1.Influence of age on prevalence rates of chronic complications of hospitalized aged patients with hypertension——analysis of 17,682 cases

Objective To understand the relationship between age and chronic complications in hospitalized aged patients with hypertension, to provide evidence for hypertension prevention and control. Methods To retrospectively analyze the clinical and laboratory data on 17,682 patients with essential hypertension during Jan 1st,1993-Dee 12th, 2008 in PLA general hospital. Results 1)Among all of the inrolled cases, those aged 60-64 account for 27.87%, 65-69 years group account for 26.55%, 70-74 years group accounted for 23.96%, 75-79 years group accounted for 14.14%, 80-84 years group accounted for 5.26%, 85-89 years group accounted for 1.69%, > 90 years accounted for 0.41%. 2) The prevalence rate of chronic complications in 60-69 years group were 31.3-31.2% for diabetes and,22.6-27.0% for cerebrovascular disease, 9.5-11.1% for myocardial infarction, 6.7-9.1% for heart failure, 5.8-6.0% for renal dysfanction 4.9-6.8% for atrial fibrillation, 0.1-0.3% for multiple organ dysfunction syndrome (MODS) in the elderly(P <0.05 ). 3) The first four complications of hypertension were diabetes(33.5%), cerebrovascular disease (31.9%), myocardial infarction(13.2%) and heart failure(12.3%) in 70-74 years group (P<0.05), cerebrovascular disease (42.8%), diabetes (32.8%), heart failure (16.5%) and myocardial infarction(15.9%) in 75-79 years group (P<0.05), cerebrovascular disease (45.4%), diabetes (35.0%), heart failure (21.1%) and myocardial infarction(15.9%) in 80-84 years group (P<0.05), cerebrovascular disease(42.5%), diabetes (35.8%), heart failure (23.1%) and renal dysfanction (17.7%) in 85-89 years group(P<0.05 ),and cerebrovascular disease (45. 2%), heart failure(31.5%), diabetes (26.0%) and renal dysfanction (20.5%) in patients more than 90 years group (P<0.05). Conclusions The prevalence rate and kinds of chronic complications in hospitalized aged patients with hypertension were changed with the increasing age, and the first kind of complication is cerebrovascular disease. It is of more importance to prevent the occurrence of renal dysfanction and heart failure in those hypertension patients who were more than 80 years old.
2.Studies on HPLC fingerprint of Radix Aconiti Kusnezoffii.

OBJECTIVETo establish the HPLC fingerprint of Radix Aconiti Kusnezoffii.
METHODThe chromatographic separation was performed on a Kromasil C18 (4.6 mm x 250 mm, 5 microm) eluted with a mobile phase of acetonitrile-ammonium acetate buffer (2. 5 per thousand acetic acid-ammonia pH 10.5) (60:40). The UV detection wavelength was set at 240 nm and the flow rate was set at 1.0 mL x min(-1).
RESULTThe RSD of precision and repeatability was less than 2%. Under the selected chromatographic conditions, good HPLC fingerprints of Radix Aconiti Kusnezoffii were obtained.
CONCLUSIONThe method was simple, accurate and repeatable. It can be used for the quality control of Radix Aconiti Kusnezoffii.

Objective To investigate the expression and clinical value of immunohistochemistry in endometrial stromal tumors.Methods Immunohistochemical technique(Envision method)was applied to de- tect the expression of CD_(10),SM-MHC,h-caldesmon,AE1/3,CD_(99),Ki-67,CD_(34),c-kit,ER and PR in 15 cases of endomertrial stromal sarcoma and 3 metastases.The clinical pathological data,including the histological characteristics,histochemical and immunohistochemical staining features,complication,differential diagnosis and prognosis of endometrial stromal tumours were analyzed.Results Among the 18 cases of endometrial stromal tumor,17 cases had shown positive for CD_(10),including 13 cases diffuse positive and 4 muitifocal,7 cases with smooth muscle differentiation,3 cases with epithelial differentiation,7 cases with sex-cord differ- entiation.13 cases of ER and 16 cases of PR were positive expression in endometrial stromal sarcoma.Ki-67 in range 36 %~78 %.Conclusion Endometrial stromal tumour can display multi-differentiation.They show various pathomorphological features,Smooth muscle and sex-coed differentiation,the most common types. CD_(10) can be expressed consistently in endometrial stromal tumors.CD_(10) with h-caldesmon and SM-MHC can be used to make differential diagnosis between the endometrial stromal tumors and cellular leiomy0ma.ER and PR should be routinely estimated and be a prognostic predictor for endometrial stromal sarcoma.
5.Incidence and risk factors of target organ damage in 17, 682 elderly hypertensive inpatients between 1993 and 2008.

