1.Expression of glucose-regulated protein in rat retina with acute high intraocular pressure and its significance

BackgroundThe progressive death of retinal ganglion cells (RGCs) is the primary pathological characteristics of visual defects in glaucomatous eye.Research showed that endoplasmic reticulum stress (ERS) is involved in this progression.Glucose-regulated protein 78 ( GRP78 ) is a special marker of ERS.To understand the change in expression of GRP78 in the retina under the pressure induced is very important for the protection of visual function.Objective The present study was to observe the expression of GRP78 in rat retina with acute high intraocular pressure (IOP) and investigate the possible effect of ERS in acute glaucoma damage. MethodsFiftysix Wistar rats were randomly assigned to normal control group,anterior chamber punctured group and 12 hours,1 day,3,7,14 days groups following acute IOP rising.The acute high IOP models were established in the right eyes of 4.0 Wistar rats by paracentesis of the anterior chamber and infusion of normal saline solution into the anterior chamber.The histopathology changes of the retina were examined by hemotoxylin and eosin staining.The expression of GRP78 protein and mRNA in the retina were detected by immunohistochemistry and real-time fluorescence quantitative PCR.ResultsThe retinal layers and cells were clear with normal alignement in the rats of the normal control group and the anterior chamber punctured group.The edema and thickening of retinas appeared in 12 hours after molding and peaked in 1 day after molding.Then the retina decreased in thickness and atrophied from 3 days through 14 days.The expression levels of GRP78 protein (A value) were 0.195±0.006 in the nounal rats and gradually increased from 12 hours through 3 days (0.268±0.017) following molding with the peak value at 1 day (0.499±0.039 ),showing significant differences in comparison with the normal control group (t =0.098,0.304,0.073,P<0.05),and the reduction in expression at 7 and 14 days were not significantly different from the normal control group ( t =0.002,0.001,P>0.05 ).The relative value of GRP78 mRNA ( 2△△C1 ) in the retina was 1.011 ±0.013 in the normal control group and gradually up-regulated from 12 hours through 3 days with the peak value at Iday (2.141±0.171 ) and then down-regulated from the third day onwards.A significant difference was seen in 2-△△C1value between the normal control group and 12 hours group,1 day group or 3 group ( t =0.525,1.130,0.409,P<0.05 ).However,the 2-△△C1 values of GRP78 mRNA at 7 and 14 days was similar to that of the normal control group (t=0.020,0.004,P>0.05).ConclusionsGRP78 participates in the process of RGCs damage following acute high IOP.The results suggest that interfering with ERS may be helpful for protecting the retina and optical nerve from pressure-induced damage.
2.Standardized patients applied to the training on neuro-ophthalmology students

AIM:To enhance the doctors` ability of disease diagnosis and treatment by using standardized patients in neuro-ophthalmology teaching.METHODS: Graduated students and visiting students accepting neuro-ophthalmology training courses in Department of Ophthalmology during 2014-2016 were enrolled as the research objects.Two groups were randomized allocated and 20 students each group.One group of students was applied standardized patients teaching method and the other group of students was applied traditional teaching method.A questionnaire and exam after the teaching were evaluated.The data were analyzed by SPSS 20.0.RESULTS: The data of questionnaire included content interest improving,satisfaction for teaching,the key points understanding,theory with practice effective linking,future practice instruction.Comparison questionnaire data between two groups were of statistical significance (P<0.05).The test scores of case analysis of two groups were statistically significant also(P<0.001).CONCLUSION: Standardized patients teaching method is helpful for students` training on the ability of clinical diagnosis and treatment.
3.Associated factors of poor vision after cataract surgery

AIM: To analyze the associated factors of poor vision after phacoemulsification operation.
METHODS:Clinical data of cases with poor vision after cataract phacoemulsification combined with IOL implantation from 341 patients(418 eyes) during Jan. 2015 to Jan. 2016 in our hospital were collected and logistic correlation analyzed.
RESULTS: The incidences of postoperative low vision were 10. 3% for one month after operation. Forty-three eyes were reported low vision, and 41 of 43 the patients with postoperative low vision had fundus oculi disease before operation, which included 20 eyes ( 48. 8%) with age-related macular degeneration, 16 eyes(39. 0%) with diabetic retinopathy, 3 eyes ( 7. 3%) with glaucomatous optic neuropathy, 1 eye(2. 4%) with pathologic myopia of retinopathy and 1 eye ( 2. 4%) with branch retinal vein occlusion.
CONCLUSION: The associated factors of poor vision after phacoemulsification operation are preoperative eye diseases and the nuclear hardness, and preoperative eye diseases may be the main causes of postoperative low vision. Comprehensive examination should be attached importance to, and be careful in the operation to decrease the harm for visual performance.
4.Effects of Huoxue huayu decoction on expression of retinal glutamate transporter in acute ocular hypertension of rats

