1.Changes of flexor-extensor strength during trunk isokinetic exercise: Angle ranges easily lead to spinal injury

BACKGROUND: Studies concerning trunk muscles mainly focus on muscle strength changes in patients with low back pain,which is rare regarding angle ranges that easily lead,to spinal injury.OBJECTIVE: To explore the mechanics characteristic of flexor-extensor and angle ranges easily lead to spinal injury. METHODS: Fourteen healthy, male, post-graduates of Soochow University were selected. Flexor-extensor strength and spinal angle of participants were measured using CON-TREX isokinetic testing system. All participants took isokinetic centrifugal exercise maximally at the speeds of 30, 60, 90, 120, 180 (°)/s, respectively, with a 5 minutes interval. Main outcome measures:①Lumbar range of motion; ②Peak torque, flexor/extensor, the angle at peak torque at isokinetic centripetal exercise; ③Average power and total work at isokinetic centripetal exercise.RESULTS AND CONCLUSION: ①During isokinetic centripetal exercise, the peak torque of extensor were decreased as the geniovelocity increased (P < 0.05), however, no inerratic changes could be found in flexor peak torque. Though the flexor/extensor was increased with geniovelocity increased, the difference had no significance (P > 0.05). ②Dudng slow isokinetic centripetal exercise underwent different geniovelocity, the angle at peak torque of flexor and extensor distributed discretely, -48.56° at 30 (°)/s; -46.18° at 90 (°)/s; when underwent fast isokinetic exercise, the angles at peak torque of flexor and extensor were very close, which were -48.71° and -51.61° at 120 (°)/s, and -54.86° and -53.11° at 180 (°)/s. ③During isokinetic centripetal exercise, when underwent different geniovelocity, total work of flexor and extensor was reduced with geniovelocity expedited, extensor total work was more than that of flexor (P < 0.05); the average power of flexor and extensor was increased linearly with geniovelocity expedited, and the average power of flexor was less than that of extensor all along (P < 0.01). During isokinetic centripetal exercise, trunk muscle strength descended with the geniovelocity expedited, and the stability of trunk is preferable in slow exercise. During fast isokinetic centripetal exercise, angles at flexor and extensor peak torque are more discrete, which easily result in muscle damages and spine unstable when suffered a suddenly outside attack. Total work of flexor and extensor are descended with the geniovelocity expediting during isokinetic exercise, but the intramuscular eruption increased.

Depression is one of the most common mental disorders.Since 1960s,researchers have proposed a number of etiological hypotheses from the aspects of genetics,biochemistry,neuroendocrine and immune,but so far it has not been fully under-stood.Toll like receptor 4(TLR4),as a bridge between innate immunity and adaptive immunity,could induce inflammatory by activating related signal transduction pathways and promoting the expression of inflammatory factors in c-ells.More and more studies showed that the occurrence of depression was related to inflammation.Reviewing relationship of TLR4 and depressive disorder and exploring the effects of some interventions for depression on TLR4.Research on TLR4 shed light on pathological mechanism of depressive order.TLR4 could play an important role in inhibiting the production of 5-serotonin,the disorders of HPA axis,interfering with the hippocampus and cortical nerve regeneration,the autophagy of hippocampus.
3.Investigation and analysis of cognitive demnds of the discharge patients with T tube

Objective To investigate the situation of cognitive demands for disease of discharged patients with T tube,in order to adopt a more rational and effective interventions to meet the needs of patients.Methods 60 patients were investigated with the questionnaire survey designed to gauge patients'cognitive demands for disease and health knowledge.Results The patients had higher cognitive demands for T tube retention time,how to make self-observation and influence of T tube retention on normal work and life,among these patients,they laid different particular emphasis to some items between different sexes.Conclusions Nurses can provide patients with comprehensive,prioritized and individualized health guidance according to cognitive demands of patients,so that the health education quality and work efficiency can be improved.
4.Kindred-like Nursing for Severe Craniocerebral Trauma Patients

Objective To observe the effect of kindred-like nursing on coma after severe craniocerebral trauma. Methods 48 cases of sereve craniocerebral trauma were divided into two groups: the control group (n=24) who accepted routine nursing, and the observation group (n=24) who accepted the kindred-like nursing in addition. Results More patients revived in the observation group than in the control group (P<0.05) 14 and 28 d after admission. Conclusion Kindred-like nursing can facilitate the reviving from coma after severe craniocerebral trauma.

