2.Progress of surgical management of radiation proctitis

Radiation proctitis is one of the complications after radiotherapy for pelvic malignancies.Surgery is not necessary for the most of radiation proctitis.However, surgery is unavoidable when rectal obstruction, uncontrollable bleeding, and fistula occur. This article reviews the progress in surgical treatment of radiation proctitis.
3.Clinicopathological features of renal cell carcinoma : a primary study of 705 cases

Objective To investigate the clinicopathological features of renal cell carcinoma (RCC).Methods From December 1956 to August 2012,the clinicopathological features of RCC were studied in 705 cases and related literatures were reviewed.Results The diameter of RCC ranged from 0.6 to 18.0 cm,which the average size was 4.6 cm.The proportions of the clear cell,papillary,multilocular clear cell,chromophobe and unclassified histologic subtype were 88.9% (627/705),4.1% (29/705),3.3% (23/705),1.3% (9/705) and 2.4% (17/705),respectively.According to the Fuhrman grading system,the proportions of grade 1,2,3,4 were 19.0% (116/612),58.3% (357/612),18.1% (111/612)and 4.6% (28/612),respectively.The rates of invasion into the renal pelvis,perirenal fat and vascular were 10.9% (66/603),10.6% (64/603) and 4.8% (29/603),respectively.Of 705 cases,464 (76.6%)cases were in T1,65 (10.7%) cases in T2,73 (12.0%) cases in T3,and 4 (0.7%) cases in T4.As to the lymph node and distant metastasis,the rate was 2.8% (17/606) and 3.5% (21/606).The percentages of stage Ⅰ,Ⅱ,Ⅲ and Ⅳ RCC were 74.3% (450/606),9.9% (60/606),11.7% (71/606) and 4.1%(25/606),respectively.The 3-,5-,10-and 15-year disease-specific survival rate for RCC was 92.8%,86.9%,76.8% and 55.5%,respectively.To those patients with clear cell RCC,the disease-specific survival at the same time point was 92.8%,88.1%,77.4% and 55.4%,respectively.Multivariate analysis showed that the stage was the only independent prognostic factor for RCC.Conclusions Tumor stage of RCC is the independent prognostic factor for disease-specific survival.The evaluation of renal sinus invasion and lymph node should be noted in the diagnosis of RCC.
4.Affected muscle fibers in Nonaka myopathy with endoplasmic reticulum stress

ObjectiveTo investigate the characteristic of pathology in Chinese patients with Nonaka myopathy.MethodsThirteen patients (7 males and 6 females) diagnosed with Nonaka myopathy in our laboratory from January 2002 to March 2011 were included in this study.Their mean age was 39.5 years old and the mean duration of illness was 4.15 years.The most common symptoms were weakness of raising feet with sparing of quadriceps femoris muscles in the early stage of disease.One patient presented the initial symptoms of upper limb weakness. Muscles biopsies were obtained from all these 13 patients. Histology study including immunohistochemical (IHC) staining with antibody against amyloid 3,phosphorylated tau protein,ubiquitin,glucose-regulated protein of molecular weight 78 000(GRP78),calnexin,caspase-12and Bax were performed.Skeletal muscle samples from 3 chronic fatigue syndrome patients,2 myofibrillar myopathy patients were used for control in the IHC staining. All coding exons of uridinediphospho-N-acetylglucosamine 2-epimerase gene were directly sequenced in genomic DNA from these patients.Results The main pathological changes of tibialis anterior muscle in 12 cases were muscle dystrophy with rimmed vacuoles.The rimmed vacuoles were positive for anti-β-amyloid,tau protein and ubiquitin in IHC studies.In the atrophy fibers,IHC showed the increased expression of endoplasmic reticulum stress related proteins GRP78 and calnexin,and apoptosis proteins of caspase-12 and Bax.ConclusionsThere is accumulation of abnormal proteins in muscle fibers in Chinese patients with Nonaka myopahty.These proteins may stimulate endoplasmic reticulum stress and apoptosis,which may be a mechanism responsible for muscle damage.
5.Extranodal NK/T-cell lymphoma, nasal type:a clinicopathological analysis of 28 cases

