Ischemic stroke has the characteristics of high morbidity,high mortality and high disability rate,which brings to the society and family burden and pain.Therefore,effective prevention is especially important.Primary prevention of stroke is to prevent maiden stroke,mainly to improve lifestyle,and control all kinds of controllable risk factors positively.Secondary prevention of stroke is to prevent the recurrence of stroke,in addition to the causes,prevention,antiplatelet therapy,and anticoagulant therapy are included.

3.Clinical Observation of Shengxuening Tablets Reducing the Risk of Microinflammation in Maintenance Ho-modialys with Renal Anemia

OBJECTIVE:To observe the clinical efficacy of Shengxuening tablets reducing the risk of microinflammation in maintenance hemodialysis(MHD)patients with renal anemia. METHODS:60 MHD patients were randomly divided into observa-tion group and control group with 30 cases in each group. The total dialysis times of all the patients were 12 h every week at blood flow of 200-250 ml/min and dialysis flow of 500 ml/min. Both groups received routine treatment;observation group was additional-ly given Shengxuening tablets orally 1.0 g,tid;control group was given ferrous succinate 0.2 g,tid,orally;both groups were giv-en subcutaneous injection of erythropoietin (EPO),10 000 IU/time,once a week;treatment for all of the patients lasted for 3 months. The fasting venous blood sample were collected at the beginning of treatment and in the morning every one months. The levels of hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor α(TNF-α),IL-6,hemoglobin(Hb),and hematocrit (Hct)were detected. RESULTS:After 3 months of treatment,total effective rate of 2 groups were 90.00%,the levels of hs-CRP、TNF-α、IL-6 in observation group were much lower than that before treatment and control group after treatment,with statistical sig-nificance(P<0.05 or P<0.01). No severe ADR was found in 2 groups,and the incidence of ADR in observation group(6.67%) was significantly lower than in control group (13.33%),with statistical significance (P<0.05). CONCLUSIONS:Shengxuening tablets have good curative effect,and they can reduce the risk of microinflammation due to the long-term use of EPO and chalybeate.

Apogossypolone (ApoG2), a novel derivative of gossypol, has been shown to be a potent inhibitor of antiapoptotic Bcl-2 family proteins and to have antitumor activity in multiple types of cancer cells. Recent reports suggest that gossypol stimulates the generation of cellular reactive oxygen species (ROS) in leukemia and colorectal carcinoma cells; however, gossypol-mediated cell death in leukemia cells was reported to be ROS-independent. This study was conducted to clarify the effect of ApoG2-induced ROS on mitochondria and cell viability, and to further evaluate its utility as a treatment for nasopharyngeal carcinoma (NPC). We tested the photocytotoxicity of ApoG2 to the poorly differentiated NPC cell line CNE-2 using the ROS-generating TL/10 illumination system. The rapid ApoG2-induced cell death was partially reversed by the antioxidant N-acetyl-L-cysteine (NAC), but the ApoG2-induced reduction of mitochondrial membrane potential (MMP) was not reversed by NAC. In the presence of TL/10 illumination, ApoG2 generated massive amounts of singlet oxygen and was more effective in inhibiting cell growth than in the absence of illumination. We also determined the influence of light on the anti-proliferative activity of ApoG2 using a CNE-2-xenograft mouse model. ApoG2 under TL/10 illumination healed tumor wounds and suppressed tumor growth more effectively than ApoG2 treatment alone. These results indicate that the ApoG2-induced CNE-2 cell death is partly ROS-dependent. ApoG2 may be used with photodynamic therapy (PDT) to treat NPC.

5.Application of navigator IDEAL-IQ in liver fat quantification:a comparative study with MRS

