1.Suppression of MDR1 gene expression and reversal of cisplatin resistance in renal carcinoma cells by RNA interference

Objective:To investigate the inhibitory effect of RNA interference (RNAi) on the expression of multidrug resistance (MDR1) gene and analyze the altered sensitivities of human renal carcinoma cell line to cisplatin.Methods:Three small interfering RNA (siRNA) sequences targeted MDR1 gene were synthesized and transfected into renal carcinoma A498 cells. The expression level of MDRl mRNA was measured by RT-PCR to identify the most effective siRNA sequence. The recombinant plasmid was packed by lentivirus and transfected into A498 cells. RT-PCR was used to screen the A498 cells with the optimal silencing efficacy. The MDR1 protein expression level in the cloned cells was verified by Western blotting. The inhibitory effect of cisplatin on the proliferation of A498 cells was assessed by MTT assay and the IC_(50) value was calculated. Results:The 3 siRNA sequences suppressed MDR1 gene expression at different degrees. The siRNA 1 sequence silenced MDR1 gene more effectively with a significant reduction of 67%. The MDR1 protein expression greatly decreased in screened A498 cells compared with non-transfected cells (P<0.01), and the IC_(50) value of cisplatin on screened A498 cells was significantly decreased by 83.37% (P<0.01). Conclusion: The RNAi could effectively inhibit the expression of MDR1 gene and increase the sensibility to cisplatin in human renal carcinoma A498 cell line, which make it possible to reverse the resistance of renal carcinoma to chemotherapy.
2.Quality problems of community home care service in Beijing: a qualitative observatory study

Objective:To investigate the quality issues of community home care service in Beijing.Methods:A study on quality of community home care service was conducted from March to May in 2021 in six urban districts of Beijing, and 10 community health service centers were selected as study sites by purpose sampling method. The quality issues of home care service were studied by observing the process of 17 home care services, interviewing medical staff and reviewing relevant medical documents.Results:The study extracted 3 structural quality problems and 6 process quality problems of home care service. The structural quality problems included unclear service scheduling and insufficient qualification of the personnel; lacking special transportations; and lacking home health service standards in some centers. The process quality problems included incomplete material preparation, wearing jewelry during medical procedures, no physical check-up before procedures, not strictly following the aseptic principle in the operation, not properly disposing medical wastes, wrong or missing filling of medical documents.Conclusions:There are some problems in the implementation of community home care service in Beijing, such as insufficient medical personnel and material resources, unclear service scheduling, lack of unified medical service standards, and insufficient quality management. Therefore, these problems should corrected to ensure the quality of community home care services in Beijing.
3.Research progress on measurement tools of patient participation in medication safety

Patient medication safety is a global concern. Patient participation in the medication process can effectively reduce the occurrence of medication errors. Assessing the patient′s ability to participate in medication safety is conducive to identify problems in the participation process; therefore, to provide reference for corresponding intervention measures and further improve patient participation ability. This article introduces the currently used measurement tools of patient participation in medication safety, and provides reference for improving measurement index system in China.
4.Effect of patient engagement on medication safety for chronic disease patients: a systematic review

Objective:To explore the effect of patient engagement on medication safety for patients with chronic disease through a systematic review.Methods:Relevant randomized controlled trials of patient engagement on medication safety were searched from PubMed, EMBASE, The Cochrane Library, CNKI, Wanfang and VIP database. The literature was screened according to inclusion and exclusion criteria, and data extraction and literature quality evaluation was conducted on the selected literature.Results:A total of 15 eligible studies was included. Most of the studies occurred in outpatient and home settings, and the subjects were patients with chronic diseases. Patient engagement strategies can be divided into three categories: (1) patient engagement in medical decision-making; (2) patient engagement in medication adjustment; (3) patient engagement in medication management. The outcomes of medication safety included medication adherence, medication knowledge, medication beliefs, adverse events and medication errors. Intervention strategies for patients to actively engage in medication safety significantly improved patients′ medication knowledge and beliefs, but did not improve medication adherence of patients.Conclusion:Promoting patient active engagement is an effective intervention measure to improve patients ′ perception of medication safety. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusions.
5.Expression of lncRNA RP3-340N1.2 in breast cancer tissues and its effect on proliferation and migration of MCF-7 cells

Objective: To observe the expression of long-chain non-coding RNA (lncRNA) RP3-340N1.2 in breast cancer tissues and its effect on proliferation and migration of breast cancer MCF-7 cells, and to explore the possible mechanism. Methods: 13 pairs of breast cancer tissues and adjacent tissues from breast cancer patients, who underwent radical surgery at the Cancer Center of theAffiliated People’s Hospital of Hubei University of Medicine from Jan. 2017 to Sep. 2017, were collected for this study. qRT-PCR was used to detect the differential expression of RP3-340N1.2 in collected tissue samples and breast cancer cell lines and normal breast epithelial cell line. RP3-340N1.2 plasmid (experimental group) and the negative control plasmid (control group) were transfected into breast cancer MCF-7 cells using Lipofectamine 3000. Cell counting (CCK-8) and Transwell migration assay were used to examine the effect of RP3-340N1.2 over-expression on proliferation and migration of MCF7 cells, the effect of RP3-340N1.2 over-expression on the mRNA expression of miR-134-5p and OPCML was detected by qRT-PCR, and Western blotting was used to detect the expression of OPCML protein. Results: The expression of RP3-340N1.2 in breast cancer tissues was significantly lower than that in adjacent tissues ( P <0.01), and the expression of RP3-340N1.2 in breast cancer cell lines was significantly lower than that in normal breast epithelial cells ( P < 0.01). Up-regulation of RP3-340N1.2 decreased the proliferation and migration of MCF7 cells (all P <0.05). After over-expression of RP3-340N1.2 in MCF7 cells, the expression of miR-134-5p obviously decreased ( P <0.01); moreover, the mRNA and protein expressions of OPCML significantly increased ( P <0.01) while the expressions of cell cycle regulatory proteins (CDK4, Cyclin D2) and cell migration regulatory proteins (Vimentin and N-cadherin) decreased significantly (all
P <0.01). Conclusion: RP3-340N1.2 is low expressed in breast cancer tissues and cell lines. Up-regulation of RP3-340N1.2 expression can lead to decreased expression of miR-1345p and increased expression of OPCML gene, thereby inhibiting the proliferation and migration of breast cancer cells.
