1.Imbalance of Th_1/Th_2 cells in thyroid tissues of patients with autoimmune thyroid diseases

Objective To investigate the role of the imbalance of Th 1/Th 2 cells in the pathogenesis of autoimmune thyroid diseases (AITD). Methods Thirteen patients with Graves′ disease (GD), 12 patients with Hashimoto′s thyroiditis (HT) were recruited for the study, and 8 patients with non-toxic nodular goiter served as controls. The expressions of interferon-? (IFN-?) and interleukin-4 (IL-4) of intrathyroidal mononuclear cells (ITMC), representative cytokines of Th 1 and Th 2 cells respectively, were assayed by immunohistochemical technique. 2.IMMUNOCYTOCHEMICAL STUDY ON THE PP CELL OF RAT PANCREATIC ISLETS DURING POSTNATAL DEVELOPMENT

The pancreatic polypeptide (PP) cells of rat pancreatic islets were demonstrated by immunocytochemical method. The distribution and morphology of PP cells were investigated in order to observe their changes during postnatal development. At the lst day and 7th day of rat after birth, the PP cells of islets were different from that in adult, the processes of PP cells were not evident, and the immunoreactivity was weaker. In addition, the PP cells were also presented in the duodenum epithtlium. After 18th day, the distribution and morphology of the PP cells did not show obvious difference from that in adult. The percentage of PP cells of islets was less in lst day, and approximated to adult level in 18th day, and over adult level in 28 th day. The above changes of percentage of the PP cells showed mainly in islets in the head of pancreas. The changes of volume density and numerical density per area corresponded with the changes of percentage of the PP cells. Immunoelectron microscopic study showed that the secretory granules (SGs) and gold la belled particles on the SGs of the PP cells in 7th day were less than that in 28th day and adult. Based upon the morphology of the SGs, the PP cells can be classified into two types. The type Ⅰ cells contained SGs with medium electron density or empty-like cores, and larger SGs were more. The type Ⅱ cells contained SGs with lower electron density cores, and small SGs were more. The gold labelled particles in SGs in the type Ⅰ were more than that in the type Ⅱ. The results suggest that the PP cells of rat islets continue to change after birth.
3.Heart Involvement in Behet′s disease

Objective To know the heart involvement in Behet′s disease.Methods Echocardiography (UCG) and clinical characteristic evaluation were performed in 30 patients with Behet′s disease.UCG findings were compared with those in 30 control subjects.Results Valve abnormalities were common on the UCG.Valve prolapse was observed in 10% and proximal aorta dilatation in 7% of patients.The positive rate of antinuclear antibodies was very low (7%) without differences between both groups.Conclusion Heart valve diseases are common in patients with Behet′s disease.

5.Value and measuring methods of elasticity contrast index in the diagnosis of thyroid nodules

Objective:To explore the value of the elasticity contrast index (ECI) in differential diagnosis of thyroid nodules, and contrast the effect of 4 different methods on measuring ECI.Methods:A total of 122 patients with 131 thyroid nodules in Binzhou Medical University Hospital from May 2018 to May 2019 were enrolled, and elastography was performed in 4 different ways such as in axial plane for internal (AI), axial plane for periintranodular (AP), longitudinal plane for internal (LI) and longitudinal plane for periintranodular (LP). The cut-off values for predicting malignant nodules in 4 different ways were determined separately using receiver operating characteristic (ROC) curve analysis.Results:There were 54 benign and 77 malignant ones in 131 nodules. The ECI in AI, AP, LI and LP in benign thyroid nodules was significantly lower than that in malignant ones: 2.10 (1.48, 2.34) vs. 3.07 (2.73, 3.87), 1.91 (1.64, 2.18) vs. 2.62 (2.24, 3.07), 2.19 (1.59, 2.39) vs. 3.00 (2.72, 3.63) and 1.89 (1.71, 2.16) vs. 2.66 (2.21, 3.10), and there was statistical difference ( P<0.05). Among the 4 different ways, the largest area under curve was achieved in AI with 0.925, and the corresponding optimal diagnostic threshold was 2.43 (with 90.9% sensitivity, 87.0% specificity, 88.5% accuracy, 89.7% positive predictive value and 86.8% negative predictive value). Conclusions:ECI is helpful for conventional ultrasound to diagnose thyroid nodules as malignant or benign. The best value is obtained in AI.
6.Clinical Observation of Astragalus Injection Combined with Neoadjuvant Chemotherapy and Three Dimen-sional Conformal Radiotherapy in the Treatment of Colorectal Cancer