OBJECTIVEThe aim of our study was to investigate the prevalence of target organ damage (TOD) in elderly hypertensive inpatients.
METHODSData of the present retrospective survey were collected and analyzed from the computerized medical records of 17 682 aged 60 years or older inpatients with the diagnosis of essential hypertension (EH) from January 1993 to December 2008 in our hospital. The evidences of hypertensive TOD and associated risk factors with TOD including age, gender, presence of diabetes (DM), body mass index (BMI), systolic blood pressure (SBP) and diastolic blood pressure (DBP), serum lipids were analyzed.
RESULTSThe overall prevalence of stroke, coronary artery disease (CAD), chronic kidney disease (CKD) and aortic dissection (AD) was 32.19%, 27.33%, 10.12% and 0.77%, respectively. Incidence of TOD was 68.03% in male and 31.70% in female patients. CKD stage 3-5 was more prevalent in males than in females (12.75% vs. 5.40%, P < 0.01), while the prevalence of CAD (31.31% vs. 27.96%, P = 0.06), Stroke (28.23% vs. 25.81%, P = 0.08) and AD (0.89% vs.0.74%, P = 0.72) was similar between men and women. One TOD was presented in 23.20% patients and two or more TODs were found in 47.19% patients. Higher age and BMI, longer history and lower control rate of hypertension, severe degree of hypertension and higher level of SBP, pulse pressure, TC, LDL-C, estimated GFR (eGFR) and Hcy were risk factors for TOD. BMI, fasting plasma glucose, incidence of DM, prevalence of stage 1 and 2 hypertension, control rate of hypertension, eGFR and TG levels were all significantly higher while the prevalence of hypertension stage 3 and level of TC and LDL-C were significantly lower in female TOD patients than in male TOD patients (all P < 0.05). In patients without TOD, TG was significantly higher while SBP, fasting plasma glucose and LDL-C were significantly lower and history of hypertension was significantly shorter in female patients than in male patients (all P < 0.05). The prevalence of CAD, stroke and CKD increased with age (P < 0.001).
CONCLUSIONThe prevalence of TOD is high in elderly hypertensive inpatients and higher age and BMI, longer history and lower control rate of hypertension, severe degree of hypertension and higher level of SBP, pulse pressure, TC, LDL-C, eGFR and Hcy are risk factors for TOD.

6.Cytotoxicity of MICA-reactive V delta 1 gamma delta T cells towards epithelial tumor cells.

OBJECTIVETo confirm whether human MHC class I chain-related A (MICA) induces the amplification of V delta 1 gamma delta tumor-infiltrating lymphocytes (TILs) in vitro and to identify the cytotoxicity of MICA-reactive V delta 1 gamma delta TILs towards epithelial tumor cells.
METHODSMICA protein was prokaryoticly expressed and purified by molecular cloning technology. The purified recombined MICA (rMICA) was used to induce V delta 1 gamma delta T cells from tumor tissues in vitro and the cytotoxicity of these V delta 1 gamma delta TILs were tested by 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide (MTT).
RESULTSThe rMICA was expressed in prokaryocyte with pET30 as a vector. The immobilized rMICA protein could markedly induce the amplification of V delta 1 gamma delta T cells from tumor tissue in vitro. These V delta 1 gamma delta T cells showed strong cytolytic activities towards tumor cell lines expressing MICA.
CONCLUSIONThe MICA-reactive V delta 1 gamma delta T cell may be a candidate for adoptive cellular therapy of tumors.

