1.Clinical application and biocompatibility of the biliary tract stents

OBJECTIVE:To introduce the research progress of biliary tract prosthesis,to analyze the reason of complication following stent implantation,and to explore the biocompatibility of various biliary tracts and host. METHODS:We retrieved Medline database (1989/2009) and CNKI (1989/2009) using the computer. According to inclusion and exclusion criteria,the literatures were screened by the first author,and the data were collected,and the quality was evaluated. We described and arranged the treatment progress of biliary tract prosthesis implantation,complication of stent implantation and biocompatibility of biliary tract prosthesis to investigate treatment state,complication and biocompatibility of the biliary tract prosthesis. RESULTS:The literatures were summarized. The bilitary tract prosthesis contains plastic stent,mental stent,silica gel tract stent and degradable polylactic acid stent. The complications of stent implantation included transudation,hemorrhage,perforation and stenosis. Among various stents,the biocompatibility of plastic stent was poor,easy to be blocked,but the price was low,and easy to be taken out. The diameter of mental stent following distension could reach 7-10 mm,with long unobstructed time,but the price was higher compared with the plastic stent. The biocompatibility of silica gel tract stent was good. The silica gel tract had strong decay resistance and pliable texture,which could extend tumor excision range and elevate curative effects. Moreover,complications did not occur such as infection and stenosis of the biliary tract. Biocompatibility of nickel-titanium alloy biliary ducts was significantly better than the stent products,without complications. With the exception of price factor,the biocompatibility of degradable polylactic acid stent was the best,but further studies were needed to observe and study the formation of biliary sludge,stent displacement and blockage,stent degradation products. CONCLUSION:Biliary tract prosthesis implantation can be an important treatment method for stenosis of biliary tract. During treatment,enough preparation is needed prior to implantation according to various clinical symptoms to select suitable stent type. Indications are followed strictly to avoid the occurrence of complications. The biocompatibility of silica gel stent and Ni-Ti alloy stent is good,but the biocompatibility of other stents should be enhanced.
2.Application of element-tagged immunoassay combined with inductively coupled plasma mass spectrometry in HCG detection

Objective:To establish an inductively coupled plasma mass spectrometry (ICP-MS) based immunoassay method for the quantitative detection of human chorionic gonadotropin (HCG), and evaluate the clinical applicability of this method.Methods:Sm was selected as element tags, and the HCG quantitative detection system was established by double antibody sandwich method. The dosage of biotinylated antibody and reaction time were optimized. According to EP documents of Clinical and Laboratory Standards Institute (CLSI) and related standards, the analytical performance was evaluated after the establishment of the assay, including the limit of blank (LOB), linearity, precision, recovery, cross reactivity and interference test. And 88 clinical samples were measured using the new method compared to the electrochemical immunoassay (ECLIA) method.Results:Total process completed within 30 min after optimization, and the optimal biotinylated antibody dosage was 0.5 μl. The LOB was 0.29 mIU/ml. The linearity was good within the range of 1.16-8 365.62 mIU/ml with the linear correlation coefficient greater than 0.995 ( R2=0.998 0), the recovery was 97.53%-102.01%. Both intra-and inter-assay coefficients of variation of the high-value sample and the low-value sample were less than 10%. And there was no significant cross-reaction with Luteinizing hormone (LH), follicular stimulating hormone (FSH), and thyroid-stimulating hormone (TSH). The interference bias caused by different concentrations of interference substances was less than 10%. When compared with the ECLIA method for clinical sample detection, the proposed method showed a significant correlation( R2=0.960 0). Conclusion:The proposed ICP-MS base immunoassay for HCG detection has good accuracy, high sensitivity and specificity, and the results of analytical performance verification meet the clinical requirements, which provides experimental basis for the clinical application of this method.
3.Chinese medicine Jiangzhuo mixture regulates glucose and lipid metabolism in obese rats through TLR4/I κB α/NF- κB signaling pathway.

OBJECTIVES:
To explore the mechanism of Chinese medicine Jiangzhuo mixture regulating glucose and lipid metabolism in obese rats.
METHODS:
Thirty healthy male SD rats were randomly divided into normal control group, model control group, and Jiangzhuo mixture treatment group, with 10 rats in each group. The rats in the normal control group were fed with normal diet, the obesity model was induced by feeding high-fat diet in the model control group and the Jiangzhuo mixture treatment group, the rats in the treatment group were given with Jiangzhuo mixture 50 g/kg by gavage. After 8 weeks of intervention, the blood glucose (GLU), total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) levels were measured in the three groups. Quantitative reverse transcription PCR were used to detect the expression levels of PR domain containing 16 (PRDM16) and uncoupling protein 1 (UCP1) in white and brown adipose tissues of the rats in each group; Western blotting was used to detect the expression of PRDM16 in the white and brown adipose tissue of rats, and Toll-like receptor 4 (TLR4), nuclear factor-κB (NF-κB) and inhibitor of NF-κB alpha (IκBα) in the white adipose tissue; immunohistochemistry was used to detect the expression of UCP1 protein in white and brown adipose tissues.
RESULTS:
Compared with the normal control group, the white fat weight (P<0.01), white fat coefficient (P<0.05) and Lee's coefficient (P<0.01) were significantly increased in the model control group; the contents of GLU, TC, TG and LDL-C were all increased, and the content of TG was significantly increased (P<0.05) in the model control group. The mRNA and protein expression levels of PRDM16 and UCP1 in white fat and brown fat were significantly decreased (P<0.05) in the model control group. Compared with the model control group, the white fat weight and white fat coefficient and Lee's coefficient were significantly reduced in the Jiangzhuo mixture treatment group (all P<0.01), the levels of GLU, TC, TG, and LDL-C in the the treatment group were all reduced, and the content of TG was reduced more obviously (P<0.01); expression levels of PRDM16 and UCP1 mRNA and protein were increased in brown and white adipose tissue. Compared with the normal control group, the expression levels of TLR4, phospho-IκBα and NF-κB-p65 proteins in white adipose tissue of the model control group were significantly increased (all P<0.01), while the expression levels of these proteins in the treatment group were significantly lower than those in the model control group (all P<0.05).
CONCLUSIONS
Jiangzhuo mixture can alleviate high-fat diet-induced increase in body fat, abnormal expression of biochemical indexes and promote the expression of key proteins including UCP1 and PRDM16 in white and brown adipose tissues by regulating TLR4/IκBα/NF-κB signaling pathway.
