1.Signal control of directional differentiation of neural stem cells

OBJECTIVE: To prove that the neural stem cell can be separated from neural tissue in adult mammalian. Stem cells can separate and proliferate continually, and can divide further into neuron and glial cell. The uptodate researches of neural stem cells suggest a prospective approach to treat neurological diseases, i.e. the dysfunctional cells may be replaced with neural stem cells by transplantation for functional reconstruction. Therefore,many kinds of directional differentiative signal controlling of neural stem cells to differentiate at a certain direction.DATA SOURCES: Using the key term neural stem cell, differentiation,signal and cell transplantation, we searched the MEDLINE database plus the relevant articles in English language from January 1995 to August 2004. Meanwhile, using the key terms mentioned above, we searched Chinese journal full-text database, Wanfang database in Chinese language from January 1995 to August 2004.STUDY SELECTION: The articles on extracellular signal of the differentiation of neural stem cell with full summarization were selected.DATA EXTRACTION: A total of 30 articles were eligible, and the relevant contents were compared and summarized. Eighteen articles were selected at last.DATA SYNTHESIS: All 18 articles contained different aspects of signal regulation of neural stem cell, including various key cellular factors and autogene regulation.CONCLUSION: Both cellular signals in vivo and in vitro can affect the cell differentiation. The vital problem to be solved is how to induce neural stem cells to differentiate at a certain direction so as to realize the aim of transplantation.
2.Role of Tissue-Type Plasminogen Activator in Neonatal Rat with Hypoxia-Ischemia Brain Damage

Objective To explore the role of tissue-type plasminogen activator(tPA) in neonatal rat with hypoxia-ischemia brain damage(HIBD).Methods Seven-day-old Wistar rat pups were used for the Vannucci model of HIBD.Reverse transcription-polymerase chain reaction(RT-PCR),terminal deoxynucleotidyl transferase mediated dUTP nick end labeling(TUNEL),double immunostaining and immunoblot analysis were adapted to determin the expression of tPA at acute phase after HIBD and neural cell apoptosis and the blood-brain-barrier(BBB) damage.Results Neonatal hypoxia-ischemia triggers persistent induction of the plasminogen system.The increase of tPA activity induced the degradation of laminin and occludin which would aggravate the BBB damage.The number of neural cell apoptosis after HIBD increased progressively with the reperfusion time.Conclusions The increase of tPA at the acute phase after HIBD can help clot to dissolve,while its extravascular proteolysis will induce cell apoptosis and BBB damage which will aggravate brain injury.
3.Determination of Deoxynivalenol in Four Traditional Chinese Herbal Medicines by LC-MS-MS

Objective:To develop a method for determining deoxynivalenol in four kinds of traditional Chinese herbal medicines. Methods:After being extracted by water, purified by an immunoaffinity column, deoxynivalenol was analyzed by LC-MS-MS. Results:The calibration curve was linear within the range of 2-50 ng·ml-1 for deoxynivalenol. The recovery was 68. 7%-88. 3%. Conclusion:The method is simple, sensitive and accurate in the determination of deoxynivalenol in Semen Ziziphi Spinosae, Semen Sojae Praepara-tum, Semen Coicis and Fructus Psoraleae.
4.Quality Standard for Anweiyang Capsules

Objective:To improve the quality standard for Anweiyang capsules. Methods: TLC was used to identify Glycyrrhiza inflata Bat. Licochalcone A was determined by RP-HPLC. Using a C18 Column, the mobile phase consisted of acetonitrile-0. 1% phos-phoric acid(40:60), and the detection wavelength was at 376 nm. Results:The herb could be identified by TLC. For licochalcone A, the linear range was from 25. 563 to 1 533. 798 ng, and the average recovery was 99. 8%(n=9). Conclusion:The method is simple and accurate, which can be used to improve and control the quality of Anweiyang capsules.
5.Analysis of long-term care service system in the United States and its implications to China

This paper summarizes the kinds of long term care facilities, service delivery organization, service model, service role and experience of long-term service system in the United States. It provides referral implications for China to explore and establish its own long-term care service system. . There is a variety of long-term care services such as short and long-term stay, adult day care, and home care services, with a gradual change from institutional services to home and community ones. Long-term care services are provided by paid both formal and mostly unpaid informal caregivers. Informal family caregivers are most frequently daughters (29. 3%), followed by spouses with (21. 2%). With the increased availability of the family and community-based services, the family caregivers are more likely to share long term care services with the paid formal ones. The United States has a well-structured long-term care service system whose access mechanism is rigorous and systematic, emphasizing the evaluation of service quality and effectiveness. Moreover, the U. S. long term care service system pays more attention on elders' wishes, respecting their preferences and rights of choice on the services. Based on the experience of the United States, China should primarily focuses on establishing a family and community-based long term care service system with a variety of service delivery facilities, give a full play to the community health facilities, social partners, and families. The Chi-nese government should not only give a focus to the service institutions and content diversification, but should as well establish a sound and detailed assessment and evaluation standard system to access the services and assure the effec-tiveness of service quality with a humanized concern.
6.Perioperative Management of Fracture with Hemophilia A

