Objective To explore the safety,minimal invasion and cosmetic effect of endoscopic thyroidectomy.Methods From May 2007 to April 2008,among 413 cases of thyroid operations,238 cases were operated by endoscopic techniguc and 175 cases were operated by traditional methods.the operation time,intra operative blood loss,incision cosmetic score,postoperative hospitalization time,cost of hospitalization,and surgical complications between the two mehods were compared.Results Both groups of patients were operated successfully without postoperative bleeding,hoarseness,or hypocalcemia.Compared with traditional thyroid surgery,endoscopic thyroidectomy had less blood loss,shorter operation time,and higher-satisfaction cosmetic effect without significant scars or skin color changes.Conclusions Compared with traditional thyroid surgery,endoscopic thyroidectomy has the desired advantages of excelent cosmetic result,safety,and quick recovery.It is an ideal operation method for treatment of benign thyroid diseases.

After the concept of symptoms in traditional Chinese medicine was defined, the classification of symp-toms in traditional Chinese medicine and the characteristics of clinical diagnosis and treatment in traditional Chinese medicine were described .It was suggested that the symptoms in traditional Chinese medicine should be classified into 3 kinds, namely symptoms and signs according to the 4 diagnostic methods, symptoms and signs according to the diagnostic devices, symptoms and signs according to the physical constitutions of patients in order to lay a foun-dation for improving the classification in clinical term system of traditional Chinese medicine.

ObjectiveTo improve nursing quality by paying attention to management of nurses' psychology and behavior and changing individual and collective behavior.MethodsGuided by dynamic field theory,giving nursing staff continuous periodic training and counseling,establishing a staff interview system,giving systematic support and enhancement to develop a high-performance team.A case-control study was carried out in eight wards to evaluate implementation effect by comparison study.ResultsBefore and after introducing dynamic field theory,there were significant differences in job satisfaction of nurse staff (P<0.01),in patients' satisfaction(P<0.05),in evaluation of nursing staff management efficiency(P<0.05).ConclusionsApplication of dynamic field theory in nursing management can improve job satisfaction of nurse staff,patients' satisfaction and nurses' post suitability,and obtain continuous nursing quality improvement.

ObjectiveTo assess the value of plain X-ray radiography,CT and MRI in the diagnosis of synovial oateochondromatosis.MethodsTwenty patients with synovial osteochondromatosis confirmed with surgery and pathology were enrolled.The images of X-rays (n= 18),CT (n=20) and MRI (n= 10) were analyzed retrospectively.ResultsDefinite diagnosis was made in 15 patients according to plain X-ray radiography,in 18 patients according to CT findings,while in 8 patients according to MRI manifestations.Plain X-ray radiography,CT and MRI could show loose bodies around/within the involved joints in various shape,size and different amount.Plain X-ray radiography and CT could show calcification of loose bodies.MRI could show early cartilage loose bodies.CT and MRI could display effusion of the involved joints.ConclusionPlain X-ray radiography,CT and MRI are able to diagnose synovial osteochondromatosis.CT and MRI can depict lesions and characteristics that impossible for X-ray radiography.

The eight scalp needles, founded by Professor QIN Liang-fu, and its clinical experience for treatment of Parkinson's disease (PD) are introduced. Based on his years of clinical experience, it is proposed by Professor QIN that the Governor Vessel is mainly for miscellaneous disease and disease of limbs. Combined with distribution of cephalic motor region and meridian, an acupuncture treatment plan that is full of innovativeness is proposed, which is called Qin's eight scalp needles. It includes bilateral Fengchi (GB 20), Shuaigu (GB 8), Toulinqi (GB 15) as well as Yintang (GV 29) and Baihui (GV 20), mainly for treatment of nervous system diseases, such as PD and multiple sclerosis and so on. Besides, some outpatient cases are introduced to explain that eight scalp needle could alleviate the progression of PD, improve patients' motor, cognitive and affective disorders, reduce the suffering of patients, and improve the patient's quality of life.

OBJECTIVE: To study the chemical constituents of Scirpus yagara Ohwi. METHODS: Column chromatography techniques were applied to isolate the constituents. The chemical structures of the constituents were elucidated on the basis of physicochemical properties and spectral data. RESULTS: Forteen compounds were isolated and identified as sanleng diphenyllactone (1), β-sitosterol (2), ferulic acid(3), succinic acid(4), azelaic acid(5), docosanoic acid (6), 6, 7, 10-trihydroxy-8-octadecenoic acid(7), 4, 4-dime-thyl-1, 7-pimelic acid(8), vanillic acid(9), 3, 5-dihydroxy-4-methoxy-benzoic acid(10), p-hydroxybenzaldehyde(II), 3, 4-dihydroxy-benzoic acid(12), 2, 7-dihydroxy xanthone(13) and daucosterol(14). CONCLUSION: Compounds 1-10 and 12-14 are obtained from this plant for the first time, and compounds 1 and 13 were obtained from the genus Scirpus for the first time.

The damage to the barrier of colonic mucosal epithelial cells is the fundamental cause of bacterial translocation.The tight junction among epithelial cells forms the most important constituent part of the mechanical barrier of the organism to prevent bacterial translocation from happening in the intestinal canal.The para-colonic-mucosal-epithelial-cell space is closed by protein complexes which are considered to be used for regulating the permeability between cells.ZO-1,Claudin-1 and Occludin are the most representative among the numerous protein complexes.They have become the important index to judge the damage degree of the barrier of colonic mucosal epithelial cells in test.The endotoxin is the product released by bacteria.Anti-endotoxin is one of the methods to protect the colonic mucosal barrier,to prevent and treat bacterial translocation.There are anti-endotoxin drugs.However,there has not been any drug widely recognized clinically up to now.Qingyi Decoction and Chaiqin Chengqi Decoction are the traditional Chinese medicine with the function of protecting the intestinal mucosal barrier and suppressing the transloeation of enteric bacteria.Oxymatrine has many pharmacological effects such as anti-bacterial,anti-inflammatory and immunoragulation.The recent animal experiments showed that oxymatrine has protective effect on the intestinal mucosa during severe infection.However,the mechanism of the drug action has not been completely known and needs to be studied in depth.Our traditional Chinese medicine applies extensively in clinical application of the prevention of the damage to the colonic mucosal epithelial cells during severe infection.
