1.The influence of Maixuekang therapy on hematoma absorption , cerebral edema and neural function in patients with acute intracerebral hemorrhage

Objective To observe the influence of Maixuekang therapy on hematoma absorption,cerebral edema and neural function in patients with acute intracerebral hemorrhage within 3 weeks.Methods A total of 89 patients with acute intracerebral hemorrhage were divided into control group (45 cases) and treatment group (44 cases) by random digits table method.The two groups were implemented routine therapy and individualized rehabilitation treatment.Furthermore,the patients in treatment group were treated with Maixuekang (1.0 g,thrice a day for 3 weeks),if reexamination result of cerebral CT showed no expanding hematoma after 24 h from the onset of intracerebral hemorrhage.Cerebral hematoma and edema volume was measured by brain CT before therapy and on the 7th,14th and 21st day after therapy,and neurologic impairment was scored at the same time points by neurological impairment scale(NIS).The results were analyzed with statistics.Results The cerebral hematoma volume,edema volume and NIS before therapy,on the 7th,14th and 21st day after therapy in two groups respectively was (22.4 ± 8.4) ml,(9.5 ±2.6) ml,(25.4 ±3.8) scores,(21.7 ±7.6) ml,(19.9 ±7.1) ml,(25.3 ±3.4) scores,(18.9 ±6.7) ml,(18.3 ± 5.7) ml,(23.8 ± 3.0) scores,(15.3 ± 5.4) ml,(14.5 ± 4.8) ml,(21.7 ± 2.5) scores in control group; (22.3 ± 8.6) ml,(9.4 ± 2.5) ml,(25.3 ±3.6) scores,(19.6 ± 7.0) ml,(17.3 ± 6.4) ml,(23.9 ± 3.2) scores,(15.5 ±5.9) ml,(15.1 ±4.5) ml,(20.5 ±2.8) scores,(11.2 ±4.5) ml,(10.7 ±3.3) ml,(17.4 ± 2.1) scores in treatment group.The cerebral hematoma volume and edema volume was significantly smaller in treatment group than that in control group after therapy (P < 0.05 or < 0.01),but did not show significant difference compared with that before therapy (P > 0.05).Conclusion Maixuekang therapy can significantly reduce cerebral edema and contribute to hematoma absorption and nerve functional recovery in patients with acute intracerebral hemorrhage.
2.Pathogenic Bacteria in Emergency Department and Antibiotic Treatment

OBJECTIVE To study the characteristic of pathogenic bacteria isolated from patients in the emergency department and to investigate the relativity of experience and targeted antibiotic treatment.METHODS The pathogenic bacteria and drug sensitivity results of all samples in 2005 were analyzed and the relativity of experience and targeted antibiotic treatment was summarized.RESULTS Totally 1061 specimens were acquired from 463 patients including sputum,venous blood,middle urine,purulent fluid on wound surface and 312 strains pathogenic bacteria were isolated.52.09% of them were Gram-positive bacteria,28.29% were Gram-negative bacteria and 19.16% were fungi.Of all pathogenic bacteria,there were 44 strains of drug-resistant bacteria.The relativity of experience and targeted antibiotic treatment was 89.67%.CONCLUSIONS The patients admitted to the emergency department are in severe and complicated conditions,with multiple system diseases.The pathogenic bacteria of emergency department are different from the bacteria of other units.Antibiotic treatment by experience should combined with bacteria monitoring and drug sensitivity.Infection caused by fungi should be paid great attention.
3.Effectiveness of integrated teaching designed according to nurse's core competency

Objective To explore the effects of the integrated teaching," teaching,studying and doing",which designed according to the nurse's core competency. Methods The integrated teaching of "teaching,studying and doing" was applied to the experimental class. The basal portions of the integrated teaching were as follows:searching for the Internet information,Sharing information between groups,per-forming cleaning care for the elders living in welfare agency according to nursing process,writing Nursing History or reflections. Results Comparison of mean scores between the experimental class and the con-trol one predicted significant differcences on theoretical and practical test,and the former was better than the Last. Conclusions The integrated teaching designed according to the nurse's core competency was more effective than the traditional teaching method,since the students from the experimental class valued the knowledge and skills immanently,and were involved in applying,reviewing them in time.
4.Significance of expression of HMGB1 and Smac/DIABLO protein in epithelial ovarian cancer

