1.Study on clinical effect of viable bifidobacterium combined with montmorillonite powder in the treatment of infantile diarrhea

Objective To study the clinical efficacy of viable bifidobacterium combined with montmorillonite powder in the treatment of infantile diarrhea.Methods 100 patients with infantile diarrhea disease were randomly divided into control group and observation group,50 cases in each group.The control group was given montmorillonitc powder treatment,while the observation group was added viable bifidobacterium treatment based on control group.Effi-cacy,serum C reactive protein (CRP)level,clinical symptom improvement time and adverse reactions rate were com-pared between two groups.Results The total effective rates of clinical treatment in the control group and observation group were 78%(39 /50)and 96%(48 /50)(P1 =0.026).After treatment,the treatment time,diarrhea stopping time,body temperature returned to normal,frequency of defecate recovery time,shape of defecate recovery time and scrum CRP levels in the control group were (8.73 ±1.92)d,(2.32 ±0.85)d,(27.53 ±7.24)h,(2.52 ±0.85)d, (3.62 ±1.23)d and (13.63 ±2.15)mg/L,which in observation group were (6.15 ±1.26)d,(1.53 ±0.62)d, (19.65 ±5.71)h,(1.75 ±0.76)d,(2.56 ±0.85)d and (7.52 ±1.26)mg/L.The observation group were better than the control group (P2 =0.026,P3 =0.018,P4 =0.021,P5 =0.012,P6 =0.031,P10 =0.002).The incidence of adverse reactions between two groups had no obvious difference.Conclusion Viable bifidobacterium combined with montmorillonite powder can effectively treat infantile diarrhea,with advantages of significant clinical efficacy,quickly recovery of clinical symptoms,rapidly reduce of CRP level,and low adverse reactions,it is worthy of promotion.
2.The clinical application value of flow-cytometry combined with colposcope ultra high frequency electric knife for diagnosis of uterine cervix carcinoma and cervical intraepithelial neoplasia

Objective To explore the clinical application value of flow -cytometry combined with colpo-scope ultra high frequency electric knife for diagnosis of uterine cervix carcinoma ( UCC) and cervical intraepitheli-al neoplasia(CIN).Methods A total of 705 cases was included in this study who were screened from April 2014 to April 2016 and accepted colposcopy biopsy .DNA flow cytometry was performed to determine DNA index ( DI) and proliferative index ( PI) .Results Colposcopy diagnosis results showed that the detection rate of CINⅠ,CINⅡ,CINⅢ/CIS and UCC were 62.2%,78.0%,62.2% and 78.0% respectively.The sensitivity and specificity were 87.7%(213/243)and 90.2%(417/462)for DNA ploidy test using flow-cytometry.DI and PI of CINⅠ, CINⅡ,CINⅢ/CIS and UCC increased gradually with significant differences (P<0.05).Conclusion Flow-cytometry combined with colposcopy biopsy may be a useful tool for cervical cancer screening in developing coun -tries and has a competitive sensitivity and specificity ,which is worthy of popularization and application .
3.The Expression and Significance of IGF-1 and survivin in Reactive and Neoplastic Astrocytes

Objective To study the difference of IGF-1,Bcl-2,survivin and Ki-67 protein expression in reactive and neoplastic astrocytes and the significance of them.Methods Immunohistochemistry and tissue microarray techniques were used to determinate the expression of IGF-1,Bcl-2,survivin and Ki-67 protein in normal brain tissues, astrocytes proliferation,low-grade astrocytomas, and high-grade astrocytomas.Results The expression of IGF-1,Bcl-2,survivin and Ki-67 protein were all negative in control group.The positive expression rates of them in reactive astrocytes were 28.9%, 26.7%, 26.7% and 22.2%,respectvely; and in low-grade astrocytomas were 63.8%, 50.0%, 53.2%,70.2%; in high-grade astrocytomas were 88.9%, 79.2%, 88.1%,95.2%.IGF-1,Bcl-2,survivin and Ki-67 all had significant difference among the three experimental groups(P
4.Study of the types of matrix metalloproteinases involved in dentin bonding interface degradation.

OBJECTIVETo study the types of matrix metalloproteinases (MMPs) involved in dentin bonding interface degradation.
METHODSDentin slices were prepared and treated with two adhesive systems (Single Bond 2 or Clearfil S3 Bond). The dentin surface was bonded with composite resin. All specimens were immersed in sterile artificial saliva for 0 or 6 months, and their micro-shear bond strength (muSBS) were measured. The fracture modes were observed through field emission scanning electron microscope (FE-SEM). Dentin slices with 4 mm x 3 mm x 1 mm dimensions were prepared. The slices were divided into three groups according to the treatment modes (negative control, Single Bond 2, and Clearfil S3 Bond). All specimens were stored in sterile artificial saliva for 0 or 6 months. The concentrations of MMP-1, -2, -3, -8, and -9 of each group were detected through fluorescent microsphere immunoassay.
RESULTSThe muSBS of both adhesive systems significantly decreased after storage aging. Significant differences in failure modes within the four groups tested in this study were observed. Compared with the negative control, the concentrations of MMP-1 and MMP-3 in different adhesive groups showed no significant difference after storage aging. However, the concentrations of MMP-2, -8, and -9 in Single Bond 2 group and the concentrations of MMP-8 and -9 in Clearfil S3 Bond group significantly decreased after 6 months of storage aging.
CONCLUSIONSignificant degradation occur in the dentin bonding interface of both adhesive groups under 6 months aging challenge. The concentrations ofdentinal MMP-2, -8, and -9 significantly decrease after treatment with adhesives and aging, indicating that these MMPs have an important function in dentin bonding interface degradation.

