1.PHARMACOKINETICS OF MELOXICAM IN HEALTHY CHINESE VOLUNTEERS

AIM To assess the pharmacokinetic profile of single doses of meloxicam in healthy Chinese volunteers. METHODS The plasma concentrations of meloxicam after an oral dose of 15 mg to twenty healthy male volunteers were analized by means of a validated HPLC method. The pharmacokinetic parameters were subjected to Shapiro-Wilk test to determine whether these data were fitted to a normal distribution. RESULTS The twenty volunteers can be classified into extensive metabolizers and poor metabolizers according to pharmacokinetic parameters. The main parameters in the two groups obtained were as follows: T1/2 were 21±4 and 38±9 h, AUC0-∞ were 49±10 and 110±8 μg*h*mL-1, respectively. Even the AUC data in extensive metabolizers were 1.7 times as that reported in White volunteers following the same doses of meloxicam. CONCLUSION There were significant individual differences in the pharmacokinetics of meloxicam in Chinese volunteers, which may be due to the genetic polymorphism of CYP2C9.
2.Breast conserving surgery for early breast cancer: a report of 86 cases

ObjectiveTo evaluate the breast-conserving surgery for early breast cancer.MethodFrom 1993 to 2000, 86 female cases of stage ⅠⅡa breast cancer received breast conserving comprehensive therapy. Clinical data were analyzed retrospectively. ResultsThe 5 year survival rate of this group was 96.5% and the 5 year recurrence rate was 5.2%.The postoperative contour of the breast was satisfactory.ConclusionBreast-conserving surgery is safe and effective therapy for early breast cancer, when strict indication and necessary safe margin of resection is obeyed by. Postoperative radiotherapy and chemotherapy should be properly arranged and the patients should be closely followed-up.
3.Isolation of cDNA Clones Using PAC and BAC Probes

Purpose To introduce a method to isolate cDNA clones using bacteriophage P1-derived artificial chromosome (PAC) or bacterial artificial chromosome (BAC) as probe for hybridization and try to find some novel genes related to hepatocellular carcinoma. Methods PAC 579 (D17S926 locus) and BAC 1529 (D17S1529 locus) in the deletion region of chromosome 17p13.3 in human hepatocellular carcinoma were chosen to screening the human liver cDNA library as probe for hybridization. The isolated positive cDNA clones were partially sequenced, then analyzed by computer comparison and Southern blot. Results After three cycles of screening, 78 and 8 candidate positive cDNA clones were isolated using PAC 579 and BAC 1529 probes respectively. Further analysis indicated 18 cDNA clones isolated by PAC 579 probe and 5 cDNA isolated by BAC 1529 probe were potential novel genes related to hepatocellular carcinoma. Conclusions The isolation of cDNA clones using PAC and BAC probes is effective and practical.
4.Parents′ knowledge, attitudes and practice on antibiotics use in children

Objective:To analyze parents' knowledge, attitudes and practice (KAP) on antibiotics use in children.Methods:A survey of parents' KAP on antibiotics use in children was conducted among 482 parents in Tangqiao community who were visiting the vaccination unit of our center during January to June 2018. The influencing factors of patients' KAP on antibiotics use in children were investigated with univariater and multivariate logistic regression analysis.Results:In terms of knowledge, 89.0% (429/482) of parents agreed that common cold did not require antibiotics treatment; 88.8% (428/482) of parents knew that antibiotics had adverse reactions; 82.0% (395/482) of parents knew that overuse of antibiotics would increase the possibility of drug resistance of pathogenic bacteria; and 69.7% (327/482) of the relevant knowledge acquisition came from doctors. In terms of attitudes, 84.1% (405/482) of the parents agreed with the phenomenon of excessive use of antibiotics in our country; 68.2% (405/482) of the parents agreed that they should follow the doctor's instruction but not ask the doctor to prescribe drugs; 30.7% (148/482) parents thought they had enough knowledge about antibiotics drugs; 24.7% (148/482) parents were willing to use more expensive antibiotics for their children. In terms of practice, 13.9% (67/482) of parents would buy antibiotics by themselves; 35.7% (172/482) of parents had antibiotics at home. Multi-factor regression analysis results showed that parents with high annual family income are likely to better understand antibiotics ( OR=2.619, 95 %CI: 1.431—4.793); parents who had antibiotics at home ( OR=4.713, 95 %CI: 1.964—11.309)and purchased antibiotics by themselves ( OR=2.768, 95 %CI: 1.237—6.194) were more likely to use antibiotics for their children. Conclusion:There are still some problems in parents' knowledge, attitude and behavior of using antibiotics in children in the community, and public education on rational use of antibiotics in children is necessary.
5.Radical hepatectomy in patients with initially unresectable liver cancer after conversion therapy

Objective:To explore the clinical efficacy of radical surgery after successful conversion therapy for liver cancer.Methods:We retrospectively analyzed the clinical data of 10 patients with liver cancer who underwent successful conversion therapy and subsequent radical surgery in Peking University People's Hospital from Nov 2019 to Dec 2021.Results:The median age of the 10 patients was 64 (51.25,68.50) years. The median number of conversion therapy cycles was 11 (4.75,25.00). No serious adverse reactions were found in the patients during conversion therapy. After conversion therapy, 8 patients underwent partial hepatectomy, and 2 patients underwent radiofrequency ablation. Postoperative complications occurred in 4 patients. All complications were classified as Clavien-Dindo grade Ⅰ or Ⅱ. The median follow-up time was 13 (9.75,49.75) weeks. Three patients had tumor recurrence after surgery. Among the patients with tumor recurrence, 1 patient died of liver failure.Conclusions:Conversion therapy is an effective treatment for patients with clinically unresectable liver cancer. The incidence of serious adverse reactions in conversion therapy for liver cancer is low. The radical surgery can be safely performed in patients with good general condition and liver function. Radical surgery after conversion therapy can prolong the survival time of patients for unresectable liver cancer.
6.Comprehensive treatment for acute iliac femoral vein thrombosis complicating left iliac vein compression

