1.Advance in Quality of Life of Patients with Chronic Heart Failure and Primary Caregivers (review)

Chronic heart failure is the end stage of different cardiovascular diseases and usually has a high readmission rate and mortality, and the current optimal treatment is dissatisfied. The quality of life of patients with chronic heart failure and their primary caregivers is at a medium-to-low level, especially in physical, psychological and social dimensions. Furthermore, chronic heart failure patients and their primary caregivers usually suffered from a relatively higher degree of anxiety and depression, which further impaired their quality of life. The quality of life about chronic heart failure patients could be improved by using accurate drug therapy, cardiac resynchronization therapy, implantable cardioverter defibrillator and proper exercise rehabilitation according to the recent studies.
2.Chemical constituents of steroidal saponins in rhizome of Paris polyphylla var. yunnanensis cultured in vitro.

In this study, liquid chromatography-mass spectrometry(LC-MS) and high performance liquid chromatography(HPLC) were employed for qualitative and quantitative analysis of the steroidal saponins in rhizomes of Paris polyphylla var. yunnanensis from three different habitats cultured in vitro, in an attempt to explore whether the rhizomes of the medicinal herb cultured in vitro can synthesize the steroidal saponins, including polyphyllinsⅠ, Ⅱ, and Ⅶ, the quality markers specified in Chinese Pharmacopoeia(2020 edition). A total of 20 steroidal saponins were identified in the rhizomes from Changxin, Yunlong(S1), Fengyi, Dali(S2), and Niujie, Eryuan(S3): parisyunnanoside A and parisyunnanoside D or E, proto-polyphyllin Ⅱ, polyphyllins G and H, polyphyllinsⅠ, Ⅱ, Ⅴ, Ⅵ, and Ⅶ, dioscin, gracillin, prosapogenin A, Tg, isomer of Th, saponin Th, reclinatoside, proto-pairs D, pseudoproto-dioscin, and 23-O-glc-(23S,25R)-spirost-5-en-3β,23α,27-triol-3-O-rha-(1→2)-[ara(1→4)]-glc or 27-O-glc-(23S,25R)-spirost-5-en-3β,27α-diol-3-O-rha-(1→2)-[ara(1→4)]-glc. Among them, polyphyllinsⅠ, Ⅱ, and Ⅶ were detected in the rhizomes from S1, with the mass fraction of 0.109 1%, 0.165 2%, and 0.051 03%, respectively(total 0.325 3%). Polyphyllins Ⅱ and Ⅶ were identified in the rhizomes from S2 with the respective mass fraction of 0.192 2% and 0.074 23% and total content of 0.266 5%. Moreover, polyphyllins Ⅱ and Ⅶ were also found in the rhizomes from S3, which had the mass fraction of 0.207 7% and 0.186 9%, separately, with the total content of 0.394 6%. Thus, steroidal saponins, including the quality makers polyphyllins Ⅰ, Ⅱ, and Ⅶ recorded in Chinese Pharmacopoeia(2020 edition) can be synthesized in rhizomes of Paris polyphylla var. yunnanensis cultured in vitro, but their total content fails to meet the standard(0.60% in Chinese Pharmacopoeia). Therefore, in vitro culture of the Paris polyphylla var. yunnanensis is feasible, but the culture conditions need to be further improved.

3.Test-retest Reliability and Validity of 3-dimensional Digital Goniometer for Cervical Range of Motion

Objective To evaluate the test-retest reliability and validity of the 3-dimensional Digital Goniometer for Cervical (3DDGC) in measurement of cervical range of motion. Methods 39 healthy participants were measured of cervical range of motion 2 times in 1 hour with 3DDGC by one observer, and with cervical range of motion (CROM) device once. The intraclass correlation coefficient (ICC) of test-retest and the Pearson correlation coefficient between devices were caculated. The measurement errors were evaluated with standard er-ror of mean (SEM). Results The ICC was 0.89 of 3DDGC as the cervical rotation to the left, and it was 0.90-0.98 of the other directions, with the SEM of 2.07-3.85° . The Pearson correlation coefficient was 0.73-0.92, with the SEM of 1.66-3.17° . Conclusion 3DDGC is valid and reliable in test-retest in measuring cervical range of motion, which need more research clinically.
4.Study progress of malignant tracheoesophageal fistula

