1.The expression and clinical significance of mineral dust-induced gene in malignant and tuberculosis pleural effusion

Objective To investigate the expression and clinical significance of mineral dust-induced gene(MDIG) in malignant pleural effusions (MPE) and tuberculosis pleural effusions (TBPE).Methods Fifty-four patients with MPE (MPE group) and 50 patients with TBPE (TBPE group) were collected.The MDIG protein was detected by enzyme-linked immunosorbent assay and the relative quantification of MDIG mRNA was measured by real-time quantitation polymerase chain reaction.The cutoff value,sensitivity and specificity of the MDIG protein and MDIG mRNA to diagnose the MPE were evaluated by receiver operating characteristic (ROC) curve analysis.By Spearman correlation analysis,the correlation of MDIG protein and MDIG mRNA was evaluated.Results The MDIG protein in MPE group was significantly higher than that in TBPE group [(304.38 ± 228.47) ng/L vs.(44.43 ± 40.57) ng/L],and there was statistical difference (P < 0.01).The MDIG mRNA in MPE group was significantly higher than that in TBPE group (6.27 ± 3.54 vs.1.82 ± 0.64),and there was statistical difference (P < 0.01).With a cutoff point of 114.23 ng/L,MDIG protein had a sensitivity of 77.8% and a specificity of 94.0% for differential diagnosis.With a cutoff point of 2.75,MDIG mRNA had a sensitivity of 88.9% and a specificity of 92.0% for differential diagnosis.There was a positive correction between MDIG protein and MDIG mRNA (r =0.915,P < 0.01).Conclusions The MDIG protein and MDIG mRNA are highly expressed in MPE with a good sensitivity and specificity.MDIG protein and MDIG mRNA maybe a good clinical indicator in the diagnosis of malignant pleural effusions.
2.Mice with focal segmental glomerular sclerosis are induced by adriamycin and its mechanism

Objective To establish adriamycin-induced focal segmental glomerular sclerosis(FSGS) mice model,and observe the expressions of and relation between oxidative stress and p38 MAPK signal pathway in renal injury.Methods Eight-week-old male Balb/c mice were randomly divided into FSGS group (n=20) and control group (n=20).In FSGS group mice were intravenously injected with a single dose of adriamycin (0.01 rag/g),and mice in control group were received saline with the same dose.At day 3,7,14,22 and 32,urine protein-to-urine creatinine ratio (P/C) was detected.At day 22 and 32,serum creatinine,blood urea nitrogen,nitric oxide (NO) and reactive oxygen species (ROS) in blood and urine,and ROS in kidney tissues were detected;changes of pathological morphology in renal tissue were analyzed by HE stain;the expressions of NF-κB,CD36,IL-13,BAX and Bcl-2 mRNA were detected by real time quantitative PCR;the expressions of NF-κB,p-p38 and p-ERK1/2 protein were detected by Western blotting.Results Compared with that in control group,P/C was gradually increasing in FSGS group,and peaked at day 22 (P < 0.05).At day 22 and 32,mice had higher creatinine,serum creatinine,urea nitrogen,ROS and NO in FSGS group than those in control group (all P < 0.05).There were mild hyperplasia of mesangial cells and mesangial matrix,segment with moderate exacerbations,podocytes with significant proliferation,and the capillary loops of the stenosed in the glomerular in FSGS group at day 32.Compared with those in control group,the mRNA expression of NF-κB,BAX,IL-13 and CD36,and the protein expressions of NF-κB and p-p38 MAPK were gradually increased in FSGS group,all showed statistical differences at day 32 (all P< 0.05);the expression of p-ERK1/2 was increased at day 22 (P < 0.05) but was reduced at day 32 (P < 0.05).Conclusions Adriamycin has induced FSGS in mice successfully,which may through oxidative stress activating p38,up-regulating NF-κB,increasing the inflammatory cytokines and inducing apoptosis pathways.
3.Application of self-made negative pressure drainage device for extremity soft tissue trauma

Objective To investigate the clinical value of self-made dressing negative pressure drainage technique in the treatment of the four limbs soft tissue trauma.Methods A total of 60 patients with the four limbs soft tissue trauma in our hospital from December 2013 to December 2014 were randomly divided into the observation group and control group.The observation group with 30 cases were treated with the self-made negative pressure drainage technique,and the control group with 30 cases received the conventional treatment.The pain score,wound healing rate,complication rate between two groups were compared.Results The pain score of the observation group was(1.17±0.22),which was lower than(3.17±0.49) of control group,the difference was statistically significant(P<0.05).The wound healing rate of observation group(73.33%) was higher than that of the control group(40%),the difference was statistically significant(P<0.05).The complication rate of observation group(3.33%) was lower than that of control group(23.33%),the difference was statistically significant(P<0.05).Conclusion The self-made dressing negative pressure drainage technique applied to the patients of four limbs soft tissue trauma can reduce the pain,shorten the healing time,and reduce the incidence of complications.
4.Fatty acid-induced insulin resistance in 3T3-L1 adipocytes