Objective To study the perioperative treatment of the patients having fracture with hemophilia A. Methods Seven male hemophilia A patients with fractures were treated in our hospital from Jan. 1988 to Apr. 2003. All the patients received coagulation factor replacement treatment. The plasma level of factor Ⅷ was measured at preoperation, intraoperation and postoperation, fracture was treated by internal fixation, and the mean follow-up period was 16 months. Results In this series there were 1 severe, 2 moderate, 2 mild, and 2 pre-clinic hemophilia A patients, all patients had bone union during the follow-up, and no complications such as hematoma, infection and loosening or breaking of internal fixation happened. Conclusion The surgical treatment of fractures with hemophilia A can obtain excellent clinical outcome throug careful perioperative preparation.
7.International comparative study on high-value drugs payment system and its implications for China

Objective:The aim of the present study was to conduct an international comparison of the high-val-ue drugs payment systems of some selected countries, and introduce them to the Chinese context. Australia, America and Germany's high-value drugs payment systems were selected to serve as references for China. Methods: The main content and characteristics of three countries' payment systems were analyzed in four aspects:expenditure copayment mode, paid-scope selection, drug price negotiation, and drug rational use management which were then compared with the Chinese status. Results:Through the difference in healthcare system tradition, the above countries have vari-ous copayment mode, but they all select paid-drug by cost-effectiveness analysis with corresponding control measures. Conclusions:China should: establish a co-payment mode for the high-value drugs on the basis of a critical illness medical insurance found;build a national pharmaco-economic evaluation index system and improve the national nego-tiation superiority, while emphasizing on risk-sharing mechanism; and make series specifications to guarantee the drug to be rationally used.

9.Mindfulness-based therapy combined with risperidone treatment for emotional disorder in schizophrenic patients during the recovery period

Objective:To investigate the clinical effect of mindfulness-based therapy combined with risperidone treatment on emotional disorder in schizophrenic patients during the recovery period.Methods:A total of 106 schizophrenic patients during the recovery period who received treatment from March 2017 to February 2019 in The Second Hospital of Jinhua,China were included in this study.They were randomly assigned to undergo mindfulness-based therapy combined with risperidone treatment alone (study group, n = 53) or risperidone treatment (control group, n = 53).The Positive and Negative Syndrome Scale (PANSS) score and Scale of Social Function in Psychosis Inpatients (SSPI) score were compared between study and control groups before and after treatment. Results:Before treatment,there were no significant differences in PANSS total score,negative scale score,positive scale score,general psychopathology scale score between study and control groups (all P > 0.05).After treatment,negative scale score,positive scale score,general psychopathology scale score and PANSS total score in the study group were (11.54 ± 1.63) points,(11.56 ± 1.61) points,(21.71 ± 1.85) points,(44.66 ± 3.01) points respectively,and they were (15.31 ± 1.91) points,(15.29 ± 1.89) points,(23.51 ± 2.11) points,(52.06 ± 3.32) points,respectively in the control group.After treatment,negative scale score,positive scale score,general psychopathology scale score and PANSS total score in each group were significantly lower than those before treatment (study group: t = 10.352,10.337,8.419,15.630; control group: t = 2.607,2.601,4.534,7.131,all P < 0.05).After treatment,negative scale score,positive scale score,general psychopathology scale score and PANSS total score in the study group were significantly lower than those in the control group ( t = 10.930,10.937,4.670,12.022,all P < 0.05).Before treatment,there were no significant differences in scores of activities of daily living,social skills and initiative and communication between the two groups (all P > 0.05). After treatment,scores of social skills,activities of daily living and initiative and communication in the study group were (12.88 ± 1.62) points,(10.58 ± 1.54) points,(13.14 ± 1.97) points respectively,and they were (11.92 ± 1.56) points,(9.01 ± 1.43) points,and (11.46 ± 1.69) points respectively in the control group.After treatment,scores of social skills,activities of daily living and initiative and communication in each group were significantly higher than those before treatment (study group: t = 6.783,5.475,4.430; control group: t = 3.956,2.263,2.009,all P < 0.05).After treatment,scores of social skills,activities of daily living and initiative and communication in the study group were significantly higher than those in the control group ( t = 3.108,5.439,4.712,all P < 0.05). Conclusion:Mindfulness-based therapy combined with risperidone treatment for emotional disorder in schizophrenic patients during the recovery period can effectively improve the emotional disorder,reduce the schizophrenia symptoms,and improve the social function of patients.
10.The expression of HER-2 and its prognostic implication in patients with non-small cell lung cancer

Background and purpose:The prognostic role of human epidermal growth factor receptor 2 (HER-2) remains controversial in patients with non-small cell lung cancer (NSCLC). This study aimed to evaluate the expression of HER-2 and its prognostic implication in patients with NSCLC.Methods:Four hundred and twenty-ifve NSCLC patients’ specimens were obtained from Zhejiang Cancer Hospital. HER-2 protein expression was determined by immunohistochemistry (IHC). The relationship between HER-2 and clinicopathological parameters and patients’ prognosis was analyzed by Chi-square test and COX proportional hazards regression model.Results:Eighty-three patients were positive expression of HER-2 (83/425, 19.5%). The expression of HER-2 was closely related with histological type (P=0.051), most patients with HER-2 positive were adenocarcinoma, then were squamous carcinoma or other histological types. However, HER-2 was not an independent prognostic indicator for the overall survival of patients with NSCLC.Conclusion:We found that the expression of HER-2 was closely connected with histological type, but it was not a predictive marker for the prognosis in patients with NSCLC.
