1.Septum strut implantation for rhinoplasty simultaneous with cleft alveolar bone grafting in cleft lip and palate patients

Objective To pursue a more efficient and effective treatment for cleft lip and palate deformities. Methods Patients with unilateral complete cleft lip and palate at their age of 9 years after were chosen for simultaneous cleft alveolar repair and nasal deformity correction. Muco-periosteal pocket and iliacgranular bone was prepared, and bone grafting was performed conventionally. At the same time of iliac cancellous bone harvesting, a cortical plate was taken and sculpted into a strut of 18 mm in length, 6 mm in width, and 1.5 mm in thickness. A flying bird incision was made at the alar ram and across columella in a V-shape. Then the alar cartilage was detached from the overlying skin, a socket was made at the site of anterior nasal spine. The strut was inserted into the socket between the two medial crura of the alar cartilage. The medial crura was lift 3 mm above the superior edge of the strut, and mattress suture technique was used to secure the bilateral medial crura to the strut graft. Results 24 patients were treated by this technique. All the patients healed uneventfully. Depressed alar base, tilted columella and lower nasal tip were corrected satisfactorily. Conclusion There is no interference in simultaneous cleft alveolar bone grafting and rhinoplasty. Septum strut can provide favorable support for tilted nasal structure and satisfactorily correct nasal deformities. Simultaneous with alveolar grafting, it is much easier in harvesting, and the time of anesthesia and operation is also decreased.[
2.Application value of anti-CCP antibody ,RF ,ESR and CRP detection in diagnosis of RA

Objective To investigate the changes of anti-cyclic citrullinated peptide antibody ,rheumatoid factor (RF) ,erythrocyte sedimentation rate(ESR) and C reactive protein(CRP) levels in the patients with early rheumatoid arthritis (RA) and their applica-tion value in the diagnosis and treatment.Methods A total of 210 cases of early RA in sixth affiliated hospital of Guangzhou Medi-cal University from December 2015 to June 2016 were selected as the RA group ,including 28 males and 182 females ,at the same time 115 cases of other rheumatism were selected as the non-RA group ,male ,including 38 males and 77 females.85 persons under-going the healthy physical examination were taken as the health group ,including 46 males and 39 females.The latex enhanced im-mune turbidimetry was adopted to quantitatively detect anti-cyclic citrullinated peptide antibody.The RF and CRP levels were quan-titatively detected by using the immune turbidimetry.The Westergren method was used to test the ESR ,their content changes in RA ,non-RA arthritis and other autoimmune diseases were detected.Results The sensitivity ,specificity ,positive predictive value , negative predictive value and Youden index of anti-cyclic citrullinated peptide antibody in the RA group were 60.95% ,97.64% , 98.46% ,50.30% and 0.59 respectively ,which of RF test were 52.38% ,95.29% ,96.49% ,44.75% and 0.48 respectively ,which of CRP test were 30.48% ,89.41% ,87.67% ,87.67% ,34.23% and 0.20 respectively ,which of ESR test were 49.52% ,87.06% , 90.43% ,41.11% and 0.37 respectively.Anti-cyclic citrullinated peptide antibody detection in the RA group was obviously higher than that of the RF test ,their test results were obviously higher than those of the non-RA group and healthy group ,the differences were statistically significant (P<0.05).CRP and ESR detection had no statistical difference between the RA group and non-RA group (P>0.05).Conclusion Anti-cyclic citrullinated peptide antibody level is higher in the patients with RA ,which is signifi-cantly superior to the RF test ,and anti-cyclic citrullinated peptide antibody detection has higher value for early diagnosis of RA.
3.Effect of preoperative regional intra-arterial chemotherapy on serum vascular endothelial growth factor and endotatin levels in patients with colorectal carcinoma

Objective To determine serum vascular endothelial growth factor (s-VEGF) and serum endostatin (s-endostatin) levels in patients with colorectal cancer after preoperative regional intraarterial chemotherapy (PRAC). Methods Peripheral venous blood was sampled preoperatively and postoperatively, and was assayed for VEGF and endostatin by ELISA. Results Patients' preoperative level of s-VEGF and s-endostatin elevated significantly, which was in close correlation with Dukes' stage. The level of s-VEGF and s-endostatin before chemotherapy elevated significantly compared with that on day 7 after chemotherapy and on postoperational day 1 and 14 after operation. In patients undergoing palliative resection, only the level of s-endostatin before chemotherapy was significantly higher than that on day 1 after operation. Conclusions PRAC, inhibiting angiogenesis, is of antitumor effect, the decrease of s-endostatin after chemotherapy may suggest the combination of chemotherapy and antiangiogenesis for a better antitumor effect.
4.Genotyping of Japanese encephalitis viruses isolated in Yunnan.