7.Clinical and electrophysiological study of Miller-Fisher syndrome and Bickerstaff' s brainstem encephalitis

Objective To investigate the underlying mechanisms of Miller-Fisher syndrome (MFS) and Bickerstaff' s brainstem encephalitis (BBE) by studying their clinical and electrophysiological characteristics.Methods The clinical and electrophysiological characteristics of 13 MFS and 7 BBE cases in Peking Union Medical College Hospital between 2000 and 2011 were retrospectively analyzed.The electrophysiological parameters included sensory and motor nerve conduction,electromyography,F wave,sympathetic skin response and brainstem auditory evoked potential and blink reflex.Results MFS and BBE had similar clinical characteristics:respiratory symptoms were the most common infectious symptoms before disease onset; Ophthalmoplegia,facial palsy and bulbar symptoms were common; They both had cerebrospinal fluid albuminocytological dissociation and positive serum anti-GQ1b antibody.However,BBE had more central nervous system lesion signs clinically such as conscious disturbance,positive Babinski' s sign and central facial palsy.Electrophysiologically,MFS and BBE also had similar electrophysiological features:sensory nerve abnormality ratios were 6/13,2/7 respectively,with prominently reduced sensory nerve active potential amplitude,normal or slightly slowed sensory conduction velocity; Motor nerves abnormality ratios were 2/13,1/7 respectively,with slightly prolonged distal motor latency and normal compound muscle action potential; Electromyography abnormality ratios were 1/7,0/4 respectively; F wave frequency abnormality ratios were 3/13,5/7 respectively,and in some cases,F wave frequency would restore; Sympathetic skin response abnormality ratios were 1/2,1/3 respectively; Blink reflex abnormalityratios were 1/2,1/1 respectively,with central involvement in BBE; Brainstem auditory evoked potential abnormality ratios were 3/5,1/4 respectively,with wave Ⅰ latency or amplitude abnormality.Conclusion The similarities of clinical and electrophysiological features suggest that MFS and BBE have the same mechanism and they form a continuous spectrum with variable central nervous system and peripheral nervous system involvement.
8.A case report of human thelaziasis and its epidemiology in China

A case of Thelazia callipaeda infection in a 7 months old infant in Dalian was reported.The epidemiology of human thelaziasis in China was investigated in this report.China is the nation with the most reported human Thelazia callipaeda cases in the world,up to 626 case reports of human Thelazia callipaeda infection that were distributed in 28 provinces from 1917 to 2016.We analyzed more than 400 Chinese thelaziosis cases with clinical data from 1917 to 2016.Ages of the patients are from 2 months to 88 years old.Male and female cases are 60.22% and 39.78% relatively.The occupation of patients was analyzed,that the preschool children occupied 55.38 %,farmers occupied 32.27 %;workers occupied 4.78 %,and the others occupied the rest 7.57%.This study showed that it is of great significance to strengthen the research on the biology and epidemiology of the sucking nematode,to prevent the spread and epidemic of the disease.
10.Movement coordination ability among preschool children in Shanghai

Objective:
To investigate the preschool children’s movement coordination ability in Shanghai and to provide the evidence for formulating a scientific sports strategy
Methods:
During May to June of 2018, 9 833 preschool children aged 3-6 years were selected from 174 kindergartens of 4 districts (Jing’an, Pudong, Songjiang and Minhang) in Shanghai. Motor coordination ability was assessed by using Little Developmental Coordination Disorder Questionnaire.
Results:
n this study, the total scores of movement coordination ability, motor control, fine motor/writing and general coordination were 67.96±8.44,23.01±2.92,22.85±2.97 and 22.10±3.12,respectively,which gradually increased with age(P<0.01), and girls sorced higher than boys(each P<0.01). The incidence of developmental coordination disorder was 4.4% and the rate of suspected developmental coordination disorder was 7.9%. The incidences of developmental coordination disorder in students in the age group between 3 and 6 were 7.0%,4.7%,3.5% and 2.7% , which gradually decreased with age(χ2=92.04, P<0.01). The incidence rates of boys and girls were 5.3% and 3.4%, respectively,boys had a higher incidence of developmental coordination disorders than that of girls(χ2=31.70, P<0.01).
Conclusion
Preschool children in Shanghai have fine movement coordination ability which varies by age and sex.