OBJECTIVE:To observe clinical efficacy and safety of Astragalus injection combined with neoadjuvant chemother-apy and three dimensional conformal radiotherapy in the treatment of colorectal cancer. METHODS:A total of 80 patients with me-dium and advanced colorectal cancer were selected from oncology department of our hospital during Jun. 2010 to Jun. 2014,and di-vided into observation group and control group according to random number table,with 40 cases in each group. Control group re-ceived neoadjuvant chemotherapy combined with three-dimensional conformal radiotherapy. Observation group was additionally giv-en Astragalus injection 10-20 mL added into 5% Glucose injection 250-500 mL,ivgtt,qd,on the basis of control group. A treat-ment course lasted for 21 d,and both groups received 3 courses. Short-term efficacies as well as immune indexes [CD3+,CD4+, CD4+/CD8+ and NK cells] before and after treatment were observed in 2 groups. The occurrence of ADR and survival situation were compared between 2 groups. RESULTS:Clinical response rate of observation group was 87.5%,which was slightly higher than that of control group(77.5%),but there was no statistical significance(P>0.05). Before treatment,there was no statistical signifi-cance in immune index levels between 2 groups(P>0.05). After treatment,NK cells,CD3+,CD4+ and CD4+/CD8+ levels of con-trol group were decreased significantly,which was significantly lower than observation group,with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). Two-year survival rate of observa-tion group was significantly higher than that of control group;the recurrence rate and distant metastasis rate were significantly low-er than control group,with statistical significance(P<0.05). CONCLUSIONS:Astragalus injection not only can improve therapeu-tic efficacy of neoadjuvant chemotherapy in patients with colorectal cancer,balance inflammatory state,improve survival rate,re-duce recurrence rate and distant metastasis rate with good safety.
7.Application of allostatic load in evaluating the effect of management for patients with type 2 diabetes mellitus

Objective:To explore the application of Allostatic Load (AL) in evaluation the effect of management for patients with type 2 diabetes.Methods:According to the control objectives of each indicator in the Guidelines for the Prevention and Treatment of Type 2 Diabetes in China, the P75 and P80 of the subjects before intervention, AL was calculated, which were AL1-1, AL1-2 and AL1-3 respectively. On the basis of the above three indicators, the number of complications was taken as the numerical sum to obtain AL2-1, AL2-2 and AL2-3. Study participants were from a one-year randomized controlled intervention trial in patients with type 2 diabetes. Patients in the control group were treated with the integrated diabetes management mode used bg the tier-three medical institutions, while the intervention group added the intervention based on " Your Doctor" App on the basis of the above strategy. The changes of 6 types of AL and HbA 1C in the two groups before and after intervention were compared. Two groups of patients were treated with propensity score matching method to ensure the balanced distribution of covariates between groups. Multiple regression analysis was used to explore the factors influencing the effect of AL intervention. Two independent samples t-test, paired t-test and chi-square test were used to analyze the efficacy of the intervention. Results:After propensity score matching, the control group and the intervention group each included 483 patients. Among the six types of AL, the score of AL2-1 was the highest, while the score of AL1-3 was the lowest. Compared with baseline, all other indicators in the two groups decreased after intervention except ALL-3 in control group ( P<0.05).Patients who were residing in rural areas, having less co-habitants and more diabetic complications, and lower annual household income, shown higher AL ( P<0.05). The number of complications and the situation of urban or rural areas were the factors influencing the effect of AL intervention. Conclusions:More research is needed on AL as an early evaluation index of the effect of comprehensive management of diabetes.

The external stimuli and other reasons may result in hyperplasia and abnormal angle of the upper corner of the thyroid cartilage. The upper corner of the thyroid cartilage has anatomical variations. To oppression stimulate carotid artery, cervical sympathetic dry section and so on, could causing pharyngeal foreign body sensation and sore throat. By surgical removal of the thyroid cartilage hyperplasia upper corner, postoperative symptoms would be resolved.

Objective To explore the status quo of job performance and its influencing factors among clinical nurses.Methods 903 of clinical registered nurses from 5 comprehensive hospitals in Jilin province were investigated by the questionnaire.Results The level of job performance of clinical nurses from these 5 comprehensive hospitals in Jilin province was high,the average score of it was (3.345±0.442).The score of six dimensions followed by high and low were as followed:critical care,leadership,professional development,communication,planning and teaching.Some factors such as the title of a technical post,possession of spouse,classification of hospital,empowerment and organizational support were considered as the related factors via multiple regression analysis.Conclusions The leader should pay attention to empowerment and organizational support,so as to promote the job performance.
