1.Clinical classification and surgical management of palpebral bags

Objective To investigate the classification and surgical management of patients with palpebral bags. Methods A total of 200 cases with palpebral bags who underwent surgical treatment between May 2003 to August 2008 were retrospectively analyzed. The palpebral bags were classified according to skin flaccidness, bulging of excessive orbital septum fat and tarsus relaxation. The postoperative cosmetic results were also assessed. Results Of the 200 treated cases, the postoperative cosmetic results were assessed as very satisfactory in 178 (89 % ) cases, as satisfactory in 18 (9 % ) cases, and as dissatisfactory in4 (2 % ) cases. Conclusion Patients with palpebral bags should be treated with different surgical methods according to their different types.
2.Effect of Compound Tongfu Granule on Intestinal Permeability in Elderly Sepsis Patients.

OBJECTIVETo explore the effect of Compound Tongtu Granule (CTG) on intestinal permeability in elderly sepsis patients.
METHODSEighty elderly sepsis patients were randomly assigned to the experimental group and the control group by randomized double blinded method, 40 in each group. On the basis of conventional antiseptic treatment program, patients in the experimental group took CTG, while those in the control group took placebos. The dosage for CTG or placebos was 14.3 g each package, one package each time, twice daily for 14 successive days. Patients' abdominal symptoms and signs, levels of serum inflammatory factors (high-sensitivity C-reactive protein and procalcitonin), levels of plasma endotoxin, and the intestinal permeability (IP, represented by urinary lactulose/mannitol excretion rate) were compared between the two groups before and after treatment.
RESULTSAfter 14-day treatment, patients in the experimental group had improved abdominal symptoms, increased frequency of defecation, significantly decreased levels of plasma endotoxin and IP, when compared with the control group (P < 0.05).
CONCLUSIONCTG could improve the intestinal barrier function in elderly sepsis patients.

3.Study on the pathological change and the expression of neuron specific enolase in the hippocampus dentategyrus granular cell layer of epilepsy patients

Objective To find out pathologieal change and the expression of neuron specific enolage (NSE)in the hippocampus dentate gyrus granular cell layer of epilepsy patients,to investigate the relationship between the pathological change and the cause of the epilepsy.Methods The specimens of hippocampus were from 9 epilepsy patients and 20 normal persons and the pathological change were investigated under the staining of the hematoxylin and eosin staining.The expression of NSE in the hippocampns denmte gyrus granular cell layer of epilepsy patients was detected by immunohistochemistry(IHC)with specific antibody,and the rate of NSE positive nenrons was evaluated.Results The nuclear pyknosis was observed in all of hippocampus from the epilepsy patients and some neurons were swelling.The positive of NSE was showed to have yellow granule;the rate of NSE positive was 28.66%.The expression of NSE of the neurons in the hippocampus dentate gyrus granular cell layer was.significantly reduced in epilepsy patients(7.9±5.6)%.The normal neuron nuclear was big and round in the middle of the cell and the nucleolus could be seen easily.The expression of NSE of the neurons in the hippocampus dentate gyrus granular cell layer was(39.0±17.4)%.Compared with normal group,the number of neurons with yellow granule was reduced.The difference of the NSE expression rate between the two group was statistically significant(t=-5.13,P<0.01).Conclusions The result suggests that the pathological abnormality and the reduced expression of NSE on the hippocampus dentate gyrus granular cell layer neurons could be one of the main reasons of the occurring of epilepsy.
4.An investigation on the situation of disability and its influencing factors among the elderly in community

Objective To learn the status of disabled elderly in community,and to analyze the influencing factors on activities of daily living. Methods With the method of cluster stratified random sampling, a self -designed questionnaire and Modified Barthel Index (MBI)was used in investigation of survival status and activities of daily living (ADL)of the elderly in community of Sijiqing Street,Jianggan District,Hangzhou City,and the logistic regression model was used to analyze its influencing factors.Results A total of 883 valid questionnaires were completed and analyzed,and 1 91 interviewees was found to be with disability according to the disability standards with the percentage 21 . 6%.Logistic regression analysis suggested that age(OR=4. 99,95%CI:4. 52-5. 66),chronic disease situation(OR=2. 1 9,95%CI:1. 74-2. 72),stroke(OR=3. 78,95%CI:2. 65 -5. 06),osteoarthritis(OR=1. 87,95%CI:1. 55 -2. 39),chronic bronchitis(OR=2. 1 7,95%CI:1 . 73-2. 91 ),visual(OR=1 . 73,95%CI:1 . 37 -2. 28),dementia(OR=1 . 92,95%CI:1 . 23-2. 69 ),lumbocrural pain (OR =2. 04,95%CI:1 . 47 -2. 89 )were the risk factors of disability.Educational background(OR=0. 87,95%CI:0. 82-0. 95),income(OR=0. 81 ,95%CI:0. 76 -0. 87),outdoor activity(OR=0. 69, 95%CI:0. 63-0. 81 ),physical exercise(OR=0. 67,95%CI:0. 56 -0. 79)were protective factors.Conclusion The status of disabled in community of Hangzhou affected by various factors,and it is necessary to provide them health management and comprehensive intervention.
5.Mechanism of cinnamic aldehyde-inducing apoptosis of chronic myeloid leukemic cells in vitro.