Background Hypertension,ischemia and hypoxia induce the increase of glutamate level in eye tissue and furthermore produce excitatory damage and apoptosis of retinal ganglion cells(RGCs).At present,the study on the protection of traditional Chinese medicine on glutamate-induced retinal excitatory damage is lack.objective Present study was to explore the protective effects of Huoxue huayu decoction,a Chinese herbal recipe,on RGCs in acute ocular hypertension model. Methods Fony-five healthy clean Wistar rats were divided into three groups randomly.The acute ocular hypertension models were established in 40 Wistar rats by injecting the normal saline solution into the anterior chamber to elevate the intraocular pressure for 60 minutes.Huoxue huayu decoction of 4 ml was administered intragastrically once per day in 20 model rats.Other matched 5 normal Wistar rats served as normal control group.The rats were sacrificed on 1,3,7 and 14 days after modeling and the retinas were isolated for the histopathologieal and uhrastructure examination.Expression of glutamate transporter in the retina of acute ocular hypertension and normal rat retina were detected using quantitative real-time polymerase chain reaction.The utilization of animals followed the Association for Research in vision and Ophthalmology. Results After acute ocular hypertension.the retina thickness attenuated and the numbers of RGCs decreased under the light microscope in 1,3,7 and 14 days after modeling in comparison with normal control rats.The degradation of the organelle and edema as well as changes of cell nuclei were seen in model rats under the transmission electron microscopy.The expression of glutamate transporter mRNA in model group and glutamate transporter group was rapidly elevated in the first day (P<0.05),descended at the third days(P<0.01)and returned to the normal level 7 days later(P>0.05).Conclusion Huoxue huayu decoction can protect the retina against RGCs damage in the acute ocular hypertension by elevating the expression of glutamate transporters in retina.

6.Preliminary analysis on the treatment of infection caused by pandrug-resistant Acinetobacter baumannii

Objective To analyze the clinical features of pandrug-resistant Acinetobacter baumannii (PDR-Ab) in a hospital and compare the efficacy of different antibiotic treatments on patients with pneumonia caused by PDR-Ab.Methods Data were ret- rospectively collected from all isolated PDR-Ab strains in our hospital from February 2004 to March 2005.The clinical features and outcomes were reviewed.Results A total of 77 strains of PDR-Ab were collected, 45 of which were pathogens causing clini- cal infections (35 strains from lower respiratory tract, 6 from bloodstream, 3 from drainage fluid, and 1 from wounds).Lower respiratory tract was the most common source of PDR-Ab.More than 90% of the isolated PDR-Ab strains produced OXA-23 type?-lactamase.Cefoperazone-sulbactam plus minocyeline showed good efficacy for patients with PDR-Ab pneumonia.The total clinical cure rate was 68.4%.Bacterial eradication rate was 42.1%.The factors influencing bacterial clearance were pro- longed mechanical ventilation prior to positive culture (17.5 d vs 5.5 d).mixed infection (100% vs 12.5%) and lower GCS score (9.1?0.7 vs 13.2?2.1).Concomitant septic shock (OR=13.8) and APACHEⅡscore (OR=2.1) were independent factors of clinical outcome.Conclusions Nosocomial infections caused by PDR-Ab are not untreatable.Our analysis suggests that cefoperazone-sulbactam plus minocycline may be an effective treatment for lower respiratory tract infections caused by PDR-Ab in our hospital.
7.Protective effects of sinomenine on renal ischemia/reperfusion injury in mice