Objective To quantitatively assess the liver fat content by Nav IDEAL-IQ and compare with conventional breath-hold IDEAL-IQ and magnetic resonance spectroscopy (MRS).Methods Twenty volunteers,twenty fatty liver patients and twenty patients with other liver diseases were enrolled in this study.IDEAL-IQ,Nav IDEAL-IQ and MRS imaging were performed and fat fraction (FF) were measured respectively.The results were compared by t-test to explore whether there were differences between these groups. Spearman correlation analysis was also used to test the correlation.Results No significant differences in the FF measured by IDEAL-IQ,Nav IDEAL-IQ and MRS in normal volunteers [(4.09±0.82)%,(3.94±0.83)%,(3.92±0.85)%],fatty liver [(15.5±6.1)%,(11.8±6.5)%,(1 2 .4 ± 7 .2 )% ] and other liver lesions patients [(4.20±0.84)%,(4.00±0.94)%,(3.97±0.96)%]were found between these groups (P>0.05). There was a significant positive correlation between Nav IDEAL-IQ and MRS,and the correlation coefficients of these groups were 0.959,0.786 and 0.945,respectively.Conclusion The results of Nav IDEAL-IQ of fat quantification are consistent with MRS results and it increases the success rate of fat quantitative testing.In a word,it is a noninvasive,convenient and accurate way to measure fat fraction.
6.Influences of structural changes after valgus impacted femoral neck fracture on hip range of motion: a 3D simulation test

Objective:To determine the influences of structural changes after valgus impacted femoral neck fracture on hip range of motion (ROM) so as to provide evidence for clinical judgment of whether reduction is necessary or not in the internal fixation of such fractures.Methods:1. 3D reconstructions of the CT hip scans were performed for the 73 patients who had been treated at Department of Orthopaedic Surgery, Shanghai Sixth People’s Hospital Affiliated to Shanghai Jiao Tong University for valgus impacted femoral neck fractures from January 2019 to April 2019.The femoral neck-shaft angle, anteversion angle, femoral offset, axial alpha angle, lateral center edge angle (LCEA), anterior center edge angle (ACEA) and center displacement were measured and compared between the affected and healthy sides to determine the influences of the fracture on the above indexes. 2. Hip motions (flexion and MIR-90°) were simulated on bilateral sides to determine the influences of structural changes after fracture on hip ROM using stepwise regression and Logistic regression. 3. The distribution of femoral-acetabular contact points on the femoral side was observed in simulation of hip flexion to detect the potential area for femoracetabular impingement (FAI) induced by the fracture displacement.Results:1. The valgus impacted femoral neck fractures had significant influences on femoral neck-shaft angle, anteversion angle, femoral offset and axial alpha angle. Compared with the healthy side, on average, the femoral neck-shaft angle increased by 5.1°, anteversion angle decreased by 6.5°, femoral offset decreased by 8.2 mm and axial alpha angle increased by 9.7° on the affected side, showing significant differences ( P<0.05).The displacements of the femoral head center averaged 9.2 mm. There was no significant difference in LCEA or ACEA between the affected and healthy sides ( P>0.05). 2. Compared with the healthy side, on average, the simulated hip flexion decreased significantly by 27.0° and the hip MIR-90° decreased significantly by 20.3° on the affected side after fracture ( P<0.05). Regression analysis showed that femoral anteversion angle, ACEA and displacement of the femoral head center had a significant influence on hip ROM, especially the anteversion angle. When the anteversion angle decreased by more than 7.1°, the hip flexion would decrease by at least 20%. 3. The points of FAI distributed more widely on the fracture side. Compared with the healthy side, the impact points extended outward and upward in hip flexion and extended inwardly in hip MIR-90° on the affected side. Conclusions:After a valgus impacted femoral neck fracture, if the femoral anteversion angle has been decreased by more than 7.1°, the hip ROM can be greatly influenced and the points of FAI can be distributed more widely. Therefore, reduction should be recommended before internal fixation of the fracture.
7.Clinical effect of endoscopic sclerosing agent and tissue glue injection combined with ligation for the treatment of rectal ectopic varices bleeding

Objective To explore the clinical effect of endoscopic sclerosing agent and tissue glue injection combined with ligation in the treatment of rectal ectopic varices(EV)bleeding.Method A retrospective analysis was conducted on 60 patients with rectal EV bleeding who received endoscopic treatment from January 2017 to January 2022.According to the surgical method,the patients were divided into two groups,with 30 cases in the control group receiving endoscopic ligation treatment;30 cases in the observation group were treated with endoscopic scleroing agent and tissue glue injection combined with ligation.Follow up was conducted according to a cycle of 1,3,6 and 12 months after surgery,and the following indicators were compared between the two groups of patients:successful surgical hemostasis rate,incidence of postoperative complications,mortality rate,improvement of postoperative varicose vein diameter,and postoperative response rate.Result The success rate of surgical hemostasis in the observation group was 93.3%,significantly higher than 66.7%in the control group,the incidence of abdominal pain and fever in the observation group was 10.0%and 10.0%,respectively,significantly lower than 33.3%and 36.7%in the control group,there were 2 deaths in the observation group and 9 deaths in the control group,the mortality rate in the observation group was 6.7%,significantly lower than the 30.0%in the control group,the preoperative venous diameter of the observation group was(1.60±0.36)cm,and the postoperative diameter continued to decrease,at the 12th month after surgery,the diameter was(0.78±0.31)cm,while in the control group,the preoperative venous diameter was(1.52±0.26)cm,the postoperative diameter continued to increase,and at the 12th month after surgery,the diameter was(1.55±0.36)cm,the postoperative effective rate of the observation group was significantly better than that of the control group,the differences were statistically significant(P<0.05).Conclusion The combination of endoscopic scleroing agent and tissue glue injection combined with ligation for the treatment of rectal EV bleeding has a higher success rate and effective rate of surgical hemostasis,fewer postoperative complications,lower mortality rate during follow-up observation,and satisfactory clinical efficacy.It is worth further promoting and applying in clinical practice.

