1.Effect of early rehabilitative intervention on prognosis of patients with hypertensive cerebral hemorrhage

Objective To explore the effect of early rehabilitative intervention on prognosis of patients with hypertensive cerebral hemorrhage.Methods 60 patients with hypertensive cerebral hemorrhage were randomly divided into the observation group and the control group,each group 30 cases.Both two groups received common therapy,while the observation group was given early rehabilitative intervention.Results After treatment for one month,the total effective rate of the observation group was obviously higher than that of the control group (90.00% vs 73.33%,x2 =2.81,P < 0.05).After treatment for three months,the living ability of experiment group was obviously better than control group (66.67 % vs 50.00%,x2 =2.69,P < 0.05).Conclusion Early rehabilitative intervention in patients with hypertensive cerebral hemorrhage can increase clinical effect and improve prognosis.
2.Relationship between quality of life and self-efficacy of patients with type-2 diabetes mellitus

Objective To investigate the relationship between self-efficacy and quality of life of patients with type 2 diabetes mellitus.Methods A convenient sample of 116 patients with type 2 diabetes was investigated by diabetes management self-efficacy scale(DMSES)and diabetes quality of life scale(DQOL)and the results underwent analysis.Results Both the level of selfefficacy and quality of life of patients with type 2 diabetes mellitus were low generally.Score of self-efficacy and quality of life of patients with different type of type 2 diabetes mellitus were statistically different.The level of diabetes self efficacy was moderately positivdy correlated with the level of diabetes quality of life(r=0.43,P<0.01).Conclusions The improvement of self-efficacy and quabty of life of patients with type 2 diabetes mellitus must be thought highly of in the clinical practice which should be based on self-efficacy theory.Various methods should be adopted to arouse the potential ability of patients in order to increase their self-efficacy,so that the quality of life of patients can he improved.
3.Characteristics and prognostic analysis of 193 patients with special type of lung adenocarcinoma

Objective: This work aimed to investigate the negative prognostic factors of bronchioloalveolar carcinoma (BAC) and adenocarcinoma with BAC characteristics, based on the 2004 pathological classification by the World Health Organization (WHO), which were further verified with the new pathological classification of lung adenocarcinoma (WHO 2011), to identify crucial factors that determine the prognosis of BAC and adenocarcinoma with BAC features, and to prove the coherence of the two pathological classi-fications in assessing clinical prognosis. Methods: Upon pathological diagnosis, some of the 193 cases of BAC or adenocarcinoma with BAC features were categorized into adenocarcinoma in situ (AIS) or minimally invasive adenocarcinoma (MIA), based on the 2011 WHO classification. Gender, age, tumor size, familial cancer history, smoking history, TNM stage, symptoms, duration of symp-toms, and the choice of treatment were recorded and analyzed for prognosis. The survival rate was calculated by Kaplan-Meier method. Log-rank test was introduced to compare the survival rate. Univariate and multivariate factors for the survival rate were analyzed by Cox proportional hazards regression model. Results:The overall 1-, 3-and 5-year survival rates were 84.3%, 60.6%, and 45.6%, respec-tively. Cox univariate analysis revealed that the tumor size, symptoms, TNM stage, pathological outcomes, and the choice of treatment were all prognostic factors. Cox multivariate analysis revealed that TNM stage was an independent prognostic factor for patients with BAC. Data from patients with AIS and MIA revealed better survival. Conclusion:The overall survival rate of BAC and adenocarcino-ma with BAC features are superior to that of other non-small cell lung cancer (NSCLC). The clinical symptoms are non-specific com-pared with other types of NSCLC. Clinical stage at diagnosis is a key prognostic factor, such that early correct diagnosis significantly improves survival. The new classification criteria of WHO, released in 2011, is more elaborate and more conducive to clinical practice.
4.Effect of Medified Xiaochaihu Decoction on NF-kB and HSP70 of BRL Cell Induced by Cisplatin

Objective To study the effect of modified Xiaochaihu Decoction on NF-kB and HSP70 of BRL cell induced by cisplatin. Methods BRL Cells were divided into control group, cisplatin group, high and low dose of modified Xiaochaihu decoction. Complete cell protein dot blot technique and immunohistochemisth analysis were appllied to detect the change of NF-kB and HSP70. Results There was a significant increase in NF-kB and decrease in HSP70 of BRL cell induced by cisplatin compared with control group. There was a significant decrease of NF-kB in modified Xiaochaihu decoction group compared with cisplatin group. There were increases of the expression of HSP70 in both high and low dose group, but only the high dose group demonstrated significant difference. Conclution The expression of NF-kB of BRL cell induced by cisplatin can be inhibited while the expression of HSP70 can be enhenced by modified Xiaochaihu decoction. It may be one of important mechanisms of relieving cell oxidize lesion and inhibiting apoptosis by modified Xiaochaihu decoction.
5.Comparison of effects of dexmedetomidine and fentanyl on median effective concentration of propofol required for inducing respiratory depression

