1.Clinical research in humidifying methods of artificial airway

Objective To explore the best humidifying methods of artificial airway by comparison of different humidifying approaches.Methods 124 patients were divided into the control group (60 cases) and the treatment group (64 cases).Two methods,continuous airway humidifying by the trace injection pump and continuous airway humidifying by oxygen atomization were adopted.The humidifying effect,tolerance rate of patients,the airway complications and nursing cost were compared between the two groups.Results The method of continuous airway humidifying by oxygen atomization had better effect,was easy to be tolerated,less complications and less cost.Conclusions Continuous airway humidifying by oxygen atomization is a safe,effective,low-cost method,it is easy to be tolerated by patients and is worthy of being popularized.
2.Villi Chromosome High-throughput Gene Sequencing of 52 Cases of Missed Abortion

Objective To understand the relationship between embryonic chromosomal abnormalities and missed abortion by high-throughput gene sequencing, so as to provide a basis for guiding the next pregnancy.Meth-ods By chromosome detection on villi chromosome of 52 cases of missed abortion by high-throughput gene sequen-cing technique, the types of chromosome abnormalities and the proportion were investigated.Results Among villi samples from the 52 cases of missed abortion, chromosomal abnormalities were founded in 28 cases, including 24 ca-ses of number abnormality (17 cases of trisomy, 6 cases of triploid and a case of monomer) and 4 cases of structural abnormalities.Conclusion Embryonic chromosomal abnormality is the main cause of missed abortion in early preg-nancy.High-throughput gene sequencing technology used to detect the villi chromosome is comprehensive, fast and accurate, helping to clarify the cause of missed abortion and guide the next pregnancy reasonably.
3.IgM nephropathy with nephrotic syndrome in children

Objective To investigate the clinical charactors and treatment of IgM nephropathy with nephrotic syndrome(NS) in children.Methods Thirty-six IgM nephropathy patients hospitalized in Hunan Children's Hospital as research group(group A),from June 2005 to June 2012.One hundred and six patients with minimal change disease (MCD) as control group (group B),followed up for 1 ~ 8 years.Results (1) Hematuria at presentation of the two groups respectly 3.8% vs 30.6% (x2 =20.403,P < 0.05).(2) Renal pathology revealed that there were 26 (72.2%)patients with minimal change disease,9 cases (72.2 %)with moderate membranoproliferative glomerulonephritis,1 cases with focal segmental glomerulosclerosis.(3) According to renal pathology,group A patients were divided into two sub groups:mild group and moderate group.To compared with group B,the steroid-resistant incidence of the 3 group were respectly 12.3%,19.2%,77.8% (x2 =24.369,P < 0.05).There was no significant difference between control group and mild group.(4)The remission rate of proteinuria in steroid-resistant patients who combined to use mycophenolate mofetil (MMF) with the two groups were respectively 50% and 85.7 % (x2 =3.60,P > 0.05).Conclusion Incidence of hematuria is higher in IgM nephropathy patients and patients with renal pathology for moderate lesions have a high steroid-resistant,and need use immunosuppressive early.MMF may be a good immunosuppressive for theses patients.
4.Storage Problems and Countermeasures for Injection Drugs Protected from Light in Inpatient Pharmacy of Our Hospital

OBJECTIVE:To provide reference for storage management of injection drugs protected from light in inpatient phar-macy of hospital. METHODS:From Jan. to Mar. 2016,the storage items of injection drugs protected from light in inpatient phar-macy of our hospital and the current storage situation were surveyed,the quality management tool PDCA circulation was adopted to improve storage situation and evaluate the effect of PDCA. RESULTS:For improper storage of injection drugs protected from light caused by employees lacking knowledge in photophobic knowledge,unfamiliar to the list of injection drugs protected from light, no packaging in returned drugs,after developing and carrying out corresponding countermeasures,including reinforcing staff train-ing,studying characteristics of the new-coming drug timely,renewal photophobic knowledge and the list of injection drugs protect-ed from light,and ameliorating working procedure,the light storage pass rate was increased from 96.5% to 100% in 310 injection drugs protected from light in inpatient pharmacy. CONCLUSIONS:Appling PDCA can effectively improve the storage management of injection drugs protected from light and provide reliable protection for the drugs'safe and effective application.
5.The effects of ammonium perchlorate (AP) on pulmonary fibrosis

Objective To explore the effects of AP on pulmonary fibrosis. Methods Alveolar macrophages (AM) were treated by AP for 24 hours. Pulmonary fibroblasts (FB) were cultured with the supernatant of AM medium. The protein of transforming growth factor beta 1 (TGF-?1) of AM, the proliferative activity and hydroxyproline (Hyp) content of FB were examined. Rats were treated with AP by intratracheal instillation and sacrificed at 3 days. The TGF-?1 mRNA content in the lung was examined. Objective To investigate the influence of two general anesthestic modes on postoperative cognitive dysfunction (POCD) in the patients undergoing laparoscopic radical hysterectomy .Methods One hundred ASA Ⅰ‐Ⅱ patients undergoing lapa‐roscopic radical hysterectomy were randomly allocated to the propofol group (group P) and sevoflurane group (group S) ,50 cases in each group .The anaesthesia time ,total dose of sufentanil ,total dose of vecuronium ,recovery time ,recovery time for regaining ori‐entation and complications during anesthetic recovery period were recorded .The cognitive function was assessed by the mini‐mental state examination (MMSE) on preoperative 1 d (T0 ) ,postoperative 1 d (T1 ) ,postoperative 3 d ,(T2 ) ,postoperative 7 d (T3 ) ,post‐operative 1 month (T4 )、postoperative 3 months (T5 ) and the POCD occurrence situation was evaluated by adopting the Z scoring . Results The total dose of sufentanil and vecuronium in the group S was lower than that in the group P (P<0 .05) ,the recovery time and time for regaining orientation in the group S was longer than that in the group P (P<0 .05);the incidence rates of shive‐ring ,dysphoria and upper respiratory tract obstruction in the group S were higher than those in the group P (P<0 .05) .There were no statistically significant difference in the MMSE scores between the two groups (F=0 .14 ,P=0 .709);the MMSE scores in each group had statistical differences among different time points (F=74 .46 ,P<0 .01) .The interaction effect existed between the gen‐eral anesthetic mode and time with MMSE score (F=7 .99 ,P<0 .01);the MMSE scores at T1 ,T2 in the group S were lower than those in the group P (P<0 .05) .The incidence rate of POCD at T1 ,T2 、T3 ,T4 in the group S was higher than that in the group P (P<0 .05) .Conclusion The incidence rate of POCD in the patients undergoing laparoscopic radical hysterectomy by adopting sevoflurane inhalation general anaesthesia is higher than that by adopting propofol anesthesia ,but which has no difference after postoperative 3 months .
7.Analysis of clinical pathology of the IgA nephropathy and purpura nephritis in children from the perspective of IgA vasculitis

