1.Clinical analysis on vulnerable coronary atherosclerotic plaque by intravascular ultrasound

Objective To find out the sensitivity and s pecificity of intravascular ultrasound (IVUS) in identification of unstable atherosclerotic plaques. Methods Fourty-three patients diagnosed coronary heart disease, receiving coronary angiography and intravascular ultrasound examination,were divided into soft plaque and hard plaque group according to the echo reflectivity of atherosclerotic plaque by IVUS. Quantitative determination was performed between the two groups. Results The sensitivity of IVUS in detecting soft plaques from patients with acute coronary artery syndrome was 81.4 %, specificity 75.0 %, accuracy 79.1 %, positive predictive value 84.6 %. The lipid core areas and lipid to plaque ratio of soft plaques were significantly larger than those of hard plaques (P 0.05 ).Conclusions IVUS had higher sensitivity and specificity in detecting coronary vulnerable plaques in live bodies.
2.Purification of a small peptide to inhibit platelet aggregation from Agkistrodon acutus venom

Aim To purify a small platide inhibiting platelet aggregation from Agkistrodon Acutus snake venom,and to identify its physical and chemical properties and its effects on the platelet aggregation stimulated by ADP, collagen, and thrombin. Methods Extract snake venom through Superdex 75 gel filtration, ultrafiltration and DEAE-Sepharose CL-6B ionexchange. The purified product was identified by HPLC C_ 18 . The molecular weight was determined by SDS-polyacrylamid gel electrphoresis. Platelet aggragation was measured by nephelometry. Results The molecular weight of the peptide was 7 862 u and its purified from Agkistrodon Acutus snake venom is oelectric point was pH 4.29. This peptide dose-dependently inhibited the platelet aggregation induced by ADP,collagen,and thrombin. Conclusion The method has been proved to be successful for the purification of low molecular weight peptide fraction that inhibits platelet aggregation.
3.Effect of peptide from agkistrodon acutus snake venom on adhesion and proliferation of human ovarian carcimona A2780 cell line

Aim To investigate the inhibitory effect of peptide from Agkistrodon Acutus snake venom(fraction K)on the growth of human ovarian carcimona A2780 cell line and its mechanism.Methods The A2780 cells were treated with fraction K at different concentrations.The proliferation of A2780 cells was assayed with MTT colorimetric method.The histologi-cal and ultras-tructural changes of cell were scored using light and electron microseopy.The apoptosis of A2780 cells induced by fraction K was studied by fluorescent staining.Crystal violet staining and MTT assay were employed to determine the effect of fraction K on A2780 cells that they had been adhered to fibronectin.Result Proliferation of A2780 cells was inhibited significantly by fraction K in a dose-and time-dependant manner.Under electron-microseop and fluorescence microscope,some A2780 cells underwent a typical apoptosis and morphology changes after 24 hours incubation with fraction K.Fraction K could inhibit the adhesion of A2780 cells to fibronectin in a dose-dependent manner.Conclusion fraction K can inhibit proliferation of A2780 cells,which may be related to the mechanism of inducing cellular apoptosis and cellular adhesion.
4.Study on the level of MDA and SOD in cerbrospinal fluid from patients with cysticercosis.

ObJective To measure the level of MDA and SOD in cerebrospinal fluid from patients with cysticercosis indifferent stages and to investigate their effects on this dieases. Methods Forty-nine patients with a single cysticercus incerebral parenchyma and twenty normal controls parlticipated in the study. According to the magnetic resonance imaging(MRI) results, patients were divided into four stages. The level of MDA,SOD in CSF were tested. The data were analyzedwith the software of SPSS. Results The amount of SOD was significantly decreased in all stages except the fourth stage,while the level of MDA was increased in all the stages,and reached its peak in the second stage. There was a negative cor-relation between SOD and MDA. Conclusion The data suggested oxygen free radicals may be a main mechanism of para-site killing.
5.Artificial dura materials in neurosurgical procedures:An analysis of 100 cases