6.Safety of Ultrasound-guided Multifetal Pregnancy Reduction in the Second Trimester

PurposeTo evaluate the safety and feasibility of transabdominal ultrasound guided multifetal pregnancy reduction performed in the second trimester.Materials and Methods Forty-three women receiving embryo implantation were divided into two groups according to whether they have received multifetal pregnancy reduction including 16 in the reduction group in the second trimester to reduce multiple fetuses to twins, and 27 with twin pregnancy and no reduction. The pregnancy outcome was compared.Results In the reduction group, 1 patient had miscarriage because of intrauterine infection; 1 patient lost one of the retained fetuses in two days after reduction and delivered the remaining fetus. In the other 14 cases, 8 delivered and 6 were still in pregnancy. This group had 17 newborns, all without neonatal death or birth defects. In the non-reduction group, there was 1 spontaneous abortion. In 2 patients, 1 of the twin embryos stopped development. 1 case had a spontaneous abortion, the other 24 cases delivered uneventfully. There were 50 newborns in this group with 3 birth defects and no neonatal death. There was no statistically significant difference in gestational age at delivery, birth weight, and the incidence of maternal and neonate complications (P>0.05).Conclusion Multifetal reduction improves pregnancy outcomes, similar to non-reduction twin pregnancy. It is safe and feasible to perform multifetal pregnancy reduction in the second trimester.
7.Analysis of application of common laboratory indexes in kidney diseases

Objective To investigate the value of laboratory testing in kidney diseases and to assess whether these marks can predict the renal histological lesions for screening the best clinical test combination.Methods The main clinical data,pathological types and laboratory examination indexes in 117 patients with renal biopsy in our hospital from December 2010 to March 2014 were collected and graded according to the WHO renal pathological classification standard and the Katafuchi semi-quantitative score standard classification (Ⅰ,Ⅱ,Ⅲ,Ⅳ).The differences in serum CysC,urine NAG,urine alpha 1-MG,urine mAlb and 24 h urine total protein (24h-UTP)level were compared among the various groups and their relations with the glomerulus was analyzed;the correlation and the statistical significance of each index in different groups was analyzed.Results NAG,alpha 1-MG and CysC had statistical differences between in the grade Ⅳgroup and the gradeⅠ,Ⅱ,Ⅲ groups (P <0.05),24h-UTP and mAlb had significant correlation in reflecting the kidney diseases (r=0.565,P =0.000);NAG and alpha 1-MG also had significant correlation in reflec-ting renal tubular interstitial disease (r=0.509,P =0.000).Conclusion The detection results of various detection indicators when rising to a certain height can be used for judging the progression of the patients clinical and pathological condition,illustrating the serious condition of illness;at the same time according to the correlation among the indexes and the detection efficiency,three indi-cators of Cys C,mAlb,alpha 1 - MG are screened out for conducting the combination detection,the detection efficiency could be greatly improved.
9.Effect of Electro-acupuncture on Mammalian Target of Rapamycin Expression in Hypoxic-ischemic Encephalopathy Newborn Rats

Objective To observe the intervention effect of electro-acupuncture(EA) on mammalian target of rapamycin(mTOR) expression in hypoxic-ischemic encephalopathy(HIE) newborn rats. Methods Seven-day-old SD rats were randomly assigned into 3 groups, namely sham-surgery group, HIE model group, and EA group. All of the groups were subdivided into 4 subgroups in terms of 4 time periods of 1, 3, 7 and 21 day(s). Hematoxylin-eosin staining was used for the observation of histological changes in the affected cerebral region. Expression levels of cerebral mTOR were detected with Western blotting method. Results The results of HE staining showed that EA group at experimental day 21 had clear organizational structure, ordered arrangement of neurons, relieved cellular swelling, well cell integrity with clear nucleolus and proliferative glial cells. Compared with the sham-surgery group, mTOR level of the model group was dramatically increased on the 1st, 3rd, 7th and 21st day(P<0.05), and the mTOR expression level of EA group was significantly higher than that in the model group at the above time points (P < 0.05). Conclusion EA can promote mTOR expression in brain tissue and can protect the brain of newborn rats from HIE.
