2.Application of Defibrillation Techniques in Cardiopulmonary Resuscitation

Objective To investigate the techniques of electrical defibrillation and their application to cardiopulmonary re-suscitation(CPR).Methods On the basis of reviewing the definition of electrical defibrillation,the main defibrillation tech-niques and their developments were introduced,as well as the selection of shock energy.The role that defibrillation played during CPR was defined according to the consensus.Results As an unfashionable technique,monophasic waveforms was inclined to be replaced by present biphasic waveform including biphasic truncated exponential(BTE) waveform and recti-linear biphasic waveform(RBW).The selected shock energy varied with the waveform that a defibrillator made use of,i.e.a monophasic defibrillator required a higher energy level as usual,whereas a biphasic one did not.It was deemed that biphasic technique presented an excellence in less damage to heart in addition to higher rate of successful defibrillation.In the critical care for sudden cardiac arrest with a shockable rhythm,the protocol of "1 shock + 5 cycles of CPR" should be recommended.Conclusion Electrical defibrillation techniques consist mainly of monophasic waveform and biphasic wave-form,between which the latter gains an advantage over the former.On the grounds of modern emergency medicine,defib-rillation has to be performed in combination with CPR.
3.Prospective Application of Automated External Defibrillator in Inferior Troops

4.Clinical observation of IMRT therapy combined with astragalus polysaccharide injection on cervicoce-rebral tumor treatment

Objective To observe the effect of IMRT combined with astragalus polysaccharide (ASP) injection on cervicocerebral tumor,radiation side effects and its influence on immune function. Methods 61 head and neck tumor patients were randomly divided into two groups.One groups received the IMRT combined with Astragalus Polysaccharide (ASP) injection (n=30) while the other group only received the IMRT(n=31). The recent effect, immune function and the side effects of radiotherapy between the 2 groups were observed and compared , changes of T-lymphocyte subsets, natural killer (NK) cell activity and the dermatitis,oral mucosa reaction,thirsty condition on patients in intraoperative or postoperative period were also observed and compared. Results The total effective rate in experimental group and the control group were 83.0% and 80.6% respectively.The cell viability of peripheral blood lymphocyte and NK cell as well as the T3、T4、NK cell value increased in IMRT-ASP group (P<0.05) after treatment. While T3、T4、NK cell value in peripheral blood decreased (P<0.05). Mucosa,dermatitis and salivary gland damage showed in IMRT-ASP group were significantly better than in control group after 3 weeks treatment (P<0.05). Conclusion ASP can improve immune function effect in head and neck tumor patients during RT, and can also eliminate the side effects of radiotherapy.
5.Plasma and Skin Tissue Fluid Levels of beta-endorphin in Patients with Vitiligo

Objective To study the role of beta endorphin (?-EP) in the pathogenesis of vitiligo. Methods Radioimmunoassay (RIA) was used to measure the levels of ?-EP in plasma from 40 patients and tissue fluid of lesional and non lesional skin from 33 patients with vitiligo. Results Plasma levels of ?-EP were significantly higher in patients with generalized(11.74?3.47 pmol/L), local(8.31?2.57) and segmental(8.61?2.61) vitiligo than those in normal controls(6.69?1.46). Tissue fluid levels of ?-EP were significantly higher in lesional skin (9.25?4.49 and 10.24?4.37) than those in non lesional skin (5.50?2.84 and 6.12?1.61) from patients with local and segmental vitiligo. Increased levels of ?-EP were observed in tissue fluid from both lesional and non lesional skin in patients with generalized vitiligo, however, no significant difference of ?-EP levels was found between the two kinds of skin tissue. Conclusion It is suggested that ?-EP might play a role in the pathogenesis of vitiligo.
6.Application of "Three-step training and Two-part Examination" in Utilization and Training of Defibrillator