Purpose To study the clinicopathologic features, diagnosis and differential diagnosis of extranodal NK/T-cell lymphoma, nasal type ( EN-NK/TCL) . Methods Twenty-eight cases of EN-NK/TCL were analyzed according to WHO classification of tumors of haematopoietic and lymphoid tissues, the cases were studied by microscopy, immunohistochemistry and in situ hybridization. Results In 28 cases, the male to female ratio was 1 ∶ 1. 2. The mean age was 46 years, with the median of 44. 5. Eighteen cases involved nasal cavity, 5 cases of skin, and 1 case of tonsil, upper palate, root of tongue, adrenal glands and gastric, respectively. The main presentation was nasal obstruction in cases with nasal involvement, and the cases with skin involvement present with rash, ulcer, plaque or nodular mass. Patients might be accompanied by B-type symptoms, lymphoadenopathy or with multiple sites involvement. The tumor cells were small, medium, large-sized, or mixed, and angiocentric and angiodestructive growth pattern was frequently pres-ent. Most cases were mixed with inflammatory cells infiltration. The typical immunophenotype was T-cell ( the most sensitive, CD3ε) , CD56 and cytotoxic markers. In situ hybridization for EBER was positive. Ten cases were followed up, and 1 case died. Conclusions The most common site of EN-NK/TCL is nasal cavity, followed by skin. EN-NK/TCL can be definitely diagnosed by integration of pathological morphology, immunophenotype and in situ hybridization of EBER.
6.Influence of sampling satisfaction using endometrial sampling device and related factors for pathology diagnostic accordance rate

Objective To observe the satisfaction of obtaining endometrial tissue in using endometrial sampling device and explore the influence for pathology diagnostic accordance rate,and to discuss the problem needs to focus in obtaining endometrial tissue with endometrial sampling device for endometrial cancer and precancerous lesion screening.Methods Totally 204 cases using endometrial sampling device with the subsequent diagnostic curettage,hysteroscopy,or hysterectomy were collected.A comparison of pathological diagnosis between two methods was made.The relationship among specimen quantity,proficiency,pathological types and diagnostic accordance rate were analyzed.Results The diagnostic accordance rate of endometrial cancer and precancerous lesion of two methods was 67% (20/30).92.2% (188/204) cases successfully got the endometrial glands,77.0% (157/204) cases got ≥5 specimens of glands and 62.3% (127/204) cases got ≥10 specimens of glands.When obtaining ≥5 but< 10 endometrial glands,the diagnostic accordance rate was 76.7% (23/30); however,obtaining ≥10 endometrial glands,the diagnostic accordance rate was 92.9% (118/127).The degree of specimen satisfaction of the first phase was lower than the second phase.Conclusions Obtaining ≥5 endometrial glands in using endometrial sampling device could satisfy the requirement of the basic pathological diagnosis.The proficiency of sampling and the pathological types could influence the degree of specimen satisfaction,and then influence the diagnostic accordance rate.
7.Pharmacokinetics of Qingyi -ⅡGranula in Rats

OBJECTIVE:To study the pharmacokinetics of emodin(an active component of Qingyi-Ⅱgranula) in rats after i.g administration of Qingyi-Ⅱgranula.METHODS:Rats were i.g administered with Qingyi -Ⅱgranula(2.5 g?kg~(-1) and 5.0 g?kg~(-1)).Serum concentrations of emodin were determined at different time points by HPLC,and the pharmacokinetic parameters were computed.RESULTS:The pharmacokinetic parameters of emodin in rats after administration of Qingyi-Ⅱgranula(2.5g?kg~(-1) and5.0g?kg~(-1)) were asfollows:t_(1/2?):(9.468?8.46) hand(21.68?17.867) h;t_(1/2?):(15.388?5.46) h and.(39.63?24.39) h;t_(max):(2.500?3.479) h and(5.333?3.266) h;C_(max):(0.058?0.004) mg?L~(-1)and(0.101?0.007) mg?L~(-1);CL:(33.027?9.365) L?h~(-1)?kg~(-1) and(9.405?5.846) L?h~(-1)?kg~(-1);AUC_(0-∞):(0.652?0.201) mg?h~(-1)?L~(-1)and (1.364?0.267) mg?h~(-1)?L~(-1).The pharmacokinetic process was in line with two-compartment model.CONCLUSION:The method for the detection of serum concentration of emodin in rats and the related pharmacokinetic parameters had been established in our study,which serves as a theoretic basis for the pharmacokinetic study of Qingyi-Ⅱgranula.
8.Analysis of 1290 Cases of ADR/ADE Induced by Dezocine Injection