The aim of this study was to investigate the apoptosis-inducing effect of cinnamic aldehyde (CA) on chronic myeloid leukemic (CML) cells and its mechanism. K562 cells and primary bone marrow mononuclear cells (MNC) from patients with CML were treated by various concentrations of CA. Flow cytometry was employed to measure the apoptosis of K562 cells and primary CML bone marrow MNC. Western blot was used to determine the expression of C-MYC and the phosphorylation of CrkL in K562 cells, and real-time polymerase chain reaction (real-time PCR) was used to quantify the expression of BCR-ABL mRNA in K562 cells. The results indicated that CA induced the apoptosis of K562 cells in a time- and dose-dependent manner. CA induced apoptosis of CML MNC dose-dependently. CA inhibited the expression of BCR-ABL mRNA and C-MYC, reduced CrkL phosphorylation levels in K562 cells. It is concluded that CA induces apoptosis of CML cells in vitro. Down-regulation of the expression and function of BCR-ABL may be one of its most important anti-leukemia mechanisms.

6.The analysis of effect of Th1/Th2 cytokine in the different prognosis in severe influenza A (H1N1).

OBJECTIVETo explore the effect of cytokine in the different prognosis of patients with severe influenza A (H1N1) infection.
METHODS28 cases with severe influenza A (H1N1) were enrolled in the study including 16 cured cases and 12 dead cases. The cytokine level in serum was detected by Luminex technology.
RESULTSThe levels of IL-2, IL-12 (P70) and IFN-gamma in dead group was lower than cured and normal control group and the difference were significant, P <0.05, respectively. IL-4 level in the dead group was significantly lower than cured group and normal control group, P value was 0.0310 and 0.0012, respectively.
CONCLUSIONSThe Thl cytokine level in the severe 2009 epidemic H1N1 influeaze cases shows decreased trend, and the trend is more obvious in dead cases. The decrease of Th1 cytokine may be one of reasons leading to severe clinical situation and related withthe bad prognosis.

7.Plastic surgical treatment for malignant soft tissue tumors and refractory wounds.

OBJECTIVETo investigate the advantages of plastic surgical treatment for malignant soft tissue tumors and refractory wounds.
METHODSAccording to the lesion type, location, and size, malignant tumors were excised including the 3 - 5 cm normal tissue around the tumor and the refractory wounds were debrided completely. The adjacent flaps or musculocutaneous flaps, or free flaps were selected to repair the defect.
RESULTSFrom 2003 to 2006, 21 cases were treated. All the flaps survived completely. The patients were followed up for 2 - 3 years with relapse in only one case with tumor. All the refractory wound healed.
CONCLUSIONSExcellent results can be achieved with plastic surgical technique for malignant soft tissue tumors and refractory wounds. Plastic surgical technique enhances the therapeutic effect of other surgical treatment.

8.A follow-up study on the pain changes trend and effects in patients diagnosed with herpes zoster in Beijing City.

Objective: To understand the changes in pain and its effects in patients with the diagnosis of herpes zoster. Methods: A total of 3 487 patients diagnosed with herpes zoster (HZ) for the first time at the outpatient department of Miyun District Hospital from January 1, 2017, to December 31, 2019, were included in the study. The information of patients was registered and issued with a record card. Patients were required to record the time of pain and rash by themselves. Telephone follow-up was conducted at 21, 90, 180 and 365 days after the onset of rashes, including hospitalization, location of rash and pain, and the time of start and end. The impact of pain on life was evaluated by the Zoster Brief Pain Inventory (ZBPI). Results: The age of 2 999 HZ patients included in the analysis were (53±16) years old, including 1 377 (45.91%) males and 1 903 (63.45%) patients aged 50 years and older. After 21 days of rash, mild, moderate and severe pain accounted for 20.87% (626 cases), 37.98% (1 139 cases) and 33.81% (1 014 cases), respectively. Only 5.07% (152 cases) had no pain or discomfort, and 2.27% (68 cases) had no pain but discomfort. Most of the pain sites were consistent with the rash sites. The chest and back and waist and abdomen were the most common, accounting for 35.58% (1 067 cases) and 29.18% (875 cases), respectively, followed by the limbs and face and neck, accounting for 16.74% (502 cases) and 16.40% (492 cases), respectively. The M (Q1, Q3) of pain days in the HZ patients was 14 (8, 20) days, and the incidence of post-herpetic neuralgia (PHN) was 6.63% (171/2 580) (excluding 419 patients who refused to visit or lost to visit on 90 days after the onset of rash). The pain score of HZ patients within 21 days after the rash was (5.19±2.73) points, and the pain score of PHN patients was (7.61±2.13) points, which was significantly higher than that of non-PHN patients [(5.04±2.69) points] (P<0.001). Daily activities, emotions, walking ability, work, social interaction, sleep and recreation were affected for 21 days after the rash in HZ patients, ranging from 60.79% to 83.83%, with sleep being the most affected (83.83%). The impact scores of pain and life dimensions in PHN patients ranged from 4.59 to 7.61 points on the ZBPI scale, which were higher than those in non-PHN patients (2.49-5.04) (t values ranged from 8.86 to 11.67, all P values <0.001). Conclusion: The proportion of pain in HZ patients after the diagnosis is high, and the pain is more obvious in patients with PHN and HZ patients aged 50 and older, which has a greater impact on their daily lives.