Objective To evaluate the protective effect of sinomenine (SIN) on renal ischemia/reperfusion (I/R) in mice. Methods In the experiment one, 12 C57BL/6 mice were randomly divided into 2 groups: SIN group (mice were injected with 200 mg/kg SIN by tail vein) and control group (mice were injected with equal volume of saline). Six and 24 hs later, the serum was collected and the contents of alanine aminotransferase (ALT) and creatinine (SCr) were determined. In the experiment two, C57BL/6 mice were randomly divided into 3 groups: sham-operated (SO) group, SIN group (mice were injected with 200 mg/kg sinomenine just before ischemia induction) and saline group (mice were injected with equal volume of saline at the same time). At the 6th h after reperfusion, the sera and renal samples subject to IR injury were collected. The SCr and BUN levels in serum were determined and renal histological changes were also examined. The apoptosis of renal tubular epithelial cells was measured by using terminal deoxynucleotidyl transferase mediated dUTP nick end labeling assay. The infiltration of F4/80 positive macrophages was measured by using immunohistochemistry and that of neutrophils with myeloperoxidase (MPO) kits. The mRNA expression of tumor necrosis factor (TNF)-α, chemokine CXC ligand (CXCL)-10, intercellular adhesion molecule (ICAM)-1 and IL-17 was detected by using real-time reverse transcription PCR. The activation of transcription factor NF-κB was measured by using Western blotting. Results In the experiment one, there was no significant difference in ALT and SCr between the two groups at 6 or 24 h. In the experiment two,levels of SCr and BUN were lower in SIN group (P<0. 05 or P<0. 01 ), histological damage was milder (P<0. 01 ), and apoptosis rate of renal tubular epithelial cells apoptosis was lower than in saline group (P<0. 05). The infiltration of macrophages, neutrophils and the mRNA expression of TNF-α, CXCL-10, ICAM-1 and IL-17 in the renal tissue in SIN group were reduced as compared with saline group (P<0. 05 or P<0. 01 ). The activation of NF-κB in SIN group was significantly downregulated as compared with saline group. Conclusion SIN can ameliorate the renal IR injury without hepatic or renal toxicity, which is associated with inhibition of acute inflammatory response induced by reperfusion.
8.Cardiopulmonary Related Index Change of Diffusing the Lung and Promoting Urination on Cor Pulmonale Rats

This study was aimed to diffuse the lung and promote urination for the observation of cardiopulmonary re-lated index change of cor pulmonale rats to further explore the treatment effect on the pulmonary function, pulmonary arterial hypertension (PAH) and the influence of right heart hypertrophy of rats, in order to further illustrate the ef-fect of diffusing the lung and promoting urination for cor pulmonale. Sixty Wistar rats were randomly divided into the control group, model group and Xiao-Qing-Long decoction (XQLD) group with 20 rats in each group. The AniRes2003 animal lung function analysis system was applied to measure the pulmonary function of rats. And the multi-guide physiological recorder was used in the recording of the pulmonary artery pressure of rats. The conven-tional weighing method was applied to calculate and obtain the change of right heart hypertrophy. The results showed that compared to the control group, symptoms in the model group became severe obviously, which include reduced activity, slow movement and occasional airway sputum sound, and the right heart hypertrophy index of the model group increased obviously (P < 0.01). Compared to the model group, the pulmonary function and pulmonary artery pressure of the XQLD group have obvious difference (P< 0.05). It was concluded that to diffuse the lung and pro-mote urination can effectively improve the pulmonary function, PAH and the right heart hypertrophy index of rats with cor pulmonale (fluid retention). The effect of this method is definite in the treatment of cor pulmonale.
9.Application of medical robot in vascular intervention: an experimental study

Objective To investigate the technical feasibility of medical robot in application to vascular intervention. Methods The independent-developed medical robot was used in the glass vessel model and vascular intervention experiments in a dog. Results The process of experiments were smooth,the system movement did not have any malfunction,and the animal experiments did not have any operative complications. The operative time was 50 minutes.Conclusions The medical robot can basically meet the requirements of cerebral angiography. It has laid a foundation for further development of intracranial vascular interventional procedures and clinical application.
10.Effects of Wenjing Tongluo Formula on GFAP Level in Perpheral Neuropathy Rats

Objective To discuss the treatment action and mechanism of Wenjing Tongluo Formula on oxaliplatin-induced perpheral neurotoxicity in rats. Methods Intraperitoneal injection was used to inject oxaliplatin 4 mg/kg to establish oxaliplatin-induced peripheral neurotoxicity rat models. Female Wistar rats were randomly divided into normal group, model group, and TCM group. TCM group was given Wenjing Tongluo Formula to soak rats’ limbs and tails. Rats in the model group were soaked with deionized water for comparison. Rats in the normal group received intraperitoneal injection with 5%glucose. Algesia hypersensitivity and anaphylaxis were detected under the mechanical stimulation and temperature. Immunohistochemical method was used to detect the expression of GFAP in L4-L6 spinal dorsal horn of rats. Explore the level of GLT-1 in L4-L6 dorsal root ganglia by RT-PCR. Results Rats in model group showed obvious behavioral changes compared with normal group (P<0.05);Rats in the TCM group improved in behavioristics compared with model group (P<0.01);number of positive cells in GFAP of rats in the model group increased compared with normal group (P<0.05);the increase in the TCM group was not obvious. Compared with normal group, astrocytes in spinal dorsal horn of model group were enlarged, protuberances increased, became coarse, and GLT-1 mRNA is decreased (P<0.05, P<0.01). Compared with model group, active cells and protuberances in the TCM group decreased (P<0.01), GLT-1 mRNA is increased (P<0.01). Conclusion Wenjing Tongluo Formula can improve behavioral changes of model rats under temperature and mechanical stimulation, probably related to harmful signal transmission induced by inhibition of astrocyte in spinal dorsal horn.
