1.Molybdenum Target X-ray Diagnosis of Ductal Carcinoma in Situ of Breast:A Report of 20 Cases

Objective To study radiologic (molybdenum target X-ray) findings of ductal carcinoma in situ (DCIS) of breast.Methods The molybdenum target X-ray findings of breast DCIS confirmed by operation or pathology in 20 cases were analyzed.All cases were examined with molybdenum target X-ray at axial and lateral oblique positions in both breasts.Results Among 20 patients,16 cases presented calcifications in the lesions,3 cases showed simple masses,1 case appeared as small dense focus.Conclusion Calcifications are mainly X-ray appearances of DCIS,special attention should be payed to the signs such as masses,small dense focus and so on.
2.Cutaneous intravascular NK/T cell lymphoma: a case report

An 18-year-old female presented with painful erythema and nodules on both legs for more than 2 years.Dermatological examination showed irregularly sized,mildly indurated,tender,deep subcutaneous nodules arising in diffused infiltrated dark erythematous patches in the inner and posterior region of the left leg.Histopathology showed no significant changes in the epidermis.There were perivascular lymphoid cell infiltrates in the dermis and subcutis.Multiple sites of necrosis of blood vessel walls with vascular occlusions were noted.The lumens of some blood vessels were filled with lymphocytes,among which were many atypical cells with hyperchromatic nuclei and pathologic mitotic figures.Immunohistochemistry showed that lymphocytes in the cavities of blood vessels were positive for CD3(+++),CD3ε(+++),CD2(+),CD56(+++),granzyme B(+++),perforin(+++),CD30(+),Ki67 (+++,100% ),but negative for CD20,CD5,CD7,CD4,CD8,TdT,anaplastic lymphoma kinase,early membrane antigen (EMA) or pan cytokeratin (pCK).The endothelial cells lining the blood vessels stained positively for CD34.The intravascular lymphocytes were also positive for EBER1/2 by in situ hybridization.A diagnosis of cutaneous intravascular NK/T cell lymphoma was made.
3.Effects of Tangbikang on the Expressions of p38 MAPK of Sciatic Nerve and Plasma TNF-αin Diabetic Rats

Objective To explore the effects of Chinese herbal compound Tangbikang on the expressions of p38 MAPK of sciatic nerve and plasma TNF-α in diabetic rats. Methods Ten of the sixty male SD rats were selected randomly as normal group, and the rest were fed with high-fat diet and low-dosage STZ was used to induce type Ⅱdiabetic rat models. Model rats were randomly divided into model group, mecobalamine group and Tangbikang low-, medium-, and high-dosage groups, 10 rats in each group. Each medication group was intervened with relevant medicine. Rat unilaterals sciatic nerves were taken after 16 weeks. The content of TNF-α in plasma was determined by radioimmunoassay. Western blot method was used to detect the expressions of p38 and p-p38 MAPK protein of sciatic nerve. Results Compared with normal group, the expressions of p38 and p-p38 protein and content of TNF-αin model group significantly increase (P<0.05, P<0.01). Compared with the model group, the expressions of p-p38 protein and the content of TNF-α significantly decreased after medicine intervention in different doses Tangbikang groups and mecobalamin group (P<0.05, P<0.01). The expression of p38 protein in Tangbikang high-dose group significantly decreased (P<0.05), with statistical significance (P<0.05). Conclusion Tangbikang can reduce the expression of p38 and p-p38 MAPK protein of the rat sciatic nerve, and reduce the content of TNF-α protein in rat plasma, which may be one of the effective targets of neuroprotection and abirritation of diabetic peripheral neuropathy.
4.Clinical research on Shenqi-Dihuang Decoction combined with conventional western medicine in the treatment of stage Ⅳ diabetic nephropathy