Objective To explore the significance of comprehensive treatment including anticoagulation,inferior vena cava filter implantation,mechanical thrombectomy,iliac vein angioplasty and stent placement and catheter-directed thrombolysis (CDT) in the treatment of acute iliac femoral vein thrombosis on the basis of left iliac vein compression.Methods In this study 61 patients were enrolled at Beijing Luhe Hospital,Capital Medical University between Mar 2013 and Mar 2019 in anticoagulation combined with CDT group and comprehensive treatment group.Short-term venous recanalization rate scores and long-term proximal valvular function of left femoral vein were compared.The survival curve was drawn using kaplan-meier method,and incidence of PTS was compared with the Log rank test.Results The short-term venous recanalization rate scores of the patients in the comprehensive treatment group were higher than scores of the patients in the traditional treatment group(t =9.872,P < 0.001).16 patients developed PTS.The incidence of PTS in the comprehensive treatment group was lower than that in the traditional treatment group(x2 =7.146,P =0.008).Patients in comprehensive treatment group kept a better function of the proximal valvular of left femoral vein than patients in the traditional treatment group (x2 =23.834,P <0.001).Conclusions The comprehensive treatment can effectively reduce the occurrence of long-term PTS in patients with acute iliac femoral vein thrombosis complicated with left iliac vein compression,and improves the quality of patients' life.
7.Lunch supply in public primary school canteens in Zhejiang Province

Objective:
To investigate lunch supply of public primary school canteens in Zhejiang Province, and to provide a basis for the scientific guidance of school lunch.
Methods:
During May to Jun. and Sept. to Oct. in 2019, lunch food supply was weighed and recorded and the number of diners in 44 public primary school canteens were summarized. Each investigation lasted for one week.
Results:
Food was mainly based on the taste of the students (40.91%) in the school canteen. About 45.45% of the schools referred to the Nutrition Guidelines of School Meals for students meals when making recipes in the school canteen. The supplies of cereals, vegetables, fruits, livestock and poultry meat, fish and shrimp, eggs, milk, soybean nuts, vegetable oil and salt were 109.05, 118.01, 0, 63.96, 9.25, 11.31, 0, 10.68, 10.47, 2.54 g. The supply of vegetable oil was basically the same as the recommended amount ( P >0.05). The supplies of energy, protein, calcium, iron, zinc, vitamin A, vitamin B 1, vitamin B 2, vitamin C, dietary fiber were 820.84 kcal, 32.79 g, 164.18 mg, 7.84 mg, 4.71 mg, 23.07 μgRAE, 0.41 mg, 0.35 mg, 20.47 mg, 2.34 g, 37.56% of energy from fat and 48.47% of energy from carbohydrate. The supply of vitamin B 1 was basically the same as the recommended amount ( P >0.05). There were no significant differences in all kinds of food and nutrients between urban and rural primary schools ( P >0.05).
Conclusion
Lunch supply is not optimistic in public primary school canteens in Zhejiang Province, with unreasonable structure and fails to agree with current nutritional recommendations. It is suggested that the scientific guidance of students meals should be carried out according to the survey results combined with the characteristics of local diet.
8.Detection of intraperitoneal free cancer cells in gastric cancer patients undergoing gastrectomy and its relationship with short-term prognosis

Objective To evaluate different methods in detecting intraperitontal free cancer cells (IFCCs) in patients with gastric cancer and to clarify the relationship between positive IFCCs and short-term prognosis.Methods A total of 119 gastric cancer patients who underwent surgical treatment were enrolled.Peritoneal lavage was performed with 300-400 ml saline respectively at three points of time:immediately after abdominal cavity entry;when surgical operation was completed;when extensive intraoperative peritoneal lavage was done.The IFCCs were detected with methods of traditional centrifugal cytology,membrane cytology,ICC and RT-PCR.The survival curve of patients with gastric cancer was drawn using Kaplan-Meier method.Results The positive rate of PLC was 16.8%,20.7% and 11.2% respectively at 3 timepoints (P < 0.05).The positive rates of ICC were 28.6%,38.8% and 20.7% respectively at 3 timepoints.The positive rates of RT-PCR were 39.3%,69.5% and 50.8% respectively at 3 time points.The positive rate of IFCCs detected through RT-PCR was higher than that of PLC and ICC (P < 0.05).The short-term prognosis of patients with positive IFCCs was worse than those with negative results detected with any three method at the timg point immediately after opening the abdomen (P < 0.05).At the timg point immediately after removing the tumors,the short-term prognosis of patients with positive IFCCs detected with PLC was worse (P < 0.05).Conclusion The short-term prognosis was poor in patients with positive IFCCs.It is the best time to detect IFCCs before radical resection.Surgical procedures increase the risk of shedding of IFCCs.