OBJECTIVEExplore the genotype of the epidemic Japanese encephalitis virus (JEV) in Yunnan Province from molecular level and to understand the molecular differences of the virus isolated from Yunnan at different time, locality and host.
METHODSThree-day suckling mice were inoculated with viruses continuously and when the disease developed and the mice were dying, the brain was taken and DNA was extracted from the supernatants of the brain after grinding; then the gene fragments of Prm-C region were amplified by RT-PCR. The viral gene sequences were compared with those of other 72 strains of JEV originated from both China and abroad at different times. Finally the genotypes were analyzed with the method which was established by Woan-Ru Chen.
RESULTSAll the 3-day suckling mice which were inoculated with the virus died within 78 h. The results of the nucleic acid-sequence analysis showed that 17 strains of the experimental virus belonged to genotype 1 and 2 strains belonged to genotype 3. The difference between genotype 1 and type 3 were more than 15%. While the difference between 17 strains of genotype 1 which were separated at different time, location and hosts were only 3.8%-5.2%.
CONCLUSIONThe above results suggest that the genotype of the epidemic JEV in Yunnan Province are type 1 and 3 and the latter is the main type.

5.Cystic tumour of the atrioventricular node: a case report and review of the literature.

Cystic tumour of the atrioventricular node is a rare primary cardiac tumour that can cause complete heart block and sudden death. Here, we describe a male case aged 42 years who suddenly died without a medical and family history of cardiac illnesses. After detailed macroscopic and microscopic examinations, a cystic mass was found in the atrioventricular nodal region. The small lesion was less than 1 cm in diameter, and consisted of small and large cystic spaces and tubular structures lined by flat, cuboidal or squamous epithelium. Immunohistochemical staining revealed the tumour epithelium positive for epithelial membrane antigen, carcinoembryonic antigen, antigen epitopes AE1/AE3, cytokeratins CK5/6 and CK7, but negative for calretinin, HBME-1, Wilms' tumor 1, factor VIII, chromogranin, synaptophysin or smooth muscle actin, suggesting an endodermal rather than mesothelial origin.

6.Studies on chemical constituents from the stems of Schisandra sphaerandra.

OBJECTIVETo investigate the chemical constituents from the stems of Schisandra sphaerandra.
METHODCompounds were isolated from ethanolic extract of the titled herb by silica gel column chromatography, and their structures were elucidated by physical and chemical evidences and spectroscopic analysis.
RESULT12 compounds were obtained and identified as wuweizisu C (1), ganwuweizic acid (2), nigranoic acid (3), catechin (4), 2 alpha,24-dihytroxyursolic acid. (5), 3 beta-O-acetylursolic acid (6), ursolic acid (7), slyceryl 26-hydroxyhexacosanoate (8), slyceryl hexacosanoate (9), fat acids (10), beta-sitosterol (11), daucosterol (12), respectirely.
CONCLUSIONThree pentacyclic triterpene carboxylic acid (5-7) were isolated from Schisandreae for the first time.

7.Molecular characteristics of the full-length genomes of Japanese encephalitis virus strains newly isolated in 2009, China.

To conduct sequencing of full-length genomes of two Japanese encephalitis virus strains (JEV) newly isolated in 2009 in China and analyze the characteristics of complete nucleotide sequences. The complete genomic sequences were obtained by RT-PCR and sequencing directly. Bioinformatics was used to analyze the nucleic acid data, deduced amino acid sequence and phylogenetic trees. The result of sequence analysis showed that the genomes of YN0911 and YN0967 strains were both 10965nt in length, which coded 3432 amino acid polyprotein. The homology of genome ranged from 83.3% to 98.9% in nt and from 94.8% to 99.7% in aa, respectively, when compared with selected JEV strains in GenBank. There were 13 amino acid divergences which were not the key virulence sites in E protein when compared with vaccine strain SA14-14-2. There were 11nt deletions in the 3' UTR region. Phylogenetic analyses based on C/ PrM, E gene and full-length genome all showed that YN0911 and YN0967 strains belonged to genotype I. The result also showed that two new JEVs had close phylogenetic relationship with the strains from Viet Nam, Sichuan Province, Guizhou Province, Guangxi Province, China. This study indicated that JEV strains newly isolated in 2009 in China were the members of JEV genotype I. The key virulence sites in E protein did not change.

