1.Effects of angiotensin Ⅱ receptor antagonist on TGF-β1 synthesis and TIMP-1 mRNA expression of hepatic stellate cells

Objective To investigate the effects of angiotensin Ⅱ (Ang Ⅱ ) and angiotensin Ⅱ type 1 receptor antagonist (AT1RA) on the transforming growth factor-β1 (TGF-β1) synthesis and tissue inhibitor of metalloproteinase-1 (TIMP-1) mRNA expression of in vitro cultured hepatic stellate cells (HSCs). Methods HSC-T6 rat hepatic stellate cell line was selected as the study model of the activated hepatic stellate cells. Cultured HSCs were randomized into control group, Ang Ⅱ group, AT1RA group and Ang Ⅱ AT1RA group. Cell culture medium was used to detect the TGF-β1 level by ELISA method. HSCs were harvested to measure the TIMP-1 mRNA expression by RT-PCR. Results TGF-β1 level of control group, Ang Ⅱ group and AngⅡ AT1RA group in cell culture medium was (7.531 ±0. 654) pg/mL, (9. 855± 1. 485)pg/mL and (7.719 ± 0.329) pg/mL respectively, Ang Ⅱ group higher than control group (P < 0.05 ), Ang Ⅱ AT1RA group lower than Ang Ⅱ group (P < 0. 05 ). TIMP-1 mRNA expression level of control group, Ang Ⅱ group and Ang Ⅱ AT1RA group in HSCs was 3. 387 ± 0. 042, 4.870 ± 0.061 and 3. 837 ± 0. 042 respectively, Ang Ⅱ group higher than control group ( P < 0. 05 ), Ang Ⅱ AT1RA group lower than Ang Ⅱ group (P < 0. 05). Conclusion AngiotensinⅡ can increase the TGF-β1 synthesis and TIMP-1 mRNA expression of hepatic stellate cells, while all these effects are inhibited by angiotensinⅡ type 1 receptor antagonist.
2.Protecting Methods of Unilateral Lung in Elderly Patients with Both-side Severe Pulmonary Infection after Operation

OBJECTIVE To approach the nursing methods for protecting of the unilateral lung in eldly patients with severe pulmonary infection of both-sides after operation.METHODS The patients which had a relatively better function in one side ie.with a higher Sat-O2 value were selected.Using postural drainage and sputum aspiration in protecting the unilateral lung,turning the body over quickly and knocking the back of the body on the other-side of the lung were adopted to gradually recovered the pulmonary function side-by-side.RESULTS Through the new nursing model the pulmonary function and the Sat-O2 value could be improved.CONCLUSIONS When the both-sides of the pulmonary infected severely,nursing the unilateral lung at the beginning by using postural drainage,turning the body over quickly and knocking the back in the other side of the lungs,can efficiently speed up the proceeding of rehabilitation of pulmonary infection.

This paper reports the postoperative managenment of 58 cases who had various types of congenital heart disease and received artificial valves due to rheumatic heart disease in our institute from 1982 to 1984. Vaso-active agents were administered with the monitoring of the cardiac function. According to the hemodynamic parameters and the criterion for clinical typing, the vaso-active agents therapy can provide a precise prognosis and the therapy is of certain value in cardiac surgery.

Objective To compare the influences of general anesthesia and intra spinal anesthesia on circulation, respiration, body temperature and anesthesia-related complications in patients undergoing percutaneous nephrolithotomy (PCNL), and assess the effectiveness and safety of both anesthesia. Methods Forty ASA Ⅰ - Ⅱ patients elective for PCNL surgery were divided into two groups by random digits table with 20 cases each:group Ⅰ (endotracheal general anesthesia) and group Ⅱ (intra spinal anesthesia). The temperature, heart rate (HR), mean arterial pressure (MAP), pulse oxygen saturation (SpO2) values and postoperative shivering, nausea, vomiting, back discomfort and the incidence of sore throat were observed and recorded. Results The anesthesia was stable, there were no changes in MAP, HR at different time in group Ⅰ . The anesthesia in group Ⅱ was effective, MAP at 15 min after anesthesia,and HR,MAP after lithotomy position and prone position were obviously changed in group Ⅱ compared with those before anesthesia and group Ⅰ (P < 0.05 ). The temperature at 30,60,90, 120 min after anesthesia decreased compared with that before anesthesia in two groups (P < 0.05 ), and the temperature at 30,60 min after anesthesia in group Ⅰ [(35.8 ±0.6), (34.8 ± 0.5)℃] was lower than that in group Ⅱ [(36.2 ± 0.6),(35.6 ± 0.5)℃](P< 0.05).During recovery,complications such as shivering, nausea,sore throat, back discomfort occurred to some extent, of which the incidence of shivering was the highest. Conclusion Both of two anesthesia are applicable to PCNL. When intra spinal anesthesia is used,the life indicators of patients need to be observed and general anesthesia is preferable for the obesity,less physical and the old with poorly compensatory function.
7.Factors of incidence of new-onset atrial fibrillation in acute myocardial infarction