Depression is a frequently-seen mental disorder that profoundly affects the survival and quality of life of individuals.Present clinical medicine therapies for depression are not fully efficacious and novel therapeutic agents and targets remain necessary.Bupleuri Radix-Paeoniae Radix(BR-PRA),an essential and crucial compo-nent of traditional antidepressant compound,possesses the beneficial effect of lowering toxicity and amplifying the antidepressant effect when utilized in combination.The underlying mechanisms of these synergistic effects may involve the suppression of inflammation and oxidative stress,the regulation of monoamine neurotransmitters,brain-derived neurotrophic factors,the modulation of the hypothalamus-pituitary-adrenal axis,and the metabolism of various amino acids and energy.This article summarizes the synergistic effects and antidepressant pharmacological effects of BR-PRA herb-pair,thereby providing valuable insights into the potential advantages of this combination and its potential mechanisms of antidepressant action.
9.Improvement mechanism study of kushenol F on ulcerative colitis mice by regulating gut microbiota and immune response

OBJECTIVE To explore the action mechanism of kushenol F (KSCF) in treating ulcerative colitis (UC) in mice. METHODS The potential targets of KSCF intervening in UC were predicted with network pharmacology and molecular docking. C57BL/6J mice were randomly divided by body weight into model group, positive control group (sulfasalazine, 703 mg/kg), KSCF group (100 mg/kg), and normal group, with 6 mice per group. The UC model of mice was induced by dextran sulfate sodium solution. During the modeling period, the mice were given relevant medicine intragastrically, once a day, for 7 consecutive days. After the last administration, the disease activity index (DAI) of the mice was scored; the length of the mice’s colon was measured; pathological changes in the colon tissue of mice were observed; the levels of lipopolysaccharide (LPS) in serum, myeloperoxidase (MPO), nitric oxide (NO) and superoxide dismutase (SOD) in the colon were detected in mice; the expression levels of occludin and ZO-1 in colon tissue of mice were detected; the proportions of CD3+T, CD4+T, and CD8+T lymphocytes in the spleen and the ratio of CD4+/CD8+ were detected; changes in colonic microbiota were analyzed by 16S rDNA sequencing. RESULTS Results of network pharmacology indicated that KSCF may treat UC by regulating signaling pathways such as phosphatidylinositol-3 kinase/protein kinase B (PI3K/AKT) and nuclear factor kappa B (NF- κB). Molecular docking results showed that KSCF bound most stably with NF-κB p65 protein. Animal experiment results demonstrated that, compared with the model group, the pathological characteristics of colon tissue in mice were improved in KSCF group. DAI scores, serum levels of LPS, the levels of MPO,NF-κB p65 phosphorylation and NLRP3 protein expression in the colon, and the proportion of CD8+T lymphocytes in the spleen were reduced significantly (P<0.05). Body weight, SOD levels, expression levels of occludin and ZO-1 in the colon, proportions of CD3+T and CD4+T lymphocytes, and the CD4+/CD8+ ratio in the spleen were significantly increased (P<0.05); the abundance of Firmicutes, Actinobacteria, Akkermansia, and Lactobacillus genera were increased, while Proteobacteria decreased; the microbial community structure tended towards that of the normal group. CONCLUSIONS KSCF alleviates UC by restoring intestinal microbial imbalance, enhancing immune response, and inhibiting colonic inflammatory responses, thereby improving intestinal barrier integrity.