Objective To compare the effects of dexmedetomidine and fentanyl on the median effective concentration (EC50) of propofol required for inducing respiratory depression.Methods ASA physical status Ⅰ or Ⅱ patients,aged 30-50 yr,weighing 50-75 kg,with body mass index of 22-28 kg/m2,scheduled for elective gynecological surgery under general anesthesia,were randomly divided into 3 groups:control group (group C),dexmedetomidine group (group D) and fentanyl group (group F).Dexmedetomidine 0.5 μg/kg and fentanyl 1.5 μg/kg in 10ml of normal saline were infused over 10 min in D and F groups,respectively,while the equal volume of normal saline was given in group C.Lidocaine was injected intravenously followed by target-controlled infusion of propofol.In C,D and F groups,the initial target concentration of propofol was 2.5,2.1 and 1.1 μg/ml,respectively,and the ratio between the two successive concentration gradients was 1.1.It was defined as positive when patients developed respiration depression.EC50 and 95% confidence interval of propofol inducing respiratory depression were calculated.BIS values and OAA/S scores were recorded after admission to operating room (T0),after infusion of dexmedetomidine,fentanyl or normal saline,at 5 and 10 min after the start of propofol target-controlled infusion and after the target effect-site and plasma concentrations were balanced.Results EC50 (95% confidence interval) of propofol required for inducing respiratory depression was 2.72 (2.55-2.91),2.15 (2.05-2.27)and 1.17(1.08-1.25) μg/ml in C,D and F groups,respectively.Compared with group C,the EC50 was significantly decreased in D and F groups and the BIS values and OAA/S scores were increased in group F (P < 0.05),and no significant changes were found in the BIS values and OAA/S scores in group D (P > 0.05).The EC50 was significantly decreased and the BIS values and OAA/S scores was increased in group F as compared with group D (P < 0.05).When the BIS values reached 65 or OAA/S scores was 3,the effect-site concentration was significantly lower in D and F groups than in group C (P < 0.05).In C,D and F groups,the percentage of patients who developed upper airway obstruction caused by glossocoma was 100%,60% and 20%,respectively,and who developed decreased respiratory rate or apnea (RR≤ 8 bpm or respiratory arrest time≥ 15 s) was 0,40% and 80%,respectively.Conclusion Although dexmedetomidine induces no respiratory depression,dexmedetomidine can enhance the potency of propofol required for inducing respiratory depression and the prtency is lower than that of small-dose fentanyl.
6.Recombinant human growth hormone activates JAK2-STAT3 pathway of human colon cells in vitro

ObjectiveTo investigate the effect of recombinant human growth hormone (rhGH) on JAK2-STAT3 signaling pathway and on the growth of human colon cancer cells at different growth hormone receptor (GHR) expression status.MethodsLOVO and HCT-8 cell lines were selected after the colon cancer cells were screened using flow cytometry according to the GHR expression status.The growth of human colon cancer cells after intervention with rhGH was detected by MTT assay.The proliferation index ( PI),cell cycle,and apoptosis were detected by flow cytometry.The expression of JAK2-STAT3 signaling pathway proteins was detected by Western blot.ResultsHCT-8 cell line showed positive GHR expression (59.6%),and LOVO cell showed negative GHR expression (3.5%).The growth rate of HCT-8 cells increased after rhGH treatment.Compared with the untreated HCT-8 cells,the rhGH-treated HCT-8 cells showed reduced apoptosis,elevated PI,and increased G2/Mphase cells ( P =0.0073).Western blot revealed that rhGH up-regulated the proteins of pJAK2,pSTAT3,VEGF,Cyclin D1,and Bcl-xL in HCT-8 cells.In contrast,no such changes were observed in LOVO cells treated with rhGH.ConclusionsrhGH may promote the growth of HCT-8,the cell line of high GHR expression,and up-regulate the expression of a number of key nodes in JAK2-STAT3 signaling pathway.However,rhGH may not affect LOVO,the cell line of low GHR expression.
7.Role of hypoxia inducible factor 1α-mediated signaling pathway in angiotensin Ⅱ-induced renal interstitial fibrosis

