Objective To explore the criterion of constitution with Yin-deficiency and provide the evidence for further development of establishing the criterion.Method According to reference documents on TCM,a questionnaire on the criterion of constitution with Yin-deficiency was developed and distributed to 51 experts of TCM in Zhejiang province. Results The mean scores of forty-six items ranged from 2.02 to 2.78 and top sixteen symptoms were dry mouth, dry stools, dry throat, feverish palms and soles, thready pulse, constipation, emaciation, yellowish urine, red tongue, rapid pulse, vertigo, thirst with preference for cold drinks, vexation, dry lips, little coating, thirst with frequent drinking.Modes of 46 items ranged from 1 to 4.Coefficients of weight for diagnosis ranged from 0.018 to 0.024 and variance of coefficients were from 0.540 to 1.572.Conclusion Forty-six items could become the content of the criterion of constitution.

Objective To evaluate the global and segmental (inflow, body and outflow) systolic functions of right ventricle in end-stage renal failure (ESRF) patients by real-time three-dimensional echocardiography(RT-3DE) and four-dimensional right ventricle quantitative analysis (4D-RVQ) and to reveal the long-term impact of renal insufficiency on right ventricular systolic function. Methods RT-3DE was performed in 48 ESRF patients and 26 healthy individuals (control group).Images were acquired and analysed off-line in TomTec 4D-RVQ station. The parameters of RV global and segmental systolic functions, including RV global and segmental end-diastolic volume (EDV),end-systolic volume(ESV),stroke volume(SV),ejection fraction(EF),the time to minimum systolic volume(T-msv) were obtained. All the variables were standardized by body surface area (BSA) or heart rate (HR).Results ① In the ESRF group,EDV,SV and EF of body compartment were significantly higher than those of the controls[EDV:(8.5±3.4)ml/m2 vs (6.4± 1.7)ml/m2,P =0.002;SV:(4.5 ± 1.7)ml/m2 vs (2.7±0.9)ml/m2,P =0.0037;EF:(45.4 ± 11.5)% vs (41.1 ± 7.1)%,P =0.0489],and so were EDV,SV of inflow compartment [EDV:(19.6± 1.0)ml/m2 vs (16.5±0.9)ml/m2,P =0.0438;SV:(12.3±4.6)ml/m2 vs (10.1±3.2) ml/m2,P =0.0433].②Compared with the control group, the global and segmental T-msv were all delayed obviously in the ESRF group (P < 0.001).③The percentage of SV of body compartment accounting for the global SV significantly increased in the ESRF group [(19.5 ± 7.3)% vs (15.5 ± 5.5)%,P =0.0222],while that of outflow compartment reduced significantly [(20.3 ± 7.0)% vs (26.8 ± 8.2)%,P =0.0005].Conclusions The global and segmental systolic functions of right ventricle were impaired in ESRF patients, especially in the body compartment. The RT-3DE and 4D-RVQ are helpful technologies in the evaluation and follow-up of right ventricular systolic function in patients with renal failure.

Objective This study aimed to explore the experience and needs of dignity-conserving care of disabled elderly people in nursing homes, and to provide a basis for formulating a quality evaluation system for dignity-conserving care. Methods This descriptive qualitative study, involving a semi-structured in-depth interview of 13 disabled elderly people based on purposive sampling, was conducted from August to November 2022 at a nursing home in Hangzhou.Based on the harmony nursing theory, the directional content analysis method was performed in order to summarize and extract themes from the interview texts. Results The dignity-conserving care experience and needs of disabled elderly people were extracted and classified into 5 themes, including physical safety(physical health maintenance, privacy protection, hygiene and image maintenance),psychological health(psychological counseling, psychological support),social communication(well communication environment, harmonious way of communication),spiritual culture(self-realization,respect for faith),needs of dignity-conserving care(individualized health needs, activity participation needs, dignified communication needs, spiritual needs of respecting the elderly). Conclusion When conducting the dignity-conserving care, nursing homes should pay attention to the physical health of the disabled elderly, actively maintain the image and privacy of the disabled elderly people, pay attention to the needs of psychological health, provide psychological counseling and support, strengthen the construction of barrier-free environments and cultural environments, build mutual respect way of communication, fully respect the religious beliefs of disabled elderly people, promote the realization of self-value of disabled elderly people, broaden the channels of personalized service and activity participation, strengthen the interaction of the dignity and spirit of the respect for the elderly of nursing homes, comprehensively improve the quality of dignity-conserving care in nursing homes.

OBJECTIVETo explore the value of electronic data capture (EDC) system in large-sample size studies on Kawasaki disease (KD).
METHODThe clinical data of 602 KD cases from 2007 to 2012 admitted to Shanghai Children's Hospital with EDC system connected with hospital information system (HIS) were retrospectively analyzed. Age, gender, acute symptoms, laboratory results, echocardiography, therapy were collected. The differences in parameters were compared between KD with and without coronary artery lesion (CAL). Furthermore, the difference between intravenous immunoglobulin (IVIG) resistant group and sensitive group were compared. Multi-factor logistic regression analyses were used to analyze the risk factors. The sensitivity and specificity of IVIG resistance parameters were detected with receiver operating characteristic curve (ROC) analysis.
RESULTThe male to female ratio of KD cases was 1.85: 1. The median age of KD cases was 2.0 years (one month to 11.7 years old); 20.1% cases (121/602) exhibited CAL. Compared with KD without CAL (n = 481), the incidence of bright red cracked lips (71.1% vs. 88.6%, P = 0.001), peeling of the skin of the toes (28.1% vs. 41.6%, P = 0.021) and perianal skin peeling (29.8% vs. 38.9%, P = 0.031) were statistically lower in KD with CAL (n = 121). The incidence of CAL in KD IVIG resistant group was significantly higher than KD IVIG sensitive group (34.6% (9/26) vs.21.3% (112/525), P = 0.05 ). Male ratio (80.8% vs. 63.4%, P = 0.05), time of IVIG ( (6 ± 2) vs. (8 ± 5) d, P = 0.009), erythrocyte sedimentation rate(ESR) ( (81 ± 2) vs. (66 ± 30) mm/1 h, P = 0.014), C-reactive protein ((107 ± 51) vs. (87 ± 52) mg/L, P = 0.017), blood platelet ( (599 ± 178) vs. (489 ± 182) ×10(9)/L, P = 0.003), hemoglobin ( (96 ± 13) vs. (102 ± 19) g/L, P = 0.032) and albumin ((34 ± 6) vs. (37 ± 6) g/L, P = 0.020) were significantly different between IVIG resistant group and sensitive group. Logistic regression analysis showed that alanine aminotransferase (ALT) ≥ 80 U/L was the independent risk factor of IVIG resistance (P = 0.012). C-reactive protein = 104 mg/L (sensitivity 61.5%, specificity 62.7%), ESR = 106 mm/1 h (sensitivity 26.9%, specificity 93.6%) and blood platelet = 187×10(9)/L (sensitivity 76.9%, specificity 53.1%) of KD in acute phase were predictive for IVIG resistance with receiver operate characteristic curve analysis.
CONCLUSIONEDC system can acquire KD clinical data quickly and accurately. It is helpful for multicenter retrospective analysis of KD.