5.Effect of Zhenganxifeng Decoction and Buyanghuanwu Decoction on Early Rehabilitation of Stroke

Objective To observe the effect of Zhenganxifeng decoction and Buyanghuanwu decoction on the early rehabilitation of the stroke.Methods 101 cases with acute stroke were randomly divided into the treatment group(n=51) and control group(n=50).All patients were treated with routine medical therapy,physiotherapy and electric stimulation for a month.The treatment group was treated with Zhenganxifeng decoction while the control group with Buyanghuanwu decoction in addition.Results The motor function and Activities of Daily Living(ADL) in both groups were improved after treatment,but were better in treatment group than in control group(P<0.01).Conclusion It is better to use Zhenganxifeng decoction in the early rehabilitation of the stroke than Buyanghuanwu decoction.
6.Anti-oxidative damage effect in Schisandrin B in mice of Alzheimer's disease and its mechanism

Objective:To investigate the preventive and protective effects of Schisandrin B in the mice of Alzheimer's disease (AD),and to clarify its mechanism.Methods:Fifty Balb/c mice were randomly divided into blank group,model group,pasitive control group,low dose of Schisandrin B group(0.1 g·kg-1)and high dose of Schisandrin B group(0.5 g·kg-1);there were 10 mice in each group.Step-through test was conducted after last administration to detect the latencies and number of errors of the mice in various groups,and the brain tissue was taken.Congo red staining was to detect the morphology changes of cells and neuronal amyloidosis in brain tissue of the mice.The levels of ROS in brain tissue of the mice were tested by Flow Cytometry.The contents of MDA,the levels of LDH,and the activities of CAT,GSH-Px and SOD in brain tissue of the mice were tested by biochemical method.Western blotting method was used to detect the expression levels of signaling pathway proteins Nrf2 and Keap1 in brain tissue of the mice.Results:Compared with model group,the latencies of the mice in low and high dose of Schisandrin B groups were increased (P<0.01) and the number of errors in step-through tests was decreased (P<0.05 or P<0.01).The Congo red staining results showed that compared with model group,the neuronal amyloidosis in brain tissue of the mice in Schisandrin B groups was decreased significantly.Compared with model group,the levels of ROS,LDH and the contents of MDA in brain tissue of the mice in low and high doses of Schisandrin B groups were decreased (P<0.05 or P<0.01),and the activities of CAT,SOD and GSH-Px were increased (P<0.01).Compared with low dose of Schisandrin B group,the content of MDA and the activities of SOD and GSH-Px in brain tissue of the mice in high dose of Schisandrin B group were increased (P<0.001).Compared with model group,the expression level of Nrf2 protein in brain tissue of the mice in low dose of Schisandrin B group was increased (P<0.01),while the expression level of Nrf2 protein in brain tissue of the mice in high dose of Schisandrin B group was decreased (P<0.01);the expression levels of Keap1 protein in brain tissue of the mice in low and high doses of Schisandrin B groups was decreased (P<0.01).Conclusion:Schisandrin B could decrease the level of peroxidation in brain tissue of the mice and reduce the oxidative damage and improve the memory function of the AD mice.The mechanism is related to the activation of Nrf2 signaling pathway which improve the activity of antioxidant enzymes.
7.Analysis of therapeutic effects of external fixator for the treatment of comminuted fracture of distal radius.

OBJECTIVETo evaluate the clinical effects of external fixator in treating comminuted fracture of distal radius.
METHODSFrom Mar.2008 to Dec.2009, 37 patients with comminuted fracture of distal radius were treated with external fixator or assisted with Kirschner wire and T-shape locking compression plate (T-LCP) fixation. There were 14 males and 23 females, ranging in age from 30 to 79 years, with an average of 59.1 years. According to AO typing, type C1 was in 3 cases, type C2 was in 11 cases and type C3 was in 23 cases. Function of wrist joint and X-ray films were observed according to Gratland-Werley system at different months.
RESULTSAll patients were followed up from 8 to 24 months with an average of 12 months. All fractures had healing with an average time of 8 weeks. According to Gratland-Werley system, 16 cases obtained excellent result, 17 good, 4 fair, the rate of excellent and good was 89.0%.
CONCLUSIONExternal fixator can obtained satisfactory clinical effect in treating comminuted fracture of distal radius.