ObjectiveThe purpose of this study was to investigate the clinical significance of HMGB1 and Smac/DIABLO protein expression in epithelial ovarian cancer.MethodsImmunohistochemistry (IHC) and terminal deoxynucleotidyl transferase-mediated dUTP nick end-labeling (TUNEL) staining were used to detect the protein expression of HMGB1,Smac/DIABLO and cell apoptosis in 45 epithelial ovarian cancers by tissue microarray.ResultsThe protein expression of HMGB1 in epithelial ovarian cancers was significant higher than benign cancers and normal ovarian tissues( P <0.05).The protein expression of HMGB1 was related to the lymph node metastasis,which had a negative correlation with the cell apoptosis index ( rs =-0.583,P =0.000).The protein expression of Smac/DIABLO in epithelial ovarian cancers was significantly lower than benign cancers and normal ovarian tissues( P <0.05) ; the protein expression of HMGB1 in epithelial ovarian cancers had a negative relationship with protein expression of Smac/DIABLO( rs =-0.40,P =0.006).ConclusionsThe over-expression of HMGB1 in epithelial ovarian cancers may play an important part in the metastasis of the epithelial ovarian cancers.The over-expression of HMGB1 and the low-expression of Smac/DIABLO may take part in the occurrence and development of the epithelial ovarian cancers through abnormal cell apoptosis.
5.Correlation with expression of HMGB1 mRNA and clinicopathological features of endometrial carcinoma

Objective To investigate the expression of HMGB1 mRNA in endometrial carcinoma and its correlation with clinicopathological features of endometrial carcinoma. Methods Semi-quantitative RT-PCR method was used to detect the expression level of HMGB1 gene in 56 cases of endometrial carcinoma and 20 cases of normal endometrium. Results The expression of HMGB1 mRNA in endometrial carcinoma (0. 3512 ± 0. 0985 ) was significantly higher than that in normal endometrium (0. 2208 ± 0. 0170 ).There was a significant difference in the two groups( P <0. 05 ). Grouping by clinicopathological features,the expression of HMGB 1 mRNA in endometrial carcinoma had a correlation with clinical-surgical stage ( P<0. 05), metastasis of lymph nodes ( P < 0. 05), and depth of myometrial invasion ( P < 0. 05 ). Conclusion The high level of HMGB1 mRNA indicated that HMGB1 might play an essential role in the genesis, growth, invasion and metastasis of endometrial carcinoma.
6.Mediating effect of emotional labor between professional values and job burnout among ICU nurses

Objective:To explore the mediating effect of emotional labor between professional values and job burnout among ICU nurses, and to provide a reference for reducing ICU nurses' job burnout and motivating their work enthusiasm.Methods:From June to July 2017, 276 ICU nurses from 3 comprehensive ClassⅢ Grade A hospitals in Nanchang were selected as the subjects by convenience sampling. The General Information Questionnaire and Nurses' Emotional Labor Scale, Professional Values Scale and Job Burnout Scale were used to investigate. Structural equation model was used to explore the mediating effect of emotional labor between professional values and job burnout.Results:A total of 276 questionnaires were issued and 249 valid questionnaires. The average score of the Nurses' Emotional Labor Scale of 249 ICU nurses was (3.55±0.45) , the average score of the Professional Values Scale was (3.63±0.41) , and the dimensions scores of the emotional exhaustion, depersonalization and personal accomplishment of the Job Burnout Scale were (2.77±1.07) , (1.54±1.26) , and (3.78±1.16) . The structural equation fitting indexes were as follows: χ 2/ df=1.155, GFI=0.976, AGFI=0.942, RMSEA=0.027. Professional values and emotional labor of ICU nurses had direct predictive effect on job burnout. Professional values also used emotional labor as an intermediary variable, which indirectly affected job burnout. Conclusions:Nursing managers should pay attention to reducing the job burnout of ICU nurses by improving the level of ICU nurses' professional values and optimizing their emotional labor strategies from the individual factor level.