Malignant tracheoesophageal fistula (MTEF) is pathological communication between the respiratory tracts such as the trachea or bronchia and the esophagus because of malignant tumor dissemination through them.Radiography is an important adjunctive technology in the diagnosis of MTEF,and the location and size of fistula often need the further diagnosis of bronchoscopy and esophagoscopy.The patients are often with an unfavourable prognosis once developed MTEF,and are treated usually with the aim of symptom palliation and life quality improvement.The individual treatment includes esophageal stenting,esophageal exclusion and esophagus bypass,fistula exclusion and repair,radiotherapy and others effective therapy according to the patients condition.These therapies will prolong the life span and improve the life quality of patients.MTEF is not absolute contraindication for chemoradiotherapy.Despite its acute toxicity,this concurrent chemoradiothe rapy protocol appears feasible and effective at closing esophageal malignant fistula,especially in patients in a good general condition and without metastasis.
5. Identification of Gentiana rhodantha and Its Adulterants by DNA Barcoding Coupled with High Resolution Melting Analysis

OBJECTIVE: To apply DNA barcoding coupled with high resolution melting analysis to distinguish Gentiana rhodantha from its adulterants. METHODS: The internal transcribed spacer 2 (ITS2) barcode was selected for HRM analysis to produce standard melting profile of the selected species. RESULTS: The ITS2 molecular regions coupled with HRM analysis can effectively differentiate five herbal species, including two Gentiana rhodantha and their four common adulterants. CONCLUSION: DNA barcoding coupled with HRM analysis is a accurate, reliable, rapid, cost-effective and robust tool, which may contribute to the quality control of traditional Chinese medicine in the natural health product industry.
6.Research progress on immunological properties of ESAT-6 secreted by Mycobacterium tuberculosis and its role in new vaccines.

Early secreted antigenic target of 6 kDa protein (ESAT-6) is the major virulence factor of Mycobacterium tuberculosis (MTB), which can resist the clearance of MTB in bodies by inhibiting macrophage phagocytosis and autophagy reaction, thus impeding the immune defense function of the body against MTB infection. In addition, ESAT-6-induced apoptosis of macrophage and massive necrosis of innate immune cells can foster MTB proliferation and colonization, leading to systemic MTB infection. Moreover, ESAT-6 hampers the protective immune response of Th1 cells, reducing the secretion of pro-inflammatory cytokines and contributing to immune dysfunction, thus accelerating the course of MTB infection. During the process, the high immunogenicity of ESAT-6 can be leveraged as a dominant antigen in the development of new TB vaccines, making it a promising candidate with broad prospects for further development.

7.Improvement Effects of Bee Venom Plastics on Experimental Cerebral Thrombosis in Rats

OBJECTIVE: To study the improvement effects of Bee venom(BV) plastics on experimental cerebral thrombosis in rats. METHODS: Totally 96 SD rats were randomly divided into sham operation group (normal saline), model group (plastics blank matrix), Nimodipine group (positive drug, 4.00 mg/kg) and BV plastics low-dose, medium-dose, high-dose groups (1.67, 3.33, 6.67 mg/kg), with 16 rats in each group. Rats in sham operation group and Nimodipine group were given medicine intragastrically, while rats in model group and BV plastics groups were given medicine by transdermal smearing. After 5 days of continuous administration, the experimental cerebral thrombosis model was established by ligating the right external carotid artery and pterygomandibular artery, and injecting compound thrombus inducer into the internal carotid artery. The wet mass ratio of right brain to left brain was measured to investigate the degree of brain edema on the infarcted side. The content of Evans blue (EB) in the left and right hemispheres of rats was determined by ultraviolet spectrophotometry to investigate the cerebral vascular permeability. Blood rheology and coagulation function indicators of rats were measured. The pathological changes of brain tissue in rats were observed by HE staining, and the number of survival neuron cells was counted. RESULTS: Compared with the indexes of sham operation group, the cerebral thrombosis model was established successfully. Compared with model group, the area of blue staining in the right brain (infarcted side) of rats in BV plastics groups was significantly reduced, and the right brain/left brain wet mass ratio and the content of EB in the right brain tissue were significantly reduced (P<0.05 or P<0.01). The whole blood viscosity and Casson viscosity of rats in BV plastics groups, and the plasma viscosity of rats in BV plastics medium-dose and high-dose groups decreased significantly (P<0.01). PT and APTT of rats were prolonged significantly in BV plastics medium-dose group (P<0.01). The pathological changes of brain tissue in rats in BV plastics groups were significantly alleviated. The arrangement of neuron cells was more orderly, the shape and structure of cells were clear, the nucleolus was clear, the membrane was intact, and the number of survival neuron cells was significantly increased (P<0.01). CONCLUSIONS: BV plastics can alleviate brain edema, inhibit cerebral vascular permeability, improve hemorheology and coagulation function indicators of rats after the formation of cerebral thrombosis, and alleviate nerve cell injury after ischemia.
8. A Comparative Study on the Fingerprint of Paris polyphylla var. Polyphylla and Paris polyphylla var. yunnanensis from Different Origins in Dali and Seven Main Steroidal Saponins by HPLC