Objective To research the applicable ways on training and examination of defibrillator utilization in grassroots units. Methods The use of the "three-step training and two-part examination" mode, that is, three-step training(theoretical instruction, grouped practice and animal experiment), a total of 83 medical officers from inferior troops of PLA were trained for the use of defibrillators, and through passing two-part test(written test and skill test), the trainees were subsequently examined so that the efficacy of the training could be evaluated. Results The trainees can be acquired satisfying achievement in the two-part test after having passed the three-step training. The respectively average scores were 86.7 ?15.9 (written test) and 90.1?14.8 (skill test). Conclusion The "three-step training and two-part test" mode is a successful defib- rillators' training mode for inferior troops.
7.Introduction of foreign hemostatic tools in tactical combat casualty care

The application of four kinds of hemostatic tools in tactical combat casualty care (TCCC) of foreign armies was introduced,including extremity tourniquet,junctional tourniquet,hemostatic dressing.and elastic bandage as well as tranexamic acid.The above hemostatic tools were described from the aspects of structure,operating process,application range and technical points,and the histories of some tourniquet were also expounded.The problems of Chinese PLA were analyzed in designing and developing hemostatic tool.Some suggestions were put forward such as enhancing the practicability and portability of hemostatic tool,rational allocation of TCCC drugs and instruments and etc.
8.Ultrastructure changes of osteosarcoma cells inactivated by alcohol in different concentrations

Objective:To study the ultrastructure changes of osteosarcoma cells(OS-732) inactivated by alcohol of two different concentrations and clinical application value.Methods:Osteosarcoma cells(OS-732) were inactivated by 75% and 95% alcohol and observed by light and electron microscope.And its viability was detected by MTT method. Results:Cell internal structure changed significantly and irreversible damage formed. MTT method proved that inactivated cells had no viability.Conclusion:These two inactivation methods were effective. Cell internal enviroment was damaged very seriously and cell was led to death. These two inactivation methods could provide choices for clinical limb protective operation.

Leucine zipper tumor suppressor 2 (LZTS2) is a novel tumor suppressor gene that has been increasingly recognized in recent years. Currently, many studies illustrate that LZTS2 gene, the important candidate tumor suppressor gene, is already involved in the inhibition of tumorigenesis and aberrant proliferation of tumor cells, and other functions of tumor cells. Information from these stud-ies can contribute to the formulation of new strategies for the treatment of tumors.
10.The clinical value of diffusion tensor imaging for predicting the recovery of walking ability after middle cerebral artery infarction

Objective:To explore the changes in the diffusion tensor imaging (DTI) parameters of the pontocerebellar tract 3 months and one year after ischemic stroke and analyze the data′s potential for predicting long-term motor outcomes.Methods:Thirty patients with middle cerebral artery infarction were prospectively studied using DTI within 3 months and 1 year after the onset. A region of interest-based analysis was performed for the fractional anisotropy (FA) of the middle cerebellar peduncles (MCP) in the pontocerebellar fibers (PCF). Neurological functioning was evaluated using the National Institutes of Health Stroke Scale and the degree of paresis was assessed at the same time using paresis grading. At one year after the onset, balance function, functional prognosis and self-care ability were evaluated using the Brunel Balance Scale, the modified Rankine Scale and the Functional Independence Scale respectively.Results:The average FA on the healthy side 3 months and 1 year after onset was significantly less than on the affected side at each time point. The ratio of the FA of the affected side to that of the unaffected side (rFA) in the PCF correlated significantly with the average paresis scores of the lower and upper extremity and the total paresis score at each time point. Moreover, the rFA of the PCF was significantly correlated with the average functional independence score, the prognosis for the upper and lower extremities as well as motor functioning. The area ratio under the ROC curve of the PCF for predicting lower extremity motor outcome was 0.84, and the optimal threshold was 0.92 (sensitivity 71.4%, specificity 73.9%). The area ratio under the ROC curve was not a significant predictor of upper extremity motor outcome.Conclusion:The DTI parameter rFA of the PCF in patients with subacute middle cerebral artery infarction can help to predict the long-term recovery of motor ability. It can serve as an important reference index for predicting the long-term motor ability of the lower limbs.
