2.Effects of regional myocardial stunning on systemic hemodynamics and myocardial oxygen balance

Objective To investigate the effects of regional myocardial stunning on systemic hemodynamics and myocardial oxygen balance Methods Thirty three open chest anesthetized dogs were subjected to 15min left anterior descending (LAD) coronary artery occlusion under normal temperature, the systemic hemodynamics, coronary blood flow (CBF), myocardial oxygen delivery(MDO 2) and consumption(MVO 2)were determined following reperfusion Results MAP decreased slightly during LAD occlusion, but returned to baseline following reperfusion CO, SV and LVSWI reduced rapidly after LAD occlusion, increased slightly 5min following reperfusion, followed by a more severe reduction , with the progressive increases of SVR and PVR during whole periods At the early stage of reperfusion (5min), CBF, MDO 2 and MVO 2 increased significantly, then decreased rapidly 10min following reperfusion Myocardial oxygen extraction rate remained stable during whole procedures Histological changes of ischemic area 30min following reperfusion showed interstitial and intracellular edema, mitochondrial vacuolization, and capillary stasis under electronic microscopy Conclusions Regional myocardial stunning may resulte in sinificant changes of systemic hemodynamics; at the early stage of reperfusion, oxygen consumption paradox occurres ,but MDO 2 matches MVO 2 10min following myocardial reperfusion

0.05). Logistic multiple regression analysis showed T242 allele, body mass index, fasting blood glucose, HbAlc, Homa-IS, systolic blood pressure were risk factors of type 2 diabetic nephropathy. Conclusions p22phox C242T gene polymorphism is associated with diabetic nephropathy in Shanghai people. p22phox subunit T allele mutation is pnobabaly one of predisposing genes of type 2 diabetic nephropathy, and p22phox A640G gene polymorphism is not associated with diabetic nephropathy in Shanghai people. T242 allele, fasting blood glucose, HbAlc, Homa-IS, systolic blood pressure are risk factors of type 2 diabetic nephropathy.

Patients with acute myocardial infarction(AMI) are often with elevated blood glucose level when admitted to hospital,this finding is associated with poor prognosis regardless of a history of diabetes.Though numerous studies have addressed some of the questions in this field,many critically important questions are still poorly understood and under debate.This article summarized current findings in hyperglycemia and its potential link withadverse outcomes in AMI patients,and addressed some of the existing controversies in this field.
5.Influential factors analysis of the removal of gastric banding after laparoscopic adjustable gastric banding

Objective To investigate what kind of obese patients appropriate to adopt the laparoscopic adjustable gastric banding volume reduction surgery.Methods A retrospective study was performed to review the clinical data of 40 patients who required reoperation to remove the gastric banding after LAGB from November 2003 to March 2013 at the Department of Minimally Invasive Surgery,Changhai Hospital.Selected 40 patients who required LAGB from January 2006 to October 2008 as control group.We conducted a case-control study to analyze.Chi-square test and multivariate and non-conditional Logistie regression analysis were used to identify the risk factors of removing of gastric banding.Results Age and gender were not statistically significant different (P > 0.05).Multiple factors of Logistic regression showed that BMI≥35 kg/m2,postoperative clinic visits per year < 3 and on the basis of gastrointestinal disease were risk factors for the removal of gastric banding (Wald =3.908,7.375,5.209,P < 0.05).Conclusion The risk factors for the removal of gastric banding include BMI,postoperative clinic visits and the basis of gastrointestinal disease.In the treatment of obesity with LAGB should take full account of the above factors.
6.Expression of tyrosine protein phosphatase 1B in adipose tissues of newly diagnosed diabetic patients