BACKGROUND:The meningeal defect patients can be treated with artificial dural materials combined neurosurgical techniques which can protect the integrity of brain tissue. OBJECTIVE:To analyze the biocompatibility and application feasibility of various artificial dura mater materials. METHODS:The artificial dura materials were classified according to the different sources, and the biocompatible and application feasibility of various artificial dura mater materials were analyzed, as wel as the advantages and disadvantages of various materials. The anticoagulant and hemolytic properties of the materials were analyzed through recalcification test and hemolysis test. 100 patients receiving col agen membrane repair selected from Department of Neurosurgery, Dongfang Hospital of Beijing University of Chinese Medicine were observed. RESULTS AND CONCLUSION:The dural repair materials mainly include autologous material, al ograft materials, dissimilar materials, synthetic materials and natural materials. Each kind of material has its advantages and disadvantages. Meningeal defect complications include cerebrospinal fluid leakage, pseudo meninges, infection and arachnoiditis. There are many problems of artificial dural materials to be solved, and the novel biological dural materials and absorbable materials are the direction of dural material research.
6.Analysis of Causes of Postoperative Acute Kidney Injury in Patients with Gastrointestinal Surgery

Objective To analyze the risk factors of acute kidney injury (AKI) in the patients undergoing gastrointestinal surgical procedures.Methods We retrospectively analyzed all gastrointestinal surgical patients nearly 2 years who were in the first affiliated hospital of Kunming Medical University and the first people's hospital of Yunnan province.Results The incidence of AKI was 2.1%.Univariate analysis and multivariate analysis indicated that: age>65 years, emergency operation and serious infection were significantly related with the incidence of AKI.Conclusion AKI is a common complication after gastrointestinal surgical procedures.This study showed that age>65 years, emergency operation, serious infection were risk factors most likely associated with postoperative AKI development.
7.Efficacy and safety of different hemostatic drugs in transurethral resection of the prostate in elderly patients

Objective To investigate the efficacy and safety of perioperative use of different hemostatic drugs in elderly patients undergoing transurethral resection of the prostate (TURP).Methods A total of 480 elderly patients with benign prostatic hyperplasia (BPH) admitted to the Departments of Urology and Gerontology of our hospital from January 2010 to October 2014 were selected and randomly divided into 6 groups (n =80,each).Patients in all groups were given preoperative oral finasteride for 1 week,with each of the non-control groups receiving perioperative heamocoagulase agkistrodon,mangabeys hemocoagulase,reptilase,ethylenediamine diaceturate,or dicynone + paraaminomethyl benzoic acid (PAMBA).Intraoperative bleeding,perioperative hematocrit,hemoglobin change,continuous bladder irrigation time,adverse reactions and other clinical parameters were recorded.Results Operations went smoothly with all patients.Changes in perioperative blood routine indexes,coagulation function,intraoperative bleeding volume and continuous bladder irrigation time had significant differences between the control group and the groups treated with hemostatic drugs (all P<0.05).No significant differences in perioperative blood routine indexes,coagulation function,intraoperative bleeding volume and continuous bladder irrigation time were found between groups treated with heamocoagulase agkistrodon,mangabeys hemocoagulase and reptilase,or between groups treated with ethylenediamine diaceturate and dicynone+PAMBA (all P> 0.05).There were significant differences in the perioperative blood routine indexes,coagulation function,intraoperative bleeding volume and continuous bladder irrigation time between the groups with and without blood coagulation enzymes (all P <0.05).Fibrinolytic responses occurred in 2 patients in the heamocoagulase agkistrodon group and were mitigated after drug withdrawal and fibrinogen infusion.Conclusions A reasonable perioperative dose of a hemostatic drug has good clinical effects in elderly patients undergoing TURP.Hemostatic drugs such as blood coagulation enzymes have certain advantages including safety over other types of hemostatic drugs,but their doses should not be too large.
8.Comparative study of coplanar and non-coplanar intensity-modulated radiotherapy in advanced lung cancer