Objective To research the risk factors of new-onset atrial fibrillation (AF) during hospitalization in patients with acute myocardial infarction.Methods Five hundred and one patients first diagnosed with acute ST-segment elevation myocardial infarction who accepted primary coronary intervention from January 2012 to May 2014 in the Second Hospital of Tianjin Medical University were selected.These patients were divided into non-AF group and new-onset AF group depending on whether happened new-onset AF during hospitalization.Data of the patients in acute myocardial infarction complicated with new-onset atrial fibrillation were collected.Univariate and multivariate logistic regression analysis were used to determine the predictors of new-onset AF.Results AF occurred in 49 cases of the 501 (9.8%) patients diagnosed as acute myocardial infarction.Logistic regression analysis showed that admission heart rate(OR =1.019,95%CI:1.004-1.034,P =0.012),left atrial diameter (OR =1.086,95% CI:1.021-1.156,P =0.009),previous stroke (OR =2.060,95% CI:1.029-4.123,P =0.041) were independent risk factors for new-onset AF in acute myocardial infarction.Use of β-blockers before coronary intervention treatment can reduce the risk of new-onset atrial fibrillation(OR=0.222,95%CI:0.064-0.768,P=0.017).Conclusion Admission heart rate and left atrial diameter are independent predictors of new-onset atrial fibrillation,and β-blockers in early stage can effectively reduce the incidence of atrial fibrillation.
8.Tracheal catheter made from nanometer antimicrobial plastics: toxicity and physical, chemical and antimicrobial properties

Objective To study the toxicity and physical, chemi ca l and antimicrobial properties of the tracheal catheter made with nanometer anti microbial plastics. Methods For examining the toxicity of the n ew tracheal catheter, 30 Kunming mice were divided into 3 groups. The mice in gr oup A received 2ml of fluid sucked through the catheter by intra-abdominal inje ction, the mice in group B received 1ml of suction fluid, and the mice in group C received 2 ml of normal saline. Multiple proportional dilutions were used to d etect the antimicrobial property of the tracheal catheter against both positive and negative Gram bacteria. Results No noxious effects or toxic ity was found in tested mice, and all the physical and chemical data of the cath eter were up to the National Standard. The catheter showed a certain antimicrobi al effects on some of common pathogenic bacteria. Conclusion Th e tracheal catheter made with nanometer antimicrobial plastics had no noxious ef fects on mice, it showed a certain antimicrobial effects on to some of common pa thogenic bacteria, and its physical and chemical properties were up to the Natio nal Standard.
9.Factors Influencing of Outcome in Cauda Equina Syndrome

Objective: We examined the cause、level of pathology、onset of symptoms、time taken to treatment and outcome of 14 patients with cauda equina syndrome(CES) to determine how these factors influenced the eventual outcome.Methods:14 patients with CES treated at our hospital between 1991 and 1997 underwent laminectomy followed by bilateral decompression.The mean time of followup was 35 months(range 22~48months).Logistical regression analysis was used to manage the data.Results:Out of 14 patients,11 had satisfactory recovery at 2 years postdecompression;3 patients were left with some residual dysfunction.Conclusion:There was a clear correlation between delayed decompression and the poor overall outcome,as well as the presence of perineal anaesthesia and significant sphicter dysfuncation.
10.Effects of angiogenesis on the development of endometrial carcinoma

ObjectiveTo study the effects of angiogenesis on the development of endometrial carcinoma. Methods Hysterectomy specimens were stained immunohistochemically by the marker of factor Ⅷ-related antigen for endothelium vessels in normal controls, patients with endometrial hyperplasia and with endometrial carcinoma. Results The microvascular density(MVD) in tumour increased gradually from normal endometrium to endometrial hyperplasia and to endometrial carcinoma(P<0.01). MVD correlated with the mvometrial invasion, histologic grades and the stages.ConclusionMYD in endometrial hyperplasia increases. MVD can be used as a prognostic factor.