Objective To explore the role of hypoxia inducible factor 1α(HIF-1α)-mediated signaling pathway in angiotensin Ⅱ(Ang Ⅱ)induced renal interstitial fibrosis. Methods Renal tubular epithelial cells were cultured and treated with different concentrations (10-9-10-6 mol/L)of Ang Ⅱ for 24 h and 48 h.Real-time quantitative PCR and Western blotting were preformed to detect the mRNA and protein expressions of HIF-1α,prolyl hydroxylase 2 (PHD2)and tissue inhibitor of metalloproteinase 1 (TIMP-1)in renal tubular epithelial cells. Results HIF-1αmRNA level was increased with Ang Ⅱ treatment in a concentration dependent manner.When cells were treated with Ang Ⅱ concentration at 10-7mol/L for 24 h,the mRNA level was markedly increased by 166%.Furthermore,by real-time quantitative PCR and Western blotting,compared with the control group,Ang Ⅱincreased the mRNA and protein levels of HIF-1α and TIMP-1 (P<0.05,respectively),while the mRNA and protein levels of PHD2 were decreased markedly (P<0.05,respectively)in renal tubular epithelial cells.Conclusion Ang Ⅱ reduces HIF-1αdegradation in renal tubular epithelial cells probably by reducing the expression of PHD2,which increases the expressions of HIF-1α and TIMP-1 involved in renal interstitial fibrosis.
8.SGK1-induced imbalance of T lymphocyte subsets in childhood asthma

Objective To study the expression of SGK1 in T lymphocytes from pediatric asthma,and the effect of SGK1 on the differentiation of T cells,also to explore the function of SGK1 regulating the differen-tiation of T subset in pediatric asthma. Methods Twenty-eight children with asthma were recruited in Xi′an children′s hospital and divided into moderate group and severe group according to diagnostic guideline of asth-ma. The serum levels of IL-4,IL-13 and IL-17A were analyzed by ELISA. The CD4 +T cells from PBMC and na?ve T cells were selected using magnetic beads. Na?ve T cells were differentiated in vitro under cytokines. SGK1 expression were analyzed with Real-time PCR. The ability of Th2 and Th17 on secreting IL-4 and IL-17A were detected after SGK1 was inhibited by siRNA. In vivo,shRNA-SGK1 Na?ve T cells were transferred into the mice asthma models by intravenous injection. The airway inflammation were observed in shRNA-SGK1 Na?ve T models. Results Compared with healthy children,the serum levels of IL-4、IL-13 and IL-17A increased signifi-cantly in the children with asthma. Importantly,the levels of these three cytokines were much higher with the de-velopment of asthma. SGK1 were up-regulated remarkably in CD4 +T cells from the children with asthma and were positively correlated with IL-13 and IL-17A. Besides,SGK1 expression increased in the differentiated Th2 and Th17 in vitro,but had no change in the differentiated Th1. The levels of IL-4 and IL-17A associated with Th2 and Th17 decreased after SGK1 was inhibited by siRNA. Similarly,In vivo,the serum levels of IL-13 and IL-17A and airway inflammation were reduced in shRNA-SGK1 Na?ve T models. Conclusion The over-expres-sion of SGK1 in pediatric asthma enhances the asthma progress by promoting the differentiation of T subsets.
9.Correlation between osteoporosis knowledge and activities and fear of falling in the community elderly

Objective To explore the relationship between osteoporosis knowledge and activities and fear of falling of elderly in the community.Method 420 elders in community were investigated with osteoporosis knowledge assessment tool (OKAT) and the modified survey of activities and fear of falling in the elderly (MSAFFE).Result The overall average score of MSAFFE was (25.36±4.58),and exercise capacity dimension scored higher (13.89±2.41).The overall average score of OKAT was (9.11±3.81).The correlation coefficient of MSAFFE and OKAT was-0.11~ 0.58(P<0.05).The level of knowledge osteoporosis could explain 24%,18%,16% variation of activities and fear of falling in each dimension.Conclusion The level of knowledge osteoporosis in the elderly is closely related to activity and fear of falling.Health education should be strengthened to improve the life quality of older people.
10.Investigation on malnutrition status among the elderly in age care institutions in Shanghai suburb and its influential factors

Objective To investigate the malnutrition status among the elderly in age care institutions in Shanghai suburb and analyze its potential influential factors.Methods The MiniNutritional Assessment (MNA) was adopted to evaluate the nutritional status of the 190 elderly people in age care institutions.The dietary supply by the institution canteen and the quantity of residual food left by the malnourished elderly people were weighted.Results In the age care institutions,the malnutrition rate reached 23.7%,47.9% of the elderly people were at the risk of malnutrition,and only 28.4% of the elderly people were well nourished.Logistic regression analysis showed that the major influential factors for malnutrition in the elderly people were food intake ability,mobile capability,chewing and swallowing ability,ageing and mental Illness.The malnourished elderly people had the most residual meat and vegetables,and insufficiency of nutrient intake was the main cause for the malnutrition in the elderly people.Conclusions The elderly people in age care institutions in Shanghai suburb have the higher risk of malnutrition.The malnutrition occurs under influence of many factors,of which some are unavoidable,however,some factors like dietary factors can be changed to improve the nutritional status of the elderly people in age care institutions.