Objective To study the dosimetry and safety of the non-coplanar IMRT plan for advanced lung cancer.Methods The two groups IMRT plans were designed with coplanar (5,7F) and non-coplanar field (5,7F-n) for patients.To compare the dosimetry of two groups and perform 4 patients F7-n IMRT plan.Results With the increase of the fields in each group PTV's CI were improved (all P =0.000),especially the 7F-n plan PTV's Dmean,Dmax,V95% and HI also were improved (P=0.001,0.001,0.009,0.000) ; in the coplanar group each lung' s V5 increased (P =0.000,0.002,0.000) and whole lung's Dmean increased (P =0.000),but non-coplanar group whole lung's and contralateral lung's V5 reduce (P =0.001,0.005).Between the groups,7F-n plan PTV's indicators were all improved to compared with 5F plan (all P =0.000),and each lung's V20 reduced (all P =0.000),and whole lung's Dmean,V30,contralateral lung' s V5 reduced (P =0.000,0.001,0.000),and spinal cord' s Dmax also reduced (P =0.033),but ipsilateral lung's V5 and heart's Dmean increased (P =0.000,0.003);with compared to 7F plan,the 7F-n's ipsilateral lung's V5 and heart's Dmean also increased (P =0.000,0.048),but whole lung' s and contralateral lung's V5 decreased (all P =0.000).Four patients were performed successfully non-coplanar IMRT treatment,no collision occurred.Conclusions 7 fields non-coplanar IMRT plan not only improve the dose distribution of PTV,but also effectively control the volume of low dose lung increase,lung V20 and Dmean reduce too.Thus recommended to use this design in patients with advanced lung cancer for radiotherapy
9.The diagnostic value of left atrial volume index for heart failure with preserved ejection fraction

Objective To explore the value of left atrium volume index (LAVI) in the diagnosis of heart failure with preserved ejection fraction (HFPEF). Methods Seventy-seven patients with HFPEF and 33 patients without HFPEF who had been treated from May 2012 to September 2013 in Guangdong General Hospital were en-rolled. The clinical data and a series of ultrasound parameters were collected and analysed. The relationship between LAVI, LAV, and other indexes of diastolic function was determined by Pearson correlation analysis. The value of LAVI and LAV for diagnosing HFPEF was compared by the ROC curve. Results LAVI and LAV of were signifi-cantly greater in HFPEF group than in non-HFPEF group. LAV and LAVI were significantly associated wtih HEPEF. The area under the ROC curve (AUC) of LAVI increased significantly as compared with the AUC of LAV (0.832 vs. 0.799, P<0.05). With a cut-off value of 30 mL/m2, the specificity and sensitivity for diagnosing HEPEF were 64.9%and 84.8%, respectively. Conclusions LAVI may be valuable in the diagnosis of HFPEF.
10.AIDS complicated with plasmablastic lymphoma: Clinical analysis of 7 cases

Objective:To analyze the clinical characteristics of AIDS complicated with plasmablastic lymphoma (PBL).Methods:Clinical data and laboratory test of 7 AIDS patients complicated with PBL admitted to Yunnan Infectious Disease Hospital from January 2016 to October 2022 were retrospectively analyzed.Results:There were 6 male patients and 1 female patient with a median age of 48 years (41-56 years). All patients had oral and maxillofacial involvement, and only 1 case was stage Ⅱ at the initial Ann Arbor stage, while 6 cases were stage Ⅲ or higher. Six patients had systemic symptoms. All patients had a Ki-67 proliferation index greater than 80% and all presented MYC gene rearrangements, and 6 patients were positive for EBER. All patients received DA-EPOCH-based first-line chemotherapy and antiretroviral therapy (ART), Five patients initiated ART at the same time as chemotherapy, and 2 patients initiated ART before chemotherapy. Four patients achieved complete remission after chemotherapy according to PET-CT evaluation, and 3 patients died.Conclusions:Active chemotherapy combined with ART can maximize the therapeutic benefits of AIDS patients with PBL. The introduction of ART in the first chemotherapy cycle can avoid the rapid disease deterioration in the patients.