8. The Clinical application of recontruction of sideburns aesthetic subunit with local flaps

Objective:
To explore the clinical application and effect of sideburns aesthetic reconstruction with local flaps.
Methods:
Between June 2012 and June 2017, there were 12 cases of patients with Benign tumor which is in the sideburn , including 7 Male patients, 5 female patients, thier ages are from 37 to 68 years old.The tumor size ranged from 0.5 cm×1.0 cm to 1.5 cm×2.5 cm, and the defects, size ranged from 1.0 cm× 1.2 cm to 1.5 cm× 3.0 cm. Single leaf flaps, double leaves flaps or local flaps which are on the ipsilateral sideburn or in the hairy area behind the ear are designed for sideburns reconstruction according to the size and shape of the defect, The flaps size ranged from 1.5 cm× 2.5 cm to 1.8 cm× 3.5 cm.
Results:
All flaps with 12 cases of patients survived completely with no flap blood supply obstacles, and the incision healed well, all patients were followed up for 6-24 months with no recurrence of the tumor , and the incision scars are hidden in the sideburns, And the sideburn showed satisfactory effect of aesthetic reconstruction with good symmetric cosmetic effect.
Conclusions
The local flaps have less injury in the surgery area , simple surgical approach, hidden incision scars, reliable blood supply and high survival rate, which do not affect the sideburns symmetry.Aesthetic result can be achieved by local flaps in the reconstruction of defect after resection of benign tumor at sideburn.
9. Advanced gourd-shaped flap with frontal branch of superficial temporal artery for defects after resection of per-auricular tumor

Objective:
To investigate the therapeutic effect of advanced gourd-shaped flap with frontal branch of superficial temporal artery for defects after resection of per-auricular tumor.
Methods:
From June 2012 to September 2016, 9 cases with pre-auricular tumor, including 7 cases of basal cell cancer and 2 cases of squamous cell carcinoma, received extended resection operation. The tumor size ranged from 0.5 cm×1.0 cm to 1.0 cm×2.0 cm.Intraoperative frozen specimen was used to make sure no invasion at margin and defect bottom. Based on the size and location of defects, the advanced gourd-shaped flap with frontal branch of superficial temporal arterywas designed to cover the defect. The wounds at donor sites were closed directly. The flap size ranged from 2.0 cm× 5.0 cm to 3.0 cm× 7.5 cm.
Results:
The flaps in all the 9 cases survived completely with primary healing and no complication, like facial paralysis or abnormal feeling. The patients were followed up for 6-36 months with inconspicuous scarandno tumor relapse. Satisfactory cosmetic and functional result can be achieved. The flaps had good match in color with the surrounding tissue.
Conclusions
The advanced gourd-shaped flap with frontal branch of superficial temporal arteryhas a reliable blood supply and flexible transverse method with satisfactory cosmetic result. It is one of the ideal method for reconstruction of pre-auricular soft tissue defects.
10.Children's pneumonia diagnosis system based on Mach-Zehnder optical fiber sensing technology

Objective:To explore a fast and accurate method to diagnose children's pneumonia according to respiratory signals, so as to avoid the cancer induction caused by traditional X-ray examination.Methods:A Mach Zehnder optical fiber sensor was used to build a respiratory signals(RSPs) detection system, and the RSPs of the monitored children were extracted according to the vibration signal generated by the children's lung rales. Preprocessing methods such as the discrete cosine transform(DCT) were used to compress and denoise the RSPs. Multi-feature extraction of RSPs was conducted through signal processing methods such as the Hilbert transform and autoregressive (AR) model spectrum estimation. A support vector machine (SVM) classification model was constructed to classify the collected RSPs.Results:The accuracy rate of the proposed RSP classification of children with or without pneumonia was 94.41%, which was higher than the previous methods.Conclusions:The children's pneumonia diagnosis system based on an optical fiber sensor has a higher detection accuracy, and is expected to be widely used in clinical practice.
