conclusions Improvement of the health level of the elderly in nursing homes can improve the happiness of them.
8.Study on Cellular Immune Responses of DNA Vaccine, rAd5 and rMVA Expressing SIV Gag/Env Gene Combined Immunization in Mice.

Therapeutic HIV vaccine was considered as a hopeful curative method for AIDS patients. However, there is still no suitable HIV animal model for vaccine study since the difference in the immune system between human and animals. To evaluate the therapeutic effect of combined immunization strategy with multiple vector vaccines in macaque models. Plasmid DNA, recombinant Ad5 and MVA vaccines which expressing SIV gag and env genes were constructed. Sequential and repeated immune strategy were applied to immunize mice with these three vaccines. Cellular immune responses in mice immunized with these three vaccines were measured by ELISPOT test in vitro and CTL assay in vivo. The results were analyzed and compared with different antigen combination, order of vaccines and intervals to choose a suitable immunization strategy for macaque immunization in future. It indicated that strong SIV-Gag/Env-specific cellular immune responses were induced by these three vector vaccines. It laid a foundation for evaluating the therapeutic effect of combined immunization strategy with multiple vector vaccines in SIV infected macaque models.

9.Clinical application of Cystatin C-based eGFR equations in the estimation of glomerular filtration rate in elderly type 2 diabetic patients

Objective To explore the most suitable equation in accessing renal function for the elderly type 2 diabetic patients, and its clinical utility in combination with hypersensitive C-reactive protein (hsCRP). Methods The new Cystatin C-based equations for estimated glomerular filtration rate (Cys-eGFR) and conventional predictive equations were compared with isotopic GFR (iGFR) by linear regression analysis, paired t-test, Bland and Altman procedures and non-parametric receiver operating characteristic (ROC) curves. The new Cys-eGFR equation and hsCRP were also incorporated for detecting renal disease in this population. Results The new Cys-eGFR equation had a better relativity with iGFR (r= 0.767, P<0.001), a less bias (bias: 0.0007, P>0.05), a higher conformance (2SD: 21.56), higher sensitivity (90.7%) and specificity (88.6%) for diagnosing moderate decrease in renal function. There was a negative relationship between the new Cys-eGFR and hsCRP (r=-0.655, P<0.01). When the new Cys-eGFR was 67.06 ml· min-1 ·1.73 m-2 and hsCRP was 5.65 mg/L, the combination of Cys-eGFR and hsCRP was better than the combination of serum creatinine and urine albumin/creatinine ratio in screening stage 3 chronic kidney disease (95%vs.46%). Conclusions The combination of new Cys-eGFR equation and hsCRP may screen an early decrease of moderate GFR.
10.Clinical analysis of 42 elderly patients with pulmonary cryptococcosis

Objective:To analyze the clinical features, imaging characteristics, diagnosis and treatment of elderly patients with pulmonary cryptococcosis, and to propose considerations for the early diagnosis and treatment of pulmonary cryptococcosis.Methods:Clinical data of 42 elderly patients with pulmonary cryptococcosis diagnosed at the First Affiliated Hospital of Zhejiang University School of Medicine from January 2015 to September 2020 were retrospectively analyzed.Differences in clinical data were compared between immunocompromised patients and immunocompetent patients by using the SPSS software.Results:The mean age of patients of with pulmonary cryptococcosis was(66.0±5.3)years.Of the cases, 27 were immunocompromised, with common underlying diseases such as autoimmune rheumatic disorders, tumors and chronic kidney disease.Main clinical manifestations were cough, sputum expectoration, fever and chest tightness.Twenty-two cases showed nodules on chest CT scans, most of which contained multiple nodules.Twenty-four cases showed unilateral lesions, mainly in the lower lobe.There was no significant difference in clinical manifestations, lesion types or location distribution between the immunocompromised and immunocompetent groups( P>0.05), while the incidence of air bronchograms was higher in the immunocompetent group than in the immunocompromised group( P<0.05). Seventeen cases were diagnosed by histopathology, and the clinical diagnosis of 25 cases was confirmed by the positive detection of capsular polysaccharide antigens.Nine cases were treated surgically, 1 case underwent clinical observation postoperatively, and 8 cases were treated with fluconazole after surgery.Thirty-three cases were treated with antifungal therapy, 6 of whom were treated with a combination of drugs.Eight cases were lost to follow-up, 29 were cured or improved, 2 progressed, and 3 died. Conclusions:Pulmonary cryptococcosis in the elderly is more common in immunocompromised patients, with atypical clinical symptoms and a high rate of misdiagnosis.Cryptococcal capsular polysaccharide antigen testing is helpful for early diagnosis, and treatment plans need to be chosen and adjusted according to the patient's immune status and specific conditions.