Objectives To analysis clinical pathology of organ speciifc IgA vasculitis (IgA nephropathy) and systemic IgA vasculitis (allergic purpura) of purpura nephritis in children. Methods Clinical and pathological data of hospitalized pediatric patients of IgA nephropathy and purpura nephritis were retrospectively analyzed from June 1993 to November 2014. Results There were 405 patients of IgA nephropathy (256 males and 149 females). The ratio of male to female was 1.7:1. The average age was 10.2±2.8 years. The nephrotic syndrome (31.6%) was the most common clinical type, followed by hematuria and proteinuria (27.9%). There were 548 patients of purpura nephritis, 329 males and 219 females. The ratio of male to female was 1.5:1. The average age was 10.2±3.1 years. The hematuria and proteinuria (61.6%) was the most common clinical type, followed by nephrotic syndrome (21.4%). None of the IgA nephropathy progressed to systemic vasculitis (allergic purpura). Conclusions The causes, onset ages and clinical manifestations of IgA nephropathy and allergic purpura may be consistent or overlap, but none of IgA nephropathy (organ speciifcity IgA vasculitis) progressed to allergic purpura (systemic IgA vasculi-tis). IgA nephropathy might have more renal immune disorder mechanisms involved in its pathogenesis.
8.Subjective well-being of coronary heart disease undergoing percutaneous coronary interventions patients and its influencing factors

Objective To investigate the current status of subjective well-being in patients with coronary heart disease (CHD) undergoing Percutaneous Coronary Interventions and its influence factors. Methods Totally 140 cases of coronary heart disease undergoing percutaneous coronary interventions were investigated with the general information questionnaire, General Well-being Scale, Self-rating Anxiety Scale and Self-rating Depression Scale, and analyzed the influence factors of subjective well-being. Results There were patients whose subjective well-being scores were lower than the average of Chinese norm among the 140 CHD patients,which took a proportion of 75.71%(106/140).The subjective well-being score in male patients was 73.33±6.43,which was lower than 75.00±15.00 of the Chinese norm (t=-2.112,P<0.05).The subjective well-being score in female patients was 65.15±8.32,which was lower than 71.00 ± 18.00 of the Chinese norm (t=-5.981, P<0.01); the self-rated health status and anxiety disorder were the main factors of subjective well-being in coronary heart disease undergoing Percutaneous Coronary Interventions patients. Conclusions The subjective well- being of patients with CHD undergoing percutaneous coronary interventions is poor; the society and family should give them much enough care and support, and effective psychological intervention measures should be taken to improve their subjective well-being.
9.Speaker gender identification based on audio fractal dimension and pitch feature.

Automatic speaker gender identification based on voice feature is an important task in voice processing and analysis fields. In this paper non-linear parameters such as fractal dimension are applied to be one part of feature space for improving the ability of describing speaker gender feature through conventional linear parameters method. Pitch is picked using lifting scheme, and audio fractal dimension is extracted. Then based on Takens theory, the time delay method is used to reconstruct the phase space of fractal dimension sequence. And fractal dimension complexity is obtained by calculating Approximate Entropy. Three dimension feature vectors, including the pitch, the fractal dimension and the fractal dimension complexity, are applied to speaker gender identification. Experiment results show that through adding non-linear parameters, compared with the linear parameter using one dimension only such as pitch, the proposed method is more accurate and robust, and thus provides a new way for speaker gender identification.

Objective To investigate the value of MRI in thmmbolytic thempy of hyperacute cerebral arterial thrombosis.Methods One hundred and sixteen patients with acute cerebral arterial thrombosis were recruited.plain CT and multi.modal MRI were performed in all patients.Thirty-three patients with hyperacute cerebral infarction were treated bv recombinant tissue plasminogen activator(rt-PA) and followed-up periodically using MRI.Results The 33 patients with thrombolysis selected by MRI demonstrated clinical improvement.90 d moclified Rankin scale scores(mRs)were less than 2 and life quality Barthal indexes(BI)were from 80 to 100.The complication included one asymptomatic parenchymal hematoma(PH1)one weeks after thrombolytic therapy and 4(12.2%)hemorrhagic infarction(HI)6 to 24 hours after thrombolytic therapy.Condusions MRI has significant clinical value for the screening and follow-up of intravenous thrombolytic therapy of hyperacute ischemic stroke.MRI-based thrombolysis is a safe and effective method for hyperacute ischemic stroke.