AIM: The purpose of the present study was to examine the effect of insulin and different kinds of free fatty acid (FFA) on glucose transport in cultured 3T3-L1 preadipocytes or adipocytes. METHODS: Following the exposure of preadipocytes to insulin at different concentrations and treated times, glucose transport was assessed as [3H]2-deoxy glucose uptake. Furthermore, 3T3-L1 preadipocytes and adipocytes with either the monounsaturated FFA oleate (C18:1) or the saturated FFA palmitate (C16:0)were used and glucose transport was examined as above. RESULTS: Insulin increased specific membrane glucose transport in 3T3-L1 preadipocytes at the time of 15 min to 1 h stimulation. 5.Improvement of magnetic resonance phase unwrapping method based on Goldstein Branch-cut algorithm.

The phase information of magnetic resonance (MR) phase image can be used in many MR imaging techniques, but phase wrapping of the images often results in inaccurate phase information and phase unwrapping is essential for MR imaging techniques. In this paper we analyze the causes of errors in phase unwrapping with the commonly used Goldstein Brunch-cut algorithm and propose an improved algorithm. During the unwrapping process, masking, filtering, dipole- remover preprocessor, and the Prim algorithm of the minimum spanning tree were introduced to optimize the residues essential for the Goldstein Brunch-cut algorithm. Experimental results showed that the residues, branch-cuts and continuous unwrapped phase surface were efficiently reduced and the quality of MR phase images was obviously improved with the proposed method.

6.Comparison of rapid detection effects of Mycobacterium tuberculosis in cerebrospinal fluid by RT-LAMP and RT-PCR

Objective To investigate the effects of reverse transcription loop-mediated isothermal amplifica-tion(RT-LAMP)method and RT-PCR method for detecting Mycobacterium tuberculosis(MTB)in cerebro-spinal fluid(CSF)to provide a basis for its rapid diagnosis and clinical pharmacodynamic evaluation.Methods Eighty-five cases of CSF sample in the Bethune International Peace Hospital of PLA from December 2015 to April 2017 were selected for conducting the study and divided into the tuberculous meningitis(TBM)group(46 cases),suspected TBM group(25 cases)and control group(16 cases).The 16S rRNA region of MTB was used to design the specific primers.Then RT-LAMP and RT-PCR detection technological systems were estab-lished.Then the detection results by using the these two methods was analyzed.Results The positive detec-tion rates of the TBM group were 97.8% and 75.0% respectively,which of the suspected TBM group were 76.0% and 40.0% respectively,and which of the control group were 0.0% and 12.0% respectively,the posi-tive detection rate of each group in the RT-LAMP method was higher than that in the RT-PCR method,the difference was statistically significant(P<0.01);in the control group,adopting RT-PCR detection found non-specific amplification,while which was not found by adopting RT-LAMP method,indicating that the specifici-ty of RT-LAMP method was stronger than that of RT-PCR;the sensitivity of RT-PCR was 10.0 CFU/mL, which was higher than 1 CFU /mL of RT-LAMP.Conclusion RT-LAMP has the advantages of simpleness,sensitivity,rapidness and detecting viable bacteria,compared with PCR,which has strong specificity,easy op-erating,low cost and short time-consuming,is expected to be a routine detection tool of basic level and field medical institutions and developing countries.
7.The clinical efficacy of Xinmailong on heart failure in coronary heart disease and on the levels of hs-C-reactive protein and N-terminal B-type natriuretic peptide

Objective To investigate the clinical efficacy of Xinmailong on heart failure in coronary heart disease and on the levels of hs-C-reactive protein (hs-CRP)and N-terminal B-type natriuretic peptide (NT-proBNP).Methods 84 elderly patients diagnosed with heart failure in coronary heart disease were enrolled in this study at our hospital from October 2014 to October 2015.All patients were randomly divided into the control group with conventional treatment(n=42) and observation group with conventional plus Xinmailong treatment(n =42).The Lee's heart failure score,cardiac function grade,quality of life score and the changes of plasma hs-CRP and NT-proBNP after one week of therapy were compared between the two groups before versus after treatment.The adverse reactions were observed during the treatment.Results The clinical efficacy rate of heart failure after treatment was significantly higher in the observation group (83.3%,35 cases)than in control group(54.7 %,23 cases) (x2 =8.016,P =0.033).The clinical efficacy of heart function after treatment was significantly higher in the observation group(80.9 %,34 cases)than in the control group (59.5 %,25 cases) (x2 =6.219,P =0.019).After treatment,the quality of life score was significantly lower in observation group (15.89 ± 3.39) than in the control group (23.80 ± 5.66) (t =7.266,P =0.026).The plasma levels of NT-proBNP and hs-CRP were significantly lower after than before treatment in NT-proBNP[(1 136.6±236.8)ng/L and (2 266.9±260.9)ng/L],and in hs-CRP[(3.06 ±0.26)mg/L and (3.56±0.29)mg/L] (t=6.608、5.106,P=0.019、0.009).There were no serious adverse reactions in both groups during the treatment.Conclusions Xinmailong can not only improve safely and effectively the quality of life of patients,but also block the atherosclerosis of the inflammatory response.
8.Antiangiogenic effect of continuous low-dose chemotherapy on Lewis lung carcinoma.