Objective:To observe the clinical efficacy of Shenqi-Dihuang Decoction combined with conventional western medicine in the treatment of stage Ⅳ Diabetic Nephropathy (DN). Methods:Made a retrospective analysis of 80 patients of stage Ⅳ DN with the syndrome of qi and yin deficiency combined with blood stasis from June 2017 to June 2018, from Beijing Hospital of Traditional Chinese Medicine and The Third Affiliated Hospital of Beijing University of Chinese Medicine. The 80 patients were divided into treatment group and control group, 40 in each group. The control group was given the conventional western treatment. The treatment group was given Shenqi-Dihuang Decoction on the basic of the control group. Both of the treatments for the two groups lasted for 12 weeks. Before and after the treatment, the changes of TCM syndrome scores were observed in each group. The automatic biochemical analyzer was used to detect the level of fasting plasma glucose, 2-hour postprandial glucose (2 hPG), Boold Urea Nitrogen (BUN), Serum Creatinine (SCr), Plasma Albumin (Alb), Total Cholesterol (TC), Triglyceride (TG), Uric Acid (UA), Superoxide Dismutase (SOD), Homocysteine (HCY) before and after treatment. The Glycosylated Hemoglobin Alc were measured by high performance liquid chromatographic (HPLC), 24-hour urine total protein quantity (24 hUTP) were measured by immunoturbidimetric assay before and after treatment, and then Glomerular Filtration Rate (eGFR) was estimated to evaluate clinical effect. Results:After the treatment, the total effective rate in the treatment group was 87.5% (35/40), and the control group was 67.5% (27/40) and the difference was statistically significant ( χ2=4.557, P=0.033). After the treatment, the scores of urine turbid, weakness of waist and knees, fatigue, edema of both lower limbs were decreased in both groups, and the treatment group was significantly better than those of the control group ( t value were -2.178, -1.675, -3.667, -1.904, -4.835, respectively, all Ps<0.01). After the treatment, the level of FPG and HbA1c were significantly lower than those of the control group ( t value were-3.781, -8.557, respectively, all Ps<0.01); the systolic blood pressure was significantly lower than that of the control group ( t=-2.883, P=0.005); the level of TC was significantly lower than that of the control group ( t=-3.321, P=0.008); the level of Alb was significantly higher than that of the control group ( t=1.510, P=0.367); the level of 24 hUTP was significantly lower than that of the control group ( t=-2.819, P=0.008); the level of HCY were significantly lower than that of the control group ( t=-2.053, P=0.043); the level of SOD was significantly higher than that of the control group ( t=2.849, P=0.018). Conclusions:Shenqi-Dihuang Decoction combined with conventional western medicine can reduce 24 hUTP quantity of Ⅳ DN patients with the syndrome of qi and yin deficiency combined with blood stasis, reduce kidney damage, delay the development of Ⅳ DN, improve clinical effect and protect the kidney function.
5.Effect of Compound Duzhong Jiangu Granules on joint function, quality of life and inflammatory factors in patients with Kashin-Beck disease

Objective:To study the effect of Compound Duzhong Jiangu Granules on joint function, quality of life and inflammatory factors in patients with Kashin-Beck disease.Methods:By group design, 135 patients with Kashin-Beck disease in Chongxin County, Pingliang City, Gansu Province, were selected and divided into intervention group (100 cases treated with Compound Duzhong Jiangu Granules, 12 g/bag, 1 bag/time, 3 times/day, treatment for 1 month) and control group (35 cases treated with ibuprofen, 0.3 g/capsule, 1 capsule/time, 2 times/day, treatment for 2 weeks) according to the randomized, single-blind principle. The changes of joint dysfunction index score, joint function improvement rate, total scores of Kashin-Beck disease quality of life (KBDQOL) and scores of each dimension were analyzed before treatment, 1 month and 3 months after treatment. The serum levels of interleukin-6 (IL-6), nuclear factor κB (NFκB)-p65, inducible nitric oxide synthase (NOS2), nitric oxide (NO), and prostaglandin E2 (PGE2) were detected by enzyme-linked immunosorbent assay before treatment and 1 month after treatment.Results:The total scores of joint dysfunction index of the intervention group and the control group 1 month after treatment and 3 months after treatment were lower than those before treatment, but 3 months after treatment was higher than 1 month after treatment ( P < 0.001). One month after treatment, the total effective rates of joint function improvement in the intervention group and the control group were 68.00% (68/100) and 54.55% (18/33), respectively; 3 months after treatment, the total effective rates of the intervention group and the control group were 36.00% (36/100) and 39.39%(13/33), respectively. The total scores and scores of each dimension of KBDQOL were not significant for the main effect of the group ( P > 0.05), but significant for the main effect at the time point ( P < 0.05), and there was no interactive effect ( P > 0.05). There were significant differences in the scores of social support and mental status dimensions in the intervention group at different time points ( P < 0.001). And in the intervention group, the total score, the scores of physical function, activity limitation, economy and overall health dimensions were statistically significant between before treatment and 1 month after treatment, 1 month after treatment and 3 months after treatment( P < 0.05); however, there was no significant difference between before treatment and 3 months after treatment ( P > 0.05). One month after treatment, the serum PGE2 levels of both groups were decreased ( P < 0.05), and there was no difference in other inflammatory factors at different groups and time points ( P > 0.05). Conclusion:Compound Duzhong Jiangu Granules can effectively inhibit the inflammatory reaction of patients with Kashin-Beck disease, promote the improvement of joint function and improve the quality of life in various aspects.