Objective:To investigate the role of protein tyrosine phosphatase 1B(PTP-1B) in pathogenesis of type 2 diabetes. Methods: Thirty-four patients were divided into 3 groups: type 2 diabetes with normal BMI group(CDM), obese type 2 diabetes group (ODM) and control group. Fasting blood glucose, fasting insulin and body mass were determined. Expression of PTP-1B in adipose tissues were determined by Western blot. Results: Compared with control group, CDM and ODM groups had insulin resistance, though there was no significant difference between CDM and ODM groups. Expression of PTP-1B in control, CDM and ODM groups were 3.39?0.94,11.57?1.89 and 36.53?14.82, respectively. AIM: To investigate the relationship between serum soluble E-selectin (sE-selectin) and insulin resistance, serum uric acid, serum lipid in essential hypertension patients. METHODS: Fasting serum sE-selectin concentration, plasma glucose, serum insulin, serum uric acid, total cholesterol, triglycerides, high density lipoprotein-cholesterol, low density lipoprotein-cholesterol were determined in 186 patients with essential hypertension (75 males, 111 females). ObjectiveTo explore the relationship between circulating tumor cells(CTCs) in the peripheral blood of patients with esophageal squamous cell carcinoma(ESCC) and the physiopathological characteristics of esophageal neoplasms.MethodsUsing negative selection system,we depleted red blood cells(RBCs) in red blood cell lysis buffer,depleted white blood cells (WBCs) with Miltenyi magnetic beads and enriched the rare cells from ESCC patients'peripheral blood.Immunofluorence staining (IF) was adopted to identify CTCs.ResultsCirculating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma was closely related to cell differentiation grade,the invasion of primary cancer,lymph node status,P-TNM stages,and was rarely related to the sex,age or the location of tumor.ConclusionThe results suggest that circulating tumor cells in the peripheral blood of patients with esophageal squamous cell carcinoma may express the development of esophageal cancer and may be served as a tumor marker to evaluate the biological behavior of esophageal cancer.
9.TNF-α up-regulates protein tyrosine phosphatase 1B via NF-kB in HepG2 cells

HepG2 ceils were treated with various concentrations of tumor necrosis factor-or (TNF-α) for 24 hours. RT-PCR and Western blot were used to measure protein tyrosine phosphatase-1 B(PTP-1B)expression, and luciferase reporter assay was used to detect NF-kB activity. The results showed that treatment of HepG2 cells with TNF-α for 24 hours led to upregulation of PTP-1B and NF-kB activity in a dose-dependent manner. Inhibition of NF-kB by PI)TC significantly attenuated TNF-α-induced PTP-IB expression in HepG2 cells. Thus, the transactivation of NF-kB seems to play an important role in the expression of PTP-1B in HepG2 cells induced by TNF-α.
10.RIP3 expression and function in the liver of rats following ischemic postconditioning

Objective:To investigate the expression and function of RIP3 in the liver of rats following ischemic postconditioning.Methods:The model of 70% hepatic ischemia and reperfusion was established,then a total of forty healthy adult male Sprague-Dawley(SD) rats were divided randomly into four groups,ten rats in each group:a sham-operation group (Sham group);an ischemia reperfusion injury group(IR group);an ischemic postconditioning group(IPO group);an ischemic postconditioning and necrostatin-1 group (Nec-1 group).The blood samples and liver tissues were collected.The serum levels of ALT and AST were detected,and the liver histological examination was performed.Western-bolt was used to detect the TNF-α and RIP3 levels.Results:Compared with the IR group,ALT and AST in serum were significantly declined in the IPO group (P<0.05);The liver damage after ischemia and reperfusion was improved obviously in the IPO group compared to which in the IR group;The Suzuki's scores was increased in the IR group compared to which in the IPO group (P<0.05);There was a low grade of TNF-α and RIP3 in the Sham group,whereas the level of TNF-α and RIP3 significantly increased in the IR and IPO and Nec-1 group(P<0.05);Compared with the IR group,the level of RIP3 was further decreased in the IPO group (P<0.05);Compared with the IPO group,the level of RIP3 was further decreased in the Nec-1 group (P<0.05).Conclusion:RIP3-mediated necroptosis was involved during hepatic ischemia postconditioning,and the protective effect of ischemia postconditioning may act as reducing necroptosis by cutting down the levels of RIP3.