BACKGROUNDRecently a number of preclinical studies have sparked interest in the concept of exploiting conventional chemotherapeutic drugs as antiangiogenics. Such antiangiogenic activity is achieved by metronomic-dosing (low-dose) protocols. This new target may have some advantages in avoiding toxicity and resistance caused by chemotherapeutic drugs. This study is to test the efficacy of continuous low-dose cyclophosphamide (CTX) for antiangiogenic effect on Lewis lung carcinoma, and investigate its antitumor effect and toxicity.
METHODSC57/BL6 mice bearing Lewis lung carcinoma were randomly divided into three groups, receiving low-dose metronomic (LDM) CTX, maximum tolerated dose (MTD) CTX therapy and saline respectively. Tumor growth, weight loss, peripheral white blood cell counts and survival of mice were monitored in each group. At the end of experiment, tumors were resected for immunohistochemical staining. Tumor microvascular density (MVD) and vascular endothelial growth factor (VEGF) level were detected by immunohistochemical staining.
RESULTSMVD and VEGF expression of tumors were much lower in the mice received LDM CTX therapy than those in control group and MTD CTX group (P < 0.05). During the experiment, growth delays of tumor were found in the mice received LDM CTX therapy, without apparent body weight loss or leukopenia, and the survival of mice was remarkably prolonged, compared with mice received MTD CTX therapy (P < 0.05).
CONCLUSIONSThe continuous low-dose regimen of CTX can significantly increase the therapeutic activity with decreased toxicity and prolonged animal survival for lung cancer. It may act as an antiangiogenic and lead to less drug resistance.
9.Clinical effect comparison of the type 2 diabetes mellitus with different BMI after the laparoscopic gastric bypass

Objective To explore impact of BMI on the clinical efficacy of treatment of type 2 diabetes mellitus after the laparoscopic gastric bypass. Methods Twenty patients were randomly selected from type 2 diabetes mellitus patients with BMI of 25 ~28 kg/m2 after laparoscopic gastric bypass surgery in our hospital from 2010 to 2013,20 patients from type 2 diabetes mellitus patients with BMI of 28~35 kg/m2 and 20 patients from type 2 diabetes mellitus patients with BMI≥35 kg/m2 . Accordingly,the total of 60 patients were divided into low BMI group,middle BMI group and high BMI group. The difference of the rate of blood glucose control and other indicators between three groups were compared and analyzed. Results The rate of blood glucose control in low BMI group was 75%,middle BMI group was 85%, and high BMI group was 90%. There was no statistical difference of the rate of blood glucose control between three groups at 12 months after laparoscopic gastric bypass surgery. The blood glucose and weight obviously improved in all patients,and fasting insulin and postprandial ser-um insulin decreased at 12 months after laparoscopic gastric bypass surgery. Conclusion All the patient of type 2 diabetes mellitus with dif-ferent BMI in three group can acquire satisfying clinical therapeutic effect after the laparoscopic gastric bypass.
10.Analysis of intervention effect of improved nursing measures in patients with thoracic laparoscopy combined with esophagectomy

Objective To investigate the intervention effect of improved nursing measures in thoracoscopic laparoscopy combined with esophageal cancer resection. Methods Thirty patients with thoracoscopic and esophageal cancer resection who underwent routine nursing from June 2015 to June 2016 were selected as the control group. From July 2016 to July 2017, 34 patients with modified thoracoscopic and esophageal cancer resection were treated as study group. Time-related indicators, complication rate, preoperative anxiety, nursing satisfaction, and postoperative quality of life scores were observed. Results The time of extubation, the time of postoperative feeding, and the length of hospital stay were shorter in the study group than in the control group. The difference was statistically significant ( t=4.059, 2.490, 10.637, P<0.05). The total complication rate was lower in the study group (5.88%, 2/34) than in the control group (26.67%, 8/30), and the difference was statistically significant (χ2=5.222, P<0.05).The preoperative SAS anxiety score of the study group [(50.24±10.36) points] was lower than that of the control group [(67.31 ± 12.72) points], the nursing satisfaction [(96.82 ± 11.49) points], and the postoperative quality of life score [(80.03 ± 9.62) points] was higher than the control group [(80.03 ± 9.62) points, (77.45 ± 8.84) points], the difference was statistically significant (t=5.913, 6.290, 4.686, P<0.05). Conclusion Improved nursing measures can reduce the preoperative anxiety level of patients undergoing thoracoscopic and laparoscopic resection of esophageal cancer, improve nursing satisfaction, and help to speed up postoperative recovery and improve quality of life.