8.Changes of the BDNF in the hippocampus of P77PMC during seizures

AIM: To quest the relationship bwteen BDNF and seizures. METHODS: BDNF and other correlative proteins in the hippocampus of audioepileptic rat (P77PMC) have been detected by immunohistochemistry at different stages of seizures including quiet stage, preconvulsive stage, convulsive stage and postconvulsive stage. Wistar rats were served as controls. RESULTS: At quiet stage, the BDNF in the hippocampus of P77PMC were less than that of control group and stepped up after stimulation, trkB in the hippocampus of P77PMC were high at every stages, but PY20 were increased only after stimulation. NPY in the hippocampus of P77PMC were in a low level before seizures. CONCLUSION: The endogenetic BDNF may alter the excitement of P77PMC by adjusting the expression of NPY in hippocampus and activating its receptor trkB.
9.Clinical characteristics and therapy analysis of Graves ophthalmopathy in children and adolescents

Background Juvenile Graves ophthalmopathy has a low prevalence and few relevant studies.Analyzing and reviewing the clinical features and therapeutic effectiveness of juvenile Graves ophthalmopathy is helpful to its diagnosis and management.Objective This study was to evaluate the clinical characteristics of Graves ophthalmopathy and its management in children and adolescents.Methods The clinical data of 54 eyes from 29 patients with Graves ophthalmopathy who were diagnosed in Shanghai Changzheng Hospital from January 2007 to December 2012 were retrospectively analyzed.The ocular manifestations,thyroid function,CT or MRI testing results were collected,and the activity of Graves ophthalmopathy was scored based on the criteria of CAS.Artificial tears was topically administered in 44 eyes with CAS ≤ 2.In the eyes with CAS ≥ 3,corticosteroids drug was systemically used in 3 patients,and orbital decompression surgery and excision of Müller muscle were performed in 3 eyes of 2 patients respectively.The follow-up was carried out for 1.5-6 years.The treatment outcomes were evaluated according to the reduction of exophthalnos and the improvement of upper eyelid retraction.Results The patients were 5-18 years old with an average age of 12.9 years old.Out of 29 Graves ophthalmopathy patients,5 males and 24 females were included.The initial clinical manifestations were proptosis,eyelid retraction and swelling,and accompanied by conjunctival congestion and hypophasis.The best corrected visual acuity (BCVA) was ≥ 0.8 in all the eyes.CAS scores wcre 0-2 in 48 eyes of 26 patients and ≥ 3 (active Graves ophthalmopathy) in 6 eyes of 3 patients.The increase of orbital adipose volume was exhibited in all the eyes and the enlargement of extraocular muscle was revealed in parts of eyes by CT/MRI.Laboratory examination showed normal thyroid function in 12 patients (41.4%),hyperthyreosis in 15 patients (51.7%) and hypothyroidism in 2 patients (6.9%).The ocular symptom was improved in 20 eyes of 11 patients (37.9%),stabilized in 29 eyes of 16 patients (55.2%) and worsen in4 eyes of 2 patients (6.9%) in following-up duration.Conclusions Graves ophthalmopathy occurs much more in female than in male.The clinical manifestations are mild,with low activity and good prognosis in children and adolescents Graves ophthalmopathy.
10.Clinicopathological characteristics of renal light and heavy chain amyloidosis

Objective:To investigate the clinicopathological characteristics of renal light and heavy chain amyloidosis (AHL).Methods:Ten patients with renal AHL diagnosed by renal biopsy in Peking University First Hospital and Institute of Nephrology of Peking University from January 2015 to June 2020 were enrolled. Clinicopathological data of these patients was collected and reviewed.Results:AHL typically affected older patients, with a male/female ratio of 7:3. The clinical manifestations were mainly edema and heavy proteinuria. At the same time, 7/10 of patients presented with nephrotic syndrome, 7/10 presented with microscopic hematuria, and 3/10 presented with renal insufficiency. Laboratory examinations showed monoclonal immunoglobulin in blood and urine in all patients, and IgGλ was the most common one (5/10). Decreased serum complement could be seen in some patients. The ratio of serum free κ light chain and free λ light chain was abnormal in all patients who underwent serum free light chain test. None of the 10 patients met the diagnostic criteria of multiple myeloma. Except for one of the 10 patients who was diagnosed as Waldenstrom's macroglobulinemia, the rest were diagnosed as monoclonal gammopathy of renal significance (MGRS). Bone marrow of 2/6 of patients were positive for amyloid. Cardiac involvement was confirmed in only one patient. Renal biopsy demonstrated amorphous eosinophilic material, which was Congo red positive, was deposited in glomerular mesangial area (10/10), capillary vessels (8/10), renal interstitium (9/10), peritubular capillary walls (9/10) and arterioles (8/10). This material showed apple green birefringence under polarized light. Immunofluorescence showed that single heavy chain and single light chain were positive at the same time, which was consistent with the results of mass spectrometry analysis. Ultrastructural evaluation revealed randomly oriented, non-branching fibrils with a diameter of 8-12 nm.Conclusions:Main clinical manifestations of AHL amyloidosis are edema and massive proteinuria, along with a high incidence of hematuria, a low portion of heart involvement and high frequency of whole molecule of monoclonal immunoglobulin (IgGλ dominant) by serum immunofixation electrophoresis. Renal pathology shows the commonly involved kidney compartments of amyloid deposits are glomerular capillary walls and peritubular capillary walls in patients with AHL amyloidosis.