OBJECTIVE:To investigate general regularity and characteristics of adverse drug reactions/events(ADR/ADE)in-duced by Dezocine injection,and to provide reference for safe and rational drug use in clinic. METHODS:Retrieved from Jiangsu Province ADR Database during Jan. 2009-Aug. 2016,ADR/ADE induced by Dezocine injection was analyzed statistically in re-spects of patient's gender and age,usage and dosage,organs/systems involved in ADR,clinical manifestations and outcome. RE-SULTS:Among 1290 ADR/ADE induced by Dezocine injection,there were 40 cases of severe ADR/ADE(3.10%);medical insti-tutions were main reporting units (1276 cases,98.81%). In ADR/ADE cases,female was more male,with ratio of male to fe-male was 1:1.45. ADR/ADE mostly occurred in 45-65 age group(43.49%). Intravenous medication was main route of administra-tion (1137 cases,88.14%). Initial dose of 577 cases was excessive dose (50.75%). ADR/ADE mainly involved gastrointestinal system (54.99%),central and peripheral nervous system (24.72%) and systemic lesions (5.20%). Severe ADR/ADE manifested as anaphylactic shock,palpitation and dyspnea,etc. But anaphylactic shock was not mentioned in drug package inserts. 1283 ADR/ADE(99.46%)were cured or improved . CONCLUSIONS:It is necessary to strengthen drug safety reevaluation of Dezocine injec-tion after being on the market,improve drug package inserts and enhance patients education as so as to reduce and avoid the occur-rence of ADR/ADE.
9.A Meta-analysis of the relationship between ACE gene L/D polymorphism and vitiligo

Objective To investigate the relationship of angiotensin converting enzyme (ACE I/D) gene I/D polymorphism with vitiligo.Methods PubMed,the Cochrane Library,China academic journal full-text database and VIP database and Wan fang database were searched according to the inclusion and exclusion criteria,and the quality of included studies was assessed.The data were quantitatively analyzed by Stata 12.0 software.Results Nine articles were included.Meta analysis results showed that the susceptibility of vitiligo difference was statistically significant in the total population of ACE I/D genotype (DD + II)vs.DI (OR=0.759,95% CI:0.643 ~0.896,P=0.001),the (DD+DI) vs.II (OR=1.523,95% CI:1.153 ~2.011,P =0.003).The genotype subgroup analysis showed the D vs.I (OR =1.381,95% CI:1.054 ~1.810,P=0.019),DDvs.II (OR=1.830,95% CI:1.110~3.017,P=0.018),(DD+ II) vs.DI (OR=0.814,95% CI:0.667 ~0.994,P=0.043),and the (DD+ID) vs.II (OR=1.690,95% CI:1.147 ~2.489,P =0.008) in Asian.The difference was statistically significantly related to vitiligo.Arab genotype (DD + II) vs.DI (OR =0.545,95% CI:0.354 ~ 0.840,P =0.006) was associated with vitiligo occurrence.Begg's inspection publication bias analysis showed all genetic types did not exist any publication bias.Conclusions The ACE gene I/D polymorphism may be associated with vitiligo susceptibility.
10.The study of hemoperfusion treatment time of patient with acute serious organophosphorus pesticide

Objective To explore the time of application of hemoperfusion (HP) for the treatment of acute serious organophosphorus pesticide (ASOPP). Methods One hundred and four patients with ASOPP were randomly divided into two groups, 46 patients accepted traditional treatment(control group), 58 patients were treated with traditional treatment and HP (HP group). The patients in HP group were again divided into three groups according the different time of treatment (time of beginning HP after poisoning), the 4-8 hours group (HP-1 group, 27 patients), the 9-16 hours group (HP-2 group, 19 patients), the 17-32 hours group (HP-3 group, 12 pafients).Tbe coma period, the dosage of atropine, the time of regaining the vitality of cholinesterase, the time of hospitalization and the rate of fatality and curing among groups were observed. Results The coma period, the dosage of atropine, the time of regaining the vitality of cholinesterase, the time of hospitalization and the rate of fatality of the HP group were less than those of the control group (P<0.05). Compared with HP-1 group, the eoma period, the dosage of atropine, the time of regaining the vitality of eholinesterase and the time of hospitalization of the HP-2 group and the HP-3 group were higher (P<0.05), but the difference of the rate of fatality and curing between the HP-1 group and the other HP groups was not statistically significant (P>0.05). The difference of all of the above indicators between HP-2 group and HP-3 group was not statistically significant (P>0.05). Conclusion Application of hemoperfusion among 4-32 hours after poisoning for the treatment of ASOPP can improve the efficacy of therapy, and the efficacy of application of hemoperfusion among 4-8 hours is the best.