OBJECTIVE: To study the fingerprints of rhizomes of Paris polyphylla Smith var. polyphylla and Paris polyphylla Smith var. yunnanensis (Franch.) Hand. -Mazz from different origins in Dali and the differences of seven main steroidal saponins. METHODS: The fingerprints of rhizomes of Paris polyphylla var. polyphylla and Paris polyphylla var. yunnanensis from different origins in Dali were established by HPLC. The similarity of fingerprints and seven main steroidal saponins were compared and analyzed. RESULTS: There were 14 common peaks in the fingerprints of rhizomes of Paris polyphylla var. polyphylla from different origins in Dali, and the similarity of the fingerprints of the samples from different habitats except Jinhua Weishan and Longjie Weishan was greater than 0.9. There were 13 common peaks in the fingerprints of rhizomes of Paris polyphylla var. yunnanensis from different origins in Dali, and the similarity of the samples from different origins was greater than 0.92. There were 11 common peaks in the fingerprints of rhizomes of Paris polyphylla var. polyphylla and Paris polyphylla var. yunnanensis from different origins in Dali, and the similarity of the fingerprints was very low, between 0.057 and 0.225. Polyphyllin Ⅰ, polyphyllin Ⅱ, polyphyllin Ⅵ and polyphyllin Ⅶ are detected in the rhizomes of Paris polyphylla var. polyphylla and Paris polyphylla var. yunnanensis from different origins in Dali. The average peak areas of polyphyllin Ⅰ, polyphyllin Ⅱ, polyphyllin Ⅵ and polyphyllin Ⅶ in Paris polyphylla var. yunnanensis from different origins in Dali were higher than those of Paris polyphylla var. polyphylla. Polyphyllin H was detected in the rhizomes of Paris polyphylla var. yunnanensis from Leqiu Nanjian, Jinhua Weishan, Fengyu Eryuan, Dengchuan Eryuan, Hongyan Midu, Xiyi Heqing and the rhizomes of Paris polyphylla var. polyphylla from Wanqiao Dali, Fengyi Dali, Xiyi Heqing, Hongyan Midu and Leqiu Nanjian. Polyphyllin Ⅲ was detected in the rhizomes of Paris polyphylla var. polyphylla from the origins except Longjie Weishan. Polyphyllin Ⅴ was also detected in the rhizomes of Paris polyphylla var. polyphylla in Fengyu Eryuan and Xiyi Heqing. CONCLUSION: Polyphyllin Ⅰ, polyphyllin Ⅱ, polyphyllin Ⅵ and polyphyllin Ⅶ are detected in the rhizomes of Paris polyphylla var. polyphylla and Paris polyphylla var. yunnanensis from different origins in Dali. The similarity of the HPLC fingerprints of the rhizomes of Paris polyphylla var. polyphylla and Paris polyphylla var. yunnanensis is very low, and there are great differences in seven main steroidal saponins. The contents of polyphyllin Ⅰ, polyphyllin Ⅱ, polyphyllin Ⅵ and polyphyllin Ⅴ in Paris polyphylla var. yunnanensis are higher than those in Paris var. polyphylla.

OBJECTIVE To prepare spider fibroin membrane loaded with Periplaneta americana extract, and investigate its characterization, in vitro drug release property and cytotoxicity. METHODS Using natural spider silk collected from Chilobrachys guangxiensis as raw material, P. americana extract as model drug, the drug-loaded spider fibroin membrane (hereinafter referred to as drug-loaded membrane) was prepared by solvent casting method. The material matrix spider fibroin membrane without P. americana extract (hereinafter referred to as blank membrane) was prepared with same method. The membrane structure was characterized by static water contact angle, Fourier infrared chromatography, X-ray diffraction and scanning electron microscopy from different angles; drug release characteristics in artificial saliva were simulated in vitro to evaluate the drug sustained-release performance. MTT assay was adopted to validate the cytotoxicity of drug-loaded membrane. RESULTS The drug-loaded membrane was prepared, and the static water contact angle was less than 90°, which was less than that of blank membrane. The drug-loaded membrane showed the characteristic absorption peak to polypeptide of P. americana extract at 1 500-1 700 cm-1. X-ray diffraction and scanning electron microscopy also proved that the drug was successfully loaded into the pellicle. The release time of the pellicle in artificial saliva was more than 200 min. The MTT test results showed that the cell proliferation rates of blank membrane and drug-loaded membrane were 84.6% and 79.4% (both greater than 70%), respectively, without significant potential cytotoxicity. CONCLUSIONS Drug-loaded membrane prepared with natural spider silk has a certain sustained-release effect in artificial saliva, which can be further developed as a drug sustained-release carrier with excellent biological characteristics and biocompatibility.
